1.Comparison of clinical effect on locking plate for proximal humeral fracture with or without application of inferomedial screws.

OBJECTIVETo compare clinical outcomes of locking plate for proximal humeral fracture whether application of inferomedial screws.
METHODSFrom January 2012 to July 2013, 46 patients with proximal humeral fracture underwent locking plates were retrospectively analyzed. There were 25 males and 21 females aged from 29 to 80 years old with an average of 55.1 years old. Among them, 25 patients were treated with inferomedial screws (support group), including 13 males and 12 females aged from 38 to 80 years old with an average of (55.8 ± 11.8) years old; 8 cases were part two fracture,10 cases were part three fracture and 7 cases were part four fracture according to Neer classification. Twenty-one patients were treated without inferomedial screws (non-support group), including 12 males and 9 females aged from 29 to 79 years old with an average of (54.2 ± 14.8)years old; 6 cases were part two fracture, 9 cases were part three fracture and 6 cases were part four fracture according to Neer classification. Operative time, fracture healing time and complications were observed and compared, Neer scoring of shoulder joint were used to evaluate clinical effect.
RESULTSAll patients were followed up from 12 to 41 months with an average of 15.6 months. Operative time and fracture healing time in support group was (1.6 ± 0.4) h and (3.0 ± 0.6) months, and (1.5 ± 0.4) h and (3.1 ± 0.6) months in non-support group, while there was no statistical difference in operative time and fracture healing time between two groups. There was significant differences in Neer score between support group (89.7± 4.9) and non-support group (83.1 ± 7.1). No complication occurred in support group,while 4 cases occurred complications in non-support group.
CONCLUSIONLocking plate with inferomedial screws for proximal humeral fracture has advantages of stable fixation, less complications, quick recovery of function and satisfied clinical effect.

2.Correlation Study between Electronic Bronchus Mirror and Chinese Medical Syndrome TVDinci of Mycoplasma pneumonia Children.

OBJECTIVETo explore the correlation between electronic bronchus mirror and Chinese medical syndrome typing of Mycoplasma pneumonia children.
METHODSTotally 198 Mycoplasma pneumonia children inpatients were assigned to three syndrome types according to Chinese medical syndrome typing and self-formulated typing standards of electronic bronchus mirror, i.e., Fei-qi accumulation of damp and heat syndrome, Fei-qi accumulation of toxicity and heat syndrome, deficient vital qi leading to lingering of pathogen syndrome. The correlation between electronic bronchus mirror and Chinese medical syndrome typing was explored.
RESULTSAs for comparison between electronic bronchus mirror and Chinese medical syndrome typing, Kappa value (K^) was 0.645 and Spearman coefficient correlation (r) was 0.653 (P < 0.01) for Fei-qi accumulation of damp and heat syndrome; K^ was 0.724 and r(s) was 0.727 (P < 0.01) for Fei-qi accumulation of toxicity and heat syndrome; K^ was 0.506 and r(s) was 0.515 (P < 0.01) for deficient vital qi leading to lingering of pathogen syndrome.
CONCLUSIONChinese medical syndrome typing of Mycoplasma pneumonia children was moderately in line with inspection typing under electronic bronchoscope with significant correlation.

Four kinds of ionic liquids [BMIM] Br, [BMIM] BF4, [BMIM] PF6, [HMIM] PF6 were used to analyze the content of oleanic acid and paeoniflorin in Paeonia lactiflora with ultrasonic-assisted extraction coupled with HPLC. The chromatographic column, Purospher star RP-C18 (4.6 mm x 250 mm, 5 μm), was used. Acetonitrile and water (90:10) as mobile phase was used to determine the content of oleanic acid with a gradient elution and flow rate at 1.00 mL · min(-1), detection wavelength at 210 nm, chromatographic column temperature at room temperature. Paeoniflorin content was determined using acetonitrile and water (18:82) as mobile phase with a gradient elution and flow rate at 1.00 mL · min(-1), detection wavelength at 250 nm, the chromatographic column temperature at room temperature. The result show that oleanic acid has the highest extraction yield when the conditions are solid-liquid ratio of 1:80 (g · mL(-1)), and the [BMIM] Br methanol solution concentration of 0.6 mol · L(-1). Under the optimal extraction conditions, the content of oleanic acid from 0.24 to 3.76 μg showed a good linearity (r = 0.9999), the average recovery was 97.20%. Paeoniflorin has the highest extraction yield when the conditions are solid-liquid ratio of 1:130 (g · mL(-1)), and the [C4 MIM] PF6 methanol solution concentration of 0.6 mol · L(-1). Under the optimal extraction conditions, paeoniflorin content from 0.42 to 4.20 μg showed a good lin- earity (r = 1.000), the average recovery was 98.84%. This method is simple and reliable, its repeatability is also very good. It has important significance in the study P. lactiflora of ionic liquid microextraction.

5.An investigation of serum and hair levels of selenium in patient with Keshan disease and in healthy controls of Keshan disease area

Objective To measure the serum selenium levels in patient with Keshan disease(KSD)and in healthy controls in Shandong,Sichuan and Inner Mongolia KSD areas,to monitor the long-term dynamic changes of hair and serum selenium levels in Shandong KSD areas,and to provide scientific basis for preventing KSD.Methods A cross-sectional survey was carried out in KSD areas of Shandong,Sichuan and Inner Mongolia in 2009.The research subjects which come from KSD areas were 77 cases and 63 healthy controls from Shandong;35 patients and 36 healthy controls from Sichuan;and 17 patients and 18 healthy controls from Inner Mongolia.Additional 33 healthy people from Jinan city were selected as controls of non-KSD areas.Blood and hair samples were collected and selenium levels were measured by 2,3-diaminonaphthalene fluorescence spectrometry.Retrospective method was used to analyze the hair and serum selenium data collected between 1976 and 2004 in Shandong KSD areas.and these data were eompard with the data of 2009 to observe the long-term dynamic changes.Results ① The serum selenium levels of KSD patients in Shandong and Inner Mongolia were significantly lower than that of healthy subjects of KSD areas[(0.0773±0.0113)vs(0.0895±0.0256),(0.0347±0.0107)vs(0.0469±0.0161),t=3.52,3.87,all P<0.01].No significant difference was found between KSD patients and healthy people in Sichuan[(0.0792±0.0162)vs(0.0774±0.0103),t=0.55,P>0.05].②The serum selenium levels of KSD patients in Shandong,Sichuan and Inner Mongolia KSD areas were lower than that of non-KSD area[(0.0988±0.0231)mg/L,q=6.74,5.83,19.47,all P<0.01].The serum selenium levels of healthy people in Sichuan and Inner Mongoha KSD areas were significantly lower than that of non-KSD area(q=6.68,16.36,all P<0.01).The serum selenium levels of healthy controls in Inner Mongolia were lower than that of in Shandong and Sichuan(q=13.63,14.74,13.62,1.46,all P<0.01).③From 1976 to 2009,the hair and serum selenium levels of Shandong resident were increased 1.68 times(0.343/0.128-1)for hair and 0.98 times(0.091/0.046-1)for serum,respectively.But there was no significant difference between the average growth rate of hair and serum selenium levels(χ2=1.38,P>0.05).Conclusions ①The hair and serum selenium levels of KSD patients are lower than that of healthy controls in non-KSD area.②The serum selenium levels of Shandong,Sichuan and Inner Mongolia are different between KSD patients and healthy controls in the diseased areas.③The hair and serum selenium data of Shandong resident show an upward vend over the past 30 years.We suggest to continue the comprehensive measures of adding selenium in KSD areas.

As a novel transdermal drug delivery technology of minimally invasive, safe and efficient, microneedles have received increasing attention. The microchannels formation by microneedles onto the skin is a prerequisite and key for microneedles to deliver drugs. However, there is still a lack of systematic evaluation in skin microchannels. This review summarized influencing factors and evaluation methods in microchannels formation and healing by microneedles, including geometric parameters, materials for preparation, drugs, penetration parameters, differences among the skin of subjects, and presence or absence of occlusion. This review provides reference for other scholars to further study the effectiveness and security of microneedle applications.

9.Analysis of the character of self-adaptation of nasal structure in patients with nasal septum deviation.

OBJECTIVETo analyze the different characters of nasal airflow-field between 10 patients with nasal septum deviation and 20 healthy Chinese people by the method of three-dimensional reconstruction of these people's nasal cavity and numerical simulation of the flow field in these nasal cavity models. The character of airflow-field was considered by analyzing the relationship between the structure and function of the human nose.
METHODSBased on the data obtained from the CT images, 10 patients with nasal septum deviation and 20 healthy Chinese people's nasal cavity models were reconstructed by the method of surface rendering. The flow field in these three-dimensional models was simulated with finite element method. The different characters of nasal airflow-field was analysed between two groups of people.
RESULTSThe airflow distribution in the nasal cavity model could be acquired from the simulation results of the velocity. The airflow for patients with nasal septum deviation mainly passed through the broad nasal cavity, especially in the middle part of meatus of nose. In the healthy people group, the airflow mainly passed through the main side of the nasal cavity, especially in the middle and inferior part of the meatus of nose. The pressure value at any point in the nasal cavity model could be obtained from the results of the pressure plot. In the patients with nasal septum deviation, the pressure mainly dropped in the part of the nasal septum deviation, accounting approximately 71.36% of the total pressure drop. In the group of healthy people, the pressure dropped mainly in the limen nasi, accounting approximately 58.78% of the total pressure drop. The nasal airway resistance of the patients with nasal septum deviation was larger than that in the group of healthy people.
CONCLUSIONSThe three-dimensional nasal airway can reflect the characters of the human nasal airway. It can be used to analyze the change of the aerodynamic in nasal cavity caused by the abnormal anatomy of the nose. This experiment can proof that human nose has the function of self-adaptation, it can build a foundation for the construction of the model of self-adaptation of the human nose.

10.Determination of mosapride in human plasma by high performance liquid chromatography tandem mass spectrometry.

To develop a sensitive and specific high performance liquid chromatography-tandem mass spectrometric (HPLC-MS/MS) method for the determination of mosapride in human plasma, mosapride and internal standard tamsulosin were extracted from plasma with liquid-liquid extraction, then separated on a Waters ACQUITY UPLC BEH C18 column (50 mm x 2.1 mm, 1.7 microm ID) with gradient elution at flow-rate of 0.25 mL x min(-1). The mobile phase was water (containing 0.3% formic acid) and acetonitrile under gradient conditions. Electrospray ionization (ESI) source was applied and operated in the positive ion mode. Multiple reaction monitoring (MRM) mode with the transitions of m/z 422 --> m/z 198 and m/z 409 --> m/z 228 were used to quantify mosapride and the internal standard, respectively. The linear calibration curve was obtained in the concentration range of 0.17 - 68.00 ng x mL(-1). The lower limit of quantification was 0.17 ng x mL(-1). The inter- and intra-day precision (RSD) was less than 13%, and the accuracy (RE) was within +/- 6.3% calculated from QC samples. The method was used to determine the concentration of mosapride in plasma after a single oral dose of 5 mg mosapride citrate to 20 healthy male Chinese volunteers. The method has been proved to be selective, sensitive, rapid and suitable for pharmacokinetic study of mosapride.
