Objective:To investigate the molecular mechanism of the disease based on the clinical characterization and genetic mutation analysis in a family with intellectual disability. Methods:The proband with intellectual disability was diagnosed at Luohe Central Hospital in December 2019. Peripheral blood samples were collected from four family members. Whole exome sequencing (WES) was used to screen the pathological mutations. Then the PCR and Sanger sequencing were used to verify the selected mutations and combine the relevant database to analyze variation loci. Results:We infer that the ARX c.1162 A>G was co-segregated with the phenotype of the family based on the results of WES. The results of sanger sequencing and WES are consistent. The mother of the proband is the carrier of the mutation. There is no mutation frequency reported in the healthy population. The mutation of the ARX c.1162A>G is harmful inferred by a variety of bioinformatics software. Combined with the phenotypic analysis of OMIM database, we infer the phenotype caused by the mutation is consistent with the patients in the family. Conclusion:The mutation of the ARX c.1162 A>G may be the cause of the intellectual disability in the family affected. And the variant has not been reported in China.

Objective:To describe the status and influencing factors of the empowerment of caregivers after gastrointestinal tumor. Methods:From March 2019 to October 2019, using convenience sampling method, 124 pairs of gastrointestinal tumor patients and caregivers who were hospitalized in the general surgery and anorectal surgery of the department of Shandong Tai′an City Central Hospital and the Second Affiliated Hospital of Shandong First Medical University were selected as the subjects. A general information questionnaire, Main Caregivers′Empowerment Measurement were used as research tools. To investigate the empowerment ability of caregivers of patients with gastrointestinal tumors after operation. Results:The total caregiver′s empowerment score was 168.81 ± 19.49. The scores of each dimension ranged from high to low: relationship with caregivers (3.83 ± 0.43), expectation of care outcomes (3.80 ± 0.43), awareness of caregiving roles (3.70 ± 0.51), caregivers′ subjectivity (3.37 ± 0.60), caregiving beliefs (3.34 ± 0.67), caregiving knowledge and skills (3.27 ± 0.73), personal resources (2.94 ± 0.65), goodwill caregiving (2.85 ± 0.65) , the scruples around (2.75 ± 0.88). Multiple linear regression results showed that, patient gender, whether the disease is first attack, whether the patient is aware of the disease, the caregiver marriage, and the caregiver working status were the influencing factors of the caregiver′s empowerment ability after gastrointestinal tumor operation ( t values were -8.15-8.72, R2=0.62, all P<0.05). Conclusions:There is room for further improvement in the empowerment of caregivers after gastrointestinal tumor surgery, especially in personal resources, goodwill care and scruples around. The results suggest that medical staff should guide caregivers to emancipate their minds, enhance the breadth of empowerment, and promote self care and fast rehabilitation of patients.

BACKGROUND:
Intra-articular injection is a classic strategy for the treatment of early osteoarthritis (OA). However, the local delivery of traditional therapeutic agents has limited benefits for alleviating OA. Exosomes, an important type of extracellular nanovesicle, show great potential for suppressing cartilage destruction in OA to replace drugs and stem cellbased administration.
METHODS:
In this study, we developed a thermosensitive, injectable hydrogel by in situ crosslinking of Pluronic F-127 and hyaluronic acid, which can be used as a slow-release carrier to durably retain primary chondrocyte-derived exosomes at damaged cartilage sites to effectively magnify their reparative effect.
RESULTS:
It was found that the hydrogel can sustainedly release exosomes, positively regulate chondrocytes on the proliferation, migration and differentiation, as well as efficiently induce polarization of M1 to M2 macrophages. Intraarticular injection of this exosomes-incorporated hydrogel significantly prevented cartilage destruction by promoting cartilage matrix formation. This strategy also displayed a regenerative immune phenotype characterized by a higher infiltration of CD163+ regenerative M2 macrophages over CD86+ M1 macrophages in synovial and chondral tissue, with a concomitant reduction in pro-inflammatory cytokines (TNF-a, IL-1b, and IL-6) and increase in anti-inflammatory cytokine (IL-10) in synovial fluid.
CONCLUSION
Our results demonstrated that local sustained-release primary chondrocyte-derived exosomes may relieve OA by promoting the phenotypic transformation of macrophages from M1 to M2, which suggesting a great potential for the application in OA.

ObjectiveTo find the different metabolites of Schisandrae Chinensis Fructus fresh fruits dried in different ways by proton nuclear magnetic resonance (1H-NMR) metabonomics technology, and to explore the possible effect of green removing on its metabolic profile. MethodFifteen samples of Schisandrae Chinensis Fructus fresh fruits were used, and each 3 samples was divided into one group. The samples were processed by five methods, including drying, drying in the sun, microwave, blanching and steaming. These samples were extracted with 50% methanol and analyzed by 1H-NMR. Combined with multivariate statistical analysis, the differences of the metabolic profile between green removing samples and dried samples, sun-dried samples were compared. ResultA total of 27 chemical components were identified by 1H-NMR and Chenomx database, mainly including amino acids, alkaloids, carbohydrates and organic acids. These metabolites were mainly involved in the energy metabolism of fruit postharvest physiology. The results of multivariate statistical analysis showed that there were significant differences among the 5 groups. The dried and sun-dried samples were used as the controls, 17 differential metabolites (asparagine, citric acid, glucose, sucrose, choline glycerophosphate, trigonelline, alanine, lysine, glycerol, leucine, isoleucine, valine, uridine, threonine, methionine, phenylalanine and fructose) were screened by variable importance in the projection (VIP) value and S-plot. One-way ANOVA was used for comparison between groups, the results showed that compared with dried and sun-dried samples, the contents of sucrose and choline glycerophosphate increased significantly (P<0.05, P<0.01) in samples treated with green removing, but the contents of valine, leucine, isoleucine, threonine, phenylalanine and methionine decreased significantly (P<0.01), the content of asparagine increased significantly (P<0.05, P<0.01) in samples treated by microwave, while the contents of trigonelline and uridine decreased significantly (P<0.01) in samples treated by blanching. ConclusionThe consumption, decomposition and/or transformation of active ingredients may be inhibited by interfering with its energy metabolism after the green removing of Schisandrae Chinensis Fructus fresh fruits.
