1.Effect of 125I seed implantation on the angiogenesis of transplanted human lung adenocarcinoma in nude mice

Objective To investigate the effect and underlying mechanism of radioactive 125I seed implantation on the angiogenesis of transplanted human lung adenocarcinoma in nude mice.Methods An animal model of transplantd human lung adenocarcinoma was established by subcutaneous implanting A549 cells into nude mice.Twenty four tumor-bearing nude mice were randomly divided into 4 groups with different irradiation doses of blank control (without any treatment) and 0 MBq,22.2 MBq,29.6 MBq and by embedding radioactive 125I seeds with an 18 G implant needle.Tumor volumes were measured every 4 days until all mice were terminated 30 d later and the tumor growth curve was drawn.The microvessel density (MVD) in the tumor tissue was detected by immunohistochemistry S-P assay.The mRNA and protein levels of VEGF and HIF-1α of each group were detected by RT-PCR and Western blot,respectively.Results After embedding of 125I seeds,the tumor volumes of 22.2 MBq group (886 ± 97) and 29.6 MBq group (590 ± 107) were significantly smaller than those of control group (2 297 ± 149) at 54 d after administration (q =14.117,17.075,P < 0.05),but there were no significant differences among 0 MBq group and control group,22.2 MBq and 29.6 MBq groups (P > 0.05).The immunohistochemical CD34-positive staining demonstrated that MVD in 22.2 MBq group (522 ± 119) and 29.6 MBq group (491 ± 121) were decreased significantly compared with control group (922 ± 260) (q =4.826,5.197,P <0.05),but there were no significant differences among 0 MBq and control groups,22.2 MBq and 29.6 MBq groups(P >0.05).The mRNA expressions of VEGF and HIF-1α in 22.2 MBq group (0.279±0.0659,0.370 ±0.0857) and 29.6 MBq group (0.215 ±0.0620,0.278 ±0.0651) were significantly lower than those in the control group (q VEGFmRNA =18.881,17.211,q HIF-1αmRNA =15.376,14.733,P <0.05),but there were no significant differences among 0 MBq and control groups,22.2 MBq and 29.6 MBq groups(P >0.05).At the same time,the expression levels of VEGF and HIF-1α protein after 125I seed implantation were also obviously decreased in 22.2 MBq and 29.6 MBq groups (qvEGr =5.848,6.263,q HIF-1α =6.560,7.576,P < 0.05),and no significant difference between 0 MBq and control groups(P > 0.05) and between 22.2 MBq and 29.6 MBq groups (P > 0.05).Conclusions Interstitial implantation with 125I seeds may potently inhibit angiogenesis in human lung adenocarcinoma xenografts of nude mice.
2.Clinical efficacy and safety of L -carnitine and coenzyme Q10 in treatment of severe chronic heart failure

Objective To evaluate the clinical effect and safety of L-carnitine and coenzyme Q10 on the treatment of chronic heart failure. Methods One hundred ninety-six patients with chronic heart failure cardiacⅢ-Ⅳlevel were paired by age and gender, and each pair were randomly divided into experimental group and the control group, every group has 98 patients.The control group was treated with metoprolol 100 mg, bid +chlorine 50 mg qd +spironolactone 40 mg bid conventional heart failure treatment.The experimental group was added with L-carni-tine (20 mg? kg -1 ,bid) and coenzyme Q10 (10 mg tid) on the basis of conventional treatment. After treatment, the improvement of cardiac function and incidence of adverse drug reactions between the two groups were compared. Results The total effective rate of treatment group (89.80%) is significantly higher than that in control group 63.27%(P<0.05).After treatment, the C reactive protein content and the brain natriuretic peptide ( BNP) level of experimental group were signifi-cantly lower than those in control group ( P<0.05).6 min walking dis-tance of patients in experimental group is longer than that in control group.The left ventricular ejection fraction ( LVEF) of treatment group was significantly higher than that in control group and the left ventricular diastolic diameter ( LVEDD) and left ventricular end systolic diameter ( LVESD) significantly lower than those of control group.The adverse reactions between the two groups have no significant difference (P>0.05).Conclusion L-carnitine combined coenzyme Q10 can significantly improve the treatment effect of severe chronic heart failure without increasing of the incidence of adverse drug reactions.
3.Efficacy of HSV-tk/GCV system on human laryngeal squamous cell cancer in vitro.

OBJECTIVEEfficacy of HSV-tk/GCV system antitumor effects was assessed on human laryngeal cancer cell line Hep-2 in vitro. To assess the HSV-tk/CGV system whether has an antitumour effect on human laryngeal squamous cell cancer Hep-2 in vitro. The mechanisms of cytotoxity were also assessed.
METHODSHep-2 cells were transfected with HSV-tk gene by lipofection. Reverse transcription polymerase chain reaction (RT-PCR) was used to detect the HSV-tk gene expression. MTT was utilized to test for the cytotoxicity of this system. The cell-circle arrest and apoptosis were analyzed by flowcytometry assay.
RESULTSHSV-tk gene transfected cells demonstrated obvious cytoreductivity followed by ganciclovir (GCV) administration and this cytoreductivity showed partial GCV dose-independent. HSV-tk gene transfected cells demonstrated obvious s-phase arrest, no apoptosis and necrosis occurred.
CONCLUSIONSThe HSV-tk/GCV system can inhabit the growth of Hep-2 cells effectively. S-phase arrest perhaps is the main reason that leads to the cell inhibition in our study. HSV-tk/GCV system has potential antitumor effects for the future clinical practice.

4.Cochlear implantation in patients with secretory otitis media

Objective To evaluate the indications and surgery time of cochlear implantation in patients with otitis media with effusion (OME). Methods Cochlear implantation was performed in 26 patients with bilateral profound sensorineural hearing loss and OME. Non-Surgical Treatment was conducted for OME before the following implantation. According to the standards of the indications and surgery time, surgery was carried out. During the process of surgery operation, related lesions of OME were completely removed and proper drainage was then constructed. After the surgery, the patients were regularly followed up, and the effective of cochlear implantation was evaluated. Results Unilateral ear of 26 patients was successfully operated. Among them, 5 cases were left ears and the remaining was right ears. All implant devices worked normally after 1 month. Had been followed up from 13 months to 4. 3 years, 19 patients with OME were healed and the remaining was turning better. No evidence showed phenomenon of the eardrum perforation and acute attack of OME. Conclusions Once the indications of cochlear implantation in patients with OME are guaranteed, the surgery should be conducted as early as possible. The complications of cochlear implantation in patients with OME can be effectively avoided through proper treatment in different stages.
5.Application of nursing sign in blood transfusion of RhD negative patients

Objective To discuss the function of nursing sign in blood transfusion of RhD negative patients.Methods From May 2010 to September 2011,nursing signs were used in RhD negative patients.The blood transfusion department distributed the report of blood type test and an "RhD(-)" sign to the nurse.After double check,the sign was hang on the patient' s bed.In the event of transfer or operation,the sign was delivered to the next department.When the patient was discharged,the sign was returned to the blood transfusion department.A questionnaire survey was conducted to investigate blood transfusion safety knowledge of RhD negative patient of the nurses and corresponding training was carried out.Results After the use of "RhD(-)" sign,76 RhD negative blood patients received blood transfusion and no blood transfusion dispute or mistake was reported.Awareness of relative knowledge of the nurses was 100% after the training.Conclusions The use of "RhD(-)" sign protects the patients' right of informed consent and can caution medical personnel and patients,prevent blood transfusion mistakes and ensure the transfusion safety.
6.Application of in-hospital emergency mode of mobile ICU in rescuing resipiratory and cardiac arrest patients

Objective To study the effect of in-hospital emergency mode of mobile ICU to rescue respiratory and cardiac arrest patients.Methods Mobile ICU infrastructure including necessary rescue equipment,sufficient amount of rescue medication and medical staff proficient in first aid techniques was established.Mobile ICU was used to rescue 22 cases of respiratory and cardiac arrest patients from January 2009to July 2011 in our hospital clinical departments,and the effects were analyzed.Results Mobile ICU arrived at the scene and set up its emergency rescue team in (2.5 ± 0.5 ) min.Out of the 22 cases,4 cases died (18.18%),12 cases were successfully rescued ( 54.55% ),6 cases were transferred to ICU for further treatment (27.27%).No medical dispute happened.Conclusions The establishment of in-hospital emergency mode of mobile ICU can elevate the level of treatment of respiratory and cardiac arrest patients,improve the successful rate,and save patients lives to the maximum extend.
7.Biological appraisal of human bone marrow mesenchymal stem cells during ex-vivo expansion.

This study was aimed to investigate the characteristics of human bone marrow mesenchymal stem cells during ex-vivo expansion, MSCs were isolated from human bone marrow. At each passage, the characteristics of proliferation kinetics, osteogenic and adipogenic differentiation potential were analyzed, and cell morphology, surface markers were investigated as well. The karyotype analysis was done in different passage cells. The infection HIV, HCV, HBV and TP were detected by ELISA. Mycoplasma contamination in vitro was detected by PCR method. HLA-SBT was used to reanalyze the results of HLA antigens and alleles. STR genetic loci were detected by PCR in the MSC1, MSC2, MSC3 and MSC4. The results indicated that the proliferative ability and osteogenic potential decreased with the increase of passage number during culture expansion. The multiple differentiation potential of MSCs was maintained during their life span. Karyotype analysis showed that MSCs from 4 groups before passage 8 were normal. The expression of CD29, CD44, CD105, CD166 and CD73 were positive. The expression of CD14, CD34, CD45, CD80, CD86 were all negative. SBT was used to identify HLA-A, B, Cw, DRB1, DRPB1, DQ alleles in the MSC1, MSC2, MSC3, MSC4. The genetype of STR in the MSC1, MSC2, MSC3, MSC4 was different. MSC 3 was examined by TP-ELISA to confirm the infectious disease of TP. MSC2 was contaminated by mycoplasma at passage 5. It is concluded that culture expansion causes MSCs to gradually lose their stem cell properties. During ex-vivo expansion of MSCs, the osteogenic differentiation potential is decreased. MSCs before passage 8 can be a valuable subject for basic research and clinical application.

8.Study of the platelet GP specific antibodies and HLA antibodies expression in platelet transfusion refractoriness patients.

OBJECTIVETo investigate the correlation between the platelet GP specific antibodies/HLA antibodies and platelet transfusion refractoriness (PTR).
METHODSSixty-five patients with PTR were selected in this study and were genotyped for HLA-A and HLA-B as well as HPA systems by standard PCR-SSP assays. The platelet GP specific antibodies and HLA antibodies in serum and platelet elution were tested with a solid phase ELISA.
RESULTSThe HLA-A/B antigens and the frequencies of HPA-1, 2, 4, 5, 6, 9, 15 antigens in PTR patients had no difference from those in healthy donors. The freguencies of HPA-3a and 3b were 0.65 and 0.35, respectively. There was statistical difference between the 65 PTR patients and the healthy donors in HPA-3 freguencies (P < 0.05). Twenty-four patients (36.9 %) only expressed HLA antibodies, and 14 (21.5%) expressed HLA and platelet GP specific antibodies. The highest expression of anti-HLA-A/B specific antibodies was -A*9(46.2 %)/-B*40(33.6%), respectively. In serum, GPIIb/IIIa was expressed (26.2%), followed by GPIa/IIa (21.5 %). In platelet elution, GPIIb/IIIa was expressed of 41.5% and GPIb/IX 41.5%. Pedigree study was carried out for 2 patients. The results showed that the platelet GP specific antibody/HLA antibody developed in PTR patients was highly related to the mismatch with the platelet antigen/HLA antigen in their parents.
CONCLUSIONThe expressions of the HLA and platelet GP specific antibodies are the most important reason in PTR, it's meaningful to explore the correlation between PTR and HLA and HLA-A/B antigen in guiding platelet transfusion.

9.Expression of anti-platelet glycoprotein specific antibodies and anti-HLA antibodies in idiopathic thrombocytopenic purpura.

In order to investigate the expression of the anti-platelet glycoprotein specific antibodies and anti-HLA antibodies in idiopathic thrombocytopenic purpura (ITP), 45 patients with ITP were selected in this study. An easy PCR-SSP assay was used to detect single-nucleotide polymorphisms or deletion in HPA and HLA systems. The anti-platelet glycoprotein specific antibodies and anti-HLA antibodies in plasma or platelet eluate were tested with a solid phase ELISA. The results indicated that the anti-platelet glycoprotein specific antibodies were detected in plasma or platelet eluate of 45 patients, among which anti-GPIIb/IIIa/and anti-GpIb/IX were most common. Both the anti-platelet glycoprotein specific antibodies and anti-HLA antibodies were found in plasma of 11 patients. Pedigree investigation in 2 patients (case 37 and case 40) was carried out, the results showed that anti-platelet glycoprotein specific antibodies and anti-HLA antibodies detected in 2 patients closely related to incompatibility with platelet antigens and HLA antigens in parents. In conclusion, the results suggested that detection of the anti-platelet glycoprotein specific antibodies and anti-HLA antibodies in plasma or platelet eluate in combination with investigation of clinical manifestation of patients is important for diagnosis of idiopathic thrombocytopenic purpura.

10.Effect of heavy metals exposure on neurobehavioral function in welders.

OBJECTIVETo explore the relationship between heavy metals exposure and neurobehavioral function impairment in welders.
METHODSThe metals exposure in 82 welders and 51 operators were investigated with blood Pb, Cd and Mn via AAS, and the nervous impairment was evaluated with neurobehavioral core test battery (NCTB).
RESULTSPb [(115.49 +/- 79.22) microg/L] and Cd [(3.67 +/- 3.19) microg/L] in welders were significantly higher than operators [(69.32 +/- 50.79) and (0.83 +/- 0.76) microg/L respectively] (P < 0.05). Welders had worse standard scores of NCTB 13 items such as depression-dejection than non-welders (P < 0.05). Significant difference of confusion-bewilderment and forward digit span in welders only existed in different groups of Pb and Mn, respectively. A dose-effect relationship was found between forward digit span and serum Mn level in welders. General linear regression analysis indicated that Pb exposure, Mn exposure and alcohol consume had negative relation with the loss of nervous system function.
CONCLUSIONThe nervous impairment in welders is attributed to occupational exposure to Pb and Mn, concomitantly.
