1.Detection of Serum Beta endorphin and its Clinical Significance in Patients with Psoriasis

In order to study the possible role of beta endorphin (? EP) in the pathogenesis of psoriasis, serum ? EP was detected by radioimmunoassay in 51 patients with psoriasis and 32 healthy controls. The results showed that serum ? EP level was markedly increased in patients with psoriasis in comparison with healthy controls. The serum ? EP level was reduced when the skin lesions of the patients were cleared after treatment, however, it was still higher than in healthy controls. Although there was a tendency towards an increase of ? EP in patients with precipitating factors in comparison with those without precipitating factors, their levels were not significantly different. There was a negative correlation between the intensity of pruritus and ? EP levels. It is suggested that ? EP involves in the pathogenisis of psoriasis. Its production and release were controlled by the central nervous system and also affected by the inflammation of skin lesions. There might be a causality between ? EP levels and inflammatory reaction of skin lesions.
2.Prediction of the risk of coronary arterial lesions in Kawasaki disease by N-terminal pro-brain natriuretic peptide.

OBJECTIVETo detect plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) in acute Kawasaki disease (KD) and analyze the relationship between NT-proBNP and other bio-markers in order to evaluate if NT-proBNP could be as a useful diagnostic marker to predict the risk of coronary arterial lesions in acute KD.
METHODTotally 106 patients with KD were recruited from January 2012 to April 2014 at Department of Pediatrics of Tongji Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology,64 were boys and 42 were girls, their age ranged from 2 months to 8 years and 4 months. Of the 106 cases, 48 had typical KD(TKD) and 58 incomplete KD(IKD). They were divided into two groups according to echocardiography results: coronary arterial lesions (KD-CAL, n = 33) and non coronary arterial lesions (KD-nCAL, n = 73). Forty children whose age and gender matched with respiratory tract infection were selected as control group, 22 were boys and 18 were girls, age range from 7 months to 7 years and 11 months. Plasma NT-proBNP levels were measured by using the enzyme-linked fluorescence analysis (ELFA) at the day of admission, meanwhile blood routine tests, liver function tests, determination of C-reactive protein (CEP), erythrocyte sedimentation rate (ESR), electrolytes were performed in these patients. Pearson's correlation analysis was used to evaluate the association. The ROC curve analysis was done to identify the threshold of coronary 'arterial lesions.
RESULTThe levels of NT-proBNP were (1 037 271) ng/L in TKD group and (1,325 ± 264) ng/L in IKD group. The levels of NT-proBNP in control group was (125 ± 22) ng/L. Both the levels of NT-proBNP in TKD and IKD group were significantly higher than that of control group (t = 3.360, 3.590; P < 0.05). The level of NT-proBNP in KD-CAL group was (2,775 ± 842) ng/L and that of KD-nCAL group was (830 ± 145) ng/L, NT-proBNP levels of KD-nCAL group was significantly higher than that of control group (t = 3.660, P = 0.007) ; moreover the level of NT-proBNP of KD-CAL group was also significantly higher than that of KD-nCAL group ( t = 3.860, P = 0.005). The levels of total white blood cell count, neutrophil percentage, platelet count, CRP and ESR of KD-CAL group were significantly higher than those of the control group, however there was no significant difference between KD-CAL group and KD-nCAL group. The levels of albumin and Na of KD-nCAL group were significantly lower than those of the control group. Plasma NT-proBNP level in KD group was positively correlated with white blood cell count, neutrophil percentage, and CRP (r = 0.239, P = 0.025; r = 0.359, P = 0.001; r = 0.474, P = 0.001), there was a negative correlation between albumin and Na (r = -0.303, P = 0.015; r = -0.338, P = 0.002). When the level of NT-proBNP was higher than 950 ng/L, the sensitivity for diagnosis of coronary arterial lesions in the KD was 88.1% and the specificity was 89.0%.
CONCLUSIONThe plasma NT-proBNP can be used as a useful parameter in early diagnosis of KD. Plasma NT-proBNP could be used to predict the risk of coronary arterial lesions in KD.

3.A RCT study on oral lidocaine plasmagel on improvement of the postoperative odynophagia in patients receiving trans-oesophageal echocardiography examination for cardiac surgery

Objective To investigate the effect of oral lidocaine plasmagel on the postoperative odynophagia in patients receiving trans-oesophageal echocardiography (TEE) examination for cardiac surgery.Methods Sixty patients for cardiac surgery were divided randomly into the control group (C group) and the oral lidocaine plasmagel group (L group) with 30 cases in each group.Patients in L group received 2% oral lidocaine plasmagel 20 ml before anesthesia,while C group did not.The incidence of postoperative odynophagia was compared between two groups.Results Postoperative odynophagia pain score were significantly higher in C Group than that in L Group.Conclusions Oral lidocaine plasmagel can alleviate the postoperative odynophagia in patients receiving trans-oesophageal echocardiography examination for cardiac surgery.
4.AN OBSERVATION ON THE CLINICAL EFFECT OF NAOMAISHU

Based on the function of nourishing blood and activating blood circulation,Naornaishu(NMS)composed of Rhizome Chuanxiong,Radix Salvice Miltiorrhizae,etc.has been used to treat cerebral arteriosclerosis and infarction.65 Patients with cerebral arteriosclerosis were treated with NMS for about two months and got a total effective rate of 98.5%.50 cases with eerebral infarction were also observed.It was shown that its effect on impro- ving myodynamies,alleviating aphasia,relieving dizziness and turning patholo- gic reflex negative was 86.4%,76.9%,100% and 38.0% respectively.No side effects were obser(?)ed during the period of more than two years of clinical applications.Therefore,NMS is regarded as an ideal medicine for preventing and treating cerebrovascular diseases.
5.Clinical effect study of magensium sulfate on the intestine preparation before colonoscopy examination

Objective We discussed the optimal method for intestinal preparation before colonoscopy examination in order to increase the success rate of intestinal examination and reduce the incidence of adverse effect. Methods We divided 120 patients who were to undergo colonoscopy examination into group A, B and C with 40 cases in each group. Group A received oral magensium sulfate 100ml and 6000ml water after that on the morning of the examination. Group B received oral magensium sulfate 50ml and 2000ml in the evening before the examination, oral magensium sulfate 50ml and 400ml water on the morning of the examination. Group C was given oral magensium sulfate 100ml and 4000~5000 water at the same time. Group A and B took part in appropriate activity and were given health education. The effect of intestinal preparation was compared between the three groups. Results The cleaning degree of group B was better than those of the other two groups (P<0.05). The adverse effect of group B was less than that of group C (P<0.01). Conclusions Oral intake of magensium sulfate in the evening before and on the morning of the examination for intestinal preparation could increase the cleaning degree of intestine, facilitate the observation of disease part and lessen adverse effect. It gave satisfying examination results and made patients satisfied.
6.A Descriptive Study on Parental Rearing Behavior and Personality Disorder

Objective: To explore the relationship between parental rearing behavior and personality disorder. Method: Descriptive study was carried out in one key-university in Beijing. Two thousand two hundred and five freshmen were investigated with Personality Diagnostic Questionnaire-Revised (PDQ-R) and EMBU scale. Fifty-five personality disorder cases were diagnosed by International Personality Disorder Examination (IPDE). Results: The scores of emotional warmth, over-protection, and favoring subject of the maternal were higher than those of the paternal. Parental scores of EMBU factors were highly correlative. The scores of rejection, overprotection, favoring subject in personality disorder cases were significantly higher than those in control group, while the score of emotional warmth was significantly lower. Conclusion: Inappropriate parental rearing behavior is a risk factor of personality disorder.
7.EFFECT OF FERMENTATION CONDITIONS ON ANTIBIOTIC PRODUCTION OF XENORHABDUS NEMATOPHILUS

The optimum medium and fermentation conditions of the Xenorhabdus nematophilus from Steinernema carpocapsae BJ strain were studied. The relationship between antibiotic activity and pH, reducing sugar, total sugar, amino-nitrogen in process of fermentation was analyzed. The optimal medium contained tryptonl. 5 %, corn powderl o4, soybean flour 3 %, sucrose1 %KH2PO4 0.02 % ,MgSO4 0. 2% and activator 0. 1%, Stock cultured for 16h, inoculum size at 4%(V/V)and primary pH of medium ranged from 6.0 to 8.0, fermentation for 72h were of benefit to the yield of antibiotic. The pH, reducing sugar, total sugar and amino nitrogen in process of fermentation were related to the antibiotic activity. The yield of antibiotic increased by 56. 3 % comparison with nutrient broth.
8.Effect of miR-19a on lipid catabolism in hepatocyte LO2

[ ABSTRACT] AIM: To observe the effect of microRNA-19a ( miR-19a) on the lipid catabolism of hepatocyte LO2, and to explore the potential mechanism.METHODS: miR-19a was over-expressed or silenced by transfection of miR-19a mimics or miR-19a inhibitor into LO2 cells, then the mRNA level of miR-19a was detected by real-time PCR.The potential target of miR-19a was found by the method of bioinformatics through internet website.The effect of miR-19a on the 3’ UTR of peroxisome proliferator-activated receptorα(PPARα) was measured by dual luciferase reporter assay, and the protein level of PPARαand its 2 major downstream rate-limiting enzymes involved in lipid catabolism, acyl-coenzyme a dehydrogenase (ACADM) and carnitine palmitoyltransferase 1A ( CPT1A), were detected by Western blotting.Mean-while, the effect of miR-19a on the generation of ketone body was measured by beta-hydroxybutyric acid (β-OHB) detec-tion assay.RESULTS:The mRNA level of miR-19a was dramatically elevated by the transfection of miR-19a mimics, and sharply decreased by the transfection of miR-19a inhibitor (P<0.05).PPARαwas found as a potential target of miR-19a, and dual luciferase reporter assay and Western blotting confirmed the regulatory effect of miR-19a on the expression of PPARα, with the protein level changes of ACADM and CPT1A.miR-19a mimics down-regulated, while miR-19a inhibitor up-regulated the concentration ofβ-OHB in LO2 cells (P<0.05).CONCLUSION:miR-19a regulates the lipid catabo-lism of hepatocytes by targeting the PPARαand its 2 downstream rate-limiting enzymes.

10.Clinical analysis of 120 cases of infective endocarditis

Objective To improve the diagnosis and treatment of infective endocarditis(IE)by exploring its causes,pathogenic microorganism and clinicsI characteristics.Methods The clinical data of 120 IE patients treated in Peking Union Medical College Hospital from October 1997 to September 2007 were analyzed retrospectively.Results Of the 120 consecutive cascs diagnosed as IE according to the Duke's new criteria,79 were male and 41 female with a average age of(43.2±16.7)years old.Twelve cases were prosthetic valve endocarditis(PVE)and 108 cases native valve endocarditis(NVE)and there were no previously known heart diseases in 29 of the cases.Seventy-nine of the 108(73.1%)NVE patients had basic cardiac abnormalifies before IE diagnosis,such as congenital cardiovascular disease(30 cases),idiopathic mitral valve prolapse(23 cases)and rheumatic heart disease(11 cases).Fever(100.0%),anemia(54.2%)and embolism(48.3%)were the most common clinical manifestations in the IE development.Of the 83 patients who had a positive blood culture result,Streptococcus vividaus(51.8%)was the most common isolated microorganism.Conclusions Congenital cardiovascular diseases and idiopathic mitral valve prolapse are the two most commonly heart diseases in IE.Blood culture and echocardiogram should always be done to exclude IE,especially presenting with fever of unknown reasons.
