1. Relationship of ecosystems and resolution of new drug research and development

OBJECTIVE: Relationship of ecosystems and resolution of new drug research and development is analyzed through a comparison of the clinical needs, industry development policy, enterprise-self factor, listing application mechanism, number and characteristics of new drug in America and China, especially, the market conditions of the me-only, me-better and me-too between America and China is analyzed importantly. The idea that the new drug is studied and developed in China is presented. METHODS: The important influencing factors that affected the ecosystems of new drug research and development were analyzed by comparison METHODS and data studies. RESULTS: The clinical availability of new drug in America is significant increased recently and over 90% applied new drugs can be finished in a limited time based on the evolution of the innovative ecological environment of new drug research and development from 1993 to 2014, new drugs are approval listing in America and China respectively. CONCLUSION: The availability of new drug is very important to a state security. The ecosystem of new drug research and development is very complicated. The relative field in America is more advanced than in China and it is used for reference to the new drug research and development in China.
2.Analysis of iodine nutrition of people and serum thyroid hormone levels of women of childbearing age in pasturing areas of Tibet in 2009

Objective To explore the iodine nutrition level of people, prevalence of iodine deficiency disorders and the thyroid function of women of childbearing age in pasturing areas of Tibet. Methods Thirty families were selected respectively in pastoral Dangxiong county and agricultural Qushui county of Lhasa in 2009,drinking water and edible salt samples were collected to test iodine content; at least 50 people from each crowd of the following populations including children aged 8 - 10, women of childbearing age of 18 - 49 old and male adults aged 18 - 60 were randomly sampled and to measure their urinary iodine content and for thyroid palpation. Direct titrimetric method was used to test salt iodine(GB/T 13025.7-1999); As3+-Ce4+ oxidation reduction process to test water iodine (GB/T 5750.1-2006); As3+-Ce4+ catalytic spectrophotometry using ammonium persulfate digestion to test urine iodine(WS/T 107-2006), and goiter examination was based on Diagnostic and Classificatory Criteria of Endemic Goiter (WS 276-2007). Results The median of water iodine was 1.3 μg/L in pasturing area and 0.7 μg/L in agricultural areas, there was no statistical significant difference between them(Z =- 1.809, P > 0.05).There was no iodized salt used in pastoral people, but iodized salt coverage rate was 90.0%(27/30) in agricultural residents. The median of urinary iodine among people of pasturing areas was 50.2 μg/L, lower than that of agricultural areas( 193.2 μg/L, Z =- 10.48, P < 0.01 ). However, the goiter rate in pasturing area[1.0%(1/100)]was significantly lower than that of agricultural areas[18.0%(18/100) , x2 =16.8, P < 0.01]. Serum level of FT4 and TT4 in pastoral population[(14.0 ± 2.0)pmol/L, (85.6 ± 17.5)nmol/L] was significantly lower than that of agricultural areas[(16.2 ± 6.3)pmol/L, (95.4 ± 21.1)nmoL/L, t =- 2.06, - 2.20, all P < 0.05]. The thyroid dysfunction rate[5.9% (2/34)]and subclinical hypothyroidism rate[2.9% (1/34)]in pastoral population was significantly lower than that of agricultural areas[25.5%(12/47), 21.3%(10/47), x2 =5.328, 5.651, all P < 0.05]. Conclusions Pastoral areas of iodine intake is significantly lower than the agricultural areas, urinary iodine levels reflect a serious iodine deficiency in pastoral people, but the blood biochemical and urinary iodine and goiter rate does not match,and shows hidden iodine hunger, which does not constitute a goiter epidemic.
3.Incidence and clinical properties of dry eye after phacoemulsification in age-related cataract patients

?AIM:To evaluate the incidence and clinical properties of dry eye after phacoemulsification in age-related cataract patients.?METHODS: Samples were collected from 145 age -related cataract patients (145 eyes).Dry eye was analyzed at 0, 7, 30, 90 and 180d after phacoemulsification by 1 ) Ocular Surface Disease Index questionnaire ( OSDI ) , 2 ) tear meniscus height ( TMH ) , 3 ) corneal fluorescein staining, 4) tear film break-up time (BUT), 5)SchirmerⅠtest( SⅠt) .?RESULTS:The symptoms and signs of dry eye, such as narrowing of TMH, shorting of BUT, decreasing of SⅠt, cornea staining by fluorescein, occurred as early as 7d post-phacoemulsification and were measured by OSDI questionnaire and 4 additional clinical tests.Over the six-month observation the severity of dry eye peaked at 30d and then gradually relieved.? CONCLUSION: The severity of dry eye after phacoemulsification peaked at 30d and gradually improved over time. Considering the characteristics of ocular surface for aged people ophthalmologists should pay more concern on evaluating the occurring of dry eye after phacoemulsification so as to improve the life quality of these people.
4.Effect of Fusion Protein Cleavage Site Mutations of NDV ZJ1 Strain on Its Fusion Activity

NDV strain ZJ1 strain , a highly virulent NDV strain, has been prevalent among the waterfowls in China mainland in the past years. Multi-basic amino acid sequence distribute in the protease cleavage site of F protein of this strain. Recombinant expressing plasmid pCI-FT, was generated by converting multi-basic amino acid sequence of 112, 115, 117 of the protease cleavage site of F_ 0 protein, to the non-basic amino acid sequence characteristic of avirulent NDV strain. The result from co-expression of mutant or parental F protein with homologous HN protein in COS-1 cells revealed that both mutant and parental F protein had fusion activity. The result from co-expression of mutant or parental F protein with homologous HN protein in CEF cells showed that the cleavage activity of mutant F protein was significantly reduced. The study built a foundation for mutagenesis of amino acid sequence of the protease cleavage site of F_ 0 protein at the full-length cDNA clone level, study on factors contributing to virulence and construction of candidate vaccine strain, and so on.

Objective To discuss effect on prevention of postpartum hemorrhage of caesarean section by us- ing calcium gluconate combined with oxytocin and misoprostol.Methods 385 cases of caesarean section were select- ed and randomized into O(Oxytocin) group and OM(Oxytocin+ Misoprostol) group and COM (Calcium gluconate+ Oxytocin+Misoprostol)group.Results The mean operative blood loss in O group and OM group and COM group were (300?50.24)ml,(220?30.83) ml,(150?45.52) ml.The amount of the mean operative blood loss of COM group was significantly lower than those of O group and OM group(P<0.05).The amount of bleeding of 2 hours after delivery in O group and OM group and COM group were (400?45.52)ml,(260?60.43)mi and(210?50.54) ml.The amount of bleeding of COM group was significantly lower than those of O group and OM group (P<0.05).Conclusion The prevention by used calcium gluconate combined with oxytocin and misoprostol is efficient in reducing the amount of postpartum hemorrhage of caesarean section.The operation of medicine is easy and safe and economic.
6.Establishment of Reverse Genetics System for NDV Isolated from Goose

Eight fragments were amplified and cloned into pCR2.1 vector with the designed primers.The fragments,amplified with primer Ⅰ to Ⅶ,were subcloned into transcription vector to construct the plasmid pNDVZJI which contained the full-length cDNA of NDV ZJI strain.The eukaryotic expression vector pCI-L was constructed by subcloning the fragments,amplified with the primer Ⅴ,Ⅵ and Ⅷ,into the expression vector pCI-neo.The full-length cDNA clone,pNDVZJI,with three helper plasmids,pCI-NP、pCI-P and pCI-L,were cotranfected into BSR-T7/5 cell expressing T7 RNA polymerase.After inoculation of transfected cell culture into embryonated chicken eggs from specific pathogen free(SPF)flock,The NDV of ZJI strain was rescued successfully,which laid a good foundation for the further related research.
7.Comparision of nutritional status of hemodialysis patients with different ages and its nursing strategy

Objective To explore the nutritional status of hemodialysis patients with different ages, and analyze the reasons of differences of nutritional status. Methods 109 hemodialysis patients were divided into the young-age group (n=26), the middle-age group (n=49) and the old-age group (n=34). The body indices, blood biochemical parameters, dietary assessment, handgrip strength and subjective global assessment were measured. The nutritional indices and relationship were compared among patients in the three groups. Results Leg circumstance, prealbumin, creatinine, handgrip strength and SGA scores were different among the three groups (P<0.05 or P<0.01). And those five factors were all statistically associated with age (P<0.05). Conclusions With the increase of age, the hemodialysis patients' nutritional status has a descending trend. It's necessary for nursing staff to timely monitor and intervene the nutrition of elderly patients so as to prevent the incidence of malnutrition.
8.Construction of a temperature inducible shuttle expression vector and its application in Streptomyces.

pHZ1080, an E. coli-Streptomyces shuttle expression vector was constructed in order to explore the utilization of lambda phage regulated expression elements in Streptomyces. A 2.7 kb polyketide synthase (PKS) gene from Streptomyces sp. FR-008 was inserted into downstream of lambda phage promoter (PR) to give the shuttle plasmid, pHZ1067. The PKS protein was expressed in Streptomyces lividans carrying pHZ1067 in a heat-dependent manner, as it did in E. coli. The PKS protein expressed in both hosts with same molecular weight was detected by SDS-PAGE and Western-blot. The successful heat-induced expression of PKS suggested that pHZ1080 was useful and convenient for heat-induced expression of heterologous genes in both E. coli and Streptomyces.

9.Observation on the clinical efficacy of spasmodic torticollis treated with matrix needling technique and acupuncture at "Wuxin points" mainly.

OBJECTIVETo compare the difference in the efficacy on spasmodic torticollis between matrix needling technique combined with puncturing "Wuxin points" and the local injection of botox type A.
METHODSForty-two cases were randomized into an acupuncture group and a botox group, 21 cases in each one. In the acupuncture group, the matrix needling technique was used and acupuncture was adopted at Fengchi (GB 20), "Wuxin points" [Shuigou (GV 26), bilateral Laogong (PC 8) and bilateral Yongquan (KI 1)] and local Ashi points. In the botox group, botox type A was injected at several spots of local region. The treatment course was 1 month in the two groups. The efficacy in each weekend and the results of follow-up visit in half a year after treatment were compared between the two groups.
RESULTSIn the 1st weekend, the markedly effective rate was 42.8% (9/21) and the total effective rate was 90.5% (19/21) in the botox group, which were better than those of 4.8% (1/21) and 28.6% (6/21) respectively in the acupuncture group (both P < 0.01). In the 2nd weekend, the markedly effective rate was 71.4% (15/21) and the total effective rate was 95.2% (20/21) in the botox group, which were better than those of 19.1% (4/21) and 61.9% (13/21) respectively in the acupuncture group (both P < 0.01). In the 3rd weekend, the markedly effective rate was 66.7% (14/21) and the total effective rate was 95.2% (20/21) in the botox group, which were better than those of 38.1% (8/21) and 71.4% (15/21) respectively in the acupuncture group (both P < 0.05). The markedly effective rate and the total effective rate were not different significantly between the two groups in the treatment of 1 month and the follow-up visit in half a year (all P > 0.05). There was no adverse reaction in the acupuncture group. There were 14 cases of adverse reactions in the botox group, in which 9 cases of neck muscular weakness and 5 cases of neck muscular pain.
CONCLUSIONThe treatment of matrix needling technique and combined with acupuncture at "Wuxin points" achieves the significant efficacy on spasmodic torticollis. Although the effect is not obtained as quickly as the local injection of botox, the efficacy of matrix needling technique is similar to that of botox in the treatment of 1 month and the follow-up visit in half a year. Moreover, this therapy has no adverse reaction.

10.Modification of in situ cryopreservation of human bone marrow mesenchymal stem cells.

The study was aimed to evaluate if the modified in situ cryopreservation could affect the biological function of mesenchymal stem cells (MSC) in vitro. Mesenchymal stem cells from human bone marrow were isolated by standard method and characterized with their morphology, cell-surface antigen profile and differentiation repertoire in vitro. The culture-expanded MSC were cryopreserved in situ with culture medium (DMEM-LG) containing 10% D MSO and 30% selected FCS in -70 degrees C. Following recovery of cryopreservation, differentiation to adipocytes, chondrocytes, and osteoblast in vitro and cell cycle analysis were performed to investigate whether the cryopreservation would change the differentiation potential of MSC. The results showed that after recovery of cryopreservation, there was no changes detected as compared with the culture-expanded MSC in both differentiation potency and growth pattern at 12 weeks. In conclusions: this optimized short term in situ cryopreservation at -70 degrees C could retain biological characteristics of human MSC for at least 3 months, and this method may be useful for cryopreservation of hum an bone marrow MSCs.
