1.The value of p16 and death-associated protein kinase methylation and autofluroscene spectrum analysis of gastric juice in diagnosing gastric carcinoma

Objective To explore the feasibility of detecting death-associated protein(DAP) kinase promoter hypermethylation in the tumor tissues and plasma of patients with gastric adenocarcino- ma,and to evaluate the effect of DNA hypermethylation and autofluroscene spectrums of gastric juice on diagnosing gastric carcinoma.Methods Primary tumor tissues and plasma and gastric juice of 50 patients with gastric adenocarcinoma were collected.Gastric mucosa tissue,plasma and gastric juice of 20 patients with chronic superficial gastritis and 20 patients with benign gastric ulcer and 30 patients with chronic atrophic gastritis were collected as controls.After sodium-bisulfite treatment,extracted DNA was amplified for DAP kinase promoter hypermethylation by methylation-specific polymerase chain reac- tion.At the same time the gastric juice autofluroscene spectrums(the excitation wavelength was 288 nm,whereas the range of emission wavelength was 300-800 nm)were detected.Results Among the samples from 50 patients,p16 and DAP kinase hypermethylation were detected in 74.4% and 68.1% of tumor tissues,52.0% and 58.0% of plasma,58.6% and 76.0% of gastric juice.No hypermethylation was detected in samples of chronic superficial gastritis and serum of patients with gastric ulcer.Among the patients with gastric ulcer,p16 and DAP kinase hypermethylation were detected in 10.0% and 20.0% of tissues,5.0% and 15.0% of gastric juice.Among the samples from 30 patients with chronic atrophic gastritis,p16 and DAP kinase were detected in 10.0% and 23.3% of tissues,3.3% and 3.3% of sera,3.3% and 20.0% of gastric juice.The intensity of gastric juice autofluroscence spectrums of patients with gastric carcinoma was much higher than those in controls.The sensitivity of p16 and DAP kinase hypermethylation and autofluorescence spectrums together were 95.6% and 97.8% respectively.Con- clusions Promoter hypermethylation of the p16 and DAP kinase gene detected in plasma and gastric juice consists with that in primary tumor tissues of patients with gastric adenocarcinoma.The combination of DNA hypermethylation and autofluorescence spectrum has a good future in diagnosing gastric carcinoma.
2.Effect of constraint-induced movement therapy on hemiplegia in children

ObjectiveTo explore the effect of constraint-induced movement therapy on hemiplegia in children.Methods30 hemiplegia children caused by cerebral palsy or trauma were divided into 2 groups.Treatment group accepts the Constraint-Induced Movement Therapy (CIMT) combined with Bobath approach. The control group accepts the Baboth approach only. Before after 12-week treatment, Ueda Test and the Brunnstrom's test were used to classify every stage of the recovery. ResultsBoth groups improved their motor function assessed either with Ueda Test or Brunnstrom's test, which was better in treatment group than in control group.ConclusionThe CIMT with Bobath approach together can remarkably raise the rate of utilization of the hemiplegia suffering limbs as well as improve the learned non-use of the suffering parts.

4.Significance of B cell activating factor of TNF family promoter polymorphisms in patients with immune thrombocytopenic purpura.

The study was aimed to examine the B cell activating factor promoter polymorphism of the TNF family (BAFF)-871 C/T in patients with immune thrombocytopenic purpura (ITP) and to explore its correlation with ITP and the relationship between the blood platelet count of newly diagnosed patients with ITP and genotypes of BAFF-871 C/T polymorphisms. Alleles specific polymerase chain reaction (ASP-PCR) and agarose gel electrophoresis were used to identify polymorphisms -871 C/T of BAFF promotor in 133 ITP patients and 117 healthy controls, and determine the genotype of subjects. Meantime, the frequency of genotype and alleles were analyzed. The results indicated that out of 133 patients with ITP, 33.1% patients exhibited C/C, 42.1% patients were heterozygous C/T, and 24.8% patients were homozygous T/T. The corresponding frequencies in 117 healthy controls were 55.6% C/C, 33.3% C/T and 11.1% T/T. The allele frequency of T in ITP patients and healthy controls were 45.9% and 27.4% respectively. There was significant difference in the BAFF-871 C/T genotypic frequency between the ITP patients and healthy controls (p < 0.05). The allele frequency of T in ITP patients was higher than that in healthy controls. There was no significant difference in the blood platelet counts between the various genotype (p > 0.05). It is concluded that the polymorphism -871 C/T of BAFF promoter is correlated with the pathogenesis of ITP. However, there is no significant difference in blood platelet counts between the various genotype, so the polymorphism -871 C/T of BAFF promoter can not be referred as the analysis index for evaluating the severity of ITP.

5.Women with poor response to ovarian stimulation have increased follicular bone morphogenetic protein-15 levels.

OBJECTIVETo evaluate the levels of bone morphogenetic protein-15 (BMP-15) in human follicular fluid (FF) and its association with response to ovarian stimulation.
METHODSWestern blotting was performed to determine the levels of BMP-15 in FF obtained from follicle aspirates in 70 patients undergoing IVF treatment. According to the response to ovarian stimulation the patients were divided into poor responder group and normal responder group.
RESULTBMP-15 levels in FF of poor responders were significantly higher than those in normal responders (1.01 +/- 0.34 vs 0.77 +/- 0.24, P<0.01).
CONCLUSIONIncreased levels of BMP-15 in FF may be associated with poor response to ovarian stimulation.

6.Cathepsin D expression in ovaries from polycystic ovarian syndrome patients.

OBJECTIVETo investigate the expression of cathepsin D in ovary of patients with polycystic ovarian syndrome (PCOS).
METHODSWestern blot was performed to detect the expression of cathepsin D and immunohistochemistry was used to detect the protein distribution in ovarian tissue.
RESULTSemi-quantity values of cathepsin D expression in PCOS and control group were 2.06 +/- 0.39 and 4.76 +/- 1.43 (P<0.05), respectively. Immunostaining for cathepsin D was obvious in both follicles and stromal cells, and the strongest immunostaining was seen in granulosa cells of follicles. Immunochemical study showed the protein was mainly located on the cytoplasm and cell membrane.
CONCLUSIONCathepsin D expression is down-regulated in ovaries of PCOS patients, which may provide a clue for the abnormality of follicle development in PCOS.

7.Value of endoscopic retrograde cholangiopancreatography in pancreaticobiliary maljunction

Objective:To explore the clinical characteristics of pancreaticobiliary maljunction (PBM) and its disease spectrum, and to evaluate therapeutic endoscopic retrograde cholangiopancreatography (ERCP).Methods:Data of 52 PBM patients who received therapeutic ERCP procedures for abdominal pain, jaundice and fever in Department of Gastroenterology of Peking University Third Hospital from June 2006 to March 2021 were collected. The clinical characteristics, typing, the change of disease spectrum and ERCP procedures were analyzed.Results:Among 52 PBM patients, female was more common. Abdominal pain and jaundice were the most common clinical manifestations, among which 20 were type Ⅰ, 25 type Ⅱ and 7 type Ⅲ. Half patients had the choledochal cyst. The mean timespan from the first onset to the final diagnosis was 12.2 years. Twenty-four cases (46.2%) had changes in PBM disease spectrum. Among 69 ERCP procedures, 5 (7.2%) failed. Difficult cannulation rate was 34.6% (18/52), and 11 patients underwent advanced cannulation techniques, while it was 15.4% (657/4 275) in the conterpart non-PBM patients in the same period, with significant difference between them ( χ2=14.455, P<0.05). Multiple therapeutic ERCP techniques including endoscopic sphincterotomy, pancreatic stent placement, removal of stones from the duct were applied with the successful rate of 92.8% (64/69). The incidence of post-ERCP pancreatitis was 15.4% (8/52). Conclusion:The chief clinical problem may be changed over time in PBM patients. Although ERCP plays an important role in PBM and its disease spectrum, there may be a higher rate of difficult cannulation and postoperative complications.
8.Work scope of Chinese dental nurses:a qualitative study

Objective To establish the work scope of Chinese dental nurses and provide theory evidence for improving the establishment of specialized education system and certification .Methods Semi-structured in-depth interviews were conducted to fifteen experts .The data was analyzed via qualitative research to establish the work scope of Chinese dental nurses .Results Work scope of dental nurses was established which contained four dimensions as patients management , clinical cooperation , clinic management and prevention and control of hospital infection;the scope also was divided into two stages: foundation certification and specialist development .Conclusions The work scope would play an important role in improving the establishment of specialized education system and certification .
9.Performance comparison of AH plus and zinc oxide clove oil paste root tip seal:a Meta-analysis

Objective To comparative evaluation of root canal filling paste AH-plus and zinc oxide root canal of clove oil seal performance. Methods By computer, we retrieved PubMed, CNKI and controlled clinical trial databases to find out the standard randomised controlled trial, and literature retrieval time from building library until October 20, 2014. According to the Cochrane literature quality evaluation method of system for the evaluating methodlogical quality of the study, and then we extracted the relevant data and analyzed by Rev Man 5. 1 software for Meta analysis. Results Eventually the study included in 23 articles of randomized controlled trials, and a total of 4104 teeth. The results showed the pain occurred after plus treatment of AH paste group which was lower than that in the control group [OR=0. 32, 95%CI(0. 26,0. 26), P<0. 01);AH plus paste group cure rate was higher than that of the control group after intervened 1 -2 years [ OR=2. 61, 95%CI(2. 03, 2. 03), P<0. 01];the paste resin class root filling AH plus length of root tip leakage was better than that of the control group, and nail polish and wax experiment method of measuring the penetration length of the tooth had statistical significance [MD=1. 28,95%CI( -1. 48,-1. 08), P<0. 01]; India ink staining solution to measure the penetration length of the tooth had statistical significance [MD=0. 82, 95% CI(-0. 90,-0. 74),P<0. 01]. Conclusions AH plus paste in root canal therapy can significantly alleviate pain, and the long-term curative effect is good, and better than biological intermiscibility, which is an ideal root canal filling material. Due to large incorporated in the analysis of the research and the quality difference, it is necessary for large sample, multicenter, randomized, double-blind, placebo-controlled trails to further confirm the clinical curative effect.
10.On nurse-physician collaborative behavior in stomatological hospitals

Objective To investigate and compare collaborative behavior between physicians and nurses in stomatological hospitals, and factors influencing nurse-physician collaboration. Methods Physicians and nurses were surveyed by Nurse-Physician Collaboration Scale ( NPSC) . Factors influencing nurse-physician collaborative level were screened out by analyzing various factors with ANOVA.Results The difference of total collaborative behavior scores between physicians and nurses was not significant ( P>0.05);among the 21 items in 3 dimensions of “information exchange among patients”,“joint participation in decision-making of treatment and nursing” and “relationship between nurses and physicians”, 7 items showed differences between the two groups ( P<0. 05 ); better nurse-physician collaborative behavior appeared between physicians with extrovert character and emotional stability, and nurses with emotional stability, 3-5 years of working and professional title as registered nurse.Conclusions The difference of nurse-physician collaborative behavior in stomatological hospitals was not significant;however, education and training of collaborative behaviors, such as nurse-physician relationship, dealing with complicated situation, etc. still need to be improved; hospital managers should build a positive collaborative environment, so as to promote team cooperation between nurses and physicians.
