1.Analysis on Drug Resistance and Pathogenic Bacteria Distribution of the Patients of Infectious Mononucleosis Combined with Bacterial Infec-tion

[Objective] To discuss the pathogenic bacteria distribution and drug resistance of patients of infectious mononucleosis combined with bacterial in-fection. [Method] Choose 38 children patients as research objects who were diagnosed as infectious mononucleosis with positive throat swab culture, and make investigation to the separated pathogenic bacteria distribution and drug resistance.[Result] In the 38 cases of samples, there ’re 44 strains of pathogenic bacteria detected(2 kinds of pathogenic bacteria detected in 6 cases respectively), the front 3 were: 14 strains of staphylococcus aureus(31.8%), 10 acineto-bacter baumannii(22.7%) and 8 klebsiel a pneumaniae(18.2%); the staphylococcus aureus had highest drug resistance to Penicilin G, occupying 85.7%; to ampicil in, cefoxitin and macrodantin, the acinetobacter baumannii had drug resistance 100%; the klebsiel a pneumaniae had drug resistance of 100% to ampicil in.[Conclusion] The pathogenic bacteria distribution and drug resistance of children patients with infectious mononucleosis combined with bacteria infection have definite features, which shal be emphasized in diagnosis and treatment.
3.Influence of Acupuncture on Cephalic Acupuncture Points Region Over the Brain-stem Evoked Potential of Patients Suffering Insufficient Blood-Supply of Vertebral Basilar Artery

Objective From the angle of electrophysiology, go further into the mechanism of action of acupuncture treatment to the insufficient blood-supply of vertebral basilar artery and at the same time, make a comparison of the effects between the acupuncture on cephalic meridian points and zonation cephalic acupuncture. Methods Through the contrast before and after adopting the self-acupuncture on the testees of 28 cases (including healthy persons of seven cases and TIA patients of 21 cases), observe the change of immediate effect of all waves latent period values of BAEP. Results The BAEP wave latent period of healthy persons (exclusive of I wave of auditory fainting region) assumes a relative delay within the physiological range and by comparison between points and region, the delay of V-wave Baihui point is evidently higher than that of auditory fainting region. However, the latent period of all waves of TIA patients is relatively moved up, especially there are evident differences before and after the I-wave acupuncture is carried out. Viewing from the comparison between points and region, there is no significant difference. Conclusion The function of acupuncture lies in the adjustment of antagonistic body state and it is most effective under the pathological state. Acupuncture will play the main role of excitation to the auditory nerve conduction of BAEP of TIA patients (from cochlea to midbrain) and the influence over the peripheral nerves is more sensitive than over the center ones. Although points and region have their own relative specificities, there is no significant difference between them.
4.A Preliminary Study on the Role of Mitochondrial Protein, Bcl-2 and Bax in Tumorigenesis

Objective: To study the expressions of mitochondrial protein (MAB1273), Bcl-2, and Bax in renal cell carcinoma and to investigate their role in the tumorigenesis. Methods: The expressions of mitochondrial protein, Bcl-2 and Bax were detected by immunohistochemistry SP method in 9 tissue samples of renal on-cocytoma, 6 samples of chromophobe carcinoma, 23 samples of clear cell carcinoma and 12 samples of normal renal tissue. Results: The expression of MAB1273 in renal oncocytoma, chromophobe carcinoma and clear cell carcinoma was much higher than that in normal renal tissue (P=0.006). No significant difference was found in MAB1273 expression among the three types of renal carcinoma. The expression of Bcl-2 was higher in the renal carcinoma groups than in the normal renal tissue group, with a significant difference (P=0.008). The expression of Bax was not different among all of the groups (P=0.057). Rank correlation analysis showed a positive correlation between MAB1273 and Bcl-2 expression (r=0.341, P=0.015). The expression of Bcl-2 was negatively correlated with Bax expression (r= -0.287, P=0.043). Conclusion: Overexpression of mitochondrial protein and Bcl-2 and underexpression of Bax may play a part in the genesis of renal oncocytoma, chromophobe carcinoma and the clear cell carcinoma, indicating that overexpression of MAB1273 may be correlated with apoptosis of renal cell carcinoma cells.
5.Bacterial Resistance of 1850 Pathogen Strains

OBJECTIVE To acquaint pathogen distribution of infection and present drug-resistance conditions of the commonly encountered pathogens.METHODS The statistics of the antimicrobial susceptibility testing of all the clinical isolates from Jan to Nov 2006 in our hospital was condncted.RESULTS Totally 1850 bacteria strains were isolated.From them 620(33.51%) were Gram-positive bacteria,1230(66.49%) were Gram-negative bacteria,25.8% of which were Escherichia coli,8.1% of which were Staphylococcus epidermidis,7.5% were Klebsiella pneumonice and 7.2% were MSSA;Statistics of drug sensibility test suggested,Gram-positive cocci isolated from the clinical issue be highly sensitive to vancomycin,teicoplanin and nitrofurantoin.They were still sensitive to rifampicin.and lower sensitive to other antibiotics.Gram-negatives were high sensitive to imipenem.They were still sensitive to piperacillin/tazobactam and the third-generation cephalosporins.CONCLUSIONS The drug resistance of the isolates is severe.It is suggested that there be ungent need for surveillance of bacterial resistance and rational use of antimicrobial agents be emphasized during clinical therapy.
6.ESBLs-producing Escherichia coli and Klebsiella pneumoniae and Their Antibiotic Resistance Changing

OBJECTIVE To investigate ESBLs-producing Escherichia coli and Klebsiella pneumoniae and their antibiotic resistance changing at the same area in different period.METHODS Using Bio-Merieux ATB susceptibility test panels microdilution broth method to detect the antibiotic resistance of E.coli and K.pneumoniae,in addition to the use of phenotypic confirmatory test to detect the ESBLs.RESULTS The ESBLs-producing rate of E.coli and K.pneumoniae during 1999-2001 compared with 2003-2005,was elevated more than double.To imipenem,cefoxitin,piperacillin/tazobactam,and amikacin,the resistant rate was relatively lower,but to other ?-lactams,aminoglycosides,quinolones and sulfonamides,the resistant rate increased significantly in both periods compared,the differences were significant(P

Infectious disease hospitals are obliged to cope with public health emergencies such as outbreak of infectious diseases.Such hospitals are required to make early detection,early containment,proper management,reasonable preplans,timely training,targeted drills,reasonable deployment of hospital resources,appropriate protection for hospital staff.General hospitals should also cope with prevention and control of infectious diseases to some extent,and work with infectious disease hospitals hand in hand to better cope with such outbreaks.
8.Effect of systemic lupus erythematosus on right heart function evaluated by parameters recommended by the American Society of Echocardiography

Objective To evaluate the right heart dysfunction in SLE patients with/ without pulmonary arterial hypertension (PAH) using the parameters recommended by the American Society of Echocardiography (ASE),and to examine whether the right heart dysfunction is directly related to elevated pulmonary arterial systolic pressure(PASP) and pulmonary vascular resistance(PVR).Methods Study population composited of 43 patients with SLE.The patients were divided into two groups according to the PASP measured by echocardiography:Group A was 24 patients with PASP ≤35 mm Hg,Group B was 19patients with PASP>35 mm Hg.Twenty-two healthy subjects with age and gender matched were set as control group.Routine transthoracic echocardiography study was performed on all patients and 26 parameters were measured in order to compare the differences of the parameters among the three groups.Results There were significant differences in 17 parameters among the three groups (P <0.05).Tricuspid annular plane systolic excurtion(TAPSE) and E/A ratio of tricuspid flow velocities in SLE patients without PAH significantly decreased compared to control group (P =0.04 and 0.03).There were significant differences in 11 parameters between SLE with and without PAH group (P <0.05).Multivariate logistic regression analysis indicated that after adjustment for age and gender,the patients with elevated PVR associated with a 6.18-fold increase in right ventricular dysfunction compared to the patients with normal PVR (P =0.02).Conclusions The impairment of right ventricular function in SLE patients was directly related to PVR and PASP.Elevation of PVR was an important predictor for right heart dysfunction.
9.Analysis of different methods of extracting DNA from paraffin-embedded tissues and the application of nest PCR.

The aim of this research was to explore the most optimal method of DNA extraction from formalin-fixed, paraffin-embedded (FFPE) tissues, and to improve the amplification of long fragments with the method. Three methods, one step method, phenol-chloroform extraction method, and genomic DNA purification kit method, were employed to extract DNA from twenty normal thyroid tissues which were fixed with formalin and embedded with paraffin. The highest proportionality of OD260/OD280 in the examples was obtained by phenol-chloroform extraction method, 1.703 +/- 0.086, compared to the results of the other two methods. As for the long DNA segments amplification, the achievement ratio of one step method, phenol-chloroform extraction method and genomic DNA purification kit method were 0%, 5% and 10%, respectively, by traditional PCR method, but 0%, 95% and 85% respectively by the nest PCR. We have found that the best process of extracting DNA from FFPE is digesting by proteinase K and purifying by phenol-chloroform, and it is effective to amplify long DNA segments from FFPE by nest PCR.

10.RNA Interference Targeting Leptin Gene Effect on Hepatic Stellate Cells

To construct the specific siRNA expression vectors and investigate their effect on leptin and collagen I in HSC, which provide a new approach to the prevent and treat hepatic fibrosis. The five siRNAs against leptin gene were transcript synthesized intracellularly by expression templates of plasmid vector psiRNA-hH1neo. The recombinant leptin siRNA plasmid vectors could express in eukaryocyte , and then to evaluate them by using enzyme cutting and sequencing. The recombinant plasmids were transfected into HSCs using Lipofectamine methods respectively. The cells were selected after growing in DMEM containing 300 μg/mL G418 for about 4 weeks. Gene expression of leptin and collagen I were showed by Western blot analysis and reverse transcription polymerase chain reaction (RT-PCR). Identification by enzyme cutting and sequencing showed that the leptin siRNA expression vectors were constructed successfully, and leptin siRNA could inhibit the leptin and collagen I gene expression effectively. It was concluded that RNA interference-mediated silencing of leptin gene diminished leptin and collagen I gene expression in HSCs. Furthermore, attenuated the extracellular matrix over-deposition at the same time. Leptin gene is ideal targets of gene therapy for liver fibrosis.
