1.Rare and atypical imaging manifestations of abdominal lesions associated with IgG4-related disease:CT and MRI findings

Objective To investigate rare and atypical imaging manifestations of abdominal lesions associated with IgG4-related disease (IgG4-RD).Methods Forty-one patients with IgG4-RD proved histopathologically or clinically were investigated retrospectively.All the patients underwent precontrast and dynamic enhancement scan of the abdomen.CT was performed in 32 patients and MR imaging in 27 patients (including both of CT and MRI in 18 patients).Whether tissue and organs of the abdomen were involved was observed,especially rare and atypical imaging manifestations including focal pancreatic lesion,pseudocyst and (or) abscess as well as calcification of pancreas,peripheral blood vessels of pancreas involvement and mass-like lesions were existed.Involvement of the organs including liver,gallbladder,spleen,gastrointestinal tract,mesentery and their manifestations were also observed.Results One to four abdominal organs were involved,the numbers of involved organs were as follows:one organ in 13 patients (31.7%),two organs in 12 patients (29.3%),three organs in 13 patients (31.7%),and four organs in 4 patients (7.3%).The most commonly involved organ of the abdomen was the pancreas which was found in 35 patients.Abdominal extrapancreatic lesions were found in 33 patients.Focal involvement of pancreas as a rare manifestation showed in 11 cases (11/35,31.4%).The patterns were focal and multifocal.Rare cases showed pseudocysts (3/35,8.6%) and abscess was found in 1 case.Capsule-like rim was present around the pancreas lesions in 25 cases.Ten cases showed thick and wide rim with unclear boundaries with adjacent blood vessels,and 7 of them showed the peripheral blood vessels surrounded.Bile duct involvement was observed in 26 patients,and gallbladder involvement was observed in only 2 of them.Rare renal involvement patient has mass-like lesion,with hypointensity on T2WI,then showed decreased enhancement and gradually delayed enhancement.One retroperitoneal fibrosis patient also showed mass-like lesion as an atypical and rare manifestation.Three IgG4-related liver inflammatory pseudotumors were found in 2 patients.Lesions showed iso or hypointensity on T2WI,continuously enhancement,with abnormal edge ring.Mesentery involvement was found in 1 patient.Conclusion Several abdominal lesions associated with IgG4-RD have rare and atypical imaging manifestations.There are specific imaging characteristics,which are helpful for most accurate diagnosis of IgG4-RD.
2.Expression and signifance of Survivin and Bcl-2 in human ampullary carcinoma

ObjectiveTo study the expressions of Survivin and Bcl-2 in carcinoma of the ampulla of Vater and their clinical significance.Methods The expressions of Survivin and Bcl-2 proteins were tested by EnVision immunohistochemistry in 40 cases of ampullary carcinomas,and 8 cases of normal ampulla of rater as control.ResultsThe positive expression of Survivin and Bcl-2 in the tissues from the ampullary carcinoma was significantly higher than that in the normal controls(82.5%,72.5% vs 0%,12.5%,P <0.05).The expression of Survivin and Bcl-2 had no relations with the sex,age,tumor size,histological types,and clinical stages( P > 0.05 ).The differences of the expression of Survivin and Bcl-2 in ampullary carcinoma of duodenal invasion,pancreatic invasion,and lymph node metastasis were significant(P < 0.05) ; Significantly positive correlation was found between the expression of Survivin and Bcl-2 by using spearman correlation analysis( r =0.647,P < 0.05 ).Conclusions Survivin and Bcl-2 may play an important role in tumorigenesis of ampullary carcinoma.Combined detection of Survivin and Bcl-2 may be useful to determine the degree of malignancy and the progress of the ampullary carcinoma.
3.Effects of HeNe laser combined with ultrashort wave therapy on pain factors and anti-inflammatory and pro-inflammatory factors in patients with chronic knee osteoarthritis

Objective:To investigate the effects of HeNe laser combined with ultrashort wave therapy on pain factors, anti-inflammatory and pro-inflammatory factors in patients with chronic knee osteoarthritis (KOA).Methods:Using prospective research methods, 108 patients with chronic KOA in Xicheng District Xichang′an Street Community Health Service Center from January 2018 to December 2019 were selected as the research objects, and randomly divided into control group and observation group, with 54 cases in each group. The control group was given shortwave treatment, while the observation group was given HeNe laser combined with ultra-short wave treatment. The clinical efficacy, levels of pain factors, anti-inflammatory and pro-inflammatory factors and knee function were compared between the two groups, and the pain factors including prostaglandin E 2 (PGE 2), substance P (SP), dopamine (DA); the anti-inflammatory and pro-inflammatory factors including transforming growth factor β1 protein (TGF-β1), tumor necrosis factor-stimulating gene (TSG-6), interleukin-10 (IL-10), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-1β (IL-1β). Results:The total effective rate of observation group was significantly higher than that of control group: 92.59% (50/54) vs. 75.93% (41/54), P<0.05. After treatment, the levels of PGE 2, SP, DA, TGF-β1, TSG-6, IL-10, TNF-α, IL-6 and IL-1β in the observation group were significantly lower than those in the control group: (2.50 ± 0.29) ng/L vs. (2.85 ± 0.32) ng/L, (1.55 ± 0.35) ng/L vs. (1.73 ± 0.36) ng/L, (12.46 ± 1.82) g/L vs. (15.25±2.20) g/L, (12.46 ± 2.06) μg/L vs. (15.58 ± 2.89) μg/L, (6.02 ± 0.89) ng/L vs. (6.84 ± 0.92) ng/L, (13.64 ± 2.92) ng/L vs. (16.50 ± 3.15) ng/L, (38.82 ± 7.15) ng/L vs. (47.05 ± 8.53) ng/L, (21.92 ± 4.19) ng/L vs. (25.41 ± 5.08) ng/L, (26.49 ± 6.74) ng/L vs. (31.53 ± 7.95) ng/L, P<0.05. After treatment, the levels of PGE2, SP, DA, TGF-β1, TSG-6, IL-10, TNF-α, IL-6 and IL-1β in the observation group were significantly lower than those in the control group ( P<0.05). The excellent rate of knee function in the observation group was significantly higher than that in the control group: 87.04% (47/54) vs. 64.81% (35/54), P<0.05. Conclusions:For patients with chronic KOA, HeNe laser combined with ultrashort wave therapy has better clinical efficacy, which can relieve pain, regulate the imbalance of body factors and improve the function of knee joint.
4.Determination of tromethamine residues in human coagulation factor Ⅷ by high performance liquid chromatography method

【Objective】 To develop and verify high performance liquid chromatography (HPLC) detection for tromethamine (Tris) residues in human coagulation factor Ⅷ. 【Methods】 Alanine was used as internal standard, and AQC for pre-column derivation. Inertsil® ODS-SP was adopted, and acetic acid-sodium acetate buffer and acetonitrile were used for gradient elution at the flow rate of 1 mL/min and column temperature of 37℃. The 25 μL sample was loaded and determined by UV-detector with detection wavelength at 248 nm. This method was then verified. 【Results】 Glycine, sodium citrate and calcium chloride showed no interference with the detection of tromethamine (Tris) residues. The recovery rate of spike samples was within 90.0%~100.8%. The RSD in repeatability test were 2.5%, 0.7% and 1.2%, respectively, and in intermediate precision test 0.8%. The tromethamine (Tris) at concentrates of (0.5~4.0)μg/mL showed good linear relationship to the peak area to internal standard, of which the regression equation was Y=0.012 9X-0.000 782, R=0.999 97.The quantitative detection limit was 0.04μg/mL. 【Conclusion】 The HPLC for determination of tromethamine (Tris) residues in human coagulation factor Ⅷ was successfully developed, which showed good linearity as well as high specificity, precision and accuracy.
5.Reversible phosphorylation of the 26S proteasome.

The 26S proteasome at the center of the ubiquitin-proteasome system (UPS) is essential for virtually all cellular processes of eukaryotes. A common misconception about the proteasome is that, once made, it remains as a static and uniform complex with spontaneous and constitutive activity for protein degradation. Recent discoveries have provided compelling evidence to support the exact opposite insomuch as the 26S proteasome undergoes dynamic and reversible phosphorylation under a variety of physiopathological conditions. In this review, we summarize the history and current understanding of proteasome phosphorylation, and advocate the idea of targeting proteasome kinases/phosphatases as a new strategy for clinical interventions of several human diseases.

6.MRI findings of primary retroperitoneal leiomyosarcoma

Objective To investigate the MRI features of primary retroperitoneal leiomyosarcoma(PRLS).Methods Thirty-three cases of pathologically confirmed PRLS were analyzed retrospectively,and their multi-parameter MRI features were analyzed qualitatively and quantitatively.Results Of the 33 PRLS,24 cases were located in the renal hilum,inferior vena cava,or the area around the bifurcation of the iliac vessels.Twenty-four cases had relatively intact capsule,25 cases had necrosis,and 15 cases had coagulated geographic necrosis.The mean diameter of the tumors was(9.9±5.1)cm,ranging from 3.0 to 20.7 cm,among which 27 cases with a diameter larger than 5 cm.The tumor parenchyma of the 33 PRLS all showed significantly high signal intensity on diffusion weighted imaging(DWI),and significantly low signal intensity on apparent diffusion coefficient(ADC)map.The tumor parenchyma showed moderate to obvious degree of progressive enhancement,and the signal intensity value of the tumor in each phase after contrast injection was higher than that of the psoas major muscle(P<0.001).Conclusion PRLS are usually large but have a relatively intact capsule and are closely related to the major retroperitoneal veins.The coagulated geographic necrosis,significantly limited diffusion of water molecules,and progressive enhancement pattern all show certain MRI characteristics.
7.Research advances in transmission dynamic models on hand, foot, and mouth disease

Hand, foot, and mouth disease (HFMD) is a common childhood infectious disease caused by various enteroviruses. China has the most significant number of reported cases and deaths of HFMD over the globe. Understanding the epidemic laws of HFMD can provide a scientific basis for designing prevention and control measures. The dynamic transmission models focus on the transmission mechanism of infectious diseases. They can simulate the actual situation to study the epidemic rules of diseases by adding, deleting, and subdividing compartments. More researchers have paid attention to dynamic models because of their high flexibility. To carry out the dynamic model of the HFMD research more effectively, a comprehensive understanding of related research progress in this field is deeply needed. In this paper, based on various researchers' different research purposes of dynamic models, the research progress was classified and summarized, providing meaningful guidance for model construction methods and future research directions and references for dynamic modeling of other models of infectious diseases. It was found that most studies used the SIR dynamic model or its extended model (such as the SEIR model), and few studies contained a complex factor compartment. Some important epidemiological parameters (such as R0) were obtained by studying the HFMD cases in a specific region, simulating different intervention scenarios to evaluate the effect of measures, or revealing the future trend by model prediction. However, there is no dynamic model simultaneously considering age structure, population moving, seasonality and periodicity, and vaccination.
8.Role of Toll⁃like receptor 4 in the early stage of acetaminophen⁃induced acute liver inj ury in mice

Objective :
To used Toll⁃like receptor 4( TLR4) inhibitor ( TAK⁃242) investigate the role of TLR4 in the early stage of acetaminophen (APAP) Ⅳinduced acute liver injury in mice.
Methods :
Fifty⁃eight male institute of cancer research( ICR) mice were randomly divided into control group ( normal control group , solvent control group , inhibitor control group) and experimental group (APAP 4 ⁃ hour group , APAP + TAK⁃242 4 ⁃ hour group , APAP 12 ⁃ hour group , APAP + TAK⁃242 12 ⁃ hour group) . Mice in the experimental group were given a single dose of APAP (300 mg/kg) and TAK⁃242 (3 mg/kg) were given intraperitoneal injection 3 ⁃ hour before APAP injection. The serum alanine aminotransferase (ALT) level and liver index of mice in each group were compared ;
HE staining showed pathological changes of liver tissue;the level of high mobility group box⁃1 (HMGB1) was determined by immunohistochemistry;the levels of TLR4 , HMGB1 and nuclear factor kappa B(NF⁃κB) protein were detected by Western blot;the levels of monocyte chemoattractant protein⁃1( MCP⁃1) , interleukin( IL) Ⅳ1β , tumor necrosis factor⁃α ( TNF⁃α ) and IL⁃6 genes were determined by real⁃time fluorescence quantitative PCR ( RT⁃qPCR) . The content of IL⁃6 in liver tissue was determined by enzyme linked immunosorbent assay (ELISA) .
Results :
HE staining showed liver tissue of mice obvious swelling and congestion of in APAP group at 4 ⁃hour and typical lobular central necrosis in APAP group at 12 ⁃ hour;the degree of liver necrosis in APAP + TAK⁃242 groups at 4 ⁃ hour and 12⁃ hour was less than that in APAP group at the same time point. Compared with the normal control group , serum ALT level , liver index , TLR4 , HMGB1 , NF⁃κB protein content , MCP⁃1 , IL⁃1β , TNF⁃α , IL⁃6 gene levels and IL⁃6 content in liver tissue of APAP 4 ⁃ hour group increased ; serum ALT level , liver index , HMGB1 protein content and IL⁃6 content in liver tissue increased in APAP 12 ⁃ hour group. Compared with APAP 4 ⁃ hour group , serum ALT level , liver index , TLR4 , HMGB1 , NF⁃κB protein content , MCP⁃1 , IL⁃1β , TNF⁃α , IL⁃6 gene level and IL⁃6 content in liver tissue decreased in APAP + TAK⁃242 4 ⁃ hour group. Compared with APAP 12 ⁃hour group , the levels of TLR4 , HMGB1 protein and MCP⁃1 , IL⁃1β , TNF⁃α genes in APAP + TAK⁃242 12 ⁃ hour group decreased.
Conclusion
nhibition of TLR4 may inhibit TLR4/HMGB1 pathway to reduce the inflammatory response in the early stage of acetaminophen⁃induced acute liver injury in mice.
9.The role of Bmi-1 gene in submandibular gland of mice

Objective To determine the role of Bmi-1 in the submandibular gland (SMG) of mice.Methods SMG of 4-week wild-type(WT) and Bmi-1 null(Bmi-1-/-) mice was analyzed on the weight,salivary flow rate,hematoxylin-eosin staining morphological differences and the changes in proliferation and aging by histology,immunohistochemistry and Western blotting.Results Compared with WT mice,the average static salivary flow rate [WT:(0.21 ±0.02) μg/min,Bmi-1-/-:(0.10 ±0.02) μg/min] (P =0.001) and the submandibular gland weight [WT:(1.89 ± 0.15) μg],Bmi-1-/-:[(1.34 ± 0.07) μg] (P =0.003) of the male Bmi-1-/-mice were significantly decreased,the number of gland duct increased,and the granular convoluted duct showed reduced diameter and branches.More senescence-associated β-galactosidase positive cells existed in SMG of Bmi-1-/-mice (WT:0.00,Bmi-1-/-:0.18 ± 0.02),and Ki-67 immunopositive cells decreased in SMG of Bmi-1-/-mice(WT:0.40 ~ 0.47,Bmi-1-/-:0.18 ~ 0.20) (P =0.000).The expression of p16 (WT:1.00 ± 0.12,Bmi-1-/-:0.00 ±0.00) (P =0.003) and p19(WT:0.97 ±0.09,Bmi-1-/-:5.09 ±0.21) (P =0.004) were up-regulated dramatically in SMG of the Bmi-1-/-mice.Conclusions Bmi-1 gene deficiency causes abnormal function of SMG by inducing senescence phenotype of SMG.
10.Establishment of kinetic detection method for PKA content

【Objective】 To develop and verify a kinetic method for the determination of prekallikrein activator (PKA) content. 【Methods】 The optimal reaction conditions were determined by comparing the factors of pH and ionic strength of different sample dilution buffers, incubation time of each procedure, and incubation temperature. The accuracy, specificity, precision, linearity, stability and durability of the method were validated. 【Results】 The sample was diluted with 0.05 mol/L Tris-HCl buffer (pH8.5, containing 0.15 mol/L NaCl) and incubated by prekallikrein (PK) at 37℃ for 20 min. After that, the substrate S-2302 was added. Within 10 min before the measurement, the absorbance change rate reached △A405/min. The validation results indicated that the linear range of the method was (0.5~4.0)IU/mL, while the recovery of calibration standard was 96.9%~103.7% with the R2 value more than 0.99. The specificity test showed that human serum albumin, excipients of intravenous human immunoglobulin (pH4), low pH and protein content had no significant effect on the detection of PKA, The recovery rates of standard sample solution in the specificity experiment were 98.0% (0.9% sodium chloride solution), 95.3% (0.46% sodium caprylate solution), 96.7% (10% maltose solution, pH4.0), 94.0%(20%BSA), and 94.0%(5%BSA, pH4.0), respectively. The accuracy and precision of the method can meet requirements in the range between 0.5 and 4.0 IU/mL. The inter-batch recovery rate of quality control samples were between 96.4%~109.5% with the coefficients of variation(CV) between 0.2%~6.9%, while the intra-batch recovery rate were between 101.5%~102.9% with the CV between 2.6%~5.9%. The linearity, accuracy and precision of the assay can meet the requirements when PK and S-2302 were placed at room temperature for less than 6 hours, with the recovery rate of quality control samples between 94.9%~109.9%. The end-point method and kinetic method were used to determine the PKA in 20 batches of human serum albumin, and the consistency showed that there was no significant difference between the two methods(P>0.05). 【Conclusion】 A kinetic method for determination of PKA content with good linearity, specificity, accuracy, precision, stability and durability has been established. Compared with the method in ChP, the new method is more convenient, accurate and rapid to determine the content of PKA in human albumin and human immunoglobulin (pH4) for intravenous injection.
