1.Ret rearrangement mutation in patients with papillary thyroid carcinoma

Objective To investigate the association of ret gene rearrangement mutation with the pathogenesis of papillary thyroid carcinoma (PTC). Methods Twenty-seven cases of PTC were analysed for expression of ret gene rearrangement (ret/PTCs) by multiplex-PCR at first, and then ret/PTC1-3 were identified by identification-PCR (ID-PCR). Finally, the specific ret/PTC was affirmed by automated direct sequencing. Ten specimens of malignant thyroid tissues of other histological types, 33 benign thyroid lesions and 30 normal thyroid specimens beside tumor (as the control) were also included. Results (1) Fifteen samples showed positive ret/PTCs, 11 of which harboured ret/PTC1,3 were positive for ret/PTC3, and 1 for ret/PTC2. All the rearrangements were clearly identified by automated direct sequencing of ID-PCR products. (2) All the 15 ret/PTC-positive tissue samples were histologically confirmed to be PTC. The prevalence of the tumor-specific ret rearrangements in 27 patients with PTC is 55.6% (15/27). 4.Changes of the elastic fibers and collagen fibers during the development and progression of experimentally induced tongue carcinoma in hamsters.

OBJECTIVETo investigate the relationship between the extracellular matrix (ECM) and neoplastic progression in hamster with tongue cancer.
METHODSForty-eight specimens of hamster tongue cancer were divided into control group (n=6) and experimental group (n=42). The pathological grade of the specimens was assessed (including 3 stages, namely atypical hyperplasia, carcinoma in situ and early invasive carcinoma). The sections of the tongue were stained with Masson and aldehyde-fuchsin (AF) staining for microscopic observation of the elastic fiber and collagen fiber changes.
RESULTSWithin the connective tissue cores (CTC) of the papillae in the control group was a framework of numerous and fine Gomrori's aldehyde fuchsin-positive elastic fibers. But in the stages of dysplasia and carcinoma in situ, these elastic fibers decreased and further diminished in the CTC in early invasive carcinoma. In dysplasia and carcinoma in situ stages, most of the elastic fibers collapsed with scattered elastic fibers, and the elastic fibers decreased significantly in early invasive carcinoma. The control group showed a significantly greater number of elastic fibers in the experimental group. The collagen fiber was obviously increased and irregularly arranged in dysplasia and carcinoma in situ stage; in early invasive carcinoma, the collagen fibers became thicker with deposition in the lamina propria.
CONCLUSIONAn excessive deposition of collagen fiber and reduction of the elastic fibers is an important factor contributing to the development of tongue carcinoma in hamsters.

5.Study on content of two sesquiterpenes in Alpinia oxyphylla.

OBJECTIVETo develop an HPLC method for determining oxyphyllenodiol A (1) and teuhetenone A (2) contained in Alpinia oxyphylla and to compare the contents of the two components contained in medicinal materials and prepared herbal medicines in pieces sold in the market and different fractions.
METHODHPLC and Waters sunfire C18 column (4. 6 mm x 250 mm, 5 microm) were adopted for gradient elute with the mobile phase of acetonitrile and water. The flow rate was 1.0 mL x min(-1) and the detection wavelength was 250 nm.
RESULT1 and 2 showed a good linear relationship within the range of 0.1296 - 0.8640 microg and 0.1635 - 1.0900 microg respectively, with the average recoveries of 99.08% and 97.80%. Their content ranges were 0.0059% - 0.0149% and 0.0080% - 0.0164% in different samples. The mean value of 1 and 2 were 0.0085% and 0.0104% in the whole fruits, and 0.0137% and 0.0157% in the seeds. They were undetected in the nutshells.
CONCLUSIONThe method is so precise, accurate and highly reproducible that it can be used to determine the contents of oxyphyllenodiol A and teuhetenone A in A. oxyphylla. The contents of the two components are mainly extracted from the seeds, with certain difference among different samples. There are a higher contents and no significant difference in the salted and raw seeds.

6.Multidrug-resistant Clinical Enterobacter cloacae Isolates: Analysis of Their ESBLs Gene Types and Molecular Epidemiology

OBJECTIVE To understand the distribution and epidemiology of ESBLs in ceftazidime or cefotaxime-(resistant) clinical Enterobacter cloacae isolates.METHODS Twenty seven ceftazidime or cefotaxime-resistant(nonrepetitive) E.cloacae were collected from 27 patients hospitalized at the Huashan Hospital,Shanghai.PCR and(sequencing) were performed to understand the distribution of ESBLs in E.cloacae;rep-PCR was(performed) to(understand) the epidemiology of ESBLs in E.cloacae.RESULTS CTX-M-3 like(ESBLs) were the most prevalent in our study(48%);this was the first report of VEB-1-like ESBLs in the member of(Enterobacteriaceae) in China,and the first report of the ESBLs VEB-1-like and CTX-M-3-like in an isolate simultaneously;the majority of(ESBLs) producers exhibited the same rep-PCR pattern,but harbored different ESBLs gene.(CONCLUSIONS) In our study,ESBLs have become prevalent in clinical E.cloacae isolates,and become an important factor of E.cloacae isolates resistant to extended-spectrum beta(-lactams).
7.Effect of continuous nursing support on breast cancer patients with radical mastectomy

Objective To investigate the effect of the continuous care support intervention on breast cancer patients with radical mastectomy.Methods Totals of 107 patients undergoing elective radical mastectomy were randomly divided into intervention group ( n =54 ) and control group ( n =53 ).Both of two groups of patients were treated on the basis of conventional care according to the doctors' advice before and after surgery.The intervention group received the continuous individual nursing intervention model,included preoperative interactive support intervention,postoperative pain assessment support intervention and rehabilitation support intervention after discharge. Intervention effects of two groups were observed and compared. Results The anxiety,positive response and negative response of two groups patients were no significant differences before intervention (P>0.05).After intervention,the scores of anxiety,positive response and negative response in intervention group respectively were (35.28 ± 5.43 ),(39.82 ±4.03 ) and (25.61 ± 5.12),and that of control group were (42.73 ± 6.72),(33.15 ± 5.62) and (31.75 ± 6.38),and the difference was statistically significant ( t =6.313,7.065,5.495,respectively; P < 0.01 ).The duration of postoperative different period pain score,first off the bed time,sleep time,upper extremity swelling and hospitalization days were statistically significant between two groups (P <0.01 ).And the postoperative recovery state of upper limb function after six months,and life quality score after one year in intervention group were significantly better than that of control group ( P<0.01 ).Conclusions Continuous individualized specialized nursing intervention support model can promote physical function in patients with early recovery and improve quality of the prognosis life and the treatment outcome.
8.Investigation and analysis of nurses' cognitive status on administration nursing risks among nurses from general hospital of Wuhan city

Objective To understand nurses' cognitive status about administration nursing risks and provide basis for management system of administration risks.Methods A total of 356 clinical nurses from three general hospitals in Wuhan were selected by convenience sampling,and investigated by self-designed recognition questionnaire on nurses' administration nursing risks.Results 72.0% of the nurses considered there was great responsibility during administration process and risks existed,and only 12.4% of the nurses believed administration risks could be avoided.Knowledge about administration risks was insufficient,the score of laws and regulations was (1.45 ± 0.37) and the score of professional medication knowledge was (1.56 ± 0.56).Nurses' self-behavior evaluation was also not optimistic,the score of correct administration was (1.83 ± 0.35),reasonable administration was (1.57 ± 0.35),and the adverse drug reaction was (1.43 ± 0.36).Conclusions Nurses should be trained on related laws and regulations and professional knowledge of drugs,so as to improve their prevention awareness and identification ability of administration risks,as well as to strengthen risk prevention and control of rational administration and adverse drug reactions.
9.Gastric carcinoma with osteoclast-like giant cells: a case report and review of the literature.

Gastric carcinoma with osteoclast-like giant cells (OGCs) is an extremely rare tumor. So far, only six cases have been reported in the literature. Here we report an additional case of this tumor in a Chinese 78-year-old man presented with abdominal pain, vomiting, and hematemesis. Physical examination and gastroscopy revealed a tumor in the gastric antrum. The biopsy and pathological findings indicated a gastric adenocarcinoma with OGCs, which were present in both the tumor and the metastatic lymph nodes. Further immunohistochemical staining indicated that OGCs were reactive with CD68, CD45, and vimentin protein, but not with pancytokeratin, carcinoembryonic antigen, or epithelial membrane antigen, suggesting the monocytic/histiocytic derivation of these OGCs. In situ hybridization for Epstein-Barr virus showed no nuclear positivity in either adenocarcinoma or OGCs. Postoperative follow-up showed that the patient had survived for at least 6 months without recurrence. Further investigation is warranted to clearly define the prognostic significance of OGCs in gastric carcinoma.

10.Influence of combined warming measure on intraoperative temperature and coagulation function in patients during gynecologic laparoscopic surgery

Objective To explore the influence of combined warming measure on intraoperative temperature and coagulation function in patients during gynecologic laparoscopic surgery. Methods A total of 70 patients undergoing gynecologic laparoscopic surgery were randomly divided into the intervention group ( n =35 ) and the control group ( n = 35 ). The intervention group received combined warming measure including using inflatable warming blanket, beating transfusion and rinses, while the regular nursing was performed in the control group. The core body temperature of the patients were recorded before operation, 1 hour after the beginning of operation and at the completion of operation. The amounts of blood loss were recorded as well. The index of coagulation function before and after operation were detected. Results Compared with the intervention group, the core body temperature 1 hour after the beginning of operation and at the completion of operation were lower; the amounts of blood loss were larger; the thrombin time prolonged; the fibrinogen and peripheral blood platelets were lower in the control group. There were statistical meanings in all of the defferences ( P < 0. 05 ).Conclusions Combined warming measure can effectively prevent intraoperative hypothermia, improve the coagulation function disorders and reduce the amounts of blood loss. The method can be used in clinic widely.
