1.Analysis and discussion on the relationship between negative chronic diarrhea and terminal ileum pathological lesions by colonoscopy

Objective:To study the relationship between TCM syndrome type of chronic diarrhea, chronic diarrhea and negative colonoscopy terminal ileum lesions by colonoscopy.Methods:104 cases in our hospital underwent colonoscopy patients (each patients colonoscopy pushed to the terminal ileum is more than or equal to 20 cm) were divided into control group and observation group by examination results, each with 52 cases. The patients in the control group had no chronic diarrhea and the observation group had negative chronic diarrhea. The observation group was given the lines of traditional Chinese medicine syndrome differentiation typing, and chronic diarrhea associated with ileum end lesions were given death drop spirit combined micro ecological preparation, and then review the lesions after 15 days.Results: In the observation group, 1 case was TCM sub type of damp heat in spleen and stomach (1.92%), 2 cases were spleen deficient deficiency (3.84%), 3 cases were liver stagnation (5.77%), 4 cases were deficiency of spleen and liver (7.69%), 8 cases were cold-dampness disturbing spleen (15.38%), 9 patients were liver stagnation and spleen deficiency (17.31%), 11 cases were spleen and kidney deficiency (21.15%), and 14 cases were spleen wet trapped (26.92%). There were 37 cases (71.15%) in the observation group of patients with ileal mucosal edema, congestion, erosion and ulceration and 12 cases in the control group (23.08%). The differences between the two groups were statistically significant (x2=24.12,P<0.05). There were 38 cases in the observation group and 7 cases in the control group with ileal follicular hyperplasia. There were 18 cases were mid hyperplasia in the observation group and 8 cases in the control group. The differences were significant(x2=37.64,x2=21.31;P<0.050). The size of patients with hyperplasia was 4.5mm in the observation group and 1.2mm in the control group. The differences between the two groups were statistically significant (Z=13.05,P<0.05). 46 cases of chronic diarrhea symptoms disappeared after treatment and 26 cases of colonoscopy terminal ileum lesions were improved. The differences before and after treatment were statistically significant (70.27%).Conclusion: Patients with chronic diarrhea syndrome are found lesions in terminal ileum. The symptom would disappear or be greatly improved after medicine treatment. It can be promoted as a reference standard of clinical medication.
2.A diffusion tensor imaging study of white matter in first episode neuroleptic-na?ve early-onset schizophrenia

Objective To investigated cerebral structural connectivity and its relationship to neuroleptic-na?ve individuals with first episode early-onset schizophrenia using diffusion tensor imaging (DTI) which could demonstrate the white matter integrity . Methods We recruited subjects with first episode DSM-Ⅳearly-onset schizophrenia who had never been exposed to antipsychotic medication(n=19) and sex ,age-matched healthy volunteers (n= 19) .All subjects received DTI and structural magnetic resonance imaging scans .Voxel-based analysis was performed to investigate brain regions fractional anisotropy (FA) values .Results Statistics revealed that schizophrenia patients showed significant FA reduction in left inferior frontal gyrus ,left temporal gyrus ,left occipital lobe and right middle temporal gyrus as compared to healthy subjects .Conclusion Deficits of white matter integrity in widespread brain regions of the first episode neuroleptic-na?ve early-onset schizophrenia patients .The presence of white matter abnormalities in the early-onset patients is suggestive of being related to the etilology of schizophrenia .
3.Pharmacokinetics and bioavailabilities of geniposide in Beagle dogs after oral administration Xingnaojing.

OBJECTIVETo establish a method for determination of geniposide in Beagle dogs plasma by high performance liquid chromatography (HPLC), and study the pharmacokinetics and bioavailability of geniposide in Beagle dogs after oral administration Xingnaojing.
METHODTo determine the geniposide in Beagle dogs plasma by HPLC after oral administration or intravenous injection Xingnaojing, and the pharmacokinetic parameters were calculated by the software of Kinetica.
RESULTThe good linearity range of geniposide was 1.24 - 158.88 mg x L(-1). The main pharmacokinetic parameters after oral administration was as follows: Cmax (11.8 +/- 0.6) mg x L(-1), Tmax (52.0 +/- 4.5) min, AUC(1280.8 +/- 172.0) mg x min x L(-1), MRT(118.7 +/- 25.4) min, and these parameters after intravenous injection was follows: Cmax 107.4 +/- 6.3) mg x L(-1), AUC(7930.1 +/- 670.0) mg x min x L(-1), MRT(92.4 +/- 5.1) min. The bioavailability of geniposide in Beagle dogs after oral administration Xingnaojing was (6.46 +/- 0.87)%.
CONCLUSIONThe HPLC method had good applicability. The extract recovery, method recovery, intra-day precision and inter-day precision of the method were all met the requirements. The stability in conditions of room temperature and freeze-thaw cycle was good. The results indicated that the oral administration bioavailability of geniposide was in low degree.

The Chinese Standard Aphasia Examination was made according to Chinese language and cultural habits by referring to Japanese Standard Language Test of Aphasia.151 normal people and patients whose brain damaged without aphasia were tested.Mean and standard deviations were calculated.The significant differences were not found by analysis of variance to age,sex,handedness,profession and education except oral instruction and describing pictures in different educational groups.Therefore,the examination is suitable for Chinese aphasics who live in different parts of China and spoken mandarin.
5.A preliminary study on the combination of group screening and opportunistic screening for gastric cancer

Objective:To evaluate the new model of group screening combined with opportunistic screening for the diagnosis and treatment of gastric cancer.Methods:Group screening combined with opportunistic screening was used for gastric cancer screening. (1) Group screening. Cluster sampling was used to screen gastric cancer by endoscopy in high-risk population (aged 40-<70 years) of rural residents in Weihai from July 2017 to December 2020, and biopsy was obtained for histopathology if necessary. Main collection parameters included the detection rate of advanced gastric cancer, early gastric cancer and high-grade intraepithelial neoplasia (HGIN). (2) Opportunistic screening. The changes of the detection rates of early gastric cancer in opportunistic screening in 2 hospitals in Weihai area were observed during the same period of time.Results:(1) In group screening, from July 2017 to December 2020, the first batch of 8 000 cases of gastric cancer screening were completed. The cases of advanced gastric cancer, early gastric cancer and HGIN were 36, 28, and 62, respectively. The detection rates of gastric cancer and early gastric cancer were 0.80% (64/8 000) and 43.75% (28/64), respectively. The proportion of early gastric cancer+HGIN who received endoscopic submucosal dissection (ESD) was 77.78% (70/90), and the rate of curative resection was 100.00%(70/70). (2) Opportunistic screening: from July 2017 to December 2020, the annual early gastric cancer detection rates in opportunistic screening in Wendeng District Traditional Chinese and Western Medicine Hospital were 16.67% (1/6), 20.00% (3/15), 23.53% (4/17), and 33.33% (6/18) in the consecutive 4 years, respectively. The annual detection rates of early gastric cancer in opportunistic screening in Ru Shan Peoples Hospital were 14.74% (14/95), 23.80% (60/252), 25.49% (65/255), and 24.04% (50/208), respectively. The detection rates of opportunistic screening for early gastric cancer in hospitals in Weihai city increased year by year.Conclusion:In areas with high incidence of gastric cancer, a certain scale of group screening can lead to a wider range of opportunistic screening, resulting in the increase of the detection rate of early gastric cancer. The new model of diagnosis and treatment of gastric cancer is worth recommendation.
