1.Efficacy of treating the early-stage chronic angle closure glaucoma with laser peripheral iridoplasty

AIM: To evaluate the safety of the laser peripheral iridoplasty ( LPIP ) for the early-stage chronic primary angle-closure glaucoma ( ECPACG) .
METHODS:Sixty-five eyes of 36 patients with ECPACG received LPIP. At preoperative and postoperative, patients were examined with intraocular pressure ( IOP ) , anterior chamber, optical coherence tomography ( OCT) , visual acuity, visual field, fundus and complications. The mean follow-up was 18. 2±6. 7mo (ranged 12~24mo).
RESULTS: The preoperative average IOP were 23. 8±5.6mmHg, postoperative 1wk, 1, 3, 6, 12mo and in the last follow-up time the average IOP were 18. 7±3. 8, 17. 9±3. 2, 17. 6±3. 5, 18. 4±3. 7, 18. 6±3. 7, and 18. 6±7. 8mmHg. There was statistical difference comparing with preoperative (P<0. 05, decreasing average 6. 5±3. 1mmHg compared with the preoperative, the difference was statistically significant (t=5. 32, P<0. 05). Postoperative 1wk, 1, 3, 6mo, the trabecular-iris angle ( TIA500 ) and the angle opening distance at 500μm ( AOD500 ) had the statistical difference comparing with preoperative ( P <0.05). At Postoperative 1a and the last follow-up time, the TIA500 and AOD500 did not have statistical difference comparing with preoperative (P>0. 05). The postoperative visual acuity, visual field, fundus had not changed. There were no complications found.
CONCLUSION:LPIP is safe, and has the short time effect in the treatment of ECPACG. With elapse of time, the effect of LPIP is weakened. We can repeat the treatment.
2.Bacteria community in different aged Coptis chinensis planting soil revealed by PCR-DGGE analysis.

In order to reveal the cause of disease occurred in the process of Coptis chinensis growth, this paper studied the bacterial species diversity index of different aged rhizospheric and non-rhizospheric soil planting normal or sick C. chinensis by using PCR-DGGE technique. The representative DGGE bands were chosen to be cloned, and sequenced, the phylogeny were constructed. The results showed that the bacterial communities were very different between the normal and diseased soil samples of C. chinensis, and the diversity index (H) of diseased soil samples were higher than that of normal soil samples. Sequencing analysis of representative cloned DGGE bands showed that the unculturable bacteria were the dominant groups, and bacteria belonged to genus Bacillus, Acidovorax, Acinetobacter, uncultured Kluyvera, and uncultured Comamonas were also existing, but the reported plant pathogenic bacteria were not found in the C. chinensis planting soil. The density and brightness of clone band d in diseased soil samples was higher than that in normal soil sample, and sequencing analysis showed that it belonged to genus Acidovorax. Obviously, during the process of C. chinensis growth, the rhizospheric bacteria population changed, and the quantity of bacteria belong Acidovorax increased, which probably resulted in the disease occurred during C. chinensis growth.

3.Mild form disseminated photocoagulation treatment for moderate non-proliferative diabetic retinopathy

·AIM:To observe the therapeutic effect of the mild form disseminated photocoagulation treatment for moderate non-proliferative diabetic retinopathy (MNPDR). ·METHODS: Prospective randomized controlled study. Totally 126 cases (209 eyes) with MNPDR were randomly assigned to two groups in ophthalmology department of Shanghai Shidong Hospital. There were 62 cases ( 103 eyes ) were treated with the mild form disseminated photocoagulation as the therapy group, 64 cases ( 106 eyes) were treated with control of blood glucose as the control group. All cases were followed up for 24mo. The best corrected visual acuity ( BCVA), fundus change and central visual field were observed and compared. · RESULTS: At 24mo after treatment, BCVA variety between two groups took on significant statistical difference which indicated that there were more eyes with improved or stable visual acuity in therapy group ( Hc=5. 942, P<0. 05). There was significant difference in fundus examination between two groups which indicated that there were more eyes with improved or stable fundus change in therapy group (Hc=12. 662, P<0. 05). There was no significant difference in average mean sensitivity (MS) between before and after laser therapy for 12mo (t=3. 1586, P>0. 05). During our follow-up, there were no complications likewise infection, hemorrhage, retinal membrane, injury of the macular fovear occurred. · CONCLUSION: The mild form disseminated photocoagulation therapy for moderate non-proliferative diabetic retinopathy is safe and effective without obvious complications, which can effectively delay the progression of diabetic retinopathy.
4.Isolation and characterization of Shewanella spp.from patients of food poisoning

Objective To identify the isolates of Shewanella spp.from specimens of food poisoning based on biological and biochemical analysis.Methods Strains were obtained from the investigation on two food poisoning episodes in September and October,2007 in Ma'anshan city,Anhui province.In accordance with the national standard protocol(GB/T 4789),all specimens were enriched and isolated on selective medium,and the suspected strains were ldentified by the VITEK-32 and API20E systems.For Shewanella spp.identified by the biochemical system,more characteristics were analyzed using auxiliary biochemical,growth,hemolytic and drug-resistance tests.DNAs of Shewanella spp.were extracted,16S rDNA was PCR amplified and sequenced with universal 16S rDNA primers.Phylogenetic tree was constructed with MEGA 4.0.Results After enrichment, all specimens were inoculated to selective medium and Shewanella spp.strains were isolated from 8 samples with single colony on both TCBS and BP media.The characteristics of growth in the Triple Sugar Iron (TSI) agar appeared to have had hydrogen sulfide production but no gas production or positive oxidase.No Shewanella spp.strain was detected in WS,SS and EMB media.The 8 strains were identified as Shewanella algae(S.algae) or Shewanella putrefaciens(S.putrefaciens) by VITEK-32,as S.putrefaciens by API20E system.No other enteropathogenic bacteria,including Vibrio cholerae,Salmonella,Vibrio parahaemolyticus,Proteus vulgaris or Staphylococcus aureua,were detected from those 8 samples.From 16S rDNA phylogenetic trees,7 out of 8 ShewaneUa spp.were identified as S.algae.1 as S.putrefaeiens.Conclusion Strains of Shewanella spp.were lsolated from samples of the food poisoning episodes,providing a possible clue to investigate the role of Shewanella spp.on food poisoning

Objective To investigate the iodine nourishment in women of child-beating age in high risk region of iodine deficiency disorders (IDD) in Qinghai Province. Methods According to The Notice to Launch a Reinforced Survey on IDD in High Risk Region issued by The Ministry of Public Health, 17 counties in 6 districts were selected as investigated area in Qinghai Province in 2007, using two stage cluster sampling and combining The National IDD Preliminary Surveillance Scheme, 30 women aged from 18 to 40 years were selected in each village, 1 or 2 villages in each town, 3 to 5 towns in each county, who were divided into newly wedding, pregnant, lactation and other women of child-bearing age. Iodine concentration in urine was detected by the method of As3+-Ce4+catalytic spectrophotometry. Results One thousand six hundreds and four urine iodine samples were analyzed. The median was 93.3 μg/L,52.1%(836/1604),31.8%(510/1604) and 12.4%(199/1604) was lower than 100,50 and 20 μg/L, respectively. It was 70.5%(527/747) and 43.0%(128/298) of women in Yushu and Haixi that had urinary iodine lower than 100 μg/L, respectively, while it was 50% of women in the 6 districts, to be specific, 88.3%(91/103) in Nangqian, 83.8% (62/74) in Zaduo and 70.7%(118/167) in Zhiduo Counties respectively. The median of urinary iodine in women who were not lactating and not pregnant was only 88.6 μg/L, of whom 53.9% (763/1415) lower than 100 μg/L. Conclusions The women of reproductive age in high risk region of IDD are deficient of iodine in Qinghai Province.

8.Bortezomib-based induction chemotherapy followed by autologous hematopoietic stem cell transplantation and maintenance in 200 patients with multiple myeloma: long-term follow-up results from single center.

Objective: To study the efficacy, safety and long-term outcomes of integrated strategy of bortezomib-based induction regimens followed by autologous hematopoietic stem cell (ASCT) and maintenance therapy in Chinese multiple myeloma (MM) patients. Methods: 200 MM patients receiving integrated strategy of bortezomib--based induction regimens followed by ASCT and maintenance therapy were retrospectively and prospectively analyzed from December 1. 2006 to April 30. 2018. Results: The complete remission rates (CR) and better than very good partial remission rates (VGPR) after induction therapy, transplantation and maintenance therapy were respectively 31% and 75.5%, 51.8% and 87.7%,73.6% and 93.4%. There was no difference between 4 cycles and more than 5 cycles induction chemotherapy. The negative rate of MRD detection by flow cytometry was 17.6% and 38.2% respectively after induction and 3 months after transplantation. The negative rate of MRD gradually increased during the maintenance therapy. The success rate of high dose CTX combined with G-CSF mobilization was 95.5% and transplantation related mortality (TRM) was zero. The median time to progress (TTP) was 75.3 months and the median overall survival (OS) was 99.5 months. TTP of patients obtaining CR and negative MRD after induction were longer that those of no CR and positive MRD. TTP and OS of patients receiving triple-drug induction and ASCT in early stage were longer than those of double-drug induction and ASCT in late stage. LDH≥240 U/L, high risk cytogenetics, ISS II+III stage and HBsAg positive were prognostic factors at diagnosis. However, only MRD and high risk cytogenetics were independent prognostic factors after transplantation and maintenance therapy. The clinical characteristics of patients of TTP ≥6 years were listed below: light-chain type M protein, ISS I stage, normal level of hemoglobin and platelet, normal LDH, HBsAg negative, chromosome 17p-negative, good response and sustained good response. Conclusions: Integrated strategy of bortezomib-based induction regimens followed by ASCT and maintenance therapy can significantly improve the short-term and long-term efficacy. The prognostic factors of TTP in different disease stages were different. Response to treatment, especially MRD, played a more important role in prognostic factors.
