The application of Standardized patient(SP) in Chinese clinical teaching is still in its infancy.The experience of SP in respiratory medicine teaching is also very limited.This article discusses the experience in applying SP in respiratory medicine teaching and several issues of SP training that should he paid attention to,and hrings forward our expectation in SP training in China.
2.YKL-40-induced IL-8 expression from bronchial epithelium leads to bronchial smooth muscle proliferation and migration

Objective:To investigate YKL-40-mediated inflammation in human bronchial epithelial cells and analyzed the soluble factors secreted by bronchial epithelial cells exposed to YKL-40 that were responsible for increasing proliferation and migration of primary normal human bronchial smooth muscle cells (BSMCs).Methods:YKL-40-induced inflammation was assayed in two human bronchial epithelial cells (BEAS-2B cell line and primary human bronchial epithelial cells ,namely HBECs).In addition,we treated BEAS-2B cells and HBECs with YKL-40,and added the conditioned culture media ( YKL-40-BEAS-2B-CM) and ( YKL-40-HBECs-CM) to BSMCs.The proliferation and migration of BSMCs were determined by premixed WST-1 cell proliferation reagent and QCM chemotaxis migration assay ,respectively.Results: Bronchial epithelial cells treated with YKL-40 resulted in a significant increase of IL-8 production,but have no effect about RANTES ,Eotaxin and TNF-α.YKL-40-BEAS-2B-CM and YKL-40-HBECs-CM induced IL-8 was found to further stimulate proliferation and migration of BSMCs ,and the effects were inhibited after neutralizing IL-8.Conclusion:Through investigating the interaction of airway epithelium and smooth muscle ,our findings implicate that YKL-40 may be involved in the inflammation of asthma by induction of IL-8 from epithelium,subsequently contributing to BSMCs proliferation and migration.Moreover, inhibition of IL-8 signaling is a potential therapeutic target for YKL-40-induced inflammation and remodeling of asthma.
4.Attempts in building a smart healthcare system at a research hospital

Described in the paper are contents and characteristics of building smart healthcare system at a research hospital,and practices of the hospital in exploring smart healthcare.Specific measures include staged building of an e-hospital emphasizing information security; closed-loop transformation with mobile and barcode technology for higher efficiency and patient safety;optimizing the service flow with smart interconnection to create an orderly outpatient environment; implementing healthcare regulations with information systems to ensure a homogeneous medical environment;and integrating the entire medical system to promote coordinated development of regional healthcare service. These measures help upgrade the information service effectiveness and quality of care of the hospital.
5.Effect of cetuximab combined with adriamycin on proliferation and apoptosis of triple negative breast cancer cells

Aim To detect the effects of cetuximab combined with adriamycin on the proliferation and ap-optosis of triple-negative breast cancer cells.Methods Cell viability was evaluated by MTT assay.The cell apoptosis was analyzed by flow cytometry with propidi-um iodide staining.JC-1 staining was used to deter-mine mitochondrial membrane potential.The expres-sions of glucose regulated protein78 (GRP-78),Bcl-2 and Caspase-3 were measured with Western blot.Re-sults MTT assay showed that cetuximab had inhibi-tion effect on the breast cancer cell MDA-MB-231 growth,and the effect was related to concentration of drug.The inhibition effect of adriamycin on MDA-MB-231 had remarkabe relationship with time and concen-tration.When combined with each other,they could re-markably increase inhibition effect.The viability of cells in combination group for 1 2 h,24 h,48 h,sig-nificantly lower than that in cetuximab or adriamycin group (P <0.05 or P <0.01 ).Apoptosis results showed that cell apoptosis was significantly increased when cetuximab combined with adriamycin,reached (43.93 ±3.59)% for 24 h,had remarkably statistical significance compared to cetuximab or adriamycin
group (P <0.01 ).JC-1 staining indicated that cetux-imab or adriamycin could reduce the mitochondrial membrane potential,but the reduction effect was more remarkable in the combination group.Western blot re-vealed that cetuximab could reduce the expression of GRP-78 and Bcl-2,and increased the expression of Caspase-3 and its activity.The expressions of Bcl-2, Caspase-3 had no significant change in adriamycin group,but GRP-78 was increased.In combination group,the expression of GRP-78 and Bcl-2 was signifi-cantly decreased,but Caspase-3 was increased nota-blely compared to adriamycin group.Conclusions The combination of cetuximab and adriamycin enhances the inhibition effect on the triple-negative breast cancer MDA-MB-231 cells,and increases cell apoptosis.The mechanism may be that cetuximab reduces the endo-plasmic reticulum stress level,then activates the mito-chondrial pathways by decreasing the expression of Bcl-2,reducing the mitochondrial membrane potential,and promoting cell apoptosis.
6.Volatile Oil Constituents of Two Acremonium Endophyte Isolates from Aquilaria sinensis

The volatile oils of endophyte isolates R1 (Acremonium sp.) and R2 (Acremonium sp.) from Aqui-laria sinensis were extracted by solvent method. Twenty and sixteen ingredients were identified from the volatile oils of R1 and R2 by GC-MS, respectively. Fatty acids, such as oleic acid, linoleic acid, hexa-decanoic acid etc., were the main components, whereas terpenoids like 1,8-cineole, zingiberene, ar-curcumene were also found in both oils. It’s the first report about the volatile oil constituents of endophyte isolates from A. sinensis.
7.The prevalence of congenital heart disease in students of Liangshan prefecture in Sichuan province

Objective To investigate the congenital heart disease (CHD) prevalence of students in Liangshan prefecture in Sichuan province,and to explore the risk factors of CHD.Methods Stratified cluster sampling was conducted in collecting demographic data.Heart auscultation was used to screening the CHD,and colour Doppler flow imaging was used to diagnose the CHD.Results A total of 10 021 student were collected in this study.About 19.74% of total cases had cardiac murmur.Sixty-eight students were diagnosed as CHD through colour Doppler flow imaging,accounting for 0.68%.In addition,the top three ranks of CHD were atrial septal defect,patent ductus arteriosus,interventricular septal defect.Our study also found that per capita income of CHD families was lower than non-CHD families,meanwhile,the CHD prevalence of Yi nationality was higher than other nationalities.There was no statistical significance in age,gender,height,weight,body mass index,head circumference,waist circumference between different types of CHD.Conclusions The CHD prevalence of students in Liangshan prefecture has been in a high level in China; therefore we need pay more attention on screening for the CHD.
8.Different metabolites of leaves between Tripterygium wilfordii and Tripterygium hypoglaucum based on UPLC-Q-TOF-MS.

To analysis the differences between Tripterygium wilfordii and T. hypoglaucum, specimens of their leaves were collected from five production regions and analyzed by ultra performance liquid chromatography coupled with quadrupole time of flight mass spectrometry (UPLC-Q-TOF-MS). The data were analyzed by multivariate statistical method, such as hierarchical cluster analysis (HCA) principal component analysis (PCA) and orthogonal signal correction partial least square discrimination (OPLS-DA). Potential markers with VIP values above 5.0 and corresponding r values above 0.85, were selected and further tested by combining mann-Whitney nonparametric. Those with P < 0.001 and AUC = 1 were confirmed as metabolite markers to discriminate them from each other. Results revealed that the two species were obviously different in their leaf metabolites. Based on their mass spectra, 23 potential metabolite markers were identified to distinguish T. wilfordii from T. hypoglaucum.

9.Study on Cytotoxic Activity and Chemical Constitunents of Marine Actinomycets Strain 124092

The hexane extract from marine actinomycetes 124092 showed potent inhibition on B16 cell line by MTT assay. The hexane extract was fractionationed on silica gel column by vacuum liquid chromatography to afford 6 fractions(Fr1~Fr6), and Fr6 showed cytotoxic activity. To determine the bioacitve components of hexane extract, Fr6 was analyzed by GC/MS. The main components were identified as palmitic acid (11.76%), oleic acid (12.16%), linoleic acid (14.77%), and lactobacillic acid (61.31%). It have been reported that palmitic acid, oleic acid, and linoleic acid possess cytotoxic activity on rat ascites tumor cells and linoleic acid have suppressive effect on human lung adenocarcinoma cells.
10.Isolation and Primary Identification of Endophytic Fungi from Cephalotaxus hainanensis L.

Seventy-two strains of endophytic fungi were isolated from the healthy bark, branches and leaves of Cephalotaxus hainanensis L.. Sixty-eight of them were morphologically classified into Fungi Imperfecti, thirty-three sporulated were identified to five genera. For those did not sporulate, one was identified to Rhizoctonia sp., the rest were tentatively classified into Mycelia Sterilia. Four were identified to Basidiomycetes. The result indicated the endophytic fungi of C. hainanensis show a degree of tissue specificity. There were significant differences about the quantity, genera and composition between the fungi isolated from bark and those from branches and leaves.
