1.Effects of sarcopenia and exercise training on muscle satellite cells:current status and prospects

BACKGROUND:The phenomenon of atrophy or reduction of muscle, causing degenerative changes of muscle functions, appears along with age. Sports training, in which muscle satelite cels are of great importance, is beneficial to increase in muscle mass and improvement of muscle function.
OBJECTIVE: To summarize regulatory mechanism of satelite cels in skeletal muscle mass; changes of satelite muscle cels in the degenerative process of muscle mass and strength; declining and reverse effects of sports training intervention; situations and problems of current research and prospective of the future.
METHODS:A computer-based online search was conducted in PubMed database by using the key words of “sarcopenia, skeletal muscle, satelite cels” from 1986 to 2015. The language was limited to English. The eligible papers were further analyzed and reviewed.
RESULTS AND CONCLUSION:A total of 168 papers were screened. Finaly, 39 papers were selected according to the titles and objectives. Skeletal muscle atrophy is shown as II type muscle fiber atrophy, and the II type muscle fiber satelite cel content decreases simultaneously. Exercise is beneficial to increase muscle mass and improve muscle function in older people. Both resistance and endurance trainings can increase the skeletal muscle, especialy the II muscle fiber satelite cel content with a further increase in the satelite cel activation and proliferation. The number and activation degree of satelite cels are related to muscle aging, and satelite cels and proliferation factors regulate muscle cel formation. Therefore, future researches should not only focus on the increase of satelite cel bank, but also explore effective ways to promote the activation of satelite cels, such as exercise training, nutrition and drugs.
2.Chromosomal analysis of 394 couples with recurrent spontaneous abortion

Objective To explore the relationship between recurrent spontaneous abortion and the chromosome abnormality by analyzing the 394 couples′chromosome karyotypes.Methods Peripheral blood lymphocytes in 394 couples of recurrent spontane-ous abortion in Guangzhou area were cultured and performed the chromosome karyotype analysis.Results Among 394 couples of recrudescence abortion,41 cases (5.2%)of abnormal chromosome karyotypes were detected including 1 case(2.44%)of chromo-some number abnormality,8 cases (19.51 %)of balanced translocation,3 cases (7.32%)of Robertsonian translocation,1 case (2.44%)of chromosome inversion and 28 cases (68.29%)of chromosomal polymorphism.Conclusion The chromosomal abnor-mality is an important reason for recurrent spontaneous abortion.Recurrent spontaneous abortion is not only associated with anoma-lies of chromosome structure,and could have a certain relationship with chromosome polymorphism.
3.Implantation of double-J stent via the guide-wire track for the treatment of ureteral stricture and obstruction

Objective To explore the technique of implantation of double-J stent via the guide-wire track, which was established through percutaneous renal puncturing, for the treatment of ureteral stricture and obstruction, and to evaluate its therapeutic effect. Methods A total of 75 patients with ureteral stricture or obstruction, who failed to respond to cystoscopic catheterization, were enrolled in this study. The lesions included benign stricture(n=60) and malignant stricture(n=15). Ureteral guide-wire track was established through percutaneous renal puncturing, which was followed by the dilatation of the stricture with balloon catheter and subsequent implantation of double-J stent via the guide-wire track. After the double-J stent was removed, the patients were followed up for 3 months to five years. Results The success rate of the procedure was 98.7%(74/75). Technical failure occurred in one patient with benign ureteral stricture. The cure rate was 70.7%(53/75), the improvement rate was 26.7%(20/75), and no improvement was seen in one case (1.35%). The total effective rate was 97.3%(73/75). Conclusion For the treatment of ureteral stricture and obstruction, the implantation of double-J stent via the guide-wire track, which is established through percutaneous renal puncturing, is a safe and effective method.
4.Interventional Therapy for Acute and Chronic Arterial Occlusion

Objective To discuss the clinic efficiency and value of intravascular infusion and PTA for treatment of acute and chronic arterial occlusion.Methods 37 patients,23 males and 14 females,aged 23~81 years,mean 52.9 years,underwent Seldinger techniqe.Percutaneous puncture femoral artery anterograde or retrocatheterism with pulsed-spray or injection thrombolysis.The concentration of urokinase was 10000 u/ml,total 300~800 thousands u,mean 550 thousands u.17 patients with chronic arterial occlusion were theated with PTA.Results The rate of success all patients with interventional therapy was 95 percent(35/37).The rate of success with acute arterial occlusion was 91 percent(21/23).In 23 cases,18 were obtained thrombolysis completely(78 percent )and thrombolysis partly were 3(13 percent )and unaffected were2(9 percent).In chronic patients thrombolysis partly was 93 percent.The rate of success with PTA was 100 percent(14/14).Conclusion The thrombolysis therapy is a first selection therapy for acute and chronic arterial occlusion and is a accessory treatment for PTA.
5.Follow-up Study of Balloon Dilatation and Stent Implantation for Benign and Malignant Esophageal Stricture(A Report of 156 Cases)

Objective To evaluate the long-term results of balloon dilatation for benign esophageal stricture and stent implantation for malignant esophageal stricture.Methods Balloon dilatation and stent implantation were performed in 156 cases,of which 12 cases received balloon dilatation in benign esophageal stricture and 134 cases received metallic stents,9 cases received balloon dilatation in malignant esophageal stricture of 143 cases.Results During a follow-up period of 1~48 months,61 cases died with a mean survival period of 8.9 months(1~33 months).Recurrent stenosis occurred in 63 cases(47%)who had to receive ballon dilatation or another stent implantation procedure,the stent was obstructed due to food and mucus in 3 cases,stricture at the upper end in 19 cases and the both ends in 8 cases.Conclusion Stent implantation is an effective palliative treatment for esophageal carcinoma with severe dysphagia.Tumor and hamartoplasia are the main cause of recurrent stricture.Covered stents can reduce the rate of recurrent stricture.
6.Impact of obesity on long-term survival after aortic valve replacement with small prosthesis

Objective This study was designed to evaluate the effect of obesity on late survival after aortic valve replacement(AVR) in patients with implantation of small aortic prosthesis(size≤21).Methods From January 1998 through December 2008,307 patients in a single center in china underwent primary AVR with smaller prosthesis survive the 30 days of surgery.Patients were defined as normal if body mass index (BM1) < 24.0 kg/m2,as overweight if BMI 24.0-27.9 kg/m2,and as obesity if BMI≥28.0 kg/m2.Data of New York Heart Association(NYHA) Functional class,effective orifice area index (EOAI),left ventricular mass index(LVMI),left ventricular ejection fraction(LVEF) were got at the 3rd month(M),6th M,1 st year(Y),3rd Y,5th Y,8th Y after operation respectively.Results At multivariable analysis,obesity was independent factor of later mortality [hazard ratio (HR):1.62 ; P =0.01].Obesity group and overweight group had poor survival (P <0.001)and higher proportion of NYHA Function Ⅲ/Ⅳ (P < 0.01) in long-term compared to normal group.EOAI were lower and LVMI were higher in obesity group and overweight group,but LVEF have no significant difference.Conclusion Obesity is associated with increased late mortality after AVR in patients with implantation of small aortic prosthesis.Obesity and overweight may also affect the NYHA function in long term.EOAI should be improved where possible as it may reduce late mortality and improve life quality in such patients.
7.Safety and efficacy of intra-arterial infusion neoadjuvant chemotherapy for local advanced bladder cancer

Objective To evaluate the safety and efficacy of intra-arterial infusion neoadiuvant chemotherapy in local advanced bladder cancer. Methods Nineteen cases with T2-T4a bladder cancer were enrolled in this study.Intra-arterial infusion chemotherapy with Gemcitabine and Cisplatin (GC)were performed for 1 to 3 cycles before radical cystectomy.Postoperative values of hematological parameters,maximum diameter of tumors,TNM(tumor,node and metastasis)stages and pathological grades were compared with preoperative parameters of the same case. Results Compared to the values before GC chemotherapy,WBC count showed no significant change post-operative,(6.63±2.58)×109/L vs(5.12±2.91)×109/L(P=0.13);RBC(4.41+0.52)×1012/L vs(3.92±0.42)×1012/L(P=0.00)and platelet count(220.50±59.86)×109/L vs(157.05±56.72)×109/L(P=0.001)showed significant decrease;ALT did not show significant decrease(20.00±8.31 vs 26.88±17.04 U/L,P=0.08);Creatltme also showed no significant change(95.82±14.57 vs 88.04±17.76μmol/L,P=0.06);Maximum diameter of tumors decreased significantly(3.72±1.23 vs 2.80±1.29 cm,P=0.02).Compared with clinical TNM stages,pathological TNM stages demonstrated significant decrease in 9 cases;While cell differentiation did not show decrease. Conclusions Intra-arterial infusion with GC regimen can reduce tumor size,decrease TNM stages,while not causing significant adverse impact to radical cystectomy.Bladder-spare treatment is an option for chemotherapy-sensitive cases.
8.Mid-and long-term result of endovascular therapy for lower extremity deep venous thrombosis

Objective To evaluate the mid-term and long-term result of mechanical aspiration thrombectomy or combined with balloon thrombectomy,venoplasty and catheter-directed thrombolysis therapy for lower extremity deep venous thrombosis.Methods Clinical data of 1 068 admitted patients were analysed retrospectively from January 2000 to June 2010.Among these patients,271 cases were treated by thrombectomy only,537 cases received thrombectomy with arterial catheter-directed thrombolysis,260 cases were treated by thrombectomy with dorsalis pedis vein thrombolysis,492 patients with stenosis or obstruction of iliac vein underwent venoplasty treatment.Results After 36 month follow-up edema,pigmentation and chronic ulcers were 10.26%,7.28%,1.55% respectively.The patency without reflux,patency with reflux,residual thrombus,occlusion rate of the iliofemoral vein were 84.25%,8.95%,4.3%,2.5% respectively; The patency,residual thrombus and occlusion rate of popliteal vein were 93.32%,5.49%,1.19%.Iliac vein stent patency rate was 92.28%.Conclusions Endovascular treatment for lower extremity deep venous thrombosis was a reasonable therapy with a satisfactory mid-term and long-term result,protecting venous valve function and reducing the morbidity of post-thrombotic syndrome.
9.Effects of on-pump and off-pump coronary artery bypass grafting on plasma adrenomedullin and endothelin-1 levels in elderly patients

Objective To investigate the effects of on-pump and off-pump coronary artery bypass grafting on plasma adrenomedullin (ADM) and endothelin-1 (ET-1) levels before and after operation in elderly patients. Methods Elderly patients aged 60 years and over with coronary artery bypass grafting were enrolled in the study and divided into off-pump group and on-pump group, each group had 20 cases. Blood samples were collected before surgery, immediately after operation and 6,24 hours, 3 days after operation. The plasma levels of ADM and ET-1 were dectecded by radio immunoassay. Results The levels of ADM and ET-1 were significantly increased immediately after operation in 2 groups of on-and off-pump. The levels of ET-1 in on-pump group and off-pump group were (67.59±10.18) ng/L and (58.73±9.64) ng/L respectively. The levels of ADM in on-and off-groups were (284.90±36.24) ng/L and (243.25±25.70) ng/L. This showed that in on-pump group, the plasma levels of ADM and ET-1 increased more significantly(P<0.05). The plasma levels of ADM and ET-1 began to decline at 6 hours after operation, and continued to 24 hours after operation. On 3 days after operation, the plasma levels of ADM and ET-1 in off-pump group returned to the preoperative level, but they were still maintained at a high level in on-pump group.Conclusions For elderly patients, both on-pump and off-pump coronary artery bypass grafting can cause endothelial dysfunction, increasing plasma levels of ET-1 and ADM. But compared with the on-pump coronary artery bypass grafting, the off-pump coronary artery bypass grafting has less influence.
10.Research on the relationship between psychological health and social support and coping style in general hospitats

Objective To study the relationship between psychological health and social support and coping style in general hospitals. Methods 756 nurses from 3 general hospitals were enrolled by cluster sampling and were investigated by Symptom Checklist 90(SCL-90),Social Support Rate Scale(SSRS)and Simplified Coping Style Questionnaire(SCSQ).The investigation results were analyzed. Results The general level of psychological health of nurses was not optimistic with a positive rate Of 37.04%. The somatizatlon factor of the head nurses was different from that Of normal nurses. Significant difference existed in the following aspects such as somatization, depression and terror factors between night shift nurses and day shift nurses. Relativity could be seen between social support and coping style and psychological health. Conclusion Hospital managers should attach importance to the psychological health of nurses. At the same time they should supply positive social support and instrucitons of coping style to improve the general level of psychological health.
