1.Sequential chemotherapy and conventional radiotherapy in treatment of 45 cases of brain metastases of lung cancer

Purpose:To evaluate the efficacy and toxic and side effects of brain metastases of lung cancer treated with chemotherapy and conventional radiotherapy. Methods:45 cases of brain metastases of lung cancer were treated with sequential chemotherapy of VM26DDP regimens and conventional radiotherapy. The complete remission and partial remission rate ,1-,2-,3-year survival rate and toxic and side effects were observed.Results:The complete remission and partial remission rate of salitary as well as multiple brain metastases of non-small-cell lung cancer and small-cell lung cancer were (92.86%) and 80.94%,75% and 83.33%, respectively . The 1-,2-,3-year survival rate of salitary and multiple brain metastases of non-small-cell lung cancer was 13.33% and 14.29%、9.5% and 4.76%、9.5% and 0. The 1-,2-,3-year survival rate of salitary and multiple brain metastases of small-cell lung cancer were 50% and 50%、25% and 33.33%、25% and 16.67%, respectively. The Ⅰ,Ⅱ,Ⅲ and Ⅳ degree myelosuppression was 33.33%,28.89%,17.78%,8.89%, respectively.Conclusions:The efficacy of brain metastases of lung cancer treated with sequential chemotherapy by VM26+DDP regimens and conventional radiotherapy is better and with less toxic and side effects.
2.Diet intervention according to Trans-theoretic Model in young and middle-aged patients with asymptomatic hyperuricemia

Objective To study intervention effect of diet habits according to Trans-theoretic Model (TTM) in young and middle-aged patients with asymptomatic hyperuricemia. Methods 120 patients were divided into the intervention group (n=60) and the control group (n=60). The nursing intervention by stages of TTM was implemented to a total of 56 patients in the intervention group in the study. Routine nursing care was taken to 57 patients in the control group. Patients were evaluated by self-designed questionnaire after 1, 3 and 6 months of the intervention in the two groups respectively, including the diets of patients, the change of patients′ eating habit, differences of uric acid, total cholesterol, triglycerides, blood sugar. Results The patients′ intake of beer, seafood, meat, edible oil in the intervention group was lower than the patients′ intake in the control group, while the patients′ intake of fruits and vegetables was higher than the patients′intake in the control group after 1, 3 and 6 months of the intervention, which was statistically significant ( P < 0.01). Compared with 27 cases in the control group, 53 cases in the intervention group were in action phase and maintenance phase after 6-month intervention, the difference was statistically significant (P < 0.01). After six months′intervention, the patients′uric acid, triglycerides in the intervention group was (418.01 ± 24.15)μmol/L and (1.52 ± 0.56) mmol/ L respectively, which was significantly lower than (435.02 ± 26.35) μmol/ L and (2.45 ± 0.74) mmol/L in the control group, t values were-3.47 and 2.20 respectively, P<0.01. Conclusions Diet intervention based on the TTM can effectively promote the change of patients′ unhealthy eating habits, improve self-management skills, and reduce uric acid, which is worthy of generalizing.
3.Nonfermenters Bacteria Infection in Lung Cancer Patients

OBJECTIVE To investigate the bacteriologic feature and the spectrum of drug resistance in lung cancer patients who got nosocomial nonfermenters bacteria infection,and to conclude the experience and how to prevent and control the nosocomial infection.METHODS A rectrospective analysis of the composition of the pathogens,the feature of drug resistance and the prognosis of 109 lung cancer inpatients with nosocomial infection was made.RESULTS Nosocomial nonfermenters bacteria infections were most happened in hospital lung cancer patients.Main strains included Pseudomonas aeruginosa 55(49.5%),Acinetobacter baumannii 23(20.7%),and Stenotrophomonas maltophilia 8(7.2%).The sensitive rate of P.aeruginosa to imipenem was 100%,but to Cotrimoxazole,the resistant rate was 100%.The sensitive rate was relatively high to ticarcillin,piperacillin and their compounds.The antimicrobial resistance rate of A.baumannii was higher than P.aeruginosa.Stenotrophomonas were resistant to most antibiotics,and the resistance rate to imipenem was 100.0%.CONCLUSIONS The resistance rate of nosocomial nonfermenters bacteria infection is high.The treatment is difficult and the prognosis is bad.The mortality of multiple infection is high.
4.Angular dose dependency of MatriXX and its calibration

Objective To characterize angular dependency of MatfiXX and develop a method for its calibration in order to verify treatment plan with original gantry angles.Methods Absolute dose calibration was carried with thimble ionization chamber on the linear accelerator.so as to make sure 1 MU=1 cGy at the depth of maximum dose(dmax).A MatriXX was put into a Mutlicube phantom,and the ionization chamber matrix was calibrated with absolute dose.In order to determine a correction factor CF as a function of gantry angle θ.open beam fields of 10 cm×10 cm size were irradiated for gantry angles θ=0°-180°(every 5°)and every 1°for lateral angles θ in the range of 85°-95°.CF was defined as the ratio of the dose measured with ionization chamber and the dose from MatriXX.Results Relatively large discrepancies in response to posterior VS.anterior fields for MatriXX detectors(up to 10%)were found during the experiment and relatively large variability of response as a function of gantry angle.The pass rate of treatment plan in lateral beams was lower than that of other beams.The isodose distribution of corrected MatriXX matched well with the outcome from the treatment planning system. Conclusions The angular dose dependency of MatriXX must be considered when it is used to verify the treatment plan with original gantry angles.
5.Cloning of human tissue-type plasminogen activator(t-PA) cDNA, construction of its ad-enovirus vector and its expression in small-diameter vascular anastomotic sites in vivo

Objectives:To study the effects of gene therapy with tissue type plasminogen activator(t PA)cDNA on the formation of thrombo embolism in vascular anastomotic sites. Methods:①The cDNA encoding t PA was amplified by RT PCR using the isolated total RNA as the template from the Bowes melanoma cells.②Recombinant plasmid pAdCMV t PA was cotransfected into 293 cells with pJMa 17 ,and the infectious but replication deficient AdCMV t PA was generated.③The rats were randomly divided into the control and treatment groups.11 0 nylone medical suture was applied to perform rat carotid artery end to end anastomoses.In the treatment group,AdCMV t PA solution was injected into the vascular anastomotic site while AdCMV (no containing t PA DNA) solution was injected into the control group. By means of RT PCR and chromogenic plasmin substrates,the following results were obtained. Results:①The t PA cDNA was successfully cloned and its eukaryotic expressing vector was constructed.②When the isolated RNA was performed with RT PCR,1.69 kb band appeared in the treatment group while the band could not be found in the control group.The t PA activity could be detected postoperatively on the 1st,2 nd,3 rd,4 th,5 th,6 th,7 th,10 th and 13 th day of the treatment,but could not be detected in the control group. Conclusions:The t PA gene can produce t PA having biological activity at anastomotic sites, possibly prevent the formation of thrombus embolism effectively and develop the anastomotic patency.
6.Evaluation of atlas - based autosegmentation with ABAS software for head - and - neck cancer

Objective To evaluate the autocontouring accuracy using the atlas-based autosegmentation of CT images for head-and-neck cancer.Methods Ten head and neck patients with contours were selected.Two groups of autocontouring atlas were tested,the first group was for patients with own atlas,for the second group we tested the autocontouring of eight patients with other two patients atlas.Dice similarity coefficient (DSC) and overlap index (OI) were introduced to evaluate the autocontours,and the discrepancy between the two groups was evaluated through paired t-test.Results Both the DSC and OIof all the organs in the first group were >0.80,the result of mandible was the highest ( >0.91 ),the DSC of the gross tumor volume (GTV) was the lowest (0.81 ),the OI of the GTV was 0.82,and the DSC and OI of the clinical target volume (node) were 0.82 and 0,79,respectively.Only the risk organ was delineated in the second group,and spinal cord and brain stem were combined to analyze.All the DSC was about 0.70,and the DSC and OI of mandible were higher than the others,which was due to its bone anatomy.The accuracy in the second group was significantly lower than that of the first group ( t =3.24 - 8.26,P =0.014 -0.000),except the right parotid (t=2.08,P=0.075).Conclusions Automatic segmentation generates contours of sufficient accuracy for adaptive planning intensity-modulated radiotherapy (IMRT) to accommodate anatomic changes during treatment.For convention planning IMRT normal structure auto-contouring,it need to select more standard atlas in order to acquire a satisfied autocontours.
7.Primary renal lymphoma of mucosa-associated lymphoid tissue: report of 2 cases and review of literatures

Background and purpose: Primary renal marginal zone B-cell lymphomas of mucosa-associated lymphoid tissue(MALT lymphoma) are rare tumors in the kidney.Till now,less than 50 cases have been reported all over the world.This article introduced 2 cases of MALT lymphoma based on the morphological and immunophenotype features,aimed to highlight this rare tumor in terms of clinical characteristics and pathology.Methods:2 cases of renal MALT lymphomas were treated in our hospital.Clinical history were collected,morphological analysis was evaluated based on the HE section,and immunohistochemistry were performed by CD20,CD79a,CD5,CD10,CD43,CD23,BCL10 and cyclinD1 antibodies.Results:2 female patients with the age of 48 and 55,respectively,both of them had a history of chronic pyelonephritis.Under the B ultrasonic and CT scanning,a lump in the kidney was found and whole nephrectomy was performed.In the macroscopic view,tumors were in the renal medulla with dark red color and ill-defined boundary.In the microscopic view,lymphoid cells infiltrate from the pelvis to the medulla of kidney,lymphoid cells mainly consisted of small lymphocytes,centrocyte-like cells,lymphoplasmacytoid and plasma cells.Reactive follicles and lymphoepithelial lesions also could be seen in the tumor.Immunohistochemistry showed that the tumor cells were CD20 and CD79a positive,CD43 was weak positive,but CD5,CD10,CD23,BCL10 and cyclinD1 were all negative.Conclusions:Primary renal MALT lymphomas are very rare diseases.According to the clinical manifestation,it is hard to differentiate from renal cell carcinoma.But the morphological features were consistent with the classic MALT lymphomas in other sites.Immunophenotype profiles were helpful for diagnosis.
9.The positioning accuracy study of the cone-beam computed tomography in combination with the sixdegree couch table

Objective To scrutinize the positioning accuracy and reproducibility of the cone-beam computed tomography system in combination with the six-degree couch table (Hexapod Robot Treatment Table, HRTT). Methods The mechanical stability of the X-ray volume imaging (XVI) system was tested,in terms of the reproducibility. And the influence of the moveable parts, including the KV panel and the source arm, on the accuracy of the XVI image registration was analyzed. The accuracy between the bone and grey value registration was compared using a head-and-neck phantom. The accuracy of the HRTT for translational, rotational, and a combination of translational and rotational corrections was investigated in consecutive measurements. Results The performance of XVI system itself was stable with translational and rotational error of below 0. 4 mm and below 0. 3°, respectively. The mean position accuracy of the XVI system in combination with the HRTT summarized over all measurements was below 0. 6 mm and below 0. 4° for translational and rotational corrections, respectively. The grey value match was more accurate than the bone match. Conclusions The XVI image acquisition and registration procedure were highly reproducible.Both translational and rotational positioning errors can be corrected very precisely with the HRTT. The HRTT is therefore well suited to complement CBCT to take full advantage of position correction in six degrees of freedom for image guided radiotherapy.
10.99mTc-MDP Bone Scintigraphy for Study on Repair of Femoral Eefect Caused by Microwave-induced Hyperthermia in Dogs

Objective To investigated the biological procedure of allograft decalcified bone matrix(DBM)and bone cement(BC)combined with bovine bone morphogenetic protein (bBMP)used for the repair of femoral defect caused by microwave- induced hyperthermia in dogsby 99mTc- MDP bone scintigraphy.Method The canine femoral defect(length 25mm,width 10mm)was caused by microwave- induced hyperthermia(50℃ ,20minutes)and the composite material was implanted .Then the canine femurs were examined by 99mTc- MDP bone scintigraphy respectively at different postoperative time and the results were compared with that of X- ray photography and histological observation.Bone cement was implanted in the other femur as a contrast.Results It could be observed at the first and the second month that the radioisotope was gathered in the place where the composite material was implanted and the amount of radioisotope gathered in was the most abundant at the third month and it was lasted to the fourth month. That of the sixth month was decreased to that of the second month.The radiation count of the first, the second, the third the fourth and the sixth month were 93.9± 12.7, 110.7± 16.4,222.1± 24.0,201.3± 26.9 and 111.6± 20.7 respectively,and the count of the third month and the fourth month were more than that of the first, the second and the sixth month(P<0.01).Conclusion The composite material could be remodeled easily and the new bone could be formed by the induction of bBMP. So it could be merged with the normal bone.While the 99mTc- MDP bone scintigraphy is the object and reliable index to determine the biological procedure of the composite material in dogs.
