2.Transport of limonin in rat intestine in situ and Caco-2 cells in vitro.

Limonin existed in citrus fruits has been shown to have anti-bacterial, anti-viral, anti-feedant, anti-nociceptive, anti-inflammatory activities and anti-carcinogenic activities. But the clinical use is limited by its low bioavailability. The aim of this study is to observe the absorption and secretion transport mechanisms of limonin in intestine which can pave the way for the further study and clinical use. The transport characteristics and mechanisms of limonin in rat were studied by in situ intestine perfusion and in vitro Caco-2 cells method. The intestinal absorption of limonin was probably via a facilitated diffusion pathway which was poor and without segment-selection. Verapamil and ketoconazole improved the absorption remarkably according to the result of in vitro Caco-2 cells study; however, probenecid had no significant effect on the absorption. The P-gp efflux and CYP3A4 metabolism were involved in the poor intestinal absorption and low bioavailability of limonin. The exploration of the intestinal absorption mechanism is crucial to the design of dosage form and clinical use of limonin.

3.Efficacy and clinical effectiveness of steroid therapy of all body and local application on hyperthyroid Graves ophthalmopathy

Objective To investigate the efficacy and clinical effectiveness of the steroid therapy with the all body and local application on hyperthyroid Graves ophthalmopathy.Methods A total of 57 patients (106 eyes)suffering from hyperthyroid Graves ophthalmopathy who hospitalized in the Department of Ophthalmology of the Second Affiliated Hospital of Harbin Medical University from Mar 2008 to Oct 2011 were enrolled in the study.Of the 57 patients,there were 21 males and 36 females,aged 18 to 57(average 37.28 ± 11.63).The clinical course was from 1 month to 5 years(average 29.63 ± 15.36 months).Based on the patient's age,sex and condition,the 57 patients were randomly divided into steroid pulse therapy with all body group(group SPT,29 patients,54 eyes)and peribulbar injection of long-term steraid group(group PI,28 patients,52 eyes).The patients in group SPT were treated with 0.5-l.0 g methylprednisolone by intravenous infusion daily for 3 days,and then stopped for 7 days,the treatment was repeated once.But the total application dose of methylprednisolone was not overruned 5.0-20.0 g.After that,the patients were treated with oral prednisone 40 mg/d.The dose of prednisone was reduced gradually to 5 mg/d.The total course of group SPT was 3 months.The patients in the group PI were treated with triamcinolone acetonide in periorbital injection of 20 mg once every two weeks for 3 months.After one month treated,all patients were visited.The patients' body weights of two groups were measured before the treatment and after one month treated.The ocular signs of the patients including visual acuity,diplopia,intraocular pressure,exophthalmos and horizontal visual palpebral fissure width were observed before and after the treatment.Results In the group SPT,the average body weight [(62.83 ± 7.23)kg]after the treatment was higher than that before the treatment[(57.03 ±7.13)kg],and also higher than that of the group PI[(59.89 ± 10.67)kg,t =25.25,2.08,all P< 0.05]after the treatment.In the group SPT,the average intraocular pressures before and after the treatment were (13.46 ± 1.78)and (13.69 ± 1.41)mm Hg(1 nm Hg =0.133 kPa),respectively.In the group PI,the average intraocular pressures before and after the treatment were (13.52 ± 2.19) and (13.77 ± 2.03)mm Hg,respectively.There was no significant differences before and after the treatment in both gr0ups(P> 0.05).In the group SPT and PI,the average exophthalmos after the treatment[(18.63 ± 2.22) and (14.85 ± 2.61)mm]were lower than those before the treatment[(20.78 ± 2.24) and (20.60 ± 2.81)mm,t =9.97,29.68,all P< 0.05],respectively.After the treatment,the average exophthalmos in the group PI was significantly lower than that of the group SPT(t =7.79,P< 0.05).In the groups SPT and PI,the average averagepalpebral fissure width after the treatment[(15.00 ± 1.80) and (11.65 ± 2.45)mm]were lower than those before the treatment[(17.22 ± 1.89) and (17.44 ± 2.37)mm,t =22.74,91.24,all P < 0.05].After treatment of the group PI,the average exophthalmos was significantly lower than that of the group SPT(t =8.01,P < 0.05).Conclusion Compared with the steroid pulse therapy with all body,the peribullbar application of long-term hormone for hyperthyroid Graves ophthalmopathy is more effective,with lcss side effects and worthy of promotion.
4.Preliminary study on the effects of an artificial liver support system in the treatment of medicamentous liver insufficiency.

OBJECTIVESTo study the effectiveness of an artificial liver support system.
METHODSThirty-two patients with medicamentous liver insufficiency were treated with an artificial liver support system in addition to the routine medicinal therapy. Thirty patients treated with routine medicinal therapy only served as controls.
RESULTSThe clinical symptoms (e.g. hepatic encephalopathy) and the laboratory indices (serum total bilirubin and prothrombin time) of the treatment group patients were obviously improved compared with those of the control group patients (P < 0.05). The cure rate and hospitalization days were 90.6% (26/32) and 47 days respectively in the treatment group, and 43.3% (13/30) and 72 days in the control group (P < 0.05).
CONCLUSIONUsing an artificial liver support system combined with routine medicinal therapy is more effective than using medication alone.

5.Impact of maternal weight gain during pregnancy on the risk of infant obesity.

OBJECTIVETo study the impact of maternal weight gain during pregnancy on the risk of infant obesity within 1 year old.
METHODSA total of 785 infants who were born in Hefei and participated children medical care in one district health center and their mothers were chosen as the research subjects from September 2010 to September 2011. Three groups were classified by weight gain during pregnancy according to the percentiles: excessive pregnancy weight gain group of 126 pairs, adequate pregnancy weight gain group of 542 pairs and inadequate pregnancy weight gain group of 117 pairs. Mother's general demographic information was collected. The height and weight were measured when the infant was 42 days, 3, 6, 9, and 12 months of physical examination. Z score was calculated. The differences of Z score in different groups were compared and the RR values of different weight gain during pregnancy on infant obesity were computed.
RESULTSThe weight-for-age Z score (WAZ) of infant at 42 days 3, 6, 9 and 12 months in excessive pregnancy weight gain group were 0.23 ± 0.93, 0.25 ± 1.03, 0.23 ± 0.99, 0.28 ± 1.09, 0.26 ± 1.14, respectively, all higher than that of the corresponding age in adequate pregnancy weight gain group (-0.04 ± 1.02, -0.07 ± 0.99, -0.05 ± 0.98, -0.06 ± 0.97, -0.07 ± 0.95, respectively). The differences were statistically significant (all P values < 0.05). In excessive pregnancy weight gain group, infant body mass index (BMI) at 9 months ((18.01 ± 0.15) kg/m(2)) and 12 months ((17.66 ± 0.15) kg/m(2)) were higher than that of adequate pregnancy weight gain group ((17.63 ± 0.13) and (17.22 ± 0.15) kg/m(2), respectively). The differences were statistically significant (all P values < 0.05). Differences of infant Height-for-age Z score (HAZ) among three groups were not statistically significant (all P values > 0.05). Compared to adequate pregnancy weight gain group, RR (95%CI) value of infant obesity in excessive pregnancy weight gain group was 1.86 (1.14 - 3.03).
CONCLUSIONExcessive maternal weight gain during pregnancy increased the risk of infant obesity within 1 year old.

6.Effects of acupuncture on body mass index and waist-hip ratio in the patient of simple obesity.

OBJECTIVETo observe the therapeutic effect of acupuncture for slimming.
METHODSEighty cases of simple obesity were randomly divided into an acupuncture group and a medication group, 40 cases in each group. The acupuncture group were treated with body acupuncture, electroacupuncture and auricular acupoint taping and pressing, with Tianshu (ST 25), Guanyuan (CV 4), Sanyinjiao (SP 6) etc. selected for body acupuncture, and auricular acupoints Shenmen, Neifenmi (endocrine), Pi (spleen) etc. selected for ear acupoint taping and pressing. And the medication group were treated with oral administration of Sibutramine. Body weight, body mass index, waist and hip circumferences and waist-hip ratio were determined before and after treatment to evaluate therapeutic effect of slimming.
RESULTSThe total effective rate of 87.5% in the acupuncture group was similar to 82.5% in the medication group (P > 0.05). After treatment, body weight, body mass index, waist and hip circumferences and waist-hip ratio significantly decreased in the two groups, but the improvement of waist circumferences and waist-hip ratio in the acupuncture group was better than that in the medication group (P < 0. 05).
CONCLUSIONAcupuncture has a definite therapeutic effect in slimming and it can effectively improve body weight, body mass index, waist and hip circumferences and waist-hip ratio in the patient of simple obesity.

7.Effect of Tangshenkang Granule containing serum on renal mesangial cells' proliferation and TGF-β1/Smad2/3 pathway in the high glucose condition.

OBJECTIVETo study the effect of Tangshenkang Granule (TG) containing serum on renal mesangial cells' (RMCs) proliferation and TGF-β1/Smad2/3 pathway in the high glucose condition.
METHODSTwelve SD rats were randomly divided into four groups, i.e., the low dose TG group, the middle dose TG group, the high dose TG group, and the blank control group, 3 in each group. After 7-day gastrogavage via portal vein blood, rats were sacrificed and their serum samples were collected. RMCs were cultured in common rat serum and TG containing serum respectively. The proliferation of mesangial cells was determined by methly thiazolyl tetrazolium (MTT) assay to determine the optimal TG containing serum concentration. Expression levels of TGF-β1 mRNA and protein were determined by real time quantitative PCR and ELISA. Smad2/3 protein expression and phosphorylation were determined by Western blot and immunofluorescence.
RESULTSTG containing serum at different doses could inhibit high glucose induced RMC cells' proliferation, TGF-β1 over-expression and Smad2/3 phosphorylation.
CONCLUSIONTG containing serum could inhibit high glucose induced RMC cells' proliferation, and its mechanism might be possibly associated with inhibiting TGF-β1/Smad2/3 signaling pathway.

8.An optoelectronic cervical cancer screening system for screening cervical cancer: comparison with cervical cytology.

OBJECTIVETo study the clinical value of optoelectronic cervical cancer screening system (TruScreen, TS) in the screening of cervical cancer in comparison with cervical cytology test.
METHODSA total of 392 patients were screened by TS, Pap, TCT, and HPV using the pathological and colposcopical results as the golden standard. The sensitivity, specificity, Kappa value and the area of under ROC of each method and their combinations (parallel tests) were compared.
RESULTSThe sensitivity of TS, Pap, TCT and HPV were 32.2%, 42.2%, 74.4% and 47.8%, with specificity of 96.7%, 93.7%, 78.8% and 84.8% in detecting cervical cancer, respectively. The sensitivity of the parallel tests, namely TCT/HPV, TCT/TS, Pap/TS and HPV/TS were 65.6%, 87.8%, 82.2% and 86.7%, with the specificity of 81.1%, 74.5%, 75.8% and 67.2%, respectively. In light of the areas of under ROC, significant differences were noted between the parallel tests of TS/Pap and TS/TCT (P<0.05), but not between TCT/Pap and TCT/TS (P>0.05); significant differences were found between the parallel tests with TS and those without TS (P<0.05), but not between TS alone and the parallel tests incorporating TS (P>0.05), nor between the 4 parallel tests (P>0.05).
CONCLUSIONAs a new modality for early screening of cervical carcinoma, TS offers a means for real-time cancer detection with better diagnostic efficacy than Pap and HPV and equivalent efficacy to TCT. The combination of TS and cytological tests can further enhance the diagnostic accuracy.

9.Analysis of microRNA in drug-resistant breast cancer cell line MCF-7/ADR.

OBJECTIVETo analyze the difference in microRNAs expression between MCF-7 and MCF-7/ADR cells and explore the association between microRNA and drug resistance of breast cancer.
METHODSThe drug resistance of MCF-7/ADR cells was evaluated using MTT assay and flow cytometry. Microarray technique and RT-PCR were used to analyze the differential expressions of the microRNA between MCF-7 and MCF-7/ADR cells.
RESULTSThe drug resistance index of MCF-7/ADR cells relative to the parental MCF-7 cells was 33.2. The percentages of the side population in MCF-7/ADR and MCF-7 cells were (9.50-/+0.9)% and (0.85-/+0.2)%, respectively. Microarray analysis of MCF-7 to MCF-7/ADR cells identified 36 differentially expressed genes, including 16 up-regulated and 20 down-regulated genes in MCF-7/ADR cells. RT-PCR identified 14 microRNAs that were differentially expressed between MCF-7 and MCF-7/ADR cells, including 7 up-regulated and 7 down-regulated ones in MCF-7/ADR cells. Of these differentially expressed microRNAs, mir-221, mir222, mir-130a, and mir-155 showed significantly increased expression, and mir200a, mir-200b, mir-200c, and mir-421 showed significantly lowered expression in MCF-7/ADR cells as indicated by the results of microarray analysis and RT-PCR.
CONCLUSIONMCF-7/ADR cells show a different microRNA expression profile from its parental MCF-7 cells, suggesting the involvement of microRNAs in tumor cell drug resistance. This finding provides a experimental basis for further study of mechanism underlying the drug resistance of breast cancer.

10.The effects of high fat diet and endurance exercise on the aorta wall structure of experimental animal.

OBJECTIVEBased on high fat diet induced the model of atherosclerosis (AS) in C57BL/6J mice, authors studied the effect of endurance exercise on the atherosclerostic formation.
METHODSForty eight 8-week-old C57BL/6J mice were divided into four groups randomly (n=12): control (group N), 12-week atherosclerosis model group(group H), 12-week atherosclerosis model plus 11-week treadmill training group (group H + E) and 22-week atherosclerosis model group (group HS). Then, we observed the effects of endurance exercise on the ultra structure of aorta by electron microscope.
RESULTSTwenty weeks of high fat diet could result in serious AS in mice while endurance exercise could significantly antagonize or restrain the occur of AS. In addition, 10 weeks of endurance exercise could alleviate the symptom of pathological changes which already happened on aorta wall.
CONCLUSIONIt indicated that endurance exercise could effectively prevent and cure AS that induced by high fat diet.
