1.Nursing of patients undergoing 64-slice CT urography

Objective To discuss the value of nursing work in 64-slice CT urography. Methods 240 patients participated in the 64-slice CT urography, inquiring medical history and iodine allergic history, paying attention to their psychological state and finishing iodine preliminary test before examination, ob-serving patients carefully, pre-judging any possible adverse effect and formulating corresponding measures during the examination, giving expectant treatment according to the various condition and nursing instruc-tion. Results 239 patients passed through the examination smoothly, agents exosmosis happened in one case and led to failure, but satisfactory image was obtained after rescanning. Conclusions Nursing oper-ation penetrates every step of 64-slice CT urography, which is a non-traumatic, low-expense and high-safety examination. Effect nursing directly influences the accuracy of the results, proficient nursing opera-tion and patient psychological nursing is the important part of it.
2.Clinical Observafion of Simvastatin Combining Ezetimibe for Treating the Patients of Acute Coronary Syndrome With Impaired Glucose Tolerance

Objective: To observe the inlfuence of simvastatin combining ezetimibe on blood levels of glucose and lipids in patients of acute coronary syndrome (ACS) with impaired glucose tolerance (IGT).
Methods: A total of 316 patients with ACS and IGT were randomly divided into 2 groups: Treatment group, the patients received simvastatin 20 mg/day with ezetimibe 10 mg/day,n=160 and Control group, the patients received simvastatin 20 mg/day,n=156. All patients were treated for 24 months. Blood levels of total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C) were observed at 6, 12 and 24 months of medication; fasting blood glucose (FBG), 2 hour postprandial blood glucose (2hPG), glycosylated hemoglobin (HbA1c) and the number of patients with new onset diabetes were examined at 6 days and 12, 24 months of medication.
Results:①In Treatment group, at 6 months of medication, blood levels of TC (mmol/L) and LDL-C (mmol/L) were (3.5 ± 0.5) and (2.1 ± 0.4) which were both lower than baseline levels (5.2±1.2) and (3.5±0.5),P<0.05; at 12 and 24 months, TC and LDL-C were (3.1 ± 1.0), (1.8 ± 0.6) and (3.0 ± 0.6), (1.8 ± 0.5), TC and LDL-C were consistently decreased, all P<0.05. In Control group, at 6 months of medication, TC and LDL-C were (4.0 ± 0.5) and (2.4 ± 0.5) which were both lower than baseline levels (5.3 ± 0.8) and (3.1 ± 0.4),P<0.05; at 12 and 24 months, TC and LDL-C were (3.8 ± 0.6), (2.3 ± 0.6) and (3.7 ± 0.5), (2.1 ± 0.7), allP<0.05. TC and LDL-C levels in Treatment group were lower than Control group at 6, 12 and 24 months of medication, allP<0.05.②In Treatment group, FBG (mmol/L) levels at 12 and 24 months were similar to 6 days,P>0.05; HbA1c at 12 and 24 months were similar to baseline,P>0.05; 2hPG (mmol/L) level at 24 months was higher than 6 days (9.5 ± 1.1) vs (8.7 ± 1.0),P<0.05; there were 26 patients with new onset diabetes at 24 months after medication. In Control group, 2hPG level at 24 months was higher than 6 days (9.6 ± 0.8) vs (8.7±0.7); there were 25 patients with new onset diabetes at 24 months after medication. The levels of FBG, 2hPG, HbA1c and the number of patients with new onset diabetes were similar between 2 groups, allP>0.05.
Conclusion: Ezetimibe combining simvastatin may better reduce blood lipids, while it had no real effect on blood glucose metabolism in patients with ACS and IGT.
3.Change of endothelial progenitor cells in the bone marrow and peripheral blood of patients with acute leukemia and its clinical significance

Objectives To evaluate the count of endothelial progenitor cells (EPCs) in peripheral blood (PB) and bone marrow (BM) of acute leukemia (AL) patients and explore its clinical significance.Methods EPCs were detected by flow cytometry procedures in 43 AL patients and in 10 benign hematologic patients as control group.Results The absolute counts of EPCs in AL patients before the treatment [(119.46± 72.23)/μl in BM and (13.69±8.26)/pl in PB] were significantly higher than those in control group [(23.21 ± 12.59)/pl in BM and (1.86±1.18)/μl in PB] (P < 0.01).The absolute counts of EPCs were significandy higher in BM than those in BP in AL patients before the treatment (P < 0.001).After the treatment, the absolute counts of EPCs in no remission (NR) group [(110.02±67.28)/μl in BM and (10.04±9.51)/μ1 in PB] were significantly higher than those in control group (P < 0.05), while the counts of EPCs in complete remission (CR) group were no significant difference compared with those in control group (P > 0.05).After the treatment ,the absolute counts of EPCs both in BM and in BP of CR group [(26.32±17.44)/μl and (2.54±2.12)/μl, respectively] were significantly lower than those before treatment [(113.18±69.22)/μl and (14.45±10.76)/μl, respectively] (P < 0.05), however those of NR group were no significant difference than before (P > 0.05).The absolute counts of EPCs whether in PB or in BM were no significant difference between acute myeloid leukemia (AML) and acute lymphoid leukemia (ALL) (P > 0.05).The absolute counts of EPCs in PB of AL had a positive correlation with β2-MG and LDH (P < 0.05).Conclusions EPC levels are significantly increased in BM and BP of AL patients and may correlate with disease status, response to treatment and prognosis.
4.Primary effects of comprehensive lifestyle modification on status of pre-chronic diseases

Objective To evaluate the effectiveness of comprehensive lifestyle modification in the primary health care system on status of pre-chronic diseases in a conventional healthcare program.Methods Two hundred and twenty-nine adult volunteers in 5 conventional healthcare centers were divided into 4 groups:group of prehypertension(n=67),group of prediabetes(n=38),group of abnormity serum cholesterol(n=71)and group of osteopenia(n=53).A muhicomponent behavioral intervention including education,physical activity,dietary practice,cease smoke,calcium supplement and exposure to sunlight was conducted for 12 months.The main outcome of each group was blood pressure,blood glucose,serum cholesterol and bone mineral density.Results Over 12 months,compared with the status before intervention,participants in each group were statistically significantly improved,the mean net reduction in systolic BP/diastolic BP was 9.4 mm Hg/6.6 mm Hg(1 mm Hg=0.133 kPa,t=5.93/8.29.P<0.05)in group of prehypertension;the reductions of blood glucose,serum cholesterol were 0.9 mmol/L(t=3.69,P<0.05)and 0.5 mmol/L(t=4.09,P<0.05)in group of prediabetes and group of abnormity serum cholesterol,and the increase of bone mineral density on lumbar spine was 0.015 g/cm2(t=2.03,P<0.05)in group of osteopenia,Conclusions For chronic disease prevention,ater one year the comprehensive lifestyle intervention produced beneficial changes in diet,physical activity,and biochemical parameters.This type of intervention is a feasible option to prevent chronic disease and should be implemented in the primary health care system.
5.Application of Serum VEGF-C, VEGFR-2 and VEGFR-3 Expression Level in Monitoring Curative Effects and Prognosis of Acute Leukemia

Objective To observe expression level of serum vascular endothelial growth factor-C (VEGF-C),VEGF-C receptor-2 and VEGF-C receptor-3 in patients with acute leukemia (AL) and to explore its clinical significance.Methods Enzyme-linked immuno sorbent assay (ELISA) was used to detect the serum expression levels of VEGF-C,VEGFR-2,and VEGFR-3 of 51 patients diagnosed with acute leukemia,43 patients under medical treatment and 16 healthy blood donors.Results (1) Serum VEGF-C,VEGFR-2,and VEGFR-3 expression levels in AL patients were significantly higher than those in normal control group.(2) Serum VEGF-C and VEGFR-2 expression levels in complete remission (CR) group significantly declined after treatment.Serum VEGF-C and VEGFR-2 expression levels in non-complete remission (NR) group slightly declined after treatment but no significant difference was found (P>0.05).(3) No significant difference was found in serum VEGFR-3 expression levels both in CR group and NR group after treatment (P>0.05).(4) Serum VEGF-C,VEGFR-2,and VEGFR-3 expression levels in NR group were significantly higher than those in CR group before treatment (P<0.08).Conclusions Observing serum expression level of VEGF-C,VEGFR-2,and VEGFR-3 of AL patients may be helpful in monitoring curative effects and prognosis of acute leukemia.
6.Expression and Significance of Telomerase Activity in Cervical Cancer

Objective: Our aim was to study the role of telomerase activation in the course of cervical carcinogenesis and progression.Methods:Telomeric repeat amplification protocol(TRAP) assay was used to measure telomerase activity in tissue samples with various cervical conditions:40 with cervical cancer, 50 with cervical intraepithelial neoplasia(CIN), 20 with normal cervice. Results:The positive rate of telomerase activity was 95.0%,44.0%, and 10.0% in cervical cancer, CIN, and normal cervices, respectively, which was significantly higher in cervical cancer than that in CIN and normal cervices, so was that in CIN than that in normal cervices (P<0.01) . The positive rate was 22.2%, 37.5%, and 75.0% in CINⅠ,Ⅱ, and Ⅲ, respectively, which was significantly higher in CINⅢ than that in CIN Ⅱand CINⅠ (P<0.01).Conclusion:Telomerase activation may relate to cervical carcinogenesis, which correlates well with the grade of cervical lesions.
7.Safety of cleaning and disinfection of duodenoscope for endoscopic retrograde cholangiopancreatography

Objective To investigate the safety of cleaning and disinfection of duodenoscope used in endoscopic retrograde cholangiopancreatography (ERCP).Methods The duodenoscopes,which were used in Endoscopy Center of Digestive Disease Hospital,Heilongjiang Provincial Hospital from May 4 to May 31 2016,were cleaned manually and disinfected with sterilization powder.The samples from elevator and elevator channel were collected for bacterial culture to evaluate the safety of clinical application of duodenoscope.Results Among 60 high level disinfections,one was excluded because disinfectant concentration wasn't effective.Among 59 elevator samples no positive cultures were detected,while among 59 channel samples,4(6.8%) cultures were positive and all from the same duodenoscope,including 3 cases of Klebsiella pneumonia and 1 case of Pseudomonas aeruginosa.Conclusion Sterilization powder is effective for disinfection of duodenoscope used for ERCP.Residual organic material can't be excluded from elevator sample even if it is negative for culture.Duodenoscope with high level disinfection still has the risk of transmitted infection,and should be monitored regularly by culture and reprocessed in time.
8.Application of ovarian transposition during hysterectomy.

OBJECTIVETo study the optimal position and method for ovarian transposition and its benefits and indications.
METHODSWe performed ovarian transposition in 34 patients from August 1989 to December 2000. Twelve patients were diagnosed with stage Ib to IIa cervical cancer, 4 had stage Ia endometrial carcinoma, 12 had stage III to IV endometriosis, 4 had myoma of uterus, 1 had dysfunctional uterine bleeding, and 1 had an ovarian granulosa cell tumor. Surgery went as follows: the ovary was dissociated by clamp, the skin was incised and a tunnel was made, then the ovary was translocated to the subcutaneous site. In the cases of benign lesions, the ovarian vessel pedicel went in through the abdominal cavity, but in malignant tumors, it went out through the peritoneum.
RESULTSIn both cases (benign lesions or malignant tumors), the short-term and long-term endocrine function of the translocated ovary remained normal. Furthermore, patients could supervise their translocated ovary themselves.
CONCLUSIONSSubcutaneous ovary transposition might prevent not only implantation of gastrointestinal cancer but also the extension of pelvic carcinoma to the ovary. Because of the shallow transposition and the incision scar, it is easy for patients to supervise themselves. Moreover, the site of the ovary is easy to locate for ultrasound examinations. Thus, it can obtain the goal of early prevention for cancer. Subcutaneous ovarian transposition with skin incision is the optimal selection and suitable for all patients with various gynecologic diseases in which ovary removal is not necessary.

9.Clinical application of serum follicular statin 1 in the diagnosis of chronic heart failure

Objective To study the application value of serum follicular statin 1(FSTL-1) in the diagnosis of chronic heart fail-ure(CHF) .Methods A total of 60 CHF children treated from January 2013 to January 2016 in hospital were selected as observa-tion group .According to New York Heart Association (HYHA) ,the subjects were divided into Type Ⅱ group ,Type Ⅲ group and Type Ⅳ group ,20 cases in each group .In addition ,60 healthy children were selected as control group .FSTL-1 ,NT-proBNP and LVMI were detected and analyzed .Results FSTL-1 ,NT-proBNP and LVMI in Type Ⅱ group ,Type Ⅲ group and Type Ⅳ group were higher than those of control group(P< 0 .05) .As the grade of cardiac function increasing ,FSTL-1 ,NT-proBNP and LVMI were higher(P<0 .05) .FSTL-1 had positive correlations with NT-proBNP and LVMI .The positive rate of FSTL-1 for diagnosis CHF was 96 .67% ,which had no significant difference with gold standard (P>0 .05) .Conclusion With the progress of illness ,the level of FSTL-1 increases ,which is importance in the diagnosis and monitoring of CHF .It is worthy of further application and pro-motion in clinical practices .
10. Study of nursing guidance on improving the radiation protection effect of accompanying examiners in CT examination

Objective:
To survey the current situation of wearing protective articles by accompanying examiners in the nursing conditions of intervention and non-intervention during CT diagnosis in a tertiary A hospital.
Methods:
A control group and an intervention group were set up to investigate the situation of wearing protective devices. Attempt was made to use mobile lead screens in place of personal protective devices and their effects were investigated.
Results:
A total of 4 890 unavoidable accompanying examiners wearing protective equipment during CT examination were investigated. After nursing guidance, the wearing rate increased from 73% to 94%, and the complete wearing rate increased from 19% to 81%. Refusal to wear protective devices was mostly due to limited time. Refusal rate of emergency accompanying examiners was significantly higher than that of outpatient and inpatient accompanying examiners. After using mobile lead screen, the probability of personnel protection increased to 99%.
Conclusions
Nursing intervention can effectively improve the wearing rate and complete wearing rate of protective articles for accompanying examiners. Moving glass lead screen is conducive to the improvement of protection level for accompanying examiners..
