1.Observation of efficacy of ESWL in different period in management of ureteral stones

Objective To compare the efficacy of the emergency ESWL within 24 hours after the onset of renal colic and elective ESWL at 5 ~ 14 days after the onset of renal colic in the treatment for ureteral stones.Methods 280 patients in the People's Hospital of Pingding,according to the wishes of patients with ureteral stones were admitted to our department for renal colic.All patients onset to treatment time 0.5 ~ 10h,They underwent emergency ESWL or elective ESWL with WD-ESWL Vi Guangdong Weida energy electromagnetic shock wave lithotripter.Success rate of fragmentation,stone-free rate after 2 weeks of operation,pain recurrence rate,and complications were evaluated.Results The success rate of fragmentation and the stone-free rate after 2 weeks of emergency ESWL were 97.18% and 96.60%,respectively,which were higher than those(88.75% and 87.61%,respectively)of elective ESWL(P < 0.05).The postoperative pain recurrence rate of emergency ESWL(5.12%)was lower than that (10.24%)of elective ESWL(P <0.05).There was no difference of total complications between emergency ESWL (3.44%)and elective ESWL(2.98%)group(P > 0.05).Conclusion Emergency ESWL is superior to selective ESWL and is the safe and effective method for the management of ureteral stones.
2.Expression and significance of Yes-associated protein and large tumor suppressor gene 1 in renal clear cell carcinoma

Objective To explore the expressions and significance of Yes-associated protein (YAP) and large tumor suppressor gene 1 ( LATS1 ) in renal clear cell carcinoma (RCCC).Methods There are 30 cases of RCCC,15 males and 15 females.The median age was 63 (36 - 77) years.There were 17 cases of stage Ⅰ - Ⅱ,13 cases of stage Ⅲ - Ⅳ.There were 8 cases well differentiated,13 cases moderately differentiated and 9 cases low differentiated.The expressions of YAP,LATS1 mRNA and proteins in 30 cases of RCCC and normal renal tissues were tested by RT-PCR and immunohistochemistry,and were analyzed with clinical data.ResultsThe expression level of YAP mRNA in RCCC was higher than that in normal renal tissues (0.569 ± 0.066 vs 0.515 ±0.068,P =0.003 ),and the level of LATS1 mRNA in RCCC was lower than that in normal renal tissues (0.454 ± 0.115 vs 0.514 ± 0.093,P =0.029 ).Thepositive rate of YAP proteins in RCCC was obviously higher than that in normal renal tissues (63.3% vs 33.3%,P =0.020),and the positive rate of LATS1 proteins in RCCC was lower than that in normal renal tissues (46.7% vs 76.7%,P =0.017).The positive rate of YAP proteins was 33.3% in well differentiated tissues,61.5% in moderately differentiated tissues and 88.9% in low differentiated tissues ( P =0.018),and the positive rate of LATS1 proteins was 75.0% in well differentiated tissues,53.8% in moderately differentiated tissues and 11.1% in low differentiated tissues (P =0.024).The positive rate of YAP proteins was 47.1% in stage Ⅰ - Ⅱ and 84.6% in stage Ⅲ - Ⅳ ( P =0.034 ),and the positive rate of LATS1 proteins was 64.7% in stage Ⅰ - Ⅱ,23.1% in stage Ⅲ - Ⅳ ( P =0.024 ).ConclusionYAP and LATS1 play important roles in the development of renal clear carcinoma,and may become new targets in the treatment of RCCC.

5.Changes of adrenal cortical function in children with growth hormone deficiency before and after treatment with recombinant human growth hormone

Objective To observe the changes of adrenal cortical function in children with growth hormone deficiency before and after treatment with recombinant human growth hormone. Methods Seventy-two children diagnosed with GHD who received rhGH treatment for no less than 6 month were included, among whom there were 32 children who were accompanied with adrenal cortical hormone (ACTH) deficiency. The changes of fasting plasma cortisol (COR) and ACTH levels before and 3 and 6 months after treatment with rhGH were retrospectively analyzed. Results Thirty-two children with ACTH deficiency were treated with rhGH after COR level reached normal by the supplementing exogenous hydrocortisone. There was negative correlation between COR level before treatment and the HC dose that make the COR reach normal lowest limit level (r?=?-0.899, P?0.01). One month after HC treatment only, the level of COR was obviously increased and ACTH was obviously decreased (P all?0.001). The levels of COR or ACTH at 3 or 6 months after rhGH and HC replacement therapy were not different from those at one month after HC treatment only (P all?>?0.05). The level of COR was obviously decreased after treatment with rhGH, and there was statistical difference compared with that before treatment (P?0.01) in 40 children without ACTH deficiency, among whom 10 children with abnormal hypothalamic pituitary MRI had lower level of COR. Conclusion Adrenal cortical function can be reduced during the treatment with rhGH in children with GHD, especially in children with abnormal hypophysis MRI. Thus, during the treatment period, adrenal cortical function should be monitored, so as to make early intervention.
6.Study of the Relationship between Angiogenic Factors and Mechanism of Preeclampsia

Objective:To study the relationship between serum soluble intercellular adhesion molecule-1(sICAM-1) and vascular endothelial growth factor(VEGF), placenta growth factor(PLGF), vascular endothelial growth factor receptor 1(VEGFR1, Flt-1), soluble vascular endothelial growth factor receptor 1(sVEGFR1, sFlt-1) mRNA expression in placenta tissue of preeclampsia(PE). Methods: The serum level of sICAM-1 was measured by enzyme-linked immunosorbent assay and gene expression of placenta tissue was detected by quantitative reverse transcriptase-polymerase chain reaction(RT-PCR). Results:(1)The average serum level of sICAM-1 was(218.45±29.93) μg/L in PE group compared with (168.84±19.39) μg/L in controls(P < 0.01).(2)The mRNA expressions of VEGF, PLGF, Flt-1 and sFlt-1 were increased in placenta of PE than those in controls (P < 0.01).(3)There was a positive correlation between the serum level of sICAM-1 and the sFlt-1 mRNA expression in placenta tissue(r = 0.90, P < 0.01). Conclusion: The serum level of sICAM was increased in the patients with PE. The expressions of VEGF, PLGF, Flt-1 and sFlt-1mRNA were increased remarkably in the placenta tissue of palients with PE,especial for sFlt-1. The remarkable increase for expression of sFlt-1mRNA may contribute to endothelial dysfunction, hypertension and proteinuria.

8.Protection of curcumin against tumor necrotic factor-alpha-mediated inflammatory injury of vascular endothelial cells

BACKGROUND:Previous studies have shown that, curcumin plays a crucial role on the inflammation in cells caused by oxidative stress.
OBJECTIVE:To elucidate the biological effect and mechanism of curcumin in the pathological inflammation reaction in vascular endothelial cells.
METHODHuman vascular endothelial cells were taken as the cellmodels. Tumor necrosis factor (10μg/L) treatment was used to induce the inflammation of cells. Curcumin (0, 50, 100μg/L) treatment for 24 hours was used to intervene the cells. The intercellular hyperpermeability of the vascular endothelial cellmonolayers was examined by HRP-conjugated bovine serum albumin, and intercellular filamentous actin stress fiber formation was examined by rhodamin-phal oidin staining. ELISA assay was used to detect the secretion of interleukin-1βin vascular endothelial cells. Immunoflurensece staining was applied to investigate the expression and translocalization of nuclear factor-κB. Western blot analysis reflected the expression of NRLP3 and caspase-1.
RESULTS AND CONCLUSION:HRP-bovine serum albumin detection results showed that, curcumin inhibited the intercellular hyperpermeability of the vascular endothelial cellmonolayers and the formation of robust intercellular filamentous actin in a dose-dependent manner. ELISA assay showed that curcumin protected vascular endothelial cells against tumor necrotic factor-α-induced interleukin-1βsecretion in a dose-dependent manner. Meanwhile, the nuclear factor-κB expression was increased and the translocation of nuclear factor-κB into the nuclei was obviously seen in vascular endothelial cells induced by tumor necrosis factor-α, but the translocation was not changed in 100μg/L curcumin-treated cells. Furthermore, western blot analysis revealed that the expression of NRLP3 and caspase-1 were inhibited in curcumin-treated cells. Curcumin can inhibit tumor necrosis factor-α-induced activation of inflammasome and secretion of interleukin-1βin vascular endothelial cells by down-regulating the expression of nuclear factor-κB, thus prevent pathological inflammatory injury in cells.
9.Analysis the therapy effect of clavicular hook plate on Neer Ⅱ distal clavicle fractures and tossy Ⅲ acromioclavicular joint dislocation

Objective To investigate therapy effect and its clinical complications of the clavicular hook plate internal fixation for the treatment of Type Neer Ⅱ distal clavicle fracture and tossy Ⅲ acromioclavicular joint dislocation.Methods Twenty-two patients of Neer Ⅱ distal clavicle fracture and 16 patients of Tossy Ⅲ acromioclavicular joint dislocation were selected as our subjects.All patients were hospitalized and treated by using clavicular hook plate fixation form August 2007 to Februay 2012.The clinical effect and complications were analyzed retrospectively.Results All patients were showed the primary healing in terms of incision.Of all patients,30 cases were being following up,and follow-up periods was 6.0 to 18.0 months (average of (10.45 ±3.78) months).All patients underwent implant removal surgery at postoperative 6-14 months.No refracture or acromioclavicular joint separation again occurred.Four patients were with complications,of which,2 patients were clavicular hook plate decoupling at 7 days or 3 weeks after operation; One patient occurred clavicle stress fracture at more than 3 months after operation.All mentioned 3 cases were treated with re-operation.1 patient was with shoulder abnormal sound after operation and abnormal sound disappear after internal fixation removed.The shoulder functions were evaluated according to University of California-Los Angeles (UCLA) score system at before and after clavicular hook plate was taken out.The scores of pain and function of the shoulder,forward lateral flexion range were significant difference between before and after clavicular hook plate removed ((7.90 ±1.20) vs.(9.20±1.03),t =-2.327,P=0.045;(8.00±0.94) vs.(9.40 ±0.97),t=-3.280,P=0.001 ; (4.00 ±0.47) vs.(4.70 ± 0.48),t =-4.583,P =0.001).After the hook plate was removed,all shoulder functions showed improvement.The overall efficacy was excellent in 15 cases,good in 13 cases,and poor in 2 cases,and the recovery of excellent and good rate were 93.33%.Conclusion The clinical efficacy is well in terms of using clavicular hook plate internal fixation to treat Type Neer Ⅱ distal clavicle fracture and Tossy Ⅲ acromioclavicular joint dislocation.Prevention and treatment of complications should paid more attention.
10.Effects of maternal deprivation on hippocampal PP1Cγ gene expression and neurobehavior of rats

Objective To investigate the effect of maternal deprivation (MD) on neurobehavior and PP1Cγgene expression in hippocampus. Methods Male pups were randomly divided into MD group(thirty-five)and control group(twenty-four). From PND 1 to PND 21 ,pups in the MD groups underwent daily maternal deprivation for 3 h ( Postnatal day). Neurobehavior was observed to investigate neurodevelopment, Morris water maze was used to measure spatial learning and memory,and Real-Time quantitative PCR was employed to analyze PP1Cγ gene expression. Results Several significant deficiencies were observed in bodyweight and grasping reflex while a great enhancement in hot-plate test in rat pups suffering from MD( (26.23 ± 2.81 )g vs. (30. 38 ± 3.85 )g;( 19.37 ± 11.89) s vs. (22.39 ± 17.62 ) s; (4.36 ± 1.76 ) s vs. ( 5.26 ± 2.55 ) s; P < 0. 05 ), but deficiencies in neurological reflexes were subtle ( ( 0.83 ±- 0.30 ) s vs. ( 0. 83 ± 0. 34 ) s; ( 3.68 ± 1.63 ) s vs. ( 5.61 ± 3. 01 ) s;( 3.00 ± 0.00 ) vs. ( 3.00 ± 0. 00); P > 0. 05 ). MD had a subtle influence on spatial learning and memory (P >0.05). Meanwhile,MD could lead to PP1Cγ expression down-regulation on PND 22 ( (2.19 ±0.62) vs. (3.52 ±0.86), P<0. 05)which was in line with early neurobehavior results. No difference was found compared with MD group and control group on PND60 ( ( 1.73 ± 0. 78 ) vs. ( 1.33 ± 0. 34); P > 0.05 ). However, there was the up-regulation of PP1Cγexpression on PND 90 ( (2.85 ± 0. 34) vs. ( 1.34 ± 0.93 ); P < 0.05 ). Conclusion MD alters early neurobehavior and hippocampal PP1Cγgene expression in the Wistar rats,but has a subtle effect on learning and memory. At the same time,MD can make PP1Cγexpression in the hippocampus varying with the age.
