1.The application of mechanical stimulation to the endometrium in the frozen thawed embryo transfer of hormone replacement therapy

Objective To analyze the mechanical stimulation to the endometrium on the clinical pregnancy rate and embryo implantation rate in the frozen thawed embryo transfer of hormone replacement therapy.Methods There were 193 patients undergoing the frozen thawed embryo transfer of hormone replacement therapy.According to the endometrial condition on the second or third day of the menstrual cycle,they were divided into three groups.Group A,54 patients whose endometrial thickness was no less than 0.6 cm or abnormal endometrium were operated the mechanical stimulation.Group B,55 patients whose endometrial thickness was no less than 0.6 cm or abnormal endometrial were not operated the mechanical stimulation.Group C,84 patients whose endometrial thickness was less than 0.6 cm and normal endometrium.Each group was divided into blastomere transplantation and blastula transplantation.Group A was 23,31 patients,group B was 24,31 patients,group C was 39,45 patients.The age,number of embryo,level of estradiol when injecting chorionic gonadotrophin,clinical pregnancy rate and embryo implantation rate were compared.Results (1) The clinical feature of blastula transplantation.The age,number of embryo,embryo implantation rate among three groups had no significant difference (P > 0.05).The clinical pregnancy rate in group A and group C was significantly higher than that in group B [73.91% (17/23),69.23% (27/39)vs.37.50%(9/24)] (P < 0.05),and there was no significant difference between group A and group C (P > 0.05).The level of estradiol when injecting chorionic gonadotrophin in group C was significantly higher than that in group B [(841.56 ± 802.73) μ g/L vs.(293.34 ± 69.14) μ g/L] (P < 0.05).There was no significant difference between group A and group B,group A and group C (P >0.05).(2)The clinical feature of blastomere transplantation.The age,number of embryo,level of estradiol when injecting chorionic gonadotrophin,embryo implantation rate and clinical pregnancy rate among three groups had no significant difference (P > 0.05).Conclusions Mechanical stimulation to the endometrium can significantly improve the clinical pregnancy rate of blastula transplantation.It has no improvement for embryo implantation rate and clinical pregnancy rate of blastomere transplantation.
2.Immunotherapy of biliary tract cancer

Biliary tract cancer (BTC)is one of the most aggressive malignancies with only 1 0% early-stage diagnosis rate.For advanced BTC,regimen of gemcitabine and cisplatin has been regarded as the first line standard combination,but the curative effect is still unsatisfied.Infiltration of immune cells and immune-related microenvironment can inhibit various types of cancers.In BTC,higher frequencies of tumor-infiltrating CD8 + cytotoxic T cells and CD4 + T cells are closely associated with favorable prognosis.These findings have provided the rationale for further development of immunotherapies as a novel treatment modality against BTC.
3.Protective effects of L-Arginine in the diabetic vascular dysfunction

Objective To explore the effects of L-Arginine on diabetic vascular endothelial function. Methods 60 patients with Diabetes were randomly divided into two group:placebo control group(30 patients)and L-Arginine group(30 patients). Patients in L-Arginine group were taken L-Arginine 7g a day. All patients were treated for 28days and other 7days for elution. The plasma concentration of nitric oxide(NO) and endothelin (ET), vWF were measured in each group before and after the experiment. Flow Mediated Dilation(FMD) function of brachial artery of all patients were measured by high resolution vascular ultrasound. Results After the experiment, the plasma concentration of NO in L-Arginine group was higher than that of placebo group, meanwhile the plasma concentration of ET and vWF were lower than placebo group (P < 0. 05). Moreover, FDM in L-Arginine group were significant higher than placebo group(P < 0. 05). Conclusion L-Arginine could protect the vascular function in the diabetic patients.

Objective To investigate the effects of ursolic acid on the expression of extracellular signal regulated kinase(ERK1),C-Jun,C-Myc,Cyclin D1 in transplanted tumor of malignant glioblastoma cell line C6 in nude mice and the related mechanisms.Methods C6 glioblastoma cells were subcutaneously injected into nude mice to establish the subcutaneous model of glioblastoma in nude mice,and then the mouse models were divided into 3 groups: blank control group,ursolic acid group(50 mg?kg-1?d-1,intra-abdominal injection,20 d),PD98059 group(2 mg?kg-1?d-1,intra-abdominal injection,7 d).Survival of nude mice,growth and histopathological changes of the tumors were observed.The expressions of ERK1,C-Jun,C-Myc,and Cyclin D1 in the tumor tissues and the expression of ERK1 mRNA in the tumor tissues were detected by immunohistochemical technique(IHC) and in suit hybridization(ISH),respectively.Results Slow growth of the implanted tumor was found in ursolic acid group and PD98059 group.The mean volume and weight of tumors in the two groups were significantly smaller than those in the blank control group(P

Objective To explore the distribution,fundamental diseases,risk factors and drug-resistance of hospital- acquired infection caused by fungi in Department of Respiratory Medicine.Methods The clinical data of 142 patients were retrospectively reviewed,and then with statistical analysis was performed.Results The main pathogen which caused fungal hospital-acquired infection was Candida albicans. The most three possible fundamental diseases were chronic obstructive pulmonary disease,bronchiectasis and lung cancer. The main risk factors were lower resistibility of patients and invasive diagnostic and therapeutic measure. The fungus resistance was gradually rising.Conclusion The hospital infection caused by fungus should be emphasized in respiratory medicine department. In order to effectively prevent the hospital infection in respiratory medicine department,we should improve the basic conditions of patients,rationally diagnose and treat,and rationally use the drug against fungi.
8.Effect of advanced glycosylation end products on expression of connective tissue growth factor in cultured rat aortic smooth muscle cells

Objective To explore the effect of advanced glycosylation end products (AGEs) on expression of connective tissue growth factor (CTGF) in cultured rat vascular smooth muscle cells (VSMCs) and the mechanism of accelerated atherosclerosis in diabetes. Methods VSMCs were isolated and cultured from the thoracic aorta of rat. Reverse transcription polymerase chain reaction (RT-PCR) and Western immunoblot analysis were used to detect the expression level of CTGF mRNA and protein. Results The expression of CTGF mRNA and protein were increased by AGE-BSA in the time- and dose-dependent manners(P<0.05). Conclusions AGEs may increase the expression activity of CTGF in cultured vascular smooth muscle cells.
9.Analysis on feasibility of extended hepatectomy for huge hepatic carcinoma

ObjectiveTo investigate the feasibility of extended hepatectomy for huge hepatic carcinoma.MethodsA retrospective analysis was made on the clinical data of 28 patients with huge hepatic carcinoma who underwent extended hepatectomy from December 2006 to June 2011.Twenty-two cases were Child-Pugh grade A,6 cases were Child-Pugh grade B,19 cases were indocyanine green retention at 15 minutes (ICGR15)≤10%,5 cases were 10% < ICGR15 ≤15%,4 cases were 15% < ICGR15 ≤20%.ResultsThe operative death rate was 7.1% (2/28) with Child-Pugh grade B,15% < ICGR15 ≤20% and liver resection volume 60%-70%.Postoperative complications were found on 15 cases,but all recovered after treatment.ConclusionExtended hepatectomy for huge hepatic carcinoma can be feasible in the patients with enough reserve of the liver function provided.
10.Contrast analysis of different laparoscopic surgical treatments for tubal pregnancy outcome and repregnancy

ObjectiveTo investigate the outcome and repregnancy after different laparoscopicsurgical treatments for tubal pregnancy,and analyse the influential factors. MethodsIn 56 tubal pregnancypatients,28 cases performed laparoscopic salpingostomy (group A) and 28 cases peoformed laparoscopicsalpingectomy (group B). The perioperative condition, the rate of repregnancy and re-ectopic pregnancy was compared and analyzed. Logistic regression analysis was used to detect the effect on subsequent repregnancyof influential factors such as pelvic adhesion. ResultsMore bleeding and longer operative time were needed in group A than group B, there were significant differences between two groups (P < 0.05 ). In follow-up of 6months to 6 yeats,the rate of repregnancy in group A and group B was 46.4%(13/28) and 32.1%(9/28)respectively,there was no significant difference between two groups (P >0.05). The rate of re-ectopic pregnancy in group A and group B was 10.7% (3/28) and 28.6% (8/28) respectively,there was significant difference between two groups (P <0.05). In the single factor analysis,the repregnancy in group A was significantly associated to pelvic adhesion and patency of the contralateral oviduct (P < 0.05 ). Conclusions The rate of repregnancy of laparoscopic salpingostomy is higher than laparoscopic salpingectomy for tubal pregnancy. Each of pelvic adhesion and the patency of the contralateral oviduct is a factor that affects the postoperative fertility. The conservation operation is not recommended for those patients with extensive pelvic adhesion or seriously destroyed tube but normal contralateral oviduct.
