1.Neural Mechanisms of Forward Conditional Reasoning:A Functional MRI Study

Purpose Two kinds of forward conditional reasoning including modus ponens (MP) and denial of the antecedent (DA) are the most common deductive reasoning in daily life. The present study aimed to explore the difference between the two neural machanisms. Materials and Methods Three kinds of tasks including MP, DA, and baseline were administrated to 14 undergraduates [7 males and 7 females, mean age (23.4±1.3) years] by using functional magnetic resonance imaging (fMRI) technique. fMRI data was analyzed. Results The reaction times were significantly slower for MP and DA than that of BS (P<0.001), while the effect was not significant for accuracy. In contrast to baseline, both MP and DA tasks activated the left inferior frontal cortex (BA 9), inferior parietal lobule (BA 40), and postcentral gyrus. Bilateral caudate significant activation was found in MP task when compared with DA, whereas the frontal, parietal, occipital lobe and anterior cingulate cortex were acivated more in the DA than in MP task. Conclusion Both forward conditional reasoning of MP and DA commonly activate the left frontoparietal cortex, while significant dissociations can be seen in the regions of caudate and bilateral frontal, parietal and occipital. These results firstly demonstrates the different neural mechanism underlying the two forward conditional reasoning, which might help to further uncover the brain mechanism of conditional reasoning.
2.Parameters of Median Nerve Electrical Stimulation for Coma after Traumatic Brain Injury

Objective To compare the effects of median nerve electrical stimulation on coma patients after traumatic brain injury with different settings. Methods From 2013 to 2015, 161 patients with traumatic brain injury were randomly divided into control group (n=40), experimental group 1 (n=41), experimental group 2 (n=39) and experimental group 3 (n=41). The control group received routine conscious-ness-promoted methods, and the experimental groups received median nerve electrical stimulation with 200μs and 30 Hz, 100 Hz and 50 Hz in sequence, 60 minutes a day for 90 days. They were assessed with Glasgow Coma Scale (GCS) and Coma Recovery Scale-Revised (CRS-R) before, 30 days and 90 days after treatment. Results There was significant difference in the scores of CCS and CRS-R, times of treatment, number of sobered patients and coma time among groups (P<0.01), that the experimental groups were better than the control group (P<0.05), and no significant difference was found between the experimental groups 1 and 2 (P>0.05). The experimental group 3 was better than the experimental groups 1 and 2 (P<0.05). Conclusion Median nerve electrical stimulation with 200μs, 50 Hz could promote co-ma patients to wake up optimally.
3.Neisseria gonorrhoeae multi-antigen sequence typing using urine samples

Objective To determine Neisseria gonorrhoeae multi-antigen sequence typing (NG-MAST) sequence types in different geographical areas of China,including Changzhou and Yangzhou cities of Jiangsu province,Wuzhou and Hezhou cities in Guangxi Zhuang Autonomous region,Sanya and Qionghai cities of Hainan province,Jiangmen and Maoming cities of Guangdong province.Methods DNA was extracted using Qiagen DX extraction kits from 88 urine samples which were collected from male patients attending sexually transmitted disease (STD) clinics and positive for nucleic acid amplification test (NAAT) for N.gonorrhoeae.Two rounds of PCR were carried out to amplify the porB and tbpB genes of N.gonorrhoeae followed by gene sequencing.Sequence alignment was performed on the NG-MAST website (http://www.ng-mast.net) to determine the genotype of N.gonorrhoeae.Results The first-round PCR yielded positive results for porB and tbpB in 13.6% (12/88) and 14.8% (13/88),respectively,of these urine specimens,and 12 samples were successfully genotyped with the efficiency of genotyping being 13.6%.The amplification efficiency of second-round PCR was enhanced to 71.6% and 72.7% for porB and tbpB,respectively,and the efficiency of genotyping increased to 70.5% (62/88).Compared with the first-round PCR,the second-round PCR showed an increase in amplification efficiency for porB and tbpB by 58.0% and 57.9% respectively,as well as in genotyping efficiency by 56.9%.Forty-five genotypes were identified in the 62 samples,including 40 known genotypes and 5 novel genotypes.Of these genotypes,ST1866 was the most abundant (6/62),followed by ST1972 (4/62) and ST3356 (4/62),all of which were from Jiangsu province.The ST532 genotype was identified in 3 samples from Guangdong province,ST2221 genotype in 2 samples from Guangxi Zhuang Autonomous region.Each of the remaining genotypes was identified in only 1 sample and scattered in all of these cities.The 5 novel MAST-genotypes were as follows:porB-892 and tbpB-46 (98% similarity),porB-130 and tbpB-504 (96% similarity),porB-2790 and tbpB-32 (99% similarity),porB-1053 and tbpB-856 (99% similarity).Conclusions Urine samples can be used for NG-MAST analysis,and two rounds of PCR can enhance the efficiency of genotyping.NG-MAST genotypes appear to be diverse in different geographical areas of China.
4.Correlation between ultrasonography and relevant techniques in traumatic lipohemarthrosis

Objective To explore the correlation between ultrasonographic findings and relevant technical factors in traumatic lipohemarthrosis. Methods Fifty-two patients with traumatic lipohemarthrosis underwent ultrasonic examination in supine position (26 in unbent, 16 in external rotary position, 10 in unbent and 45° rotation) before operation, and 22 of them underwent dynamic examination. Findings of ultrasography were compared with manifestations of MRI and CT regarded as gold standard.Results Fifty-two patients with traumatic lipohemarthrosis were diagnosed correctly. The best effect of ultrasonic examination of lipohemarthrosis was observed on suprapatellar bursa. Superior border of patella were showed well in supine straight position, then turned to the level of patella, and the structure below patella were showed badly. The best posture was in supine genuflex position with 45° rotation when the joint effusion presented, by which thicker layer liquids could be displayed than in straight position. Three layer liquids could not be displayed synchronously in mild external rotary rest position of lower extremity, and the combination of findings at internal and external side were favorable to the diagnosis. Liquids bed changed from one layer to two layers and then to three layers, from fuzzy to clear gradually in dynamic examination.Conclusion Ultrasonic manifestations of traumatic lipohemarthrosis are closely correlated to the position, posture and arresting time during examination.
5.Observation on the optimal brakingtime for ultrasonography of traumatic lipohemarthosis

Objective To investigate the braking time in diagnosis of lipohemarthrosis by ultrasound. Methods Experiment group:After 20 tubes containing fresh blood and pig marrow were uniformly mixed, they were instantly continuously detected by high-frequency linear array transducer in fixed-area at short-interval.Clinical group: Ten walking patients with lipohemarthrosis were examined with continuous detection by ultrasound,CT and MR before operation.Results Experiment group: Cloudy echo and slowly floating up of lipoids were detected at the beginning.After 2.7 to 3.7 minutes (average 3.17 minutes), the liquid was divided into two layers with vague boundary.After 5.3 to 8.0 minutes (average 6.75 minutes) ,the liquid was divided into 3 layers with thickening serum laye.Clinical group: Ultrasound findings: Cloudy medium echo was detected at first.After 2.0 to 4.0 minutes (average 3.08 minutes),the liquid was divided into two layers.After 4.4 to 10.0 minutes (average 6.92 minutes) , the liquid was divided into 3 layers with thickening serum layers.Three knees showed fracture line and 2 knees with occult fracture were diagnosed as lipohemarthrosis.CT and MR findings: Ten knees showed fracture line in CT examination,of which 7 knees showed double liquid-liquid layer and 3 knees showed single liquid-liquid layer in the suprapatellar bursa.Ten knees showed fracture line in MR examination,6 knees showed double liquid-liquid layer and 4 knees showed single liquid-liquid layer in the suprapatellar bursa. Conclusions The best braking time in diagnosing lipohemarthrosis by ultrasound can be shorten to 10 minutes and the necessary braking time is 2 to 4 minutes.
6.The study on the influencing factors of rheumatism's treatment

Objective To study influencing factors in patients with rheumatic diseases.Methods One hundred and twenty patients with rheumatic diseases were selected from November 2013 to February 2014 in our hospital for the study.The influencing factors in patients with rheumatic diseases were obtained by proposed questionnaires,and then the univariate and multivariate Logistic regression analysis were performed to study factors affecting the treatment.Results Patients older than 50 of the observation group accounted for 38% (18/48),the proportion of farmer,patients with primary school education level or below,and monthly income <3 000 RMB was 35%(17/48),60%(29/48),and 46% (22/48) respectively.Compared with the control group,they were significantly higher than the control group,which were 64%(46/72),58% (42/72),79%(57/72),65%(47/72) (x2=8.058,P=0.005; x2=6.025,P=0.014; x2=4.986,P=0.226; x2=4.456,P=0.035) respectively.Using the treatment as the dependent variable,age ≥50 years of age,farmer,primary school and lower education level,and monthly income <3 000 RMB as the independent variable,we carried out the regression analysis.Age (OR=1.124,95%CI:1.084-7.236),occupation (OR=1.871,95%CI:1.054-7.243),education level (OR=1.982,95%CI:1.157-6.256),monthly income (OR=1.363,95%CI:1.012-8.227) are the influencing factors of rheumatism's treatment.Conclusion The treatment of rheumatic diseases are influenced by many factors,and more societal support should be provided to help patients with rheumatic disease receive professional treatment and better control of the disease.
7.Diagnostic value of Copenhagen index in premenopausal and postmenopausal ovarian cancer

Objective:To compare the diagnostic value of Copenhagen index (CPH-I), serum carbohydrate antigen 125 (CA125), human epididymis protein 4 (HE4), and risk of ovarian malignancy algorithm (ROMA) for diagnosis of ovarian cancer in premenopausal and postmenopausal women.Methods:The clinical data of 239 patients with ovarian tumor treated in People's Hospital of Rizhao in Shandong Province from January 2017 to January 2020 were retrospectively analyzed. The patients were divided into ovarian benign disease group (152 cases) and ovarian cancer group (87 cases) according to postoperative pathology. The receiver-operating characteristic curve (ROC) was drawn with surgical pathology as the gold standard; the area under the curve (AUC) and the sensitivity and specificity of CPH-I, CA125, HE4, ROMA were calculated. The diagnostic performance of CA125, HE4, ROMA and CPH-I for diagnosis of ovarian cancer was compared in overall, premenopausal and postmenopausal patients.Results:The CA125 level, HE4 level, ROMA index, and CPH-I predicted probability (PP) values of ovarian cancer group were higher than those of ovarian benign disease group, and the differences were statistically significant (all P < 0.01). The AUC of CA125, HE4, ROMA, and CPH-I in the overall patients was 0.935 (95% CI 0.896-0.963), 0.940 (95% CI 0.901-0.966), 0.964 (95% CI0.932-0.984), 0.964 (95% CI 0.932-0.984); the AUC differences of CA125 and ROMA, CA125 and CPH-I (PP values), HE4 and ROMA, HE4 and CPH-I PP values were statistically significant (P values were 0.036, 0.009, 0.018, 0.019). The AUC of HE4, ROMA, and CPH-I in the premenopausal patients was 0.947 (95% CI 0.896-0.978), 0.949 (95% CI 0.898-0.979), 0.944 (95% CI 0.893-0.976), which were all larger than AUC of CA125 (0.921) (95% CI 0.863-0.960), the differences were statistically significant (P values were 0.036, 0.036, 0.026); AUC of CA125, ROMA, CPH-I PP values in postmenopausal patients was 0.953 (95% CI 0.891-0.986), 0.947 (95% CI 0.882-0.982), 0.943 (95% CI 0.877-0.980), all of which were larger than AUC of HE4 (0.889) (95% CI 0.810-0.944), and the differences were statistically significant (P values were 0.029, 0.014, 0.015). Conclusions:The diagnostic efficacy of CPH-I and ROMA for ovarian cancer is comparable, and regardless of menopause or not, the diagnostic efficacy of CPH-I and ROMA is higher. The diagnostic efficacy of CPH-I and ROMA for ovarian cancer is better than that of CA125 and HE4 in overall patients, the diagnostic efficacy of CA125 is the lowest in premenopausal patients, and the diagnostic efficacy of HE4 is the lowest in postmenopausal patients.
8.Screening molecular markers in early breast cancer of the same pathological types but with different prognoses using Agilent gene chip.

OBJECTIVETo screen molecular markers in early breast cancer and establish gene subtyping-based diagnostic criteria for predicting the prognosis of early breast cancers.
METHODSTumor tissue specimens were obtained from 8 patients with early breast cancer for analysis of the differentially expressed genes using Agilent custom 8×15 000 chips in combination with the prognostic data of the patients. Another 42 tumor tissue specimens were used to validate the differential genes by real-time fluorescent quantitative PCR.
RESULTSGene microarray analysis identified 132 differentially expressed genes between the patients with favorable and poor prognosis, and 44 of these genes were significantly up-regulated (by over two folds) and 88 down-regulated in patients with poor prognoses.
CONCLUSIONThe gene expression profiles differ in early breast cancer tissues of the same pathological type but with different clinical stages and prognoses, and CD44, MKI67, NTRK2, Nek2, C16orf60, TOP2A, ANCCA, and RRM2 genes can be used as the prognostic markers for early breast cancer.

9.Analysis of genomic characteristics and virulence factors of emetic-type Bacillus cereus strains isola-ted in Hangzhou

Objective To investigate the genomic characteristics and virulence factors of emetic-type Bacillus cereus strains isolated from food in Hangzhou for better understanding their pathogenic potential. Methods Real-time PCR was performed to detect the ces gene cluster ( cereulide) in 132 Bacillus cereus strains isolated from food from 2015 to 2017. Genomes of cereulide-positive strains were sequenced using Illumina MiSeq sequencing platform. Genome annotation, virulence factor detection, comparative and evolu-tionary analysis were performed after the sequences of genomes were assembled. Results Twelve strains (9. 09%) carried the ces gene. Their genome sizes ranged from 5. 35 to 5. 75 Mb and GC contents from 35. 25 to 35. 43 mol%. All of them harbored the full cereulide biosynthesis gene cluster, nonhemolytic ente-rotoxin ( NHE)-encoding gene cluster ( nheA, nheB and nheC) and hemolysinⅢ( hlyⅢ) . The average nu-cleotide identity ( ANI ) between the 12 isolates and the reference strain NC7401 ( Accession number:AP007209) was over 99. 35%. Phylogenetic analysis demonstrated these strains were clustered into the same branch with local clinical isolates and the emetic-type Bacillus cereus strains of NC7401 and AH187. Con-clusions The genomic sequences of the emetic-type Bacillus cereus strains isolated from food in Hangzhou area were highly similar to that of the reference strain NC7401. Results of the genomic analysis suggested that these isolates carried many virulence factors that were related to pathogenicity.
10.Heparin inhibits burn-induced spleen cell apoptosis by suppressing interleukin-1 expression.

BACKGROUNDEpidermal burn injury may trigger significant apoptosis of the spleen cells, which might be caused by a burn-induced systemic inflammatory reaction. Heparin has been shown to possess anti-inflammatory properties. Interleukin 1 (IL-1) is centrally important among pro-inflammatory cytokines. We hypothesized that heparin might inhibit burn-induced apoptosis in the spleen via suppression of the IL-1 pathway.
METHODSBurn injury was performed on IL-1 R+/+ ( IL-1 receptor wild-type mouse) and IL-1 R-/- (IL-1 receptor knock-out mouse) mice, and they were then treated with heparin, saline or IL-1 receptor antagonist IL-Ra. Apoptosis, IL-1α and IL-1β expression were assessed in the spleens and serum. Survival curve analysis was further applied to elucidate the mechanism of heparin's protective properties.
RESULTSBurn induced significant apoptosis (sham: 3.6%± 2.1% vs. burn: 28.8%± 5.9%; P < 0.001) and remarkable expression o IL-1α and IL-1β in the mouse spleens and serum. Heparin reduced the burn-induced apoptosis in the spleens (heparin treated: 8.6%± 3.4%, P < 0.005), which could be blocked by IL-1Ra. Heparin markedly decreased both IL-1α and IL-1β expression in the spleens and serum of burned mice. IL-1 R-/- mice demonstrated considerably less apoptosis in the spleens and had a higher survival rate after burns. Heparin did not significantly decrease apoptosis in the spleen and the mortality rate in IL-1 R-/- mice after burns.
CONCLUSIONHeparin inhibits burn-induced apoptosis of the spleen cells by suppressing IL-1 expression in mice.
