This research is to explore the feasibility of construction of real-time network teach-ing platform in anesthesia resident standardization training. The real-time network teaching were car-ried out by platformconstruction and teaching organization. The teaching effect was evaluated. Using the real-time network teaching platform with the four functions of learning knowledge, cultivating ability, communication and supervision can achieve good teaching effect. It was a beneficial supplement of traditional teaching.

BACKGROUND:Hydroxyapatite/gel nano-composite has the same mechanical strength to the natural bone, but its ability to repair bone defects and osteogenic effect need to be confirmed by further studies.
OBJECTIVE:To explore the repair effect of hydroxyapatite/gel bionic composite in skul defects of rabbits.
METHODS: The hole-like calvarium defect models were established in rabbits, and treated with hydroxyapatite/ gel composites (hydroxyapatite/gel group), autologous skul as positive control (autologous bone group) and
nothing as negative control (blank group). The repairing condition in the skul defect areas were observed and analyzed by X-ray and CT at 4, 8, 12 weeks after implantation.
RESULTS AND CONCLUSION: After 8 weeks, X-ray assessment showed that normal-like bone tissue appeared in the defect region of the autologous bone group; in the hydroxyapatite/gel group, dense bone with similar
morphology to normal bone tissue was found in the central site of defect region, and the boundary was slightly blurred. After 12 weeks, the hydroxyapatite/gel showed blurred edge compared with autologous bone, and the center of the composite was disconnected; in the blank group, a clear and regular transmitted shadow was
observed. After 12 weeks, CT examination showed that the hydroxyapatite/gel was connected tightly with the surrounding normal bone tissue. As a new bionic composite, the hydroxyapatite/gel can achieve good effect in repairing skul defects of rabbits.

BACKGROUND: MyoD gene is one of family members of muscle transcription factors. Transfection MyoD gene can switch on the procedure of differentiation of muscles, and transit non-muscle cells into muscle cells.The MyoD gene only expresses in skeletal muscles. Based on the same contractive structure in myocardial cells and skeletal muscle cells, it is imagined that the conversion from exogenous MyoD gene-induced fibroblast in local myocardium into skeletal muscle cells that had contractive function may become another method in the treatment of congestive heart failure on clinic.OBJECTIVE: To construct and identify a recombinant adenoviral vector carrying MyoD gene for further studies on the recovery function of MyoD gene in myocardial injury.DESIGN: Single sample experiment.SETTING: Department of Cardiology, First Affiliated Hospital, Nanjing Medical University.MATERIALS: The experiment was conducted at the Laboratory of Microbiology and Immunology, Nanjing Medical University between September 2004 and September 2005. MyoD gene and non-replicating form expressive vector of adenovirus were taken as research materials.METHODS: MyoD cDNA fragments were extracted from plasmids pEMSV-MyoD with polymerase chain reaction (PCR), and PCR was used to clone the whole-length gene of MyoD. After adding CACC sequence at 5' end, MyoD gene was cloned by orient topology into transfer ventor, pENTR/D-TOPO. Objective gene was transferred into adenoviral expression vector DNA via pENTR/D-TOPO vector. The recombinant adenoviral vectors transfected into HEK293A cells by using lipofectamine were packaged and amplified.MAIN OUTCOME MEASURES: Evaluation of PCR and DNA sequencing were used for confirming the size of segment and correctness of rank of MyoD cDNA and detecting the titre of virus.RESULTS: MyoD recombinant adenovirus contained target segment with precise length confirmed by PCR and DNA sequence that was correct. The titre of virus was 1.3×1011 pfu/mL.CONCLUSION: The recombinant adenoviral vector carrying MyoD gene is constructed successfully.

The synchronous fluorescence spectra of myoglobin were studies for the first time. The fluorescence peaks observed in the spectra were assigned. When the wavelength interval (Δλ) is 80 nm, the main peak at 335 nm is originated from the tryptophan residues in the myoglobin molecule. When Δλis 20 mn, the peak at 308 nm is mainly due to the tyrosine residues in the myoglobin molecule and in a small part due to the tryptophan residues.Two peaks at 322 and 596 nm were observed in the spectrum of myoglobin for Δλ = 40 nm. The peak at 322 nm is due to both tyrosine and tryptophan residues. The peak at 596 nm is attributed to the heme group in the myoglobin molecule .

Objective:To investigate the effects of continuous substaneous insulin infusion (CSII) on islet β cell function in newly diagnosed diabetic patients.Methods:Forty-six newly diagnosed diabetic patients who received treatment in Taishan People's Hospital from July 2011 to June 2014 were included in this study. They were treated with CSII for 14 days and followed up for 5 years. Before and after treatment, fasting blood glucose (FPG), 2-h postprandial blood glucose (2hPG), triglyceride (TG), fasting insulin (FINS), 2-h postprandial insulin (2hINS), glycosylated hemoglobin (HbA1c), superoxide dismutase (SOD), malondialdehyde (MDA) and the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) index, Homeostasis Model Assessment for beta-cell function (HOMA-β) index were compared between before treatment and 5 years after treatment.Results:Five years after treatment, the levels of FPG, 2hPG, TG, HbA1c, MDA and HOMA-IR were lower than those before treatment [FPG: (11.3 ± 1.2) mmol/L vs. (5.9 ± 0.4) mmol/L, t = 15.35, P < 0.01; 2hPG: (18.1 ± 4.2) mmol/L vs. (8.1 ± 1.6) mmol/L, t = 16.83, P < 0.01; TG: (2.9 ± 1.1) mmol/L vs. (1.5 ± 0.6) mmol/L, t = 9.81, P < 0.01; HbA1c: (11.2 ± 2.5)% vs. (5.6 ± 1.0)%, t = 11.48, P < 0.01; MDA: (4.6 ± 1.2) μmol/L vs. (2.7 ± 0.9) μmmol/L, t = 16.37, P < 0.01; HOMA-IR: (2.81 ± 0.35) vs. (1.87 ± 0.32), t = 9.37, P < 0.01]. Five years after treatment, the levels of FINS, 2hINS, SOD and HOMA-β were significantly higher than those before treatment [FINS: (5.6 ± 1.3) mU/L vs. (7.4 ± 1.5) mU/L, t = - 6.15, P < 0.01; 2hINS: (15.8 ± 7.5) mU/L vs. (25.8 ± 9.1) mU/L, t = - 5.65, P < 0.01; SOD: (28.9 ± 7.6) U/L vs. (39.6 ± 7.8) U/L, t = - 7.93, P < 0.01; HOMA-β: (14.36 ± 3.82) vs. (65.67 ± 6.67), t = - 18.72, P < 0.01]. Linear regression analysis showed that HOMA-β was positively correlated with SOD level ( R2 = 0.319, P < 0.01). Five years after treatment, the final outcome was insulin therapy in 3 cases (6.5%), oral medication in 25 cases (54.4%), and lifestyle intervention in 18 cases (39.1%). Conclusion:CS II for the treatment of newly diagnosed diabetes mellitus can effectively inhibit oxidative stress, improve the function of islet β cells, and exhibit long-term effects.

Objective To investigate the levels and clinical value of the peripheral blood CD154 and CD4+CD25+FoxP3+ regulatory T cells in patients with ankylosing spondylitis (AS).Methods 20 patients with AS and 20 healthy donors were studied.Flow cytometry was used to analyze the levels of the peripheral blood CD154,CD4+CD25+FoxP3+ regulatory T cells and HLA-B27.And T-test,Spearman correlation analysis were used in statistical analysis.Results The level of the peripheral blood CD154 was significantly higher in AS patients than that in normal controls [(4.38 ± 1.64) ％ vs.(1.92 ± 1.13) ％ (t =5.836,P ＜ 0.05)],and the level of the peripheral blood CD4+CD25+FoxP3+ regulatory T cells was lower in AS patients than that in normal controls [(3.87 ± 1.11) ％ vs.(6.30 ± 1.46) ％ (t =6.254,P ＜ 0.05)].There was negative correlation between the peripheral blood CD154 and CD4+CD25+FoxP3+ regulatory T cells in AS (r =-0.539,P ＜ 0.05).No relationship of the peripheral blood CD154,CD4+CD25+FoxP3+ regulatory T cells with laboratory parameters were found in AS.Conclusion The levels of the peripheral blood CD154 and CD4+CD225+FoxP3+ regulatory T cells in AS are abnormal,and may be involved in the immunological pathogenesis of AS.

Objective To investigate the effects of neutrophil gelatinase associated lipocalin(NGAL) on renal tubular epithelial cells apoptosis and apoptosis-regulated protein fas,bcl-2 in rat renal ischemia reperfusion injury (IRI).Methods Renal IRI models of rats were established.Rats were randomly divided into 3 groups,including control group,IRI model group and NGAL group.The pathological change of kidney tissue was investigated by hemotoxylin-eosin staining.Renal tubular epithelial cells apoptosis was detected by TUNEL method.Expression of fas and bcl-2 was measured by real-time PCR and Western blotting.Results Compared with IRI model group,NGAL group showed a decreased number of renal tubular epithelial cells apoptosis [(8.6±3.4)/HP vs (20.8±3.7)/HP,P＜0.05],down-regulated fas mRNA (2.34±0.51 vs 6.84±2.34,P＜ 0.05),fas protein (0.65±0.05 vs 0.95±0.08,P＜0.05) and up-regulated bcl-2 protein (0.33±0.05 vs 0.24±0.03,P＜0.05),but the bcl-2 mRNA had no significant change.Conclusion NGAL can protect renal tubular epithelial cells in renal IRI,which may be associated with decreasing cell apoptosis and adjusting protein expression by apoptosis-regulated cytokines.

Objective To observe the influence of epalrestat combined with insulin therapy on islet beta cell function in newly diagnosed type 2 diabetic patients.Methods 45 newly diagnosed type 2 diabetic patients were randomly treated with 4 times of subcutaneous insulin therapy(RI group) or epalrestat plus 4 times of subcutaneous insulin therapy(RI + EP group).Patients were followed up for 3 months.The fasting blood-glucose (FPG),the 2 hour postprandial blood glucose (2 h PG),fasting insulin (FINS),the 2 hour postprandial blood insulin (2 h INS),glycated hemoglobin (HbA1 C),superoxide dismutase (SOD),malondialdehyde (MDA),insulin resistance index (HOMA-IR) and insulin release index(HOMA-β) were observed at 3th month after the initiation of therapy.Results Follow-up evaluation of 22 cases in RI group,23 cases in group RI + EP were completed 3 months of treatment.After treatment,FPG,2 h PG,HbA1 C,MDA and HOMA-IR in the two groups were decreased than those before treatment,the serum FINS,2 h INS,SOD and HOMA-β were higher than those before treatment,the differences were statistically significant (all P ＜0.05).After treatment,FINS,2 h INS,SOD and HOMA-β of RI + EP group were higher than those in RI group,MDA was lower than that of RI group,the differences were statistically significant (t =3.228,2.536,3.021,2.343,2.122,all P ＜ 0.05).FPG,2 h PG,HbA1 C,HOMA-IR between the two groups had no significant differences (all P ＞ 0.05).Linear regression analysis showed that HOMA-β was positively correlated with SOD level (r =0.888,r2 =0.783,all P ＜ 0.01).Conclusion The results suggest that epalrestat combined with insulin therapy can inhibit oxidative stress,and improve islet beta cell function in newly diagnosed type 2 diabetic patients,and its clinical effect is better than monotherapy with insulin.

Objective To analyze the genotypes of human herpesvirus 8 (HHV-8) by using the polymorphisms in open reading frame(ORF) 26 gene in patients with squamous cell carcinoma (SCC)in Xinjiang Uygur Autonomous Region.MethodsDNA was extracted from paraffin-embeded tissue specimens from 41 patients with skin SCC and 46 patients with esophagus SCC,and subjected to nested-PCR for the amplification of the ORF26 gene of HHV-8 followed by bidirectional sequencing.Phylogenetic analysis was carried out to determine the genotype of HHV-8 by using the DNASTAR software,Clustal W program,and PHYLIP package.The data were analyzed by SPSS 17.0 software.ResultsHHV-8 DNA was detected in 9 (21.95％) of 41 skin SCC specimens and 10 (21.74％) of 46 esophagus SCC specimens (x2 =0.09,P＞ 0.05).As phylogenetic analysis showed,7 HHV-8 isolates from skin SCC specimens belonged to ORF26 subtype A,2 subtype C; 7 HHV-8 isolates from esophagus SCC specimens belonged to ORF26 subtype A,and 3 subtype C.Conclusions ORF26 subtype A and C are the predominate genotypes of HHV in patients with SCC in Xinjiang Uygur Autonomous Region,with the prevalence of subtype A higher than that of subtype C.The distribution of HHV subtypes seems unrelated to the location of SCC.
