1.The effect of insulin dialogue toolkit on psychological insulin resistance of type 2 diabetes

Objective To explore the effectiveness of insulin dialogue toolkit on psychological insulin resistance of type 2 diabetes. Methods A total of 134 type 2 diabetes with psychological insulin resistance were selected by the inclusion and exclusion criteria . The participates were randomly assigned to control group and intervention group by computer with 67 cases each. The control group was given conventional health education, the intervention group was given insulin dialogue toolkit instead of individual health education based on conventional health education. The intervention was lasted 2 weeks.The total score and each dimension score of my opinion of insulin and the blood glucose before and after intervention were observed. Results Before intervention, the total score in my opinion of insulin in intervention group and control group was (66.40±8.51), (66.91±7.27) scores, and there was no significant difference between two groups, P>0.05.After intervention, the total score in my opinion of insulin in intervention group and control group was(76.93±5.31), (71.09±7.77) scores, and there was significant difference between two groups, Z=-5.585, P<0.01. There was significant difference in each dimension score, P<0.05 or 0.01. After intervention, the fasting blood glucose in intervention group and control group was (7.35±1.76), (7.54±1.70) mmol/L,the postprandial blood glucose 2 h in intervention group and control group was (9.47±2.26), (8.79±2.04 ) mmol/L, and there were no significant differences, P>0.05. Conclusions The insulin dialogue toolkit has an impact on psychological insulin resistance of type 2 diabetes. Insulin dialogue toolkit is a way to improve the diabetes′attitude of insulin, but the effect on blood sugar are not clarity.
2.The status and influencing factors of psychological insulin resistance of type 2 diabetes mellitus

Objective To explore the status and influencing factor of psychological insulin resis tance (PIR) of type 2 diabetes.Methods 137 in-patients with type 2 diabetes were investigated by selfdesigned questionnaire to get general data and Insulin Perceptions Questionnaire to get PIR data.Multiple linear regression were used to find influencing factor.Results 134 participates had PIR among the 137 in-patients.The older of age,using insulin for longer time and receiving insulin treatment positively had the higher scores of main questionnaire,then had less serious psychological insulin resistance.The duration of illness course,complications,work and using insulin for the first time or not had no relationship with their PIR.Conclusions The findings indicated that the type 2 diabetes using insulin have psychological in sulin resistance generally.Age,the time using insulin,accepting or receiving insulin positively are the independent factors of psychological insulin resistance
3.Effects of Dobutamine on Related Indexes of Patients with ARDS Caused by Septic Shock

OBJECTIVE:To explore the effects of dobutamine on hemodynamics,tissue perfusion and respiratory function in patients with ARDS caused by septic shock. METHODS:Totally 26 patients with ARDS caused by septic shock were given Dobu-tamine hydrochloride injection 5 μg/(kg·min) by continuous pump for 20 min ,increasing by 5 μg/(kg·min) every 20 min to 15μg/(kg·min)for 6 h. Mean artery pressure(MAP)maintained at about 65 mmHg during continuous pump. Hemodynamic index-es [HR,MAP,cardiac output(CO),stroke volume(SV),total end diastolic volume(GEDV),intrathoracic blood volume index (ITBI),extravascular lung water index (ELWI),systemic vascular resistance index (SVRI)],tissue perfusion indexes (norepi-nephrine,urine volume,blood oxygen saturation,blood lactate)and ventilator parameter indexes(positive breathing pressure,min-ute ventilation volume,respiratory rate,peak inspiratory pressure and plateau pressure)of all patients were recorded before medica-tion,6,24,48 h after medication. RESULTS:There were no statistical significances in HR,MAP,GEDV,ITBI,blood oxygen saturation,blood lactate and respiratory rate before and after treatment (P>0.05). CO,SV,urine volume and minute ventilation volume of all patients 6,24,48 h after medication were significantly higher than before medication,and those indexes increased gradually as medication time. ELWI,SVRI,positive breathing pressure,peak inspiratory pressure and plateau pressure of all pa-tients after medication were significantly lower than before medication,and those indexes decreased gradually as medication time, with statistical significance(P<0.05). The levels of norepinephrine in all patients 24,48 h after medication were significantly low-er than before medication and 6 h after medication,with statistical significance(P<0.05);but there was no statistical significance between before medication and 6 h after medication(P>0.05). CONCLUSIONS:The early application of dobutamine ELWI,tis-sue perfusion and respiratory function in patients with ARDS caused by septic shock,and can keep hemodynamics stable.
4.BRCA mutations and clinical pathological features of 30 patients with high-risk triple negative breast cancer in Xinjiang Uygur Autonomous

Objective Knowing the BRCA gene mutational condition of high risk triple negative breast cancer (TNBC) in Xinjiang Uygur Autonomous and acquiring the differences of clinical and pathologic characteristics between person with BRCA gene mutation and person without it by means of BRCA gene mutation testing for 30 cases of TNBC in Xinjiang Uygur Autonomous.Methods The objects of this study were 30 cases of high risk TNBC from Xinjiang.All the coded sequences of BRCA1/2 gene were amplified by means of extracting genomic DNA from peripheral venous blood.BRCA1/2 gene mutation analysis were prescreened through DHPLC.Then,the result was verified by DNA sequencing.The clinical and pathologic characteristics between person with BRCA gene mutation and person without it of 30 high risk TNBC cases were contrastively analysed.Results In all the 30 cases of BRCA gene mutation testing for TNBC in Xinjiang Uygur Autonomous,there were 5 cases of pathogenic mutations of BRCA gene (5/30,16.7 %); 4 cases of BRCA 1 mutation (4/30,13.3 %); 1 case of BRCA 2 mutation (1/30,3.3 %); and there was no mutation to be found in 25 cases of BRCA gene of TNBC (25/30,83.3 %).As compared with person without gene mutation,who with it had the characteristics of earlier of TNM,the difference was statistically significant (P =0.040).Conclusion Since the rate of BRCA1 gene mutation of high risk TNBC is higher.It is suggested that the BRCA gene of every patients with high risk TNBC should be tested.Comparing with person with BRCA gene mutation and person without it,there might have differences on clinical pathological characteristics features.Therefor,individualized treatment should be taken into consideration.
5.Clinical effects of community-based rehabilitation on chronic schizophrenia

ObjectiveTo explore the effect of community based rehabilitation on chronic schizophrenia. Methods60 patients of chronic schizophrenia were randomly divided into the community based rehabilitation group (the study group) and the inpatients group (the control group). The study used prospective design for 1 year with brief psychiatric rating scale (BPRS),nurses' observation scale for inpatients evaluation (NOSIE) and social disability screening schedule (SDSS).ResultsCompared with the control group at the end of 6 month and 1 year, scores of BPRS, NOSIE and SDSS in the study group were significantly different (P<0.05-P<0.001). The relapse rate of the study group (0%) also lowered than that of the control group( 20%).ConclusionsCommunity based rehabilitation therapy can control the chronic schizophrenia effectively. It also promotes the life quality and social function of patients, and lowers the relapse rate significantly. It is an important rehabilitation method for chronic schizophrenia.
6.Fusion expression with catalase improves the stability of multicopper oxidase and its efficiency in degrading biogenic amines.

Some enzymes belonging to the multicopper oxidase (MCO) family can degrade the hazardous biogenic amine (BA) present in food. However, the oxidation of MCO in the process of degrading BAs may reduce its activity and stability, resulting in decreased catalytic efficiency. In this work, an MCO from Lactobacillus fermentum (MCOF) was fused with a Bacillus subtilis catalase (CAT) using different strategies and the fusion enzymes were respectively expressed in Escherichia coli BL21(DE3). The tolerance of eight fused MCOFs to H2O2 increased by 51%-68%, and the stability of CAT&MCOF increased by 17%, compared to the wild type MCOF. Using 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid (ABTS) as a substrate, the substrate affinity (Km), the catalytic efficiency (kcat/Km) and the molar specific activity of CAT&MCOF increased by 1.0-fold, 1.7-fold and 1.2-fold than those of MCOF, respectively. The stability of CAT&MCOF under acidic conditions (pH 2.5-4.5) and moderate temperatures (35-55 °C) also improved. Moreover, the degradation rates of putrescine, cadaverine and histamine catalyzed by CAT&MCOF reached 31.7%, 36.0% and 57.8%, respectively, which increased by 132.5%, 45.7% and 38.9% compared to that of MCOF. The improvement on the stability and catalytic efficiency of MCOF by fusion expression with CAT provides a good example for improving the applicability of enzymes through molecular modifications.

7.SUMO1 regulates post-infarct cardiac repair based on cellular heterogeneity

Small ubiquitin-related modifier(SUMOylation)is a dynamic post-translational modification that maintains cardiac function and can protect against a hypertrophic response to cardiac pressure overload.However,the function of SUMOylation after myocardial infarction(MI)and the molecular details of heart cell responses to SUMO1 deficiency have not been determined.In this study,we demonstrated that SUMO1 protein was inconsistently abundant in different cell types and heart regions after MI.However,SUMO1 knockout significantly exacerbated systolic dysfunction and infarct size after myocardial injury.Single-nucleus RNA sequencing revealed the differential role of SUMO1 in regulating heart cells.Among cardiomyocytes,SUMO1 deletion increased the Nppa+Nppb+Ankrd1+cardiomyocyte subcluster pro-portion after MI.In addition,the conversion of fibroblasts to myofibroblasts subclusters was inhibited in SUMO1 knockout mice.Importantly,SUMO1 loss promoted proliferation of endothelial cell subsets with the ability to reconstitute neovascularization and expressed angiogenesis-related genes.Computational analysis of ligand/receptor interactions suggested putative pathways that mediate cardiomyocytes to endothelial cell communication in the myocardium.Mice preinjected with cardiomyocyte-specific AAV-SUMO1,but not the endothelial cell-specific form,and exhibited ameliorated cardiac remodeling following MI.Collectively,our results identified the role of SUMO1 in cardiomyocytes,fibroblasts,and endothelial cells after Ml.These findings provide new insights into SUMO1 involvement in the patho-genesis of MI and reveal novel therapeutic targets.
8.The application of nurse-performed point-of-care ultrasound in the management of abdominal distension in patients with enteral nutrition in ICU

Objective To explore the application of nurse-performed point-of-care ultrasound in abdominal distension of ICU patients with enteral nutrition.Methods Purposive sampling method was used to select 108 patients with enteral nutrition abdominal distension admitted to the ICU of a tertiary A general hospital in Hefei City from January to December 2022,and they were divided into an experimental group and a control group according to random number table method.The experimental group used bedside ultrasound to evaluate the stomach,intestine and abdominal cavity of patients with abdominal distension,and implemented targeted treatment measures according to the evaluation results.The control group was given corresponding treatment measures according to the abdominal sight,touch,tapping,listening and traditional syringe withdrawal method.Intra-abdominal pressure,abdominal circumference,feeding compliance rate and feeding interruption rate,ICU stay time and mechanical ventilation time were compared at the 1st,3rd and 5th day.Results 11 cases dropped out of the study due to transfer,automatic discharge,death,etc.,and 97 cases were included 51 cases in the experimental group and 46 cases in the control group.Repeated measurement ANOVA showed that intra-abdominal pressure and abdominal circumference time in the 2 groups had interaction with each other(F=41.724,P<0.001;F=17.762,P<0.001).Generalised estimating equations showed that there was only a between-group effect for the rates of feeding compliance and feeding disruption in 2 groups(Wald y2=18.033,P<0.001;Wald x2=6.567,P<0.001).Simple effect analysis showed that on the 3rd and 5th day after intervention.The experimental group had lower abdominal depression and the feeding interruption rate was lower than the control group(P<0.05).On the 5th day after interven-tion,the abdominal circumference of the experimental group was lower than that of the control group,the feeding compliance rate was higher than that of control group(P<0.05).After intervention,in the experimental group,the mechanical ventilation time was shorter than that of the control group(P<0.05).There was no significant difference in ICU stay time between the 2 groups(P=0.095).Conclusion The application of nurse-performed point-of-care ultrasound in the management of abdominal distension in patients with enteral nutrition in ICU is feasible and scientific,which is helpful for the management of patients with enteral nutrition abdominal distension in ICU,reducing internal abdominal pressure,shortening mechanical ventilation time,and avoiding the occurrence of feeding intolerance.
9.Effects of Periplaneta americana Extract YS-F on the Proliferation and Apoptosis of Human Non-small Cell Lung Cancer A 549 Cells

OBJECTIVE:To investigate the effects of Periplaneta americana extract on the proliferation and apoptosis of human non-small cell lung cancer A 549 cells as well as its possible mechanism. METHODS :The dry bodies of P. americana were soaked with 90% ethanol and eluted with gradient water-methanol by polyamide column chromatography. The 20%,30%,40%, 50%,60%,70%,80%,90% methanol elution sites (YS-A-H)were obtained. MTT method was used to screen the active site , and the inhibition rate of different doses of active site was detected. Flow cytometry was adopted to detect cell apoptosis ,cell cycle and mitochondrial membrane potential of cells after treated with different doses of active site. RESULTS :Half inhibition concentrations of YS-A-H were (95.25±8.42),(129.93±7.24),(221.28±12.68),(275.39±14.87),(276.76±16.32),(31.90± 5.34),(163.15±6.97),(122.81±8.36)μg/mL,respectively. YS-F had the strongest activity. After treated with 3,9,27,81 μg/mL YS-F for 24,48,72 h,cell proliferation inhibitory rate was increased significantly at different time points ;after treated for 48,72 h,that was significantly higher than same group after treated for 24 h;after 72 h treatment ,that was significantly higher than same group after 48 h treatment (P<0.01). There was no significant effect of 24 h treatment of 3 μg/mL YS-F and 72 h treatment of 9 μg/mL YS-F on the percentage of cells in the late stage of necrosis,24 h treatment of 3 μg/mL YS-F on the percentage of cells in G2/M phase and 48 h treatment of 3 μg/mL YS-F on the reduction rate of mitochondrial membrane potential(P>0.05). The percentage of cells in the early stage of apoptosis ,the late stage of apoptosis and the early stage of necrosis ,the late stage of necrosis,as well as the percentage of cells in the Sub-G 0/G1 and S phase at each time point were significantly increased in other different doses groups ,while the percentage of cells in G 0/G1 and G 2/M phase was decreased significantly (P<0.01). In each dose group,the percentage of cells in the early stage of apoptosis ,the late stage of apoptosis and the early stage of necrosis ,the late stage of necrosis (except for the percentage of cells in the late stage of necrosis treated with YS-F 9 μg/mL for 72 h)and the percentage of cells in Sub-G 0/G1 phase,G2/M phase (except for YS-F 27,81 μg/mL for 48 h)after treated for 48,72 h were significantly higher than same group after 24 h of treatment ;the percentage of cells in G 0/G1 phase,S phase and G 2/M phase (except for YS-F 9 μg/mL for 48 h)after treated for 48,72 h were significantly lower than same group after 24 h of treatment (P<0.01);the percentage of cells in the early stage of apoptosis ,the late stage of apoptosis and the early stage of necrosis ,the late stage of necrosis (except for the percentage of cells in the late stage of apoptosis and early stage of necrosis when treated with YS-F 27 μg/mL for 72 h,the percentage of cells in the late stage of necrosis when treated with YS-F 3,9 μg/mL for 72 h were decreased significantly )and the percentage of cells in S phase (except for YS-F 3 μg/mL for 72 h)and Sub-G 0/G1 phase after treated for 72 h were significantly higher than same group after 48 h of treatment ,while the percentage of cells in G 0/G1 and G 2/M phase were significantly lower than same group after 48 h of treatment (P<0.01). After treated with YS-F 9,27,81 μg/mL for 48 h,the reduction rate of cell mitochondrial membrane potential was increased significantly ;YS-F 27,81 μg/mL groups were significantly higher than YS-F 9 μg/mL group,and YS-F 81 μg/mL group was significantly higher than YS-F 27 μg/mL group. CONCLUSIONS:YS-F can inhibit the proliferation and promote the apoptosis of A 549 cells by preventing cell transformation from S phase to G 2/M phase ,and reducing mitochondrial membrane potential ,in time-dependent or dose-dependent manner.
