1.Effect of application of acupuncture-anesthetic composite anesthesia on hysteroscopic surgery: a clinical study.

OBJECTIVETo observe the analgesic effect and safety of acupuncture-anesthetic composite anesthesia (AACA) in hysteroscopic surgery.
METHODSTotally 93 patients undergoing hysteroscopic surgery were randomly assigned to the intravenous anesthesia group (A group, 30 cases), the AACA group (B group, 32 cases), and the acupuncture combined with intravenous anesthesia group (C group, 31 cases). Patients in Group A were anesthetized by sufentanil combined propofol. Those in Group B were anesthetized by sufentanil combined acupuncture. Those in Group C were anesthetized by sufentanil, propofol combined acupuncture. Yinlian and Ququan (LR8) were needled for patients in Group B and C. The peri-operative mean arterial pressure (MAP), heart rate (HR), and oxygen saturation (SpO2), the surgical time, the recovery time, the sufentanil and propofol dosages, adverse anesthesia reactions were observed. Meanwhile, the OAA/S score, Ramsay sedation score, and Visual Analogue Score (VAS) were also measured.
RESULTSCompared with Group A and C, patients in Group B were awake, with obvious increased OAA/S score (P < 0.01). Ramsay sedation score was significantly lower (P < 0.01).The MAP and HR were elevated (P < 0.05). The patient case of SpO2 less than 85% during the operation decreased (P < 0.05). The incidence of postoperative dizziness was reduced (P < 0.05). Compared with Group A, the propofol consumption decreased in Group C (P < 0.05). There was no statistical difference in the operation time, the sufentanil dosage, VAS score, the incidence of postoperative nause- a and vomiting among the three groups (P > 0.05).
CONCLUSIONSThe patients were awake in AACA. The intraoperative sedation was better than that obtained by intravenous anesthesia. But the analgesic effect was similar to that obtained by intravenous anesthesia.

2.Effects of estradiol on the expression of estrogen receptor in lens epithelial cell of castrated female Wistar rat

Background Recently researches indicated that estrogen plays important role in maintaining the normal metabolism of lens. Objective This study was to investigate the changes of estrogen receptor( ER ) α and β expressions in lens upon estrogen level in castrated female rat. Methods Sixty clean adult female Wistar rats were randomized into castrated group,sham operation group,ovariectomy group,ovariectomy with low-dose estradiol eyedropping group,ovariectomy with high-dose estradiol eyedropping group,ovariectomy with low-dose estradiol injecting group and ovariectomy with high-dose estradiol injecting group,and 10 rats for each.The castrated animal models were established by ovariectomy for 5 months.Then 50%,100% oestradiol benzoate eyedrops were used 4 times per day respectively and 0.2 or 0.4 mg/kg oestradiol benzoate were intramuscularly injected at two-day interval for 6 weeks in corresponding experimental group.Serum estradiol concentration was detected in the rats of various groups at 5 months after ovariectomy and 6 weeks after administration of estradiol benzoate.The animals were sacrificed using the excessive anesthesia method and the lenses were obtained for the assay of ERα and ERβ expressions.The use of the animals complied with the Statement of ARVO. Results No obvious opacification of lenses and the changes of structure and morphology in lens were seen in the rats of various groups under the slit lamp microscope and light microscope during the observing duration after ovariectomy.The significant differences were found in serum estradiol concentrations among the 6 groups ( F=15490.527,P=0.000) or between before and after usage of estradiol benzoate( F=943.236,P =0.001 ).Six weeks after usage of estradiol benzoate,the expressions of ERα and ERβ in the lenses were lower in the castrated group,ovariectomy with high-dose estradiol eyedropping group and ovariectomy with low-dose estradiol injecting group compared with the the sham operative group (P<0.05),but those in the ovariectomy with low-dose estradiol eyedropping group and ovariectomy with high-dose estradiol injecting group were elevated in comparison with above groups( P<0.05 ),and expressions of ERα and ERβ in the lenses were similar to the sham operative group ( ERα:28.04±6.80 vs.31.30±7.11 ;ERβ:27.75±7.13 vs.25.38±5.59).Mean A values of ERα and ERβ in the lenses were lower in the castrated group,ovariectomy with high-dose estradiol eyedropping group and ovariectomy with low-dose estradiol injecting group compared with the sham operative group (P<0.05),but those in the ovariectomy with low-dose estradiol eyedropping group and ovariectomy with high-dose estradiol injecting group were elevated in comparison with above groups ( P<0.05 ),and mean 4 values of ERα and ERβ in the lenses were similar to the sham operative group (ERα:0.1833 ±0.0087 vs.0.1859 ±0.0067; ERβ:0.1689±0.0059 vs.0.1686±0.0095). Conclusions The expressions of ERα and ERβ in the LECs are associated with the level of serum estradiol.The effects of estrogen on lens were different by different medication way.Low-dose estradiol eyedropping was a more feasible approach to the prevention of cataract.

4.Effects of Zhikeping on aortic oxyradical and the expression of ICAM-1 in a rat model of experimental early atherosclerosis.

OBJECTIVETo investigate the effects of Zhikepingon the oxyradical andintercellular adhesion molecule-1 (ICAM-1) of experimental early atherosclerosis.
METHODThe model of SD rat early atherosclerosis was induced by cholesterol diet. The suspension of Zhikeping and simvastatin were administered intragastrically, respectively. After 10 weeks, the serum lipids, superoxide dismutase (SOD) and maleic dialdehyde (MDA) were detected by automatic biochemistry analyzer. ICAM-1 and its expression of mRNA in aortic wall were detected- by immunohistochemistry and RT-PCR, respectively. Aortic histomorphology was cbserved by HE stainning.
RESULTThe results showed that the serum lipids, superoxide dismutase (SOD) and maleic dialdehyde (MDA) of treated groups were obviously improved as compared with those of the control group. The tissue pathological damage was improved indifferent degree, and ICAM-1 and its expression of mRNA was decreased obviously.
CONCLUSIONIt is suggested the mechanism of anti-atherosclerosis of Zhikeping have close relationship with the function of its anti-oxidizing and anti-adhesiveness that can protect aortic endothelial cell.

5.Correlation between NADPH oxidase-mediated oxidative stress and dysfunction of endothelial progenitor cell in hyperlipidemic patients.

BACKGROUND/AIMS: NADPH (nicotinamide adenine dinucleotide phosphate) oxidase (NOX)-mediated oxidative stress plays a key role in promotion of oxidative injury in the cardiovascular system. The aim of this study is to evaluate the status of NOX in endothelial progenitor cells (EPCs) of hyperlipidemic patients and to assess the correlation between NOX activity and the functions EPCs. METHODS: A total of 30 hyperlipidemic patients were enrolled for this study and 30 age-matched volunteers with normal level of plasma lipids served as controls. After the circulating EPCs were isolated, the EPC functions (migration, adhesion and tube formation) were evaluated and the status of NOX (expression and activity) was examined. RESULTS: Compared to the controls, hyperlipidemic patients showed an increase in plasma lipids and a reduction in EPC functions including the attenuated abilities in adhesion, migration and tube formation, concomitant with an increase in NOX expression (NOX2 and NOX4), NOX activity, and reactive oxygen species production. The data analysis showed negative correlations between NOX activity and EPC functions. CONCLUSIONS: There is a positive correlation between the NOX-mediated oxidative stress and the dysfunctions of circulating EPCs in hyperlipidemic patients, and suppression of NOX might offer a novel strategy to improve EPCs functions in hyperlipidemia.

6.Effect of granulocyte colony stimulating factor on myeloid-derived suppressor cells in the bone marrow and peripheral blood: a preliminary study.

OBJECTIVETo investigate the effect of granulocyte colony stimulating factor (G-CSF) on myeloid-derived suppressor cells (MDSCs) in the bone marrow and peripheral blood, and explore the relationship between MDSC and graft-versus-host disease (GVHD).
METHODSBone marrow, peripheral blood and peripheral blood stem cells were obtained from 12 healthy hemopoietic stem cell donors before and on day 5 after G-CSF mobilization. Flow cytometry was employed to examine the number of MDSC, and the relationship between MDSC number and the incidence of GVHD was analyzed.
RESULTSIn normal physiological conditions, MDSC could be detected in the peripheral blood and bone marrow with a cell percentages of (1.35±0.35)% and (2.44±1.11)%, respectively, showing a significantly higher cell percentage in the bone marrow (P=0.015). On the 5th day after G-CSF mobilization, the percentage of MDSCs increased to (4.01±1.82)% in the peripheral blood and to (4.38±2.19)% in the bone marrow, showing no significant difference between them (P=0.083). The mobilization caused a significant increase in the number of MDSCs in the peripheral blood (P=0.047) but not in the bone marrow (P=0.761). The number of MDSCs in the collected samples showed a significant inverse correlation to the incidence of GVHD (P=0.048).
CONCLUSIONSMDSCs are present in the peripheral blood and bone marrow of healthy donors, with a greater number in the bone marrow. G-CSF can mobilize the MDSCs from the bone marrow to the peripheral blood to increase number of MDSCs in the peripheral blood, which may contribute to a lowered incidence of GVHD in hematopoietic stem cell transplantation (HSCT).

7.Research development of Carpesii Fructus

Carpesii Fructus is the fruit of Carpesium abrotanoides L. and is recorded in the Chinese Pharmacopoeia (2015 Edition). Carpesium abrotanoides broadly distribute in China. Traditionally, Carpesii Fructus was used as a parasiticide,especially for ascariasis,pinworms and tapeworm disease. In ancient times,the Carpesium plants were used as traditional Chinese,Korean and Japanese herbal medicines for the treatment of several diseases.Carpesii Fructus was first recorded in the book"Newly Revised Canon of Materia Medica"in the Tang Dynasty of China.The original plant is Compositae Arte-misia santonica (Seriphidium cinum) from middle east Persian. At present, Carpesium abrotanoides issometimes confused with the Lappula family in species classification. In the Song Dynasty of China,"KaiYang Materia Medica"recorded that the best Carpesii Fructus was from Persian.The main compo-nents of Carpesii Fructusare terpenes,phenolic compounds,flavonoids and coumarins. Including telekin, 3-epi-isotelekin, 11β-13-dihydro-1-epi-inuviscolide, carabrone, carabrol, terpene lactone, gerilin, carpesia, valeric acid, oleic acid, linolenic acid, thirty-one alkane, sterol, etc. The chemical components isolated from whole plants of carpesia are more than 143. In clinical practice, Carpesii Fructus is mainly used as antiparasitic drugs and usually combined with other drugs since the poor efficacy as single drug. Its toxic reaction is closely related to the dose of the drug.Carpesia,asa main component of Carpesii Fructus, might lead to adverse reactions also. At present, Major issuesof Carpesii Fructusare the lack of phar-macological research,as well as lack of in-depth study on the material basis.Therefore,further studies are needed on the drug development and clinical usuage.
8.A 4-year follow-up study on sexual orientation, sexual behavior and AIDS related knowledge of undergraduate students in a medical university

Objective To explore the characteristics of medical sexual orientation, sexual behavior and acquired immunodeficiency syndrome(AIDS) related knowledge changing with grade. Methods A group of 876 undergraduate students of 2015 in a medical university were selected to conduct an anonymous survey once a year and a total of four times using self-compiled questionnaires on AIDS-related knowledge, sexual orientation and behavior of college students. Descriptive epidemiological methods were adopted for statistical description. Results The distribution of sexual orientation among medical students with different survey batches was statistically significant ( 2=26.223, P=0.036). The proportion of openly gay people in the total number of homosexuals showed an increasing trend ( 2trend=4.150, P=0.042). Follow-up study of Chinese medicine students on AIDS related knowledge of the overall rate of increased ( 2trend=23.462, P<0.001). The incidence of general sexual behavior and male homosexual sex among medical students in the past six months showed an increasing trend with grade growth ( 2trend=4.450, P=0.035; 2trend=6.738,P=0.009). At the second and third follow-ups, the proportion of men who used condoms occasionally and never used condoms increased, and the difference was statistically significant ( 2=7.935, P=0.047). Conclusions Due to the weak awareness of medical sexual safety protection, the school should continue to strengthen the AIDS publicity and prevention education of medical students in all grades, and improve the school sexual health education mechanism.
9.Early biological markers of manganese exposure.

OBJECTIVETo explore the biomarker of manganese exposure by analyzing the relationship between manganese exposure and concentration in some biomaterials.
METHODSThe air samples were collected through the individual air sample. According to the manganese levels in the air, workers were assigned to control group, low concentration group and high concentration group, and manganese in the hair, urine, serum, blood cell and saliva from different group were measured respectively. The correlations between concentration of external manganese exposure and manganese concentrations in biomaterials, and years of employment and concentrations in biomaterials were analyzed.
RESULTSIn the high concentration group, saliva manganese was 32.17 µg/L, hair manganese was 37.39 mg/kg, urine manganese was 2.50 µg/L, plasma manganese was 29.61 µg/L, blood manganese was 14.49 µg/L, were higher than those in the control group (10.40 µg/L, 1.60 mg/kg, 0.77 µg/L, 10.30 µg/L, 4.56 µg/L respectively) (P < 0.01). The manganese concentration in the saliva was significantly correlated with airborne manganese concentration (r = 0.649, P < 0.01), with the years of employment (r = 0.404, P < 0.01), with the total exposure of manganese (r = 0.342, P < 0.01), with the manganese concentration of plasma (r = 0.303, P < 0.01) and with the manganese concentration in blood cells (r = 0.359, P < 0.01), respectively.
CONCLUSIONSThe concentration of manganese in saliva could work as a biomarker of manganese internal exposure.

10.Analysis of the positive conversion rate of negative spouse and survival analysis on HIV sero-discordant couples

Objective To analyze the positive conversion rate of negative spouse and the survival analysis on HIV sero-discordant couples. Methods Data were collected from the case report database of the integrated AIDS prevention and control data and information system. HIV sero-discordant couples diagnosed between September 1,1997 and September 1,2018 were recruited into the cohort. The participants were divided into three groups as untreated subgroup,routine treatment subgroup and early treatment subgroup according to the situation when they started HIV treatment. The positive conversion rate of negative spouse and the survival were compared among the three subgroups. Results 598 HIV sero-discordant couples were recruited and the overall survival time of the three subgroups were 258. 92,4 987.17 and 862.25 person-year. The rate of positive conversion were 6. 18/100,0. 48/100 and 0. 23/100 person-year. Multivariate Cox regression model showed that the hazard ratio of routine treatment subgroup ( HR = 0. 052,95% CI: 0. 037-0. 074) and early treatment subgroup ( HR=0. 011,95% CI: 0. 003-0. 035) were lower than untreated subgroup. The risk of death of women was lower than that of men( HR=0. 667,95% CI: 0. 472-0. 944) ; the older the age,the higher the risk of death ( HR=1. 041,95% CI: 1. 026-1. 056) ; and the risk of death of the unemployed is higher than that of farmers ( HR=1. 571,95% CI: 1. 037-2. 381) . Conclusion Targeted interventions methods and early antiviral therapy should be taken urgently for sero-discordant couples.
