1.Validity analysis of pulmicort respulas combined with montelukast in treating children with asthma and its role in pulmonary function and T lymphocyte subset

Objective To explore the validity of oxygen atomizing inhalation pulmicort respulas combined with montelukast in treating children with asthma and its role in pulmonary function and T lymphocyte subset.Methods Totally 78 cases treated in Huizhou People's Hospital from June,2014 to June,2016 were randomly divided into observation group and control group with each group of 39 cases.The control group on the basis of routine treatment with oxygen atomizing inhalation of pulmicort 1 mg,twice daily,and the observation group based on control group added Montelukast Sodium Chewable Tablets 5 mg,once daily,one week for one course.The clinical effect,pulmonary function,and level of T lymphocyte subset were compared between two groups.Results The total effective rate in the observation group was 94.70%,which was significantly higher than control group (82.05%,P < 0.05).After therapy,the symptom scores of two groups were obviously decreased compared with those before therapy (P < 0.05),and those of the observation group were significantly lower than control group (P < 0.05);The pulmonary function of the two groups were obviously improved (P < 0.05) compared with that before therapy (P < 0.05),and the levels of FEV1,FVC,and FEV1/FVC in observation group were significantly higher than control group (P < 0.05);The level ofT lymphocyte subset in control group showed no statistical difference compared to that before therapy,and the levels of CD4+ and CD4+/CD8+ in observation group were obviously decreased,whereas CD8+ was increased.Conclusion Oxygen atomizing inhalation pulmicort respulas combined with montelukast could effectively increase the clinical effect on children with asthma,improve pulmonary function and positively regulate immune function,which deserves clinical expansion.
2.Toxicity Observation on Tongbi Tablets

This paper reported the acute toxicity test and the long-term toxicity test of Tongbi Tablets in rats and dogs. The LD_(50) of mice was 5.29g/kg. For dogs following oral administration (0.513g/kg/day, which was a tenth part of mice'LD_(50)) for 6 months running, the toxic symptom of strychnine-like and an increase in BUN could be-observed after 4-month administration. Both recoveried after breaking off the administration. For 0.051g/kg/day dosage group of dogs(administered for 6 months running)and all dosage groups of rats, no toxic symptom was observed, suggesting that the clinical dosage of this medicine was safe.
3.Effect of nCPAP on ABR in Patients with OSAHS

Objective By researching the auditory brainstem response(ABR) latency and interpeak latency intervals before and after continuous positive airway pressure through the nares(nCPAP), the experiment explored the pathophysiological and anatomical basis of electric physiological change in patients with obstructive sleep apnea/hypopnea syndrome(OSAHS).Methods The ABR of 45 OSAHS patients(90 ears) and control group(32 ears)was examined,the sleep of patients with OSAHS was monitored by polysomnography(PSG).The results of ABR before and after nCPAP were compared.Results 1.The latency of wave III? V and interpeak latency intervals of I-III,III-V,I-V of patients were longer than that of the control group.2.After nCPAP,wave III and interpeak latency intervals of I-III,III-V changed obviously.Conclusion 1.The OSAHS patients' auditory brainstem structure is impaired,which has relationship with high carbonic acid and low oxygen saturation. 2.Except wave III,produced by upper nucleus olivaris that is near the breathing center,effect of nCPAP on ABR of patients with OSAHS is not obvious.
4.Clinical application of remifentanil in awake state for removal of tumors in functional brain area

Objective To investigate the clinical application of remifentanil in awake state for removal of tumors in functional brain area. Methods Twenty-four patients with tumors in functional brain area were divided into two groups by random digits table with 12 cases each:control group and remifentanil group. Infiltration anesthesia was used in all patients with 0.2% ropivacaine. Remifentanil was injected intravenously with micropump in 0.05-0.10 μg/ (kg·min) in remifentanil group and 0.9% sodium chloride was instead of remifentanil in control group. Patients remained awake during surgery in both groups. The hemodynamic changes and complications during operation were monitored. The satisfaction for surgical procedure was evaluated. Results The patients in two groups could be completed in awake state with surgery;mean artery pressure, heart rate in remifentanil group during opening or closing skull or intracranial period were significantly higher than those in control group(P< 0.05). There were no conspicuous complication in two groups such as respiratory depression, nausea, vomitting and dysphoria. The satisfaction score in remifentanil group[(3.3 ?0.6) scores] was higher than that in control group[(2.4 ?0.5) scores],there was significant difference between two groups (P<0.05). Conclusion Awake brain tumor surgery could be completed in rational use of remifentanil on the base of good local anesthesia, and hemodynamics are stable and the patients are well tolerated.
5.Protective effects of propofol pretreatment on ischemia-reperfusion injury in rabbit heart

Objective To study the protective effects of propofol pretreatment on ischemia-reperfusion injury in rabbit heart. Methods Forty New Zealand white rabbits (40 male, weighting 2. 5 ± 0. 36kg) were randomly assigned to 4 groups, including sham operation group, ischemia reperfusion (IR)group, IR + ischemia preconditioning (IP) group and IR + Propofol preconditioning (PP) group (n =10 each). The myocardial ischemia-reperfusion model of rabbits was established by coronary artery ligation.The levels of high energy phosphates (ATP, ADP and AMP) in myocardial tissue were measured by HPLC.The activities of serum lactate dehydrogenase (LDH), serum creatine kinase-MB (CK-MB) superoxide dismutase (SOD) and malondiadehyd (MDA) in myocardial tissue were assayed. Results The content of ATP, ADP, AMP, and the activity of SOD in PP group and IP group were higher than that of IR group, and the content of LDH, MDA and CK-MB in injured myocardial tissue in PP group and IP group were obviously lower than that of IR group (P <0. 05, P <0. 01). No difference was found in PP group and IP group(P>0. 05). Conclusion Propofol preconditioning as well as ischemia preconditioning protected the myocardial tissues from the ischemia-reperfusion injury by improving the myocardial energy metabolism, the debasement of the oxyradical level and the antagonism the reaction of lipid peroxides.
6.Effectiveness of community-hospital-based integrated management of diabetes patients in Xiamen, Fujian province

During 2004 to 2009, 701 patients with diabetes were under community-hospital-based integrated management at Zhonghua and Xiagang community health-care service centers, Xiamen, Fujian.Changes of clinical indicators, life-style and frequency of checks for its complications in them were compared before and after the management. All clinical indicators decreased with length of management, with fasting blood glucose from ( 7. 6 ± 2. 3 ) mmol/L before management to ( 7. 3 ± 2. 0) mmol/L half a year after management, (7. 2 ± 2. 1 ) mmol/L one year after management, (6. 9 ± 1.7 ) mmol/L two years after management. Their life-style also improved significantly and frequency of checks for complications increased with: 54. 4% for retinopathy, 52. 5% for diabetic foot, and 51.8% for neuropathy after management (P <0. 05).
7.Comparative study of intravenous medication and pelvic perfusion on patients with gynecological pelvic inflammatory disease

Objective To investigate the effects of intravenous medication and pelvic perfusion in the treatment of gynecological acute and chronic pelvic inflammatory disease.Methods 120 cases with pelvic inflammatory disease were randomly divided into two groups.The control group(60 cases) received intravenous drugs.The observation group(60 cases) were given pelvic infusion therapy.The treatment efficacy of the two groups was compared after treatment for 14 days.Results The obvious effective rate of the observation group was 80.00%,which was significantly higher than 50.00% in the control group(x2 =11.58,P < 0.05).The total effective rate in the observation group was 90.00%,which was significantly higher than 68.33% in the the control group(x2 =13.72,P < 0.05).The incidence rate of adverse reactions in the control group was 38.33%,that in the observation group was 20.00%,the difference between the two groups was significant(x2 =4.18,P < 0.05).Conclusion Pelvic perfusion therapy can improve the clinical efficacy of pelvic inflammatory disease,improve the symptoms of patients,reduce side effects,which is worthy of clinical application.

9.Cost-effectiveness analysis on Imipenem-Cilastatin Sodium and Meropenem for severe infection

Objective To evaluate the economic effects of Imipenem-Cilastatin Sodium and Meropenem for severe infection.Methods The therapeutic effects and costs of the two therapeutic schemes for severe infection were evaluated by using cost-minimization analysis.Results The total effective rates of Imipenemcilastatin group and Meropenem group in the treatment severe infection were 93.33% (28/30) and 85.71% (24/28),the difference were not statistically significant (x2 =4.89,P =0.082).The cost-effectiveness ratio (C/E) were 40.35 and 71.69 respectively.The cost of every unit increment of effectiveness for Meropenem group were 312.16 Yuan,more than that of the Imipenem-cilastatin group.Conclusion Cost-effectiveness ratio of Imipenem-cilastatin is superior to that of Meropenem,and Imipenem-cilastatin has obvious pharmacoeconomics advantage over Meropenem for severe infections.

Objective To investigate the effect and mechanism of invigorating spleen,eliminating stasis, heat-clearing drugs(milk veteh,root of red rooted salvia,Chinese goldthread)on quality of ulcer healing (QOUH).Methods The rat models of chronic gastric ulcer was induced by acetic acid,the effects of QOUH of milk veteh,root of red rooted salvia and Chinese goldthread on the model of gastric Ulcer were observed, the expression of EGFR mRNA of mucosa were measured by in situ hybridization.Results Milk veteh,root of red rooted salvia and Chinese goldthread could improve the injury of gastric mucosa.Milk veteh,root of red rooted salvia increased the expression of EGFR mRNA in the tissue around peptic ulcer(PU). Conclusion Invigorating spleen,eliminating stasis,heat-clearing drugs can improve QOUH,it is one of possible mechanisms that the drugs increased the expression of EGFR mRNA in the tissue around PU.
