1.Application of PCR-reverse dot blot hybridization in the diagnosis of vulvovaginal candidiasis and the detection of Candida albicans drug-resistant genes

Objective To evaluate the application value of PCR-reverse dot blot hybridization in the identification of Candida and the detection of Candida albicans drug-resistant genes.Methods The vaginal secretion samples from 285 patients with candidal vaginitis and 50 healthy women were collected.The identification of Candida species and their drug susceptibility were detected by the bioMérieux Yeast identification cards and MIC method(Zhengzhou Antu kit),respectively.The identification of Candida species and the mutation of Candida albicans,drug-resistant genes were also detected by the Shenzheng Yaneng test kit(PCR-reverse dot blot hybridization).The drug-resistant genes were also identified by PCR and nucleic acid sequencing.Based on the culture identification,MIC method and nucleic acid sequencing as the contrast methods,the sensitivity,specificity and accuracy of PCR-reverse dot blot hybridization in the identification of Candida species and the mutation detection of Candida albicans drug-resistant genes were evaluated.Results Compared with the bioMérieux Yeast identification method,the sensitivity,specificity,positive predictive value,negative predictive value and total coincidence rate of PCR-reverse dot blot hybridization for detecting six kinds of Candida species,including Candida albicans,Candida glabrata,Candida tropicalis,Candida parapsilosis,Candida krusei and Candida guilliermondii,were above 95%,96%,96%,98% and 97%,respectively.There was no significant difference in detecting six kinds of Candida species between the two methods (x2 =0.44,0,0,0,0 and 0,respectively,P > 0.05),and there was good consistency between them (Kappa > 0.9).Compared with the MIC method,the sensitivity,specificity,positive predictive value,negative predictive value and total coincidence rate of PCR-reverse dot blot hybridization for detecting the drug resistance of Candida albicans were 98%,88%,98%,88% and 96%,respectively.There was no significant difference in detecting the drug resistance of Candida albicans between the two methods (x2 =0.17,P > 0.05),and there was good consistency between them (Kappa > 0.8).The results of PCR-reverse dot blot hybridization in detecting the mutation sites of six kinds of Candida albicans drug-resistant genes were 100% of coincidence with that of the nucleic acid sequencing method.Conclusion The PCR-reverse dot blot hybridization has high consistency with the culture method and the nucleic acid sequencing method in the identification of Candida species and the mutation detection of Candida albicans drug-resistant genes,which is more early and rapid than the traditional detection methods,and may be applied to the auxiliary diagnosis of vulvovaginal candidiasis (VVC).
2.Status and influencing factors of learned resourcefulness in pregnant women

Objective To investigate current status of learned resourcefulness in pregnant women,and to identify its influencing factors.Methods With convenient sampling,a self-made General Data Questionnaire and the Chinese version of Self-Control Schedule (C-SCS) was used to survey 360 pregnant women in the antenatal clinic in one three-class hospital in Guangzhou.Results The mean score of learned resourcefulness in pregnant women was (115.00±14.42) and its influencing factors included education degree,gravidity,pregnancy school training and planed pregnancy.Conclusions Learned resourcefulness in pregnant women loads upper level.Medical staff should take targeted interventions to strengthen it and facilitate the mental well-being of pregnant women.
3.Gene expression profiling reveals signal transduction and abnormal expression of cytokine gene in patients with primary Sjogren syndrome

Objective To investigate signal transduction and cytokine gene expression pattern in peripheral blood mononuclear cell (PBMC) between primary Sjogren syndrome patients and healthy controls. Methods The PBMC of 3 patients with Sjogren syndrome and 3 healthy volunteers with consistent age were collected. The total RNA extracted from the PBMC was carried the reverse transcription and in vitro transcription (IVT), then labeled with Cy5/Cy3 and hybridized on the gene chips. After scanning and data extraction with LuxScan 3.0, differentially expressed genes were analyzed with SAM method. The online tools of molecule analysis system (MAS) was used for biological knowledge mining. The difference of signal transduction and cytokine gene expression of each patient and control were compared. Results Statistical difference was calculated between the patient and control group in the following four pathway: the chemokine-cytokine receptor interaction pathway[(FASLG(Fas ligand), CCL5, CCR3, IL-4R, CXCL10 (CXC chemokine ligand 10), TNFRSF19L (tumor necrosis factor supeffamily 19 ligand), CX3CR1 (CX3C chemokine receptor 1)], adhension molecule pathway [interleukin adhesion molecules-1 (ICAM-1)], Jak-STAT signal pathway [STAT4 (signal transducer and activation of transcription 4), CISH (chromogenic in situ hybridization), IL-4R], Toll-like receptor signal pathway(CCL5, CXCL10). Among these, 4 genes were up-regulated and 6 genes were down-regulated. Conclusion Understanding of differently expressed genes should help us disclose the potential molecular mechanism underlying the development process of pathogenesis of primary Sjogren syndrome.
4.Expression of regulatory T cells and helper T cells in human IgA nephropathy and its significance

Objective To investigate the effects of CD4+CD25high regulatory T cells(Treg)and the imbalance of helper T lymphocyte subsets(Th1/Th2)on the immunological mechanism of IgA nephropathy(IgAN)patients. Methods The percentage of Treg and helper T cells subpopulation (Th1/Th2)in the peripheral blood of IgAN patients and healthy controls was examined by flow cytometry.The FOXP3 expression was detected through intracellular staining.The correlation of Treg or Th1/Th2 with clinical parameters of IgAN was analyzed by Spearman or Pearson rank correlation test. Results The percentages of Treg and Th2 cells were significantly higher in peripheral blood of IgAN patients compared to that of healthy controls[Treg (2.14±0.82)%vs[1.59±0.53)%,Th2(2.57±0.72)%vs(1.81±1.10)%,all P<0.05].Th1/Th2 ratio was significantly reduced in IgAN patients(5.75±1.89 vs 12.73±9.79,P<0.05).The percentage of circulating Treg cells was positively correlated with serunl IgA concentration(r=0.397,P<0.05),and was negatively correlated with eGFR(r=-0.376,P<0.05).The percentage of circulating Th2 cells was positively correlated with serum IgA(r=0.468,P<0.05). Conclusions There is a disorder of T lymphocyte population in the peripheral blood of IgAN patients.The increased Treg and Th2 cells may play an important role in the pathogenesis of IgAN.
5.Lifestyle intervention improves oxidant stress in overweight or obese adolescents

Objective To explore the effects of diet and/or exercise intervention on oxidative stress of overweight or obese adolescents.Methods Thirty-one adolescents with normal body weight(normal control group)and 93 overweight or obese adolescents(average age(13.6 ±0.7)years; body mass index (BMI)22.4 to 34.1 kg/m2)were enrolled in this study.The overweight or obese participants were then randomly assigned to the diet group(group A),exercise group(group B),diet plus exercise group(group C),and non-intervention group(group D).Individualized caloric intake was carried out.One-hour afterschool exercise was performed once per day,4 days per week for 10 weeks.Changes of anthropometry,body composition,and metabolic biomarkers were determined.Results Compared to the normal control group,serum levels of fasting glucose,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and malondialdehyde(M DA)of the in overweight group were significant increased(all P < 0.05),although the level of SOD was significant decreased(P < 0.05).Serum MDA level was positively correlated with bodyweight,BMI,TC,and LDL-C(R values were 0.209,0.228,0.274,and 0.263,respectively ; all P <0.05),and serum SOD level was negatively correlated with bodyweight and BMI(R values were-0.334 and-0.362,respectively ; both P < 0.05).Group A,B and C showed a significant decrease in BMI,waist circumference and body fat after the intervention(all P < 0.05).The level of MDA was significantly decreased after the intervention in group A and B(both P < 0.05); the levels of TC and LDL-C were significantly decreased in group B and C(both P < 0.05).Conclusion:Oxidative stress could have existed in overweight adolescents even when their serum glucose and lipid profiles are at a normal level.Ten-week diet and/or exercise intervention may contribute to improved body weight control,lipid metabolism,and oxidative stress response in this population.
6.Effects of massage along meridians based on different stages of Brunnstrom on lower extremity function in patients with convalescent stroke

Objective To observe the effects of massage along meridians based on different stages of Brunnstrom on lower extremity function in patients with convalescent stroke. Methods Ninety patients with convalescent stroke and lower limb dysfunction who were hospitalized in our hospital's brain center from September 2016 to August 2018 were selected. They were randomly assigned to an observation group of 45 patients and a control group of 45 patients. Both groups were given basic rehabilitation therapy. The control group was given conventional exercise therapy, and the observation group was given additional massage based on different stages of Brunnstrom. The two groups were treated for6 weeks, and the FMA-L, MAS and BBS scores before and after treatment were evaluated. Results There were no significant differences in FMA-L, MAS and BBS scores between the two groups (P>0.05). After 6 weeks of treatment, the FMA-L and BBS scores of the two groups were significantly higher than the pre-treatment scores, while the MAS scores were lower than the pre-treatment scores (P<0.05 or P<0.01). Compared with the control group, the improvement of FMA-L and BBS scores and the reduction of MAS score in the observation group was more significant (P<0.05 or P<0.01). Conclusion On the basis of conventional treatment methods, the application of massage therapy based on different stages of Brunnstrom is more helpful to improve the balance function of patients during stroke recovery, reduce muscle tension of lower limbs and to improve functional activities of lower limbs.
7.Correlation of peritoneal albumin leakage with malnutrition-inflammation-atherosclerosis syndrome in continuous ambulatory peritoneal dialysis patients

Objective To investigate the impact of peritoneal albumin leakage on malnutrition-inflammation-atherosclerosis (MIA) syndrome in continuous ambulatory peritoneal dialysis (CAPD) patients. Methods A cross-sectional study of a cohort of 130 CAPD patients without edema or active infection was performed. In order to identify peritoneal transport characteristics in CAPD patients, a standard peritoneal equilibration test (PET) was carried out. For malnutrition and inflammation, serum albumin and high-sensitivity C-reactive protein (hs-CRP) levels were measured. Mean-carotid artery intima media thickness (IMT) was used to determine atherosclerosis. Residual glomerular filtration rate (rGFR) was defined as the average of 24-hour urinary urea and creatinine clearances. Results Pearson and Spearman correlation analysis showed that peritoneal albumin leakage amount was positively correlated with age, body mass index, night dwell time, blood glucose, 4 h D/P creatinine levels and hs-CRP levels (r=0.204, P<0.05 ;r=0.314, P<0.01; r=0.265, P<0.01; r=0.212, P<0.05; r=0.401, P<0.01; r=0.216, P<0.05); whereas it was negatively correlated with diastolic perssure, serum albumin levels, glucose level of dialyzate and peritoneal Kt/V (r=-0.209, P<0.05; r=-0.123, P<0.05; r=-0.271, P<0.01; r=-0.212, P<0.01). Overall, there was no correlation between peritoneal albumin leakage and IMT. Patients was significantly greater (P<0.01), and there was a positive correlation between peritoneal albumin leakage amount and IMT (r=0.650, P<0.01). Conclusions Peritoneal albumin leakage is significantly associated with peritoneal transport characteristics, malnutrition and inflammatory state in CAPD patients. High peritoneal albumin leakage amount is a risk factor for atherosclerosis in patients with rGFR less than 1 ml·min-1(1.73 m2)-1.
8.Role of DNA recognition receptors in pathogenic mechanism of hepatitis B

As a global disease,hepatitis B still threatens human health.However,the pathogenesis of hepatitis caused by HBV remains unclear.The innate immune system in the liver can detect HBV infection and use every strategy to eliminate the virus.DNA recognition receptors play an important role in this process;they recognize tlBV DNA or pgRNA in cytoplasm or nucleus,activate innate immunity through various signaling pathways to produce inflammatory cytokines and interferon,and finally exert their antiviral effect.This article summarizes the DNA recognition receptors involved in inflammation induced by HBV and HBV clearance,elaborates on their detailed pathways,and discusses the issues regarding the role of DNA recognition receptors in liver innate immunity induced by HBV and related perspectives.
9.Effects of dexmedetomidine on alveolar epithelial barrier function in rats with VILI and the role of PKC

Objective:To evaluate the effects of dexmedetomidine on alveolar epithelial barrier function in rats with ventilator-induced lung injury (VILI), and the role of protein kinase C (PKC).Methods:One hundred clean-grade male Sprague-Dawley rats, weighing 270-320 g, aged 4-5 months, were divided into 5 groups ( n=20 each) using a random number table method: control group (group C), VILI group (group V), PKC inhibitor group (group B), dexmedetomidine group (group D), and dexmedetomidine plus PKC agonist group (DP group). The VILI model was developed by mechanical ventilation with a tidal volume of 40 ml/kg for 4 h in anesthetized animals.Group C breathed air autonomously for 4 h without mechanical ventilation.Group V was mechanically ventilated for 4 h. In group B, bisindolvlmaleimide I 0.12 mg/kg was injected intramuscularly 1 h before mechanical ventilation.In D and DP groups, dxmedetomidine 5.0 μg/kg was injected intravenously at 20 min before mechanical ventilation, and dexmedetomidine was intravenously infused at the rate of 5.0 μg·kg -1·h -1 during mechanical ventilation.In group DP, PKC agonist phorbol-12-myristic acid-13-acetate 15 μg/kg was intraperitoneally injected at 30 min before mechanical ventilation.At 4 h of mechanical ventilation, oxygenation index (OI), lung permeability index (LPI) and wet/dry lung weight (W/D) ratio were measured, the pathological changes of lung tissues were observed, and lung injury was assessed and scored.The expression of PKC, occludin and ZO-1 protein was detected by Western blot, and the expression of PKC mRNA, occludin mRNA and ZO-1 mRNA was determined by real-time polymerase chain reaction. Results:Compared with group C, OI was significantly decreased, LPI, W/D ratio and lung injury score were increased, the expression of PKC protein and mRNA was up-regulated, and the expression of occludin and ZO-1 protein and mRNA was down-regulated in V and DP groups ( P<0.05), and no significant change was found in the parameters mentioned above in B and D groups ( P>0.05). Compared with group V, OI was significantly increased, LPI, W/D ratio and lung injury score were decreased, the expression of PKC protein and mRNA was down-regulated, and the expression of occludin and ZO-1 protein and mRNA was up-regulated in B, D and DP groups ( P<0.05). Compared with group D, OI was significantly decreased, LPI, W/D ratio and lung injury score were increased, the expression of PKC protein and mRNA was up-regulated, and the expression of occludin and ZO-1 protein and mRNA was down-regulated in group DP ( P<0.05). Conclusions:Dexmedetomidine can reduce the damage to alveolar epithelial barrier function in rats with VILI, and the mechanism is related to inhibition of PKC activation and up-regulation of the expression of occludin and ZO-1.
10.Transjugular intrahepatic portosystem shunt plus gastric coronary vein embolization for cirrhotics portal hypertension complicating upper gastrointestinal bleeding

Objective To evaluate the clinical efficacy of transjugular intrahepatic portosystem shunt (TIPS) and gastric coronary vein embolization (GCVE) in the treatment of patients with cirrhosis,portal hypertension and upper gastrointestinal bleeding.Methods From Jan 2014 to May 2017 72 patients were enrolled and divided into the TIPS group (36 cases,receiving TIPS) and TIPS + E group (36 cases,byTIPS+GCVE).Results Portal vein diameter (1.21 ±0.08)cm vs.(1.26 ±0.09)cm,portal pressure (23.9 ± 2.1) cmH2O vs.(25.1 ± 2.2) cmH2O and congestion index (0.06 ± 0.03) cm/s vs.(0.08 ±0.03) after 1 month of treatment in TIPS + E group was significantly lower than the TIPS group,and the portal vein velocity was significantly higher than that of the TIPS group (42 ± 6) cm/s vs.(38 ± 7) cm/s,t =2.491,2.367,2.828,t =2.343,all P < 0.05.The Child-Pugh score in the TIPS + E group was significantly lower than that in the TIPS group (7.9 ± 1.4) vs.8.6 ± 1.6,t =2.074,P =0.042).There was no statisticall different difference in postoperative hepatic encephalopathy in the two groups (17% vs.11%,x2 =0.465,P =0.496).The one-year rebleeding rates in the TIPS group and the TIPS + E group were 14% and 3%,respectively.The risk of rebleeding in the TIPS + E group was significantly lower than that in the TIPS group (HR =0.218,P =0.041).The one-year access obstruction rates in the TIPS group and the TIPS + E group were 17% and 14%,respectively.(P =0.679).The all-cause mortality rates of the TIPS group and the TIPS + E group were 8% and 3%,respectively,showing no statistically (P =0.299).Conclusions TIPS + GCVE therapy in the treatment of portal hypertensive upper gastrointestinal bleeding effectively reduces the risk of rebleeding.
