1.Effect and Ethical Consideration on New Technical Application of Negative Pressure Wound Therapy

Objective:To explore the clinical effect of negative pressure wound therapy and to discuss the ethi-cal issues in this therapy and seek a solution. Methods:Sixty patients with negative pressure wound therapy in our hospital were invited to participate in this study. Patients were all with deep pressure ulcers or diabetic foot, of which the Wagner Scores were graded as three or four. They were divided into two groups randomly. The subjects in control group received standard treatment, while those in experimental group were treated by self -designed closed negative pressure drainage settings. The wound healing time, the cost of the treatment and comfort were compared between the two groups. Results:The cost of wound care in the experimental group was less than those in the control group (P<0. 001). There was no significant difference in comfort between two groups (P>0. 05). But the wound healing time was longer in the experimental group than those in the control group. Conclusion:Self-designed closed negative pressure settings can decrease the cost of patient care with the equal patient comfort and acceptance as the standard negative pressure, however, the wound healing time has been extended. So before using negative pressure technique, the medical staff should comprehensively analyze, be guided by medical ethics and choose the appropriate treatment for different patients.
2.Ethical Thinking and Discussion on Advertising Media Operation in the Plastic and Cosmetic Industry——Based on Questionnaire Analysis in a University in Shaanxi Province

Objective:To explore the propaganda methods and propagation path of advertising media cosmetology publicity in accordance with public demand and ethical principles,through understanding college students' acceptance level for advertising media cosmetology publicity and its influence on their choice forcosmetic ways.Methods:With the method of stratified cluster sampling,an anonymous questionnaire survey was conducted among 210 college students in a university in Shaanxi province.Results:The Internet has become the main way for college students to acquire cosmetology knowledge.A total of 44.7 % of the students thought most of the media cosmetology publicity was unbelievable,46.5% of the students thought that advertising media publicity mainly considered the interests of the beauty institution,72.9% of the students would not consider the beauty institution the media recommended,62.4% of the students would choose regular public hospitals,56.3% of the students would give up the idea of cosmetology due to the news of cosmetic failure,42.2% of the students could accept minimally invasive plastic surgery,and 68.5 % of the students expected to acquire the cosmetology knowledge through expert consultation.Concltusion:At current,the credibility of media cosmetology publicity is not optimistic enough.It should further strengthen the supervision for media cosmetology publicity.Cosmetology advertising media should construct the publicity mode based on ethics,law,governance,occupation,public welfare and social responsibility.
3.The Application and Effectiveness Evaluation of Continuing Care Based on Humanistic Care for Out Patients with Facial Plastic Surgery

Objective:To evaluate the effect of continuing care in facial plastic operation .Method:Select 120 patients who had aesthetic plastic surgery in our hospital during the period from January to July in 2014 and ran-domize them into the observation group (60 cases)and the control group (60 cases).The control group merely re-ceive conventional treatment .The observation group is employed continuing care based on conventional treatment . Results:The satisfaction of two groups has no significant difference a week after operation (P>0.05).Six months after operation , the satisfaction of the observation group is obviously higher than that of the control group .The difference is statistically different ( P<0 .0001 ) .The anxiety level of the observation group is obvious lower than that of the control group and the difference is Statisticallydifferent ( P<0 .0001 ) .Conclusion: Based on the hu-manistic care, carry on the continuing care could considerately improve the satisfaction of patients in clinic facial plastic surgery and is one of the most important solution of preventing medical disputes ;mean while , it is also one of the most effective stress reduction measures which makes the patients feel humanized nursing .
4.Effects of different health education methods on patients with enterostomy: a network Meta-analysis

Objective:To explore the effects of different health education methods on patients with enterostomy using a network Meta-analysis, so as to provide a theoretical basis for patients with enterostomy to choose the health education method.Methods:Randomized controlled trials (RCTs) on the effects of different health education methods in patients with enterostomy were electronically searched in PubMed, Web of Science, Embase, Cochrane Library, Medline, CINAHL, China National Knowledge Infrastructure, China Biology Medicine disc, VIP, and WanFang Data, supplemented by other manual search methods. The search period was from database establishment to May 31, 2023. This study extracted data, conducted methodological quality evaluation, and used Stata 15.0 software for network Meta-analysis.Results:A total of 30 RCTs were included, involving 7 health education methods. The probability ranking of the network Meta-analysis showed that in terms of enhancing self-care ability, the effects were ranked from high to low as personalized education, internet platform education, nursing pathway education, visual education, mind mapping education, peer support education, and routine health education. In terms of promoting the level of stoma adaptation, the order of effects from high to low was visual education, internet platform education, peer support education, and routine health education. In terms of improving quality of life, the order of effects from high to low was internet platform education, personalized education, peer support education, and routine health education.Conclusions:Personalized education has certain advantages in enhancing the self-care ability of patients with enterostomy, visual education is advantageous in promoting the social adaptation of patients, and internet platform education has certain advantages in improving the quality of life. This study can effectively guide clinical nurses to choose appropriate health education methods based on the purpose of health education, in order to heighten the effectiveness of patient health education.
5.Establishment of the patient derived liver cancer xenograft model

Objective:To establish the patient derived xenograft (PDX) model of liver malignant tumor, analyze the related factors affecting the carcinogenesis of PDX model, and analyze the differences of biological characteristics between the primary tumor and PDX model.Methods:Fresh liver malignant tumor tissue samples were collected from the patients who received the surgery from the Tianjin First Central Hospital and the samples were inoculated subcutaneously into BALB/c-nu mice. The correlations between clinicopathological information and tumor formation rate were analyzed, and the pathological morphology and specific protein expression of PDX model and primary tumor were compared.Results:Thirty-three PDX models were successfully established from 63 cases of liver malignant tumors. The overall tumor formation rate was 52.4% (33/63), including 46.3% (25/54) of primary liver cancer and 88.9% (8/9) of liver metastasis. The main factors affecting the tumor formation rate were tumor pathological type, distant metastasis and TNM stage (all P<0.05). The pathological morphology and specific protein expression of PDX model and primary tumor were similar. Conclusion:The PDX model of liver malignant tumor was successfully constructed, and the tumor formation rate was high, and can maintain the biological characteristics of the primary tumor.
6.Alterations of hubs related to disease progression in patients with temporal lobe epilepsy: a study on topological properties of brain networks

Objective:To investigate the alterations of brain functional network related to disease progression in patients with temporal lobe epilepsy (TLE) and the potential neural mechanism of executive dysfunction.Methods:Nineteen temporal lobe epilepsy patients with a disease course of less than or equal to 5 years (TLE-SD group) and 24 temporal lobe epilepsy patients with a disease course of more than 5 years (TLE-LD group) were recruited from the Epilepsy Clinic of the First Affiliated Hospital of Guangxi Medical University from December 2019 to December 2021. At the same time, 21 healthy control individuals (healthy control group) were recruited from the society and matched with gender, age, and education level. All subjects were evaluated demographic and clinical data, and performed resting state functional magnetic resonance imaging (rs-fMRI) and attention network test (ANT). The topological properties of functional brain network of 3 groups of subjects were calculated using graph theory, and then the hubs were determined. Then the inter-group comparison of hubs properties was carried out, and the correlation analysis with executive control function was performed.Results:The distribution pattern of hub nodes in TLE-SD patients was changed. Compared with the TLE-LD and healthy control groups, the TLE-SD group significantly increased the nodal degree values in brain regions such as the ventrolateral area of the left middle frontal gyrus, the left anterior central gyrus (upper limb area), the cephalic and caudal areas of the left parietal lobule, as well as the inner parietal lobe area, the caudal side of the right parietal lobule, and the right lateral occipital cortex. The nodal values of the brain areas such as the lateral and parietal inner area, the caudal side of the right upper upper lobule, and the right lateral occipital cortex were significantly increased. TLE-SD patients had a significant increase in the middle of the right upper frontal gyrus and the head of the left upper upper lobule. The node efficiency was significantly lower than that of TLE-LD patients, and the node efficiency in the middle of the right upper frontal gyrus was significantly lower than that of the healthy control group. The node network attributes of the left upper lobe were positively correlated with the execution control efficiency, and negatively correlated with the orientation efficiency. No significant difference was detected between TLE-LD patients and healthy controls.Conclusions:In TLE patients, the hubs in the middle of the right superior frontal gyrus were first damaged, and new hubs appeared in the dorsal attention network, the left anterior central gyrus and the right occipital lobe and were overloaded. With the progression of the disease, hubs tended to be redistributed to normality. The left superior parietal lobule may play an important role in the impairment of executive control function in TLE patients.
7.The application of the growth mentor program in the standardized training of the new nurses

Objectives? To explore the effects of applying growth mentor program in the training of new nurses. Methods? By convenient sampling, the newly recruited nurses who signed official employment contract with the First Affiliated Hospital of Xi'an Jiaotong University were selected as subject. A total of 756 nurses who enrolled from 2008 to 2012 were put into the control group and 812 nurses enrolled from 2013 to 2017 were in the observation group. The control group received routine training while the observation group received the training of the growth mentor program. The two groups was compared in the resignation rate, annual assessment scores, and patients' satisfaction. Results? The differences in resignation rate between the observation group (0.37%) and the control group (1.06%) were statistical significant (P<0.01). The annual assessment scores of the observation group (88.63±2.17) and patients' satisfaction (96.78±1.76) were all higher than the control group with statistical significance (P< 0.01). Conclusions? The establishment and implementation of the new nurses' growth mentor program can significantly reduce the nurses' resignation rate, improve the nurses' professional skills and patients' satisfaction, therefore the program should be widely applied in new nurses' training in hospitals.
8.Status and influencing factors of positive feelings among caregivers of patients with enterostomy

Objective:To explore the status and influencing factors of positive feelings of caregivers of patients with enterostomy, so as to provide a reference for targeted interventions.Methods:From May to October 2021, convenience sampling was used to select 130 caregivers of patients with enterostomy who received follow-up visits in the Stoma Clinic of the First Affiliated Hospital of Xi'an Jiaotong University as the research subject. The survey was carried out using the General Information Questionnaire, the Positive Aspects of Caregiving (PAC) , and the Care Preparedness Scale (CPS) . A total of 130 questionnaires were distributed, and 127 valid questionnaires were recovered, with a valid recovery rate of 97.69%.Results:Among 127 caregivers of patients with enterostomy, the total score of PAC was (32.69±4.99) , and the self-affirmation dimension score was (18.39±2.84) , and the life outlook dimension score was (14.36±2.53) , and the total score of CPS was (17.76±4.36) . Multiple linear regression analysis showed that caregiver gender, education level, work status, and readiness were the influencing factors of positive feelings of caregivers of patients with enterostomy ( P<0.05) , which explained 43.7% of the total variance. Conclusions:The positive perception of caregivers of patients with enterostomy is at an upper-middle level. Nurses should formulate targeted humanistic care strategies according to the influencing factors, increase the positive feelings of caregivers, help patients adapt to the stoma as soon as possible, and then improve the quality of life of patients.

In order to understand the status quo of ethical review of clinical research on the defecation function of patients with rectal cancer after sphincter-preserving surgery, analyze its causes and put forward corresponding suggestions, to arouse researchers’ attention to ethical review in subsequent relevant clinical studies. The ethical review of literatures related to the defecation function of patients with rectal cancer after sphincter-preserving surgery published on CNKI in recent 10 years was sorted out and summarized. The results showed that the ethical review of clinical research papers on defecation function of patients with rectal cancer after sphincter-preserving surgery was not optimistic. We should strengthen the ethical training of researchers, improve the ethical awareness of researchers, strictly implement the ethical norms of paper publication, strengthen the ethical requirements of manuscript contracts, perfect the ethical review system, and pay attention to the examination and supervision of informed consent, so as to promote the construction of ethical examination and approval norms of clinical research documents.
