1.A clinical study on reconstruction and suture repair medial patellofemoral ligament in the treatment of patellar dislocation

Objective To explore the operation effect of isometric reconstruction of the medial patellofemoral ligament with the semitendinosus tendon and direct suture rehabilitation of the medial patellofemoral ligament in the treatment of patellar dislocation,and provide some reference for the surgical treatment of this disease.Methods Forty patients with patellar dislocation included 20 patients with suture repair (suture group) and 20 patients with reconstruction (reconstruction group).The erythrocyte sedimentation rate,C-reactive protein were determined at 3,14 d after operation in two groups.The congruence angles (CA),lateal patellofemoral angle (LPA),lateral patellar displacement(LS) before and after operation,knee joint function,efficacy after operation were observed and compared.Results There was no significant difference in CA,LPA,LS between two groups (P > 0.05).CA,LPA,LS after operation were improved compared with before operation in two groups [suture group:(9.13 ± 1.88)° vs.(16.50 ±2.83)°,(8.48 ± 1.19)° vs.(-1.28 ± 2.12)°,(0.52 ± 0.43)° vs.(0.78 ± 0.32)° ;reconstruction group:(6.69 ± 1.83)° vs.(16.95 ±3.17)°,(6.24 ± 1.33)° vs.(-0.91 ±2.00)°,(0.32 ±0.23)° vs.(0.70 ±0.16)°],and there was significant difference (P< 0.01).The improvement of CA,LPA,LS in reconstruction group were better than that in suture group,and there was significant difference (P< 0.01).The knee joint function score by Lysholm was (38.27 ± 17.06) scores in suture group,(79.44 ±9.86) scores in reconstruction group,and there was significant difference (P < 0.05).There was no significant difference in the erythrocyte sedimentation rate,C-reactive protein at 3,14 d after operation between two groups (P >0.05).Conclusions The operation effect of isometric reconstruction of the medial patellofemoral ligament with the semitendinosus tendon method is better than the direct suture rehabilitation of the medial patellofemoral ligament method,the former can effectively restore the relationship between patella and femoral trochlea,and cure the dislocation of patella from the root.
2.Sunscreen pollution of marine ecosystems and its potential impact

Sunscreen products have been widely used with the increase in public awareness of sun protection. As sunscreens are structurally stable and resistant to degradation, the concern for their biotoxicity and impact on marine environment has been heightened increasingly, and several countries and regions have successively issued bans on relevant sunscreen products. However, these bans have not yet affected the key international rules and regulations related to sunscreen products. The risks of different sunscreens to marine ecosystems and potential risks to human themselves still need to be evaluated through further multidisciplinary researches.
3.Surgical oPtion to treat tibia Plateau fracture

Objective To summarize the clinical results of treatment of tibia plateau fracture. Methods Forty-six cases of tibial plateau fractures were treated with operation from Oct. 2007 to Oct. 20l2 in General Hospital of PinGdinGshan Coal Industry Groud. AccordinG to Schatzker classification,6 cases were typeⅠ,8 were type Ⅱ,6 were type Ⅲ,4 were type Ⅳ,l2 were type Ⅴ,and l0 were type Ⅵ. Patients in type Ⅰwere treated with open reduction and fixation with screws,other types were treated with open reduction and internal fixation combined with anatomy or lockinG plates and screws. And bone Grafts were used when severe bone defect after reduction. Results Forty-six cases were followed up from 6 months to l8 months( averaGe 8. 5 months ) . AccordinG to the Rasmussen knee score standard,the satisfactory rate was 89. l%( 4l/46 ). Conclusion Different methods of operative treatment of tibia plateau fracture is recommended accordinG to the type of fractures,quality of the bone and the combination of injury. Soft tissue protection should always be kept in mind durinG operation. Delayed operation,Good restitution and fixation,early exercise of joint and late weiGht loadinG are the key factors in the treatment of tibial plateau fracture.
4.Astragalus injection strengthens biological viability of rat neural stem cells

BACKGROUND: Neuroscience and brain science researches have paid attention to the effect of astragalus membranaceus in the treatment of neurologic impairment disease and neural regeneration. Studying astragalus membranaceus effects on neural stem cells are becoming a new research direction. OBJECTIVE: To explore the effects of astragalus injection on biological viability of rat neural stem cells. METHODS: Neural stem cells of Wistar rats were separated and cultured. Immunofluorescence staining was applied to identify the neural stem cells. The purified cells were gained by the second subcultivation in vitro, and then the cells were randomly divided into control group and astragalus injection groups with various concentrations (50, 200, 400 g/L) to culture for 6, 12 and 24 hours. The activity of cells was tested by 3-(4,5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide assay, and then the immunohistochemistry was applied to detect the expressions of neuron-specific enolase and glial fibril ary acidic protein in the 50 g/L astragalus injection group after induced for 7 days. RESULTS AND CONCLUSION: The viability of neural stem cells increased significantly after intervention with different concentrations of astragalus injection for 6 hours as compared with the control group (P < 0.05). However, there was no difference in the cel viability after treated with different concentrations of astragalus injection for 24 hours (P > 0.05). Compared with the control group, the cells in the 50 g/L astragalus group differentiated rapidly, and the number of positive cells for neuron-specific enolase was increased significantly (P < 0.05). The neural stem cells proliferation was hastened, and its differentiation was promoted by the interference of astragalus injection.
5.Expression of Tim-3 in peripheral CD8 + T cells and its significance in patients with chronic hepatitis B

Objective To investigate the expression of T cell immunoglobulin-and mucin-domaincontaining molecule-3 (Tim-3) in peripheral CD8 +T cells and its significance in patients with chronic hepatitis B (CHB).Methods Fifty-eight CHB patients and 16 healthy controls were enrolled.Tim-3 expression in CDs + T cells was detected by flow cytometry,and quantities of IFNγ-producing HBV-specific cytotoxic T lymphocytes (CTLs) in HLA-A2 positive subjects were detected by enzyme-linked immunosorbent spot (ELISPOT) test before and after the blockade of Tim-3/Tim-3L pathway.Paired t test was performed to compare the quantities of CTLs before and after the blockade,and nonparametric Spearman correlation analysis was performed to explore the correlation in quantitive data.Results Tim-3 expression in CHB patients was (14.2 ± 8.98 )%,which was higher than that of healthy controls (4.80 ± 2.92)%,and the difference was of statistical significance (x2 =92.48,P < 0.05 ) Tim-3 expressions in 16 severe CHB patients and 42 mild CHB patients were ( 19.54 ± 10.95) % and (9.58 ± 7.30) %,respectively,and the difference was statistically significant (x2 =77.24,P < 0.05 ). Before the blockade of Tim-3/Tim-3L pathway,IFNγ-producing HBV-specific CTLs were 7.27 ± 3.14,and it increased to 19.62 ± 4.97 after the blockade ( t =2.95,P < 0.05 ).Conclusion The upregulation of Tim-3 on peripheral CD8 + T cells may inhibit HBV-specific CTLs,and the blockade of Tim-3 pathway can enhance the proliferation of IFNγ-producing HBV-specific CTLs,thus can enhance antiviral effect.
6.Heart sound measurement and analysis system with a stethoscope.

This paper is composed with the cardiac sound measurement and analysis system for in-home use of heart abnormality monitoring. The heart sound acquiring system is composed of a traditional chest-piece, earphone, microphone, and IC recorder. The recorded data is transmitted to a computer by USB interface for analysis based on the cardiac sound characteristic waveform (CSCW) method, which is extracted from an analytical model of single degree-of-freedom (SDOF). Furthermore, the characteristic parameters [T1, T2, T11, T12] are defined by the time intervals between the crossed points of the CSCW and a threshold value (THV), which are related to the first sound and the second sound and are used for discriminating normal and abnormal heart sounds. Also, an easy-understanding graphical representation for these parameters is considered, so that, even for an inexperienced user, he or she is able to monitor his or her cardiac status easily. Finally, a case study on the abnormal/normal cardiac sounds is demonstrated to validate the usefulness and efficiency of this proposed system and the cardiac sound characteristic waveform method.

7.An especial transition phase of hospitals: the adaptation of hospital operations to the development of COVID-19 and policy adjustments.

The ongoing pandemic coronavirus disease 19 (COVID-19) remains a significant issue for global health, economics, and society. In order to balance epidemic control and economic recovery, many countries have successively announced the gradual relaxation of some lockdown restrictions. Hospitals and medical staff constitute the backbone in this war against COVID-19. In response to this serious situation, many hospitals went into emergency and impaired healthcare access to patients with conditions other than COVID-19. Therefore, gradually promoting hospital operations and functions back to the new normal is important, especially when this outbreak has been effectively controlled. In this study, we introduce existing and potential problems that could seriously affect people's health. Additionally, we propose that an especial transition phase between the emergency and regular modes of hospitals can be well adapted to the current situation.
