1.Effects of iodine excess on TPO and NIS genes mRNA expression in rats

Objective To observe the effects of iodine excess on thyroid morphology,the expression of thyroid peroxidase and sodium iodide symporter mRNA and to explore their mechanisms.Methods One-month SD rats were divided into three groups:control iodine(CI),high iodine Ⅰ(HI Ⅰ)and high iodineⅡ(HI Ⅱ)and were fed with water containing iodine in different concentrations by adding K103(5,5000,10 000μg/L)respectively.Rats were sacrificed after being fed for six months.The morphology of thyroid was investigated under light microscopy and electron microscopy,the serum thyroid hormones and ratio of TPO/β-actin and NIS/β-actin were measured by radio-immunoassay and RT-PCR method.Results The major changes were increased follicles with colloid accumulation in HI groups.The levels of serum thyroid hormones TT3 and TT4 were decreased gradually from CI[(75.68±13.99,1.45±0.49)nmol/L]to HI Ⅰ[(73.82±16.48,1.34±0.31)nmol/L]and HIⅡ groups[(70.65±11.43,1.15±0.39)nmol/L],but there were no significant differences among three groups(F=O.371,l.163,P>0.05).The TPO and NIS mRNA expressions in HI Ⅰ(1.28±0.10,0.56±O.17)and HI Ⅱ(1.14±0.04,0.39±0.06)were significantly lower(F=30.863,62.62.675,P<O.05)than those of control group(1.39±0.08,0.71±0.13).Conclusions Chronic iodine excess leads to definite histological changes in rat thyroid,and inhibits the expressions of TPO and NIS mRNA as well as thyroid hormone synthesis,which in turn acts as a protective mechanism against iodine excess.

Objective To study the expression of p53,p16,PCNA protein in esophageal carcinoma and its relationship to sexual distinction,the location of disease,the biological level,the depth of invasion and lymph node metastasis.Methods 118 patients with esophageal carcinoma were included in the study,all of them were treated for the first time.p53,p16 and PCNA protein in the 118 cases of esophageal carcinoma were detected by immunohistochemical assay(SP technique). Results The positive expression of p53, p16, PCNA protein in 118 patients was 80 %(92/118),42%(50/118)and 97%(115/118),respectively.The positive expression of p53,PCNA protein were irrelated to the sexual distinction,the location of disease,the biological level,the depth of invasion and the lymph node metastasis.The loss of p16 was significantly related to the depth of invasion and the lymph node metastasis(P

Ten compounds were isolated from the leaf of Eucommia ulmoides by means of recrystallization and chromatographic techniques such as D-101 macroporous resin, MCI resin, ODS gel, Sephadex LH-20 and Rp-HPLC. Their structures were identified by NMR spectral analyses as kaempferide 3-O-beta-D-glucoside (1), quercetin-3-O-beta-D-glucoside (2), quercetin (3), quercetin-3-O-beta-D-xylosyl-(1-->2)-beta-D-galactoside (4), kaempferol-3-O-alpha-L-rhamnosyl-(1-->6)-beta-D-glucoside (5), (2S,3S)-taxifolin 3-O-beta-D-glucoside (6) ,4-hydroxy cinnamic acid (7), (+)-cycloolivil (8), pinoresinol beta-D-glucoside (9), squalene (10). Among them compounds 1,5-7,10 were isolated from the Eucommia genus for the first time. In the DPPH free radical scavenging assay, compound 2 exhibited significant activity (IC50 13.7 micromol x L(-1)), compared with vitamin C (IC50 59.9 micromol x L(-1)); compounds 1, 3 and 9 showed moderate activity (IC50 161,137, 214 micromol x L(-1)), compared with 2,6-di-tert-butyl-4-methylphenol (IC50 236 micromol x L(-1)); compound 4 and 6 showed weak activity (IC50 264, 299 micromol x L(-1)).

4.Effects of active components in compatible solution of Sparganii Rhizoma-Curcumae Rhizoma on pelvic adhesion in rats with chronic pelvic inflammatory disease

AIM To study the influence and action mechanism of effective components in compatible solution of Sparganii Rhizoma-Curcumae Rhizoma on pelvic adhesions of model rats with chronic pelvic inflammatory disease.METHODS Laboratory rats were randomly assigned into six groups,a sham-operated group,a model group,a positive Fukeqianjin tablets group,compatible solution groups (40 g/kg high-dose group,20 g/kg medi um-dose group,and 10 g/kg low-dose group).Except rats of the sham-operated group,those in the other groups were induced into models of chronic pelvic inflammatory disease by mechanical bacterial strain implantation,after which a 20 d corresponding drug administration by oral gavage for each group was initiated.After completion of the last drug administration,Verco criterion was taken as the reference to evaluate pelvic adhesions,HE staining was performed on uterine tissues for pathological scoring,ELASA method was applied to measuring serum IL-1β and TNF-α levels,and Western blot method was used to detect the expression levels of proteins FGF-2 and IGF-1 in uterine tissues.RESULTS Compared with the model group,the high-dose compatible solution group had a much sharper fall in Verco scores (P < 0.05),and remarkably decreased serum IL-1β and TNF-α levels (P < 0.01);both high-dose and medium-dose groups manifested with notably alleviated degenerative necrosis and lowered scores of chronic inflammation infiltration and epithelial hyperproliferation (P < 0.01 or P < 0.05),and a distinct reduction in the expression of proteins FGF-2 and IGF-1 (P < 0.05,P < 0.01).CONCLUSION The significant improvement of the chronic pelvic inflammatory disease in model rats due to the effective components in the compatibile solution of Sparganii Rhizoma-Curcumae Rhizoma may be associated with inhibited release of inflammatory mediators IL-1β and TNF-α,and the reduced expression of proteins FGF-2 and IGF-1 as well.
5.Research on output and quality of Panax notoginseng and annual change characteristics of N, P and K nutrients of planting soil under stereo-cultivation.

The output and agronomic characters of 3-year-old Panax notoginseng cultured under stereo structure (upper, middle and down layers) were investigated, and the annual change of N, P and K of its planting soil were also studied. Results showed that, compared with field cultured Panax notoginseng, growth vigour and output of stereo-cultivation were significantly lower. But the total production of the 3 layers was 1.6 times of field. The growth vigor and production of P. notoginseng was in the order of upper layer > middle layer > down layer. The content of ginsenoside in rhizome, root tuber and hair root of P. notoginseng was in the order of upper layer > field > middle layer > down layer. Organic matter content and pH of stereo-cultivation soil decreased with the prolonging of planting time, which with the same trend of yield. Organic matter content of stereo-cultivation soil was significantly higher than field, but the pH was significantly lower. Contents of total and available N, P and K in stereo-cultivation soil and field decreased with the prolonging of planting time. The content of N and P were in the order of upper layer > middle layer > yield > down layer, the content of K was in the order of upper layer > middle layer > down layer > yield. Compared with field, the proportion of N and P in the organ of underground (rhizome, root tuber and hair root) of upper layer were increased, while decreased in middle and down layers. Proportion of K in underground decreased significantly of the 3 layers. In conclusion, the agronomic characters and production of stereo-cultivation were significantly lower than that of yield. But the total production of the 3 layers were significantly higher than field of unit area. And the aim of improving land utilization efficiency was achieved. Nutritions in the soil of stereo-cultivation were enough to support the development of P. notoginseng, which was not the cause of weak growth and low production. The absorbing ability of P. notoginseng to N, P and K nutrients was decreased by stereo-cultivation mode. So, improve the growth vigour of P. notoginseng from the perspective of adjusting the stereo-cultivation mode so as to improve the nutrient absorption capacity is the future direction.

6.Chemical constituents from root of Actinidia chinensis.

OBJECTIVETo study the chemical constituents from the root of Actinidia chinensis.
METHODThe roots of A. chinensis were extracted with 95% EtOH. The EtOH extract was suspended in H2O and extracted with petroleum ether, EtOAc and BuOH successively. The compounds were isolated with column chromatography from the EtOAc fraction, and elucidated on the basis of spectral analysis(MS, IR, 1H-NMR, 13C-NMR).
RESULTSeven compounds were isolated from the root of A. chinensis, and the structures were identified as 2alpha-hydroxyoleanolic acid (1), 2alpha-hydroxyursolic acid (2), euscaphic acid (3), 23-hydroxyursolic acid (4), 3beta-O-acetylursolic acid (5), ergosta4, 6, 8, (14), 22-tetraen-3-one (6), beta-steriol (7).
CONCLUSIONAll the compounds were obtained from the root of A. chinensis for the first time.

7.Change of the thickness of macular inner and outer retina in patients with chronic primary angle-closure glaucoma

Objective To measure the thickness of macular inner and outer retina of chronic primary angle-closure glaucoma (CPACG) and observe its features.Methods Together 58 patients (58 eyes) including 14 patients with early CPACG,23 patients with moderate CPACG and 21 patients with severe CPACG were recruited as early CPACG group,moderate CPACG and severe CPACG group,respectively;additional 23 healthy volunteers were chosen as controls.The thickness of macular inner and outer retina at all parts were measured by RTVue-100 SD-OCT.Results The macular inner retinal thicknesses in fovea were (135.62 ± 2.96) μm,(124.21 ± 6.47) μm,(119.74 ± 10.67) μm and (94.95 ± 11.24) μm in the control group,early CPACG,moderate CPACG and severe CPACG group,respectively.The thicknesses of macular inner retina of fovea were significantly decreased in the early CPACG group,moderate CPACG group and severe CPACG group when compared with the control group (all P < 0.05).There were also significant differences between early CPACG group and moderate CPACG group,severe CPACG group and moderate CPACG group and early CPACG group and severe CPACG group (all P < 0.05).The thicknesses of macular outer retina of fovea,parafovea and perifovea were significantly decreased in the severe CPACG group compared with the control group (all P < 0.05).Conclusion The thickness of macular inner and outer retina are decreased at all parts in patients with the severe CPACG.The macular inner retinal thickness only in the fovea is thinner in the patients with the early CPACG.
8.Effect of aerocyst-urethral catheter on preventing perianal skin lesions in critical ill patients with fecal incontinence

Objective To study the effects of aerocyst-urethral catheter on preventing perianal skin lesions in critical ill patients with fecal incontinence.Methods Totals of 60 patients were randomly divided into three groups,first experimental group indwelled aerocyst-urethral catheter and second experimental group used skim cotton laps to clamped,while control group received cleaning after they defecated.Results The incidence of perianal skin injury in first experimental group was 10.0%,second experimental group was 30.0% and control group was 80.0,difference between three groups was statistically significant ( P < 0.01 or P < 0.05 ).Conclusions Applying aerocyst-urethral catheter in critical ill patients with fecal incontinence is a new and better nursing method,which can effectively prevent eczema and bedsore happening and be worth clinical spread application.
9.Effect of continuous nursing support on breast cancer patients with radical mastectomy

Objective To investigate the effect of the continuous care support intervention on breast cancer patients with radical mastectomy.Methods Totals of 107 patients undergoing elective radical mastectomy were randomly divided into intervention group ( n =54 ) and control group ( n =53 ).Both of two groups of patients were treated on the basis of conventional care according to the doctors' advice before and after surgery.The intervention group received the continuous individual nursing intervention model,included preoperative interactive support intervention,postoperative pain assessment support intervention and rehabilitation support intervention after discharge. Intervention effects of two groups were observed and compared. Results The anxiety,positive response and negative response of two groups patients were no significant differences before intervention (P>0.05).After intervention,the scores of anxiety,positive response and negative response in intervention group respectively were (35.28 ± 5.43 ),(39.82 ±4.03 ) and (25.61 ± 5.12),and that of control group were (42.73 ± 6.72),(33.15 ± 5.62) and (31.75 ± 6.38),and the difference was statistically significant ( t =6.313,7.065,5.495,respectively; P < 0.01 ).The duration of postoperative different period pain score,first off the bed time,sleep time,upper extremity swelling and hospitalization days were statistically significant between two groups (P <0.01 ).And the postoperative recovery state of upper limb function after six months,and life quality score after one year in intervention group were significantly better than that of control group ( P<0.01 ).Conclusions Continuous individualized specialized nursing intervention support model can promote physical function in patients with early recovery and improve quality of the prognosis life and the treatment outcome.
10.A 12-month prospective survey of perinatal outcome of liveborn neonates in Julu County, China.

BACKGROUNDPopulation based epidemiologic study on the main diseases and birth status of liveborn neonates remains scarce in China, especially in rural areas where a large number of neonates are born. The aim of this study was to establish an epidemiological basis of live births in Julu County, a representative of the northern and mid-western parts of China in terms of demography, disease pattern and women and children's health care infrastructure.
METHODSThe perinatal data of all live births were prospectively collected in three participating county-level hospitals from September 1, 2007 to August 30, 2008.
RESULTSThere were 5822 live births in these hospitals. Among all live births, 53.7% were male and 4.5% were born prematurely. Mean (SD) birth weight (BW) was (3348 ± 503) g. The low (< 2500 g) and very low BW (< 1500 g) infants accounted for 3.8% and 0.5% of the total births, with 6.5% as small for gestational age and 2.8% as multi-births. Cesarean section rate was 30.2%, of which 68.6% were elective. There were 745 infants (12.8% of the live births) admitted to local neonatal wards within 7 days of postnatal life, in which 48.3% and 19.3% were due to perinatal asphyxia and prematurity, respectively. The incidences of perinatal aspiration syndrome, transient tachypnea and respiratory distress syndrome were 4.9%, 0.6% and 0.5%, respectively. Neonatal mortality was 7.6‰ (44/5822), with 16 in delivery room and 28 in neonatal ward before discharge.
CONCLUSIONSThis study provided a population-based perinatal data of live births and neonatal mortality in a northern China county with limited resources. Neonatal disorders related to perinatal asphyxia remain a serious clinical problem, which calls for sustained education of advanced neonatal resuscitation and improvement in the quality of perinatal-neonatal care.
