1.Effect of extended nursing mainly by stoma therapists on self-care ability of permanent colostomy patients

Objective To explore the effect of extended nursing mainly by stoma therapists on the self-care ability of permanent colostomy patients group.Methods Seventy-eight patients with permanent colostomy were equally randomized by random digits table into control group and observation group.The former received routine discharge education by duty nurses and the latter the extended nursing mainly by stoma therapists. Three months after discharge,the exercise of self-care agency scale (ESCA) was used to conduct the assessments for comparisons of two groups.Result The score on ESCA in the observation group was significantly higher than that of the control group (P<0.01).Conclusion The extended nursing mainly by stoma therapists can improve colostomy patients’quality of life effectively and help them more knowledgeable about the stoma knowledge and skills to enhance self-care ability.
2.Medical Images Compression for Region of Interest Based on Curvelet Transform and SPIHT Algorithm

Purpose To propose a novel compression method for region of interest (ROI) based on Curvelet transform and SPIHT algorithm. Materials and Methods The ROI was firstly extracted without compression, and Curvelet transform was applied for the background regions. The Curvelet coefifcients were coded using SPIHT algorithm. Then the images after compression are obtained by inverse Curvelet transform. The ROI and the background were ifnally overlapped to get the full compressed image. Effect of ROI compression and overall compression were compared, as well as the Curvelet transform and wavelet transform, based on peak signal noise ratio. Results The ROI compression highlighted the region of interest and the visual effect was superior to the overall compression. The peak signal to noise of Curvelet transform was higher than that of wavelet transforms, and the compressed images were more clear for the same proportion. Conclusion ROI compression based on Curvelet transform and SPIHT algorithm can achieve efficient compression images without losing important diagnostic information, which complies with the requirement of high precision and high quality of medical image compression.
3.A new vascular tissue engineering material:Electrospun small-diameter nanofibous scaffolds

BACKGROUND:Due to the much higher requirement of biocompatibility and anticoagulant of smal-diameter vascular grafts than those of large-diameter ones, in situ blood vessel regeneration occurs as a new research direction. OBJECTIVE:To summarize the recent research development of electrospun smal-diameter scaffolds and to explore the application of in situ blood vessel regeneration and the development tendency. METHODS:The first author retrieved China National Knowledge Infrastructure database, Wanfang data and ISI Web of Knowledge foreign database to retrieve literatures addressing the fabrication of electrospun smal-diameter nanofibrous vascular grafts, surface modification and mimicking extracel ular matrix, as wel as the evaluation of biocompatibility and security after grafting. RESULTS AND CONCLUSION:Electrospun smal-diameter nanofibrous vascular grafts have emerged as promising candidates in vascular tissue engineering. By using both natural and synthetic polymers, the scaffolds can achieve a good balance between mechanical property and biocompatibility. Meanwhile, the fabrication of multi-layered vascular scaffolds, functional surface modification and mimicking extracel ular matrix structural y and functional y are now becoming attractive research directions. However, at current stage, electrospun vascular scaffolds used clinical y are basical y formed by synthetic materials, which have limited biocompatibility and anticoagulant activity. In this case, more efforts should be paid to find an optimal ratio between natural and synthetic materials for the improvement of biocompatibility and anticoagulant ability of smal-diameter vascular grafts.
4.Experimental study on restenosis after percutaneous coronary intervention: effects of fenofibrate on proliferation and apoptosis of cultured human umbilical vein endothelial cells

Objective To establish the cell model of restenosis in vitro and investigate the effects of fenofibrate on the prevention and treatment of the restenosis.Methods Human umbilical vein endothelial cells(HUVECs) were cultured in vitro.The study was designated to 5 groups:(1) control group,(2) LPC group,(3) low-concentration fenofibrate(10 ?mol/L) group,(4) midium-concentration fenofibrate(50 ?mol/L) group,and(5) high-concentration fenofibrate(100 ?mol/L) group.Proliferation and apoptosis of HUVECs were assessed by MTT assay,flow cytometry(FCM) and fluorescence microscopy respectively.Results Compared with the control group,LPC could inhibit the growth and induce apoptosis of HUVECs as well as decrease NO production in HUVECs.Fenofibrate could increase the proliferation and decrease the apoptosis of HUVECs,and also enhance NO production of HUVECs.Conclusion Fenofibrate could improve proliferation and reduce apoptosis and enhance NO production of HUVECs through lysophosphatidylcholine,which may be an important route for fenofibrate to prevent and treat the restenosis after percutaneous coronary intervention.
5.Advantage of childbirth with ball-assisted free body position

Objective To evaluate the clinical effect and safety of intravenous administration of phloroglucinol combined with delivery of ball-assisted free position in parturient women during labor.Methods One hundred and fifty-two primiparous women who were treated by vaginal delivery from June 2015 to August 2016 in Beijing Navy General Hospital were selected as the study subjects.They were given 80 mg of phloroglucinol diluted in saline injection intravenous injection.Observation group were treated with childbirth.Control group treated with traditional supine or lateral body.Results The rate of descending of the observed group was longer than that of the control group((1.09±0.22) cm/h vs.(0.83±0.15) cm/h),the first labor time,the second labor time and the total labor time of the observation group were shorter than the control group((9.57±1.71) h vs.(12.13±2.64) h,(35.75±8.68) min vs.(61.21±10.03) min,(11.34±1.96) h vs.(14.66±2.70) h),the difference between the two groups was statistically significant(t=3.611,4.204,3.307,3.456,P<0.05).While the third labor time of the control group and the observation group were (8.43±1.12) min and (7.92±0.96) min,the difference between the two groups was not statistically significant(t=1.005,P>0.05).The percentage of vaginal delivery in the observation group was 89.47%(68/76),significantly higher than that in the control group(73.68%(56/76)),the difference was statistically significant(χ2=4.514,P<0.05).The incidence of vaginal midwifery was 1.32%(1/76) and that of cesarean section was 9.21%(7/76) of observation group,significantly lower than that in control group(5.26%(4/76),21.05% (16/76)),the difference was statistically significant(χ2=5.323,4.376,P<0.05).Observation group 24 h postpartum hemorrhage was (273.12±58.23) ml,significantly less than the control group((385.42±70.13) ml),the difference between the two groups was statistically significant(t=3.744,P<0.05).The observation group had postpartum hemorrhage of 1.32%(1/76),urinary retention of 2.63%(2/76),and the percentage of neonatal asphyxia of 2.63%(2/76),were lower than that of the control group(5.26%(4/76),10.53%(8/76),9.21%(7/76)),the difference between the two groups was statistically significant(χ2=4.103,4.925,4.421,P<0.05).Conclusion The delivery of ball-assisted free-body delivery in active stage of delivery can promote natural childbirth.The medical staff should update the concept of delivery and raise awareness to encourage the use of phloroglucinol combined with free-body delivery.
6.The application effects of PDCA circulation on behavior management in discharged patients with first cerebral infarction

Objective To explore the application effects of PDCA circulation on behavior management in discharged patients with first cerebral infarction. Methods Patients with first attack of cerebral infarction were enrolled from the department of neurology of the First Hospital of Anhui Medical University from April 2013 to March 2015. According to their admission numbers, patients were divided into the observation group (94 cases) and the control group (80 cases). Besides of the regular personalized secondary prevention, the observation group was also applied for the PDCA cycle for nine months. Stroke Behavior Change Inventory was used to evaluate the changes of their lifestyle and medication compliance one year after discharge for two groups. Results After the observed patients were discharged one year, the changed extent of these five behaviors including quitting smoking, regular monitoring blood pressure and sugar, appropriate exercise, pressure control in the observation group were 0.00 (0.00, 3.00), 1.68 (1.00, 3.00), 1.00 (0.00, 2.00), 1.20 (0.75, 2.00), 1.00 (0.00, 1.00) points, and these in the control group were 0.00 (0.00, 1.0)0, 1.00 (0.00, 2.00), 0.00 (0.00, 1.00), 1.00 (1.00, 2.00), 0.00(0.00, 1.00) points. The observation group in these parameters had significantly higher than the control group (Z=-3.166--2.419, P < 0.05). The changed extent of these behaviors in the medication compliance such as reducing blood glucose, blood pressure, atrial fibrillation, cardiovascular disease, and stroke prevention in the observed group were 0.00 (0.00, 0.00), 0.00 (0.00, 2.00), 0.00 (0.00, 0.00), 0.00 (0.00, 0.00), 3.00 (3.00, 3.00) points, and these in the control group were 0.00 (0.00, 0.00), 0.00 (0.00, 0.79), 0.00 (0.00, 0.00), 0.00 (0.00, 0.80), 0.00 (0.00, 1.50) points. The observation group in these parameters had significantly higher the above parameters than the control group (Z=-8.081--2.595, P<0.05). Conclusions Application of the PDCA circulation in the patients with first attack of cerebral infarction after their discharge can help them correct lifestyle and increase their medical treatment compliance, which is beneficial for secondary prevention of cerebral infarction.

8.Effect of ghrelin on angiotensin Ⅱ induced human umbilicus vein endothelial cell oxidative stress and endothelial cell injury

Objective To determine the effect of ghrelin on protecting the human umbilical vein endothelial cells (HUVEC) from injury by angiotesin Ⅱ(AngⅡ) in vitro. Methods (1)HUVEC was incubated for 24 h with AngⅡ whose final concentration in the medium varied from 10-9 to 10-6mol/L or pretreated with 10-9to 10-6mol/L ghrelin for 2 h before incubation for 24 h with AngⅡwhose final concentration in the medium was 10-6mol/L. HUVECs were harvested to measure the cell vitality and cell apoptosis. The cell vitality was determined by MTT and cell apoptosis rates were measured by Annexin V-FITC apoptosis detection kit. (2)HUVECs were incubated for 3,6,12,or 24 h with 10-9,10-8,10-7,or 10-6mol/L AngⅡ, respectively. Before HUVECs were incubated with 10-6 mol/L AngⅡfor 24 h, ghrelin (10-9,10-8,10-7, and 10-6 mol/L) was used to pretreat the cells for 2 h. Growth hormone secregogue receptor 1a blocker [D-Lys3]GHRP-6 was added to the cells which were incubated for 24 h with 10-6mol/L AngⅡ and pretreated with 10-6 mol/L ghrelin for 2 h. Cell reactive oxygen species were measured by dichlorofluorescin (DCF) fluorescence probe method. (3)HUVECs were incubated for 24 h with 10-9,10-8,10-7, or 10-6mol/L AngⅡ and ghrelin, respectively,and then were incubated with 10-6mol/L of AngⅡ for 3,6,12,or 24 h. Furthermore,HUVECs were pretreated with 10-9,10-8,10-7, or 10-6mol/L ghrelin for 30 min,1 h,or 2 h, and then were incubated with the inhibitor of mitogen-activated protein kinase /extracellular regulated kinase (MAPK/ERK1/2),PD98059, the inhibitor of phosphoinositide-3-kinase/serine threonine kinase( PI3K/Akt)wortmannin, and [D-Lys3]GHRP-6 for 24 h. NO production was compared among groups. HUVECs were pretreated with ghrelin,PD98059, wortmannin, and [D-Lys3]GHRP-6 for 2 h and co-cultured with 10-6mol/L AngⅡfor 24 h,or pretreated with ghrelin plus PD98059, wortmannin, and [D-Lys3]GHRP-6 before incubation with AngⅡfor 24 h. NO was measured in the endothelial cell supernatants by Griess method. (4)HUVECs were cultivated with blank or AngⅡwith or without pretreatment with ghrelin or both ghrelin and wortmannin. The protein expression of eNOS and phospho-protein expression of Akt were measured by Western blot analysis. Results AngⅡinjuried the endothelial cell vitality,increased the cell apoptosis rates in HUVECs, and decreased NO production in HUVEC supernatants,whereas ghrelin protected HUVECs from AngⅡ injury. Ghrelin decreased the reactive oxygen species in HUVECs induced by AngⅡ. The effect could be attenuated by [D-Lys3]GHRP-6 pretreatment; PD98059 alleviated AngⅡ inhibition of NO production in HUVEC supernatants. Wortmannin and [D-Lys3]GHRP-6 could abolish protection of ghrelin from reducing NO production in HUVEC supernatants. AngⅡreduced the expression of eNOS,but ghrelin increased eNOS expression. Wortmannin could cancel this effect of ghrelin. Ghrelin increased p-Akt expression and reached the peak in 10 and 20 min. Conclusion Ghrelin may protect HUVECs from AngⅡinduced injury, which is related to decreasing oxidative stress, increasing the protein expression of eNOS, and activating PI3K/Akt signal pathway through GHSR1a receptor.
9.Hemorrheological Changes in Lacunar Cerebral Infarction Patients With Arteriopathy of Small Artery

【Objective】To observe the hemorrheological changes in lacunar cerebral infarction(LCI) patients with arteriopathy of middle and small arteries.【Methods】The hemorrheological changes of cerebral arteries in 85 LCI patients without arteriopathy of large artery confirmed by cervical vessels color ultrasonography and magnetic resonance image(MRI)were observed by transcranial Doppler ultrasonography(TCD).The correlation of the hemorrheological changes with age and blood pressure(BP) of arteries was investigated.The above indexes were compared with those in 25 healthy volunteers confirmed by encephalography in the same age group.【Results】The mean blood flow rates of cerebral middle,anterior and posterior artery in LCI patients were(62.09?16.90)cm?s~(-1),(50.42?13.11)(cm?s~(-1),)and(32.33?7.55)cm?s~(-1) respectively,lower than(69.65?19.20)cm?s~(-1),(57.75?16.75)cm?s~(-1),and(38.75?8.81)cm?s~(1) respectively in the healthy volunteers(P0.05).【Conclusion】The arteriopathy of small artery in LCI patients can cause the mild decrease of blood flow rate and the increase of pulsating index.There exists a correlation of pulsating index with age and arterial blood pressure.

With building up a clinical modification system model by upgrading the standard disease classification system architecture,to collect those huge amount of diagnosis names and codes in use,based on the model to screen and coordinate the relation of them,the tenth revision of the International Classification of Disease Clinical Modification of Beijing is implemented.This standard has been successfully applied for two years in all those secondary and above level hospitals in Beijing.It could well support the work of deploying Diagnosis Related Groups in Beijing and support not only those traditional applications of disease classification but also those new and advance requirements of information processing.
