1.Clinical observation of radical postoperative pain oxycodone controlled intravenous analgesia for the treatment of cervical cancer

Objective To observe the curative resection of oxycodone hydrochloride controlled intravenous analgesia pump after treatment of cervical cancer (patient-controlled-intravenous analgesia, PCIA) and the adverse effect of pain.Methods March 2016 to February 2017 undergoing open radical resection of cervical cancer patients with 108 cases, were randomly divided into observation group(n=54) and control group(n=54), the observation group was given oxycodone hydrochloride injection intravenous patient-controlled analgesia pump, the control group were treated with Sufentanil Citrate Injection patient-controlled analgesia pump, anal exhaust time, contrast two groups of patients with adverse drug reactions and postoperative 6h, 12h, 24h, 48h (NRS Numeric Rating Scale, NRS) pain score.ResultsPain score: 12h observation group compared with the control group NRS pain score (P<0.05), with statistical difference, 6h, 24h, 48h time group NRS pain scores haveno statistical difference.Adverse reactions: in the observation group, 7 cases of nausea, vomiting in 6 cases, 2 cases of hypotension;control group 16 cases of nausea and vomiting in 14 cases, 1 cases of hypotension, 1 cases of skin allergy, 1 cases of dizziness, adverse reaction rates were significantly higher than the observation group (P<0.05), the difference was statistically significant.Exhaust time: observation group (2.93±0.63) days, the control group (2.76±0.61) days, no statistical significance.ConclusionOxycodone controlled intravenous analgesia pump for analgesia in patients with radical resection of cervical carcinoma after open surgery, with less adverse reactions.
2.A survey of statins application and its efficacy in patients with ischemic stroke

Objective To investigate the clinical application of statins for secondary prevention and its efficacy in patients with ischemic stroke.Methods A total of 300 patients with ischemic stroke were divided into statins group (n=91) and control group (n=209) according to whether statins was received or not.Serum lipid parameters,liver and kidney function,carotid artery intimamedia thickness (IMT) and plaque area were compared between the two groups.Results In 209 patients not taking statins,68 cases (32.5%) were not advised by clinic doctor; 55 cases (27.8%)were reluctant to take statins in fear of adverse reactions; 77 cases (36.8%) discontinued stains when serum lipids were normal; 3 cases (1.4%) developed skin allergic reaction and the skin rash that disappeared after stains discontinuance; 3 cases (1.4%) had elevated liver enzyme levels and the enzyme levels were declined to normal after stains discontinuance.The levels of total cholesterol (TC),triglyceride (TG) and low density lipoprotein cholesterol (LDL-C) were significantly lower in statin group than in control group [(4.93±0.45) mmol/L vs.(5.87±0.38) mmol/L,(1.68±0.61)mmol/L vs.(2.01±0.58) mmol/L,(2.49±0.57) mmol/L vs.(3.76±0.44) mmol/L,t=18.60,4.46,20.94,P=0.000,0.007,0.000,respectively].High density lipoprotein (HDL-C) level was significantly higher in stain group than in control group [(1.36±0.31) mmol/L vs.(1.03±0.25)mmol/L,t=9.75,P=0.001].There were no significant differences in liver and kidney function parameters such as alanine aminotransferase,aspartate aminotransferase,alkaline phosphatase,serum total bilirubin,direct bilirubin,indirect bilirubin,glutamyl transferase,blood urea nitrogen and serum creatinine between the two groups (all P>0.05).The carotid IMT and carotid atherosclerotic plaque area were significantly lower in stain group than in control group [(1.62 ± 0.23) mm vs.(1.74±0.31) mm,(0.57±0.12) cm2 vs.(0.65±0.18) cm2,t=3.32,3.88,P=0.008,0.002,respectively].Conclusions Most patients with ischemic stroke are still not taking statin for secondary prevention.Statins can improve the blood lipid levels and carotid arteriosclerosis status,and no obvious adverse reactions are observed.

This article first elaborates the connotation of research integrity,and analyzes the causes of the problems,then enumerates the typical examples of research misconduct.Based on those analyses,we explored possible solutions to prevent such problems.Relevant departments should consummate relevant law and regulations to improve the system,to avoid the emergence of research integrity problems,guarantee high quality of research.
4.Construction and screening of a cDNA expresssion library of Clonorchis sinensis adult worm

In order to screen antigenic genes of Clonorchis sinensis (C.sinensis).Firstly,a cDNA expression library from adult worms of C.sinensis was successfully constructed with lambda ZAP express vector.Total RNA of C.sinensis adult worms were extracted by the Trizol reagent and mRNA were further purified through oligo-dT cellulose.The first strand eDNA was synthesized by using MMLV reverse transcriptase.After the synthesis of the second strand,the cDNA were purified by CHROMA SPIN-400 kit and then ligated with lambda ZAP express vector,then packaged in vitro and amplified.The original library contained 1.5 × 106 pfu cDNA clones,the titer of amplified library reached 1.5 × 1010 pfu/mL,in which about 99% clones were recombinants and most of insert DNA fragments were 0.4-2.0 kb.Secondly,immunoscreened using the naturely infected man serum from C.sinensis.The positive clones were sequenced and analyzed.From 2.0 × 105 recombinant clones of the eDNA library,41 positive clones were obtained.Sequence analysis indicated that the cDNAs encoded proteins including glycine rich antigen 2,proline rich antigen 2 and antigen Cs44 from C.sinensis,anothers were lower similarity to predicted protein from Nematostella vectensis,transcription elongation factor GreA from Bartonella quintana str.Toulouse and NM_132090 CG3446 gene product in Drosophila melanogaster from Schistosoma japonicum.These data may form a foundation for identifying recombinant antigens that can be used in the diagnosis or vaccination against clonorchiasis.
5.Clinicopathological features and prognosis of IgA nephropathy with renal arteriolosclerosis

The study was a retrospective study. The clinical data of 866 patients with IgA nephropathy (IgAN) in Beijing Anzhen Hospital, Capital Medical University from March 2010 to March 2021 were analyzed, to investigate the clinical pathology and renal prognosis of IgAN patients with intrarenal arteriolosclerosis, and to preliminarily explore whether abnormal activation of complement system is involved in the injury of arteriolosclerosis. The patients were divided into renal arteriolar lesions group and non-renal arteriolar lesions group according to the renal histopathology, and the differences of clinical pathological manifestations, prognosis between the two groups were compared. The results showed that, compared with the non-renal arteriolar lesions group ( n=236), IgAN patients in the renal arteriolar lesions group ( n=630) had higher proportions of hypertension and malignant hypertension, higher levels of urinary albumin-creatinine ratio, 24-hour urine protein quantification and serum uric acid, lower estimated glomerular filtration rate, and more severe MEST-C lesions of the Oxford classification (all P < 0.05). Cox regression analysis results showed that intrarenal arteriolosclerosis was the independent risk factor affecting the progression of IgAN to ESRD ( HR=6.437, 95% CI 2.013-20.585, P=0.002). Renal histopathology showed that the deposition of complement C3c on the wall of intrarenal arterioles in the renal arteriolar lesions group ( n=98) was stronger than that in non-renal arteriolar lesions group ( n=18, P < 0.05). IgAN patients with renal arteriolosclerosis present with serious clinical and pathological manifestations, and renal prognosis. Abnormal activation of complement system may be involved in the pathogenesis of intrarenal arteriolosclerosis.
