1.A clinical study of Maxing-Shigan decoction combined with salmeterol/fluticasone inhalation for acute exacerbation of chronic obstructive pulmonary disease

Objective To evaluate the therapeutic effect of Maxing-Shigan decoction combined with salmeterol/fluticasone inhalation for acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods A total of 86 patients with acute exacerbation of COPD were enrolled and randomly divided into a salmeterol/fluticasone group (41 patients) and a combined treatment group (45 patients). The salmeterol/fluticasone group was treated by salmeterol/fluticasone inhalation, and the combined treatment group by Maxing-Shigan decoction combined with salmeterol/fluticasone inhalation. Serum C-reactive protein (CRP) was detected by a immunonephelometric assay, and Toll-like receptor 9 (TLR9) in hemocytes was detected by flow cytometry. The score of the syndromes in traditional Chinese medicine (TCM), such as cough, sputum, gasping and shortness of breath, as well as pulmonary function and therapeutic effect were evaluateds. Results After the treatment, the serum C-reactive protein in the combined treatment group was significantly lower than that in the salmeterol/fluticasone group (4.3 ± 1.2 mg/L vs. 8.4 ± 2.5 mg/L;t=5.417, P<0.01), and the TLR9 expression was significantly higher (1.9 ± 0.7 vs. 1.6 ± 0.4;t=3.418, P<0.05). The scores of the syndromes in TCM, such as cough (1.7 ± 0.6 vs. 3.8 ± 1.1;t=2.859, P<0.05), sputum (1.6 ± 0.4 vs. 3.9 ± 1.2;t=3.027, P<0.05), gasping (1.2 ± 0.5 vs. 3.4 ± 1.3;t=3.416, P<0.05) and shortness of breath (1.5 ± 0.7 vs. 3.7 ± 1.6;t=3.468, P<0.05) in the combined treatment group were significantly lower than those in the salmeterol/fluticasone group. The forced expiratory volume in first second (75.4 ± 5.8 L vs. 62.8 ± 6.9 L;t=3.526, P<0.05) and the percentage of forced expiratory volume in first second to forced vital capacity (85.7%± 10.3%vs. 71.9%± 15.4%;t=5.648, P<0.01) in the combined treatment group were significantly higher than those in the salmeterol/fluticasone group. The time to symptoms alleviated (3.4 ± 0.7 d vs. 5.6 ± 1.2 d; t=3.256, P<0.05) and the use dose was (1.8 ± 0.2) ×103μg vs. (5.3 ± 0.4)×103μg, and use times of salmeterol/fluticasone (7.4 ± 1.3 vs. 16.5 ± 3.4;t=4.574, P<0.05) in the combined treatment group were significantly decreased than those in the salmeterol/fluticasone group. The total effective rate in in the combined treatment group were significantly decreased than those in the salmeterol/fluticasone group (84.4% vs. 73.2%; χ2=4.519, P<0.05). Conclusion Maxing-Shigan decoction combined with salmeterol/fluticasone inhalation can improve the pulmonary function in patients with acute exacerbation of COPD, its effiency is suppior to salmeterol/fluticasone inhalation alone.

3.Effects of Dopamine and Dobutamine on Endocrine of Premature Infants with Hypotension

Objective To explore the effects of dopamine and dobutamine on thyroid stimulating hormone(TSH),total thyroxine(T4) and growth hormone(GH) in preterm infants with hypotension.Methods Forty-seven preterm infants with hypotension were enrolled in the stu-dy,and 38 preterm infants remained hypotensive after two dose of isotonic saline solution,who were divided into two groups and assigned to receive dopamine or dobutamine.Drug infusion were all started at 4 ?g /(kg?min) and the dose increased by 2 ?g /(kg?min) in a stepwise fashion until either the mean blood pressure normalized or a maximal dose 20 ?g /(kg?min) was reached.Haemodynamic variables and se-rum levels of TSH,T4,and GH were assessed during the first 72 h of treatment and the first 3 d after stopping treatment.Microplate luminometer was used to test TSH,T4,GH levels.Results Necessary cumulative and mean drug doses and maximum infusion required to normalize blood pressure were significantly higher in the dobutamine group than those in the dopamine group(Pa0.05].Conclusions Dopamine and dobutamine both increase the syste-mic blood pressure,with higher drug doses in dobutamine group.Dopamine reduces levels of serum TSH,T4 and GH in preterm infants but such suppression is quickly reversed after treatment is stopped.

Monks-hood polysaccharide is one of effective components in Aconite roots. The study found that monkshood polysaccharide has effects of immune regulation, antitumor, myocardial preservation, lipid lowering, etc.
5.The current status of researches on anesthesiology in China

The research projects sponsored by grants provided by the National Natural Science Foundation of China and the articles published in the journals indexed for Science Citation Index in the field of anesthesiology from 1999 to 2009 were collected and analyzed with respect of the number, classification, impact factors etc. It has been found that the number of the grants and the articles were steadily increasing. The researches were mainly confined to the mechanism of anesthesia, pain and organ protection.
6.STUDY OF HYPOTENSIVE ACTION INDUCED BY l-STEPHO LIDINE

Intravenous injection of l-stepholildine (SPD) 2.5, 10 and 40 mg/kg decreased blood pressure(BP ) by 30, 40 and 55% respectively in anesthetized dogs. SPD also decreased BP in anesthetized rats (0.5mg/kg iv, by 29% and 2-50 mg/kg injected into duodenum, by 17-36%) and rabbits (2.5 mg/kg iv, by 24%). It could inhibit the pressor reflexes induced by occluding carotid, stimulating vagus Or sciatic nerve. Furthermore, SPD 2.5 mg/kg injected into fourth cerebroventricle of dog decreased BP by 20%, and in pithed rats SPD 10-40 mg/kg iv depreased BP by 7-40%. These results indicate that both central and peripheral mechanisms are involved in the hypotens-ive action of SPD and the peripheral seems to be more important. As showed in previous papers the effect of SPD on ?-adrenoceptors may be one of the chief mechanisms of the hypotensive action induced by SPD.
7.Detection of Vancomycin-resistant Genes of Enterococcus in Clinic and Their Molecular Epidemiology

OBJECTIVE To investigate the sensitivity to vancomycin of Enterococcus in clinic and the genotype and homology of glycopeptides-resistant strains.METHODS The sensitivity to vancomycin in 72 enterococci strains was detected with agar dilution;the existence of vanA,vanB,vanC1 and vanC2 in enterococci was detected by PCR and ERIC-PCR.RESULTS More than 50% of enterococci intermediated to vancomycin were resistant to other antimicrobial agents commonly used in clinic,except for teicoplanin and linezolid.Gene of vanC2 was present in two strains,but none with vanA and vanB.The ten strains of enterococci intermediated to vancomycin were divided into eight types,in which type A(3 strains) was isolated from the same hospital.CONCLUSIONS Multidrug resistance is observed in enterococci resistant to vancomycin,which are sensitive to teicoplanin and linezolid.Gene of vanC2 is found in some of them,suggesting clone transmission happen among strains.
8.Study of Yanshu Injection on Enhancing Effects of Chemotherapy and Reducing Toxicity and Immune Effect in Tumor-bearing Mice

Objective To investigate the effects of Yanshu injection for the chemotherapy, toxicity- reducing and immunization in mice implanted tumor. Methods Bulid up the mice models with S180 solid tumor transfered by ascites tumor, and observe the effect on tumor weight, blood WBC and bone marrow nucleated cells when combined use Yanshu injection and cyclophospamide (Cy). Determine OD value of plague forming cell by spectrophotometry, measure T-lymphocyte transformation by 3H-TDR incorporation, and survey natural killer cell (NKC) activity by colorimetric method, all these can help to study the role of Yanshu injection on the immune effect in the mice models. Results Yanshu injection can strengthen antitumor activity of Cy, antagonize the inhibition of Cy to WBC and bone marrow nucleated cell, improve OD value of plague forming cell, promote T-lymphocyte transformation, also can enhance NKC activity and killing rate. Conclusion Yanshu injection can increase the chemotherapeutic effects, reduce toxin, and enhance immune function of mice implanted tumor.
