2.Analysis and treatment of explicit and occult Descemet's membrane detachment after phacoemulsification

AIM: The reasons of explicit and occult Descemet's membrane detachment ( DMD ) after phacoemulsification were analyzed and the therapeutic measures were further provided.
METHODS:A total of 31 cases diagnosed as explicit and occult DMD between Jan. 2013 and Apr. 2016 received the injection of sterile air into anterior chamber by 1-3 times, and were followed up for 3mo to observe the recoveries of cornea.
RESULTS: A total of 30 cases recovered with no visual impairment and other related complications. One case which refused being treated resulted in bullous keratitis.CONCLUSION:Both the internal and external factors can influence the operation effect of explicit and occult DMD after phacoemulsification. Early detection and timely treatment play important roles in improving patients'visual acuity. Early examination of anterior segment OCT or UBM for intractable corneal edema is helpful to find the occult DMD with different degree and work out the targeted therapies.
3.Application of capsular tension ring combined with iris hooks in phacoemulsification of traumatic cataract with lens subluxation

AlM: To evaluate the clinical efficacy and safety of capsular tension ring ( CTR) combined with iris hooks in eyes with large traumatic zonular dialysis or weakness that underwent micro -incision coaxial phacoemulsification with posterior chamber ( PC ) intraocular lens ( lOL) implantation.METHODS: This prospective study was carried on 21 patients ( 21 eyes ) with traumatic cataract and traumaticzonular dialysis (<180 degrees determined pre-or intra-operatively ) in our hospital from January 2011 to September 2014. After CCC, 1-4 iris hooks might be used probably and a in-the-bag CTR was inserted in order to maintain or re-establish an extended capsular diaphragm before or at any time during cataract removal. Then micro-incision coaxial phacoemulsification with foldable acrylic PC lOL implantation was performed. Zonular dialysis range, posterior capsule rupture, vitreous loss, pupil size, best corrected visual acuity ( BCVA ) , intraocular pressure ( lOP ) in the pre- and post-operative periods and postoperative lOL decentration were observed after operation.RESULTS: The mean follow-up period was 3 ~12mo. No serious postoperative complication was found. BCVA in our cases: <0. 3 in 1 eye, 0. 4~0. 7 in 15 eyes ,>0. 8 in 5 eyes. lOPs were in normal range too. The lOLs in 20 eyes remained the required position to the final follow- up examination except one eye in which the PC lOL was dislocated into vitreous cavity due to a postoperative spontaneity PCR, and in which vitrectomy and lOL suture fixation was performed.CONCLUSlON: ln cases of cataract associated with traumatic zonular dialysis (<180.) , by setting the rational parameters of phacoemulsification, the CTR and iris hooks are found to be efficient in preventing lOL decentration and reducing operation complication. ln our cases, the CTR combined with iris hooks is relatively safe application.
5.Microsurgery for cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia

Objective To summarize clinical diagnosis and microsurgical treatment method of cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia.Methods Thirty-seven cases of cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia treated by microneurosurgery procedure were analyzed retrospectively.Results Complete resection was achieved in 31 cases,and subtotal was achieved in 6 cases,with no operative mortality.Postoperatively,the symptom of trigeminal neuralgia disappeared in 28 cases,was eased obviously in 6 cases,and was eased in 3 cases.The symptom of dizziness in all 6 cases vanished,and hearing in 3 cases was improved.Three cases appeared cerebrospinal fluid leakage,and 4 cases appeared oral herpes.Symptom in 1 case aggravated after 6 months,and oral medicine was ineffective.The pain symptom disappeared after reoperation.Conclusions The suboccipital retrosigmoid approach microneurosurgery is the principal treatment method for cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia.Microsurgical technique combined with surgical skill is the guarantee for increasing total removal rate of tumors and decreasing complications.
6.Clinical study of azithromycin andcefazolin in treatment of respiratory tract infections

Aim To evaluate the effect and safety of azithromycin injection produced in China and cefazolin in the treatment of respiratory tract infections.Methods 50 patients with respiratory tract infections were divided randomly into two groups. Patients in the test group recieved azithromycin in the dosage of 250 mg?d-1,qd for 5 d iv with double dosages at the first time and patients in the control group recieved cefazolin in the dosage of 2.0 g,bid,iv for 5~10 d. Results No statistical significant differences of overall clinical effect were observed between two groups .The fully recovery,effective and side reaction rates for the test group were 46.67%,90% and 3.3%,respectively,and for control group the data were 60%,95%,and 0%, respectively.Conclusion Azithromycin injection produced in China is effective and safe in the treatment of respiratory infections.
7.The Development of Information Centralization and Management Integration System for Monitors Based on Wireless Sensor Network.

Developed the information centralization and management integration system for monitors of different brands and models with wireless sensor network technologies such as wireless location and wireless communication, based on the existing wireless network. With adaptive implementation and low cost, the system which possesses the advantages of real-time, efficiency and elaboration is able to collect status and data of the monitors, locate the monitors, and provide services with web server, video server and locating server via local network. Using an intranet computer, the clinical and device management staffs can access the status and parameters of monitors. Applications of this system provide convenience and save human resource for clinical departments, as well as promote the efficiency, accuracy and elaboration for the device management. The successful achievement of this system provides solution for integrated and elaborated management of the mobile devices including ventilator and infusion pump.

8.Effect of dose-rate and leaf position tolerance on the point dose in step-and-shoot intensity-modulated radiation therapy

Objective To evaluate the effect of dose-rate and leaf position tolerance on the point dose in step-and-shoot intensity-modulated radiotherapy(IMRT). Methods IMRT plans of 2 prostate cancer patients were selected and recalculated for a water phantom.An ionization chamber was used to measure the dose of some points in the phantom at five nominal dose-rates:100 MU/min,200 MU/min,300 MU/min,400 MU/min and 500 MU/min.It was necessary to adiust the position of the water phantom to locate the ionization chamber in region where the dose gradient was very low in order to minimize the effect of dose gradient on the measured results.When measuring the effect of leaf position tolerance on point dose,the dose-rate was kept constant and the values of tolerance were 1 mm,2 mm,3 mm and 4 mm.This work was conducted on a Varian 23EX equipped with a Millennium 120-leaf multi-leat collimator(MLC).The treatment planning system was Varian Eclipse. Results As the dose-rate increased,the error between the measured dose and the calculated dose also enlarged.The difference between the maximum and the minimum was 1.2%.When MLC control system was working normally,tlle effect of leaf position tolerance on the measured point dose was very little and negligible. Conclusions The dose-rate must be selected suitably to ensure that the delivery can be finished in a short time with the radiobiological effect taking into account.It should be noted that the error between the measured dose and the calculated dose increases with the trend of inereasing more rapidly at higher dose rates.The value of leaf position tolerance should not be set too large,in order to minimize the difference between the measured dose and the planned dose in the region of steep gradient and find as early as possible when the multi-leaf collimator control system performs improperly.
9.Preconditioning effect of nitrate medicine on different parts of the heart and its signaling pathway

AIM: To study the preconditioning effect of nitrate medicine on different parts of the heart and its signaling pathway. METHODS: The cells from different parts of the heart (atrium, left ventricle, right ventricle, apex and the whole heart) were isolated and cultured. The cultured cells were treated with hypoxia and reperfusion, hypoxia preconditioning, 1 mmol/L L-arginine preconditioning and 5 mmol/L L-arginine preconditioning, respectively. The cell necrosis rate, cell apoptosis rate, LDH concentration in the medium, the [Ca 2+]i and PKC activity in the cells were determined. RESULTS: The result show that hypoxia and reperfusion increased the necrosis rate and apoptosis rate, enhanced the concentration of LDH in the medium and the [Ca 2+]i overload in the cells (P0.05). 10.Contrast enhanced ultrasonographic features of benign focal liver lesions

Objective To summarize the contrast enhanced ultrasonographic (CEUS) features of benign focal liver lesions, on and to investigate the value of contrast enhanced ultrasound techniques in the diagnosis of benign focal liver lesion. Methods The contrast enhanced ultrasonographic performance of 68 benign focal liver lesions cases which were dififcult for routine ultrasound diagnosis and conifrmed by pathology or follow-up were retrospectively analyzed. Chi-square test of four-fold table were used to compare the diagnostic coincidence rate of conventional ultrasound and contrast-enhanced ultrasound. Results The 68 cases of benign focal liver lesions included complex cysts (n=7), liver hydatids (n=2), liver abscess (n=15), focal nodular hyperplasia (n=8), angiomyolipoma (n=2), hepatocellular adenoma (n=4), focal fat accumulation (n=16), inlfammatory pseudotumor (n=12), solitary necrotic nodule (n=1), intrahepatic biliary cystadenoma (n=1). There were no enhancement among 7 complex cysts, 2 liver hydatids and 1 solitary necrotic nodule. Isoenhancement was detected in focal fat accumulation (n=16);hypoenhancement during the arterial phase and sustained enhancement during the portal or late phase was found in focal nodular hyperplasia (n=8) and angiomyolipoma (n=2). Grid-like enhancements during the arterial phase and isoenhancement or hypoenhancement during the portal phase, and hypoenhancement during the late phase was presented in liver abscess (n=15). Hyperenhancement during the arterial phase were detected in 4 cases of hepatocellular adenoma, 3 of which showed isoenhancement or hyperenhancement during the portal and delayed phase, one case showed hypoenhancement during the portal phase. Eight cases of all the inlfammatory pseudotumor showed no enhancement during all phases;3 cases showing grid enhancement during the arterial phase and the enhancement washed out rapidly;1 case showed mild edge enhancement during the arterial phase and hypoenhancement during the delayed phase. The solid part of the intrahepatic biliary cystadenoma showed hyperenhancement during the arterial phase and hypoenhancement during the portal and late phase.The central area showed no enhancement during all phase. The coincidence rate between pathology and conventional ultrasound diagnosis was 61.8%(42/68). The coincidence rate between pathology and contrast- enhanced ultrasound diagnosis was 92.6%(63/68). The coincidence rate of contrast-enhanced ultrasound diagnostic was higher than that of conventional ultrasound, with a statistically signiifcant difference (χ2=8.17, P < 0.01). Conclusion Real-time gray-scale contrast-enhanced sonography can improve the accuracy of the diagnosis and differential diagnosis for benign focal liver lesions.
