1.The value of electromyography in diagnosis of myotonic dystrophy

Objective To explore the value of electrophysiology in diagnosis of myotonic dystrophy .Methods Electromyogra-phy(EMG) and nerve conduction were detected in 26 myotonic dystrophy patients(myotonia group) .Myotonic discharges were de-tected in 25 patients with non-tonic neuromuscular disease(control group) .Results The main features of EMG in myotonia group were myotonic discharges ,myogenic damage ,and normal nerve conduction .There was no myotonic discharges in control group .The positive rate of myotonic discharges of abductor pollicis brevis was 87 .5% .The positive rate of myotonic discharges of extensor digitorum communis was 77 .8% .The positive rates of myotonic discharges of anterior tibial muscle ,quadriceps femoris and trian-gular muscle are 57 .1% ,45 .5% and 45 .5% respectively .Conclusion Myotonic dystrophy distinguished from other neuromuscular disease might be mainly based on myotonic discharges and myogenic damage .Myotonic discharges were mainly observed in the small hand muscles .However ,the positive rate of abductor pollicis brevis ,which has the highest chance of myotonic discharge ,was less than 100% .
2.Expression of endocytic receptor megalin/cubilin in embryonic mouse kidneys

Objective To investigate the expression of endocytic receptor megalin and cubilinin in the developing mouse kidneys, and the correlation between the expression and the development of the renal tubules. Methods Expression of megalin and cubilin in developing mouse kidneys was examined at different embryonic days (E) using immunohistochemistry. Meanwhile, the ultrastructure of developing proximal tubules related to endocytosis was observed at transimission electron microscope level. Results At E9.5, megalin and cubilin were co-expressed in apical plasma membrane of the mesonephric ducts and mesonephric tubules. From E11 to E18, in the metanephros, the expression of both receptors were seen at the free surface and apical plasma of uretic bud, but weakly in all renal tubules of S shaped body at early differentiatial stage. With the mature of proximal tubule development, they were both confined to the brush border and the apical plasma of the proximal tubules in juxtamedullary cortex.Conclusion The endocytic receptors, megalin and cubilin are expressed in apical part of nearly all renal tubule epithelia in early development, and confined to free surface of mature proximal tubules, suggesting that with the mature of proximal tubules, the two receptors are generally involved in collaborating to facilitate, the reabsorption of ultrafiltration.

At present,the doctor-patient relationship in China is a little tense,communication between doctors and patients is not smooth,and the doctor-patient communication skills of the young doctor is obviously insufficient and need to be improved.In this paper,we analyze the factors that affect the communication between doctors and patients in the Department of Neurology through the complexity of professional knowledge,the current background and so on.In addition,we discuss multiple methods of improving communication capability among this group doctors,hoping to strengthen young physicians' clinical basic skills training,and impart doctor-patient communication skills from the linguistic modifiers,oral expression,words and deeds,use of metaphor,simulation training and other aspects,and establish dynamic assessment system of doctor-patient communication ability.

Objective To approach the evaluative effect of respiratory variation of superior vena cava peak flow velocity measured using transthoracic echocardiography (TTE) on fluid responsiveness in patients with mechanical ventilation.Methods A prospective cohort study was conducted.All mechanical ventilated critically ill patients whose fluid therapy was planned due to hypovolemia in Department of Critical Care Medicine of Beijing Tongren Hospital of Capital Medical University from April 2011 to April 2013 were enrolled.Volume expansion was performed with 500 mL Linger solution within 30 minutes.Patients were classified as responders if pulse pressure variation (PPV) increased ≥ 13% before volume expansion.The respiratory variation in superior vena cava peak velocity was calculated as the difference between maximum and minimum values of velocity in peak A,peak S and peak D over a single respiratory circle,and their variations (ΔA,ΔS,ΔD) were also calculated.The receiver operating characteristic curve (ROC curve) was plotted to assess the evaluative effect of respiratory variation of superior vena cava peak velocity on fluid responsiveness.Results Twenty-seven patients were enrolled in this study.Volume expansion increased PPV ≥ 13% happened in 14 patients (responders).The velocity of superior vena cava in peak A,peak S,peak D was significantly increased after volume expansion compared with that before volume expansion in responders [peak A (cm/s):34.6 ± 2.2 vs.31.3 ±2.1,t=-2.493,P=0.027; peak S (cm/s):39.1 ± 1.3 vs.35.3 ±2.1,t=-2.564,P=0.024; peak D (cm/s):28.1 ± 1.2 vs.23.3 ± 1.4,t=-4.995,P=0.000],but there was no significant difference in ΔA,ΔS and ΔD between before and after volume expansion.The ΔA,ΔS and ΔD were positively correlated with PPV (r=0.040,P=0.854; r=0.350,P=0.074; r=0.749,P=0.000).The area under ROC curve (AUC) of peak S was 0.36 [95% confidence interval (95%CI):0.11-0.52],but the AUC of ΔS was 0.68 (95%CI 0.47-0.89),the AUC of peak D was 0.41 (95%CI 0.19-0.63),but the AUC of ΔD was 0.95 (95%CI 0.86-1.00),so the aberration rate of superior vena cava in respiration was better than the flow rate in superior vena cava.When the cut-off value of ΔS was 20.7% for predicting fluid responsiveness,the sensitivity was 78.6% and the specificity was 61.5%.When the cut-off value of ΔD was 12.7% for predicting fluid responsiveness,the sensitivity was 92.0% and the specificity was 92.3%.Conclusion Respiratory variations in superior vena cava peak velocity measured by TTE could assess fluid responsiveness in patients with mechanical ventilation.
7.The clinical features and MRI findings of autoimmune encephalitis

Objective To investigate the clinical symptoms and MRI imaging findings in patients with autoim-mune encephalitis and to improve clinician's understanding about the clinical and imaging characteristics of autoim-mune encephalitis. Methods We analyzed the clinical features and MRI findings of 33 patients with autoimmune en-cephalitis in our department. Results Of these 33 patients, 27 (81.8%) had psychiatric symptoms, 26 (78.8%) had seizure, 18 (54.5%) had involuntary movement, 11 (33.3%) had fever and 9 (27.3%) patients presented with cen-tral hypoventilation, the present of involuntary movement and fever was lower in group LE than in group NMDA. A total of 10 patients had positive MRI finding. Of these 10 patients, 8 had brain parenchyma lesions, 3 had meningeal involvement. The most likely affected parenchymal lesions are occipital lobe, bilateral hippocampus, frontal lobe, pari-etal lobe, temporal lobe, thalamus and cerebellum. Conclusion Psychiatric symptoms and seizure are the most com-mon neurological symptoms of autoimmune encephalitis. MRI may show abnormal signals in the limbic system. FLAIR is the most sensitive MR imaging sequence for detection of autoimmune encephalitis lesions.
8.Effects of cotransplantation of rat bone marrow mesenchymal stem cells and bone marrow on hematopoiesis

AIM: To investigate effects of rBMMSC on hematopoiesis and immune reconstitution after allo-hematopoietic stem cells transplantation (HSCT). METHODS: Allogeneic BMT model from Fischer344 rats (RT-1Al) to Wistar rats (RT-1Au) was established. The effects of MSCs on hematopoietic reconstitution and immune reconstitution were studied by observing the survival rate, peripheral blood counts, thymus counts, spleen counts, bone marrow counts and immune function analysis at 30 days after transplantation. RESULTS: 1. Cotransplantation of MSCs and bone marrow (BM) was demonstrated to improve hematopoietic reconstitution. Lymphocyte and platelet counts in peripheral blood in cotransplantation groups were higher than those in control groups. More bone marrow neucleated cells were also observed in cotransplantation groups. 2. Cotransplantation of MSCs and BM improved immune reconstitution. First, overall thymic cellularity and spleen cellularity significantly increased in cotransplantation groups at day 30. Secondly, cotransplantation improved immune functional recovery. Non-specific lymphocytes proliferation reaction induced by ConA and LPS increased in cotransplantation group, and so did for allogeneic mixed-lymphocyte reaction. CONCLUSION: Hematopoietic reconstitution and immune reconstitution were significantly enhanced by MSCs cotransplanted with BM.
9.Tracking analysis of viral nucleic acid Ct value in patients with re-positive SARS-CoV-2 infection.

OBJECTIVE:
To track analysis of viral nucleic acid test results in patients with re-positive SARS-CoV-2 infection, and provide clinical reference for nucleic acid test of re-positive cases.
METHODS:
A retrospective study was conducted. The multiple nucleic acid results of 96 cases with SARS-CoV-2 infection tested by medical laboratory of Shenzhen Luohu Hospital Group from January to September in 2022 were analyzed. The test dates and cycle threshold (Ct) values of detectable positive virus nucleic acid in the 96 cases were summarized and analyzed.
RESULTS:
A total of 96 patients with SARS-CoV-2 infection were retested re-sampled for nucleic acid testing at least 12 days after the initial positive screening. Among them, 54 cases (56.25%) had Ct value of < 35 for nucleocapsid protein gene (N) and/or open reading frame 1ab gene (ORF 1ab), 42 cases (43.75%) had Ct value ≥ 35. In the re-sampling of infected patients, N gene titers were 25.08 to 39.98 Ct cycles, and ORF 1ab gene titers were 23.16 to 39.56 Ct cycles. Compared with the positive results of the initial screening, the Ct values of N gene and/or ORF 1ab gene positive were increased in 90 cases (93.75%). Among them, the patients with the longest duration of nucleic acid positive could still be positive for double targets (the Ct value of N gene was 38.60, and the Ct value of ORF 1ab gene was 38.11) at an interval of 178 days after the initial positive screening.
CONCLUSIONS
Patients infected with SARS-CoV-2 can be sustained or repeatedly tested positive for nucleic acid for a long period of time, and most of them had Ct values < 35. But whether it is infectious needs to be comprehensively evaluated by combining epidemiology, variant type, samples with the alive virus, and clinical symptoms and signs.

Sanqi, Panax notoginseng, is a famous traditional Chinese herb, which has more than 400 years cultivation history in China. This paper has reviewed the studies on the suitable growing soil conditions for Sanqi, mineral nutrition effects on Sanqi, and the effects of nutrients uptake on the yield and quality of Sanqi by applying fertilizer. Thereafter, research needed to be put further efforts in the future has raised for discussion, and outlined the following topics for further research like the mechanism of mineral nutrition, methods of nutrients deficiency diagnosis on site, suitable cultivated soil grading special fertizer development, disease resistance, and fertilization recommended by soil testing.
