1.Effect of pravastatin on transportation of scutellarin in mouse liver and its mechanism.

This study is to investigate the transportation of scutellarin in cell and live models and study on mechanism of absorption and transport of scutellarin in mouse liver. The concentration of scutellarin in plasma and liver from control and pretreated groups was determined by high performance liquid chromatography. The uptake of scutellarin was examined in control hepatocytes group, induced hepatocytes group and induced hepatocytes plus pravastatin group. Pravastatin can affect the pharmacokinetics of scutellarin in mouse: CL is decreased while AUC is increased. The scutellarin absorption of hepatocyte induced group was higher than that of control group, but was decreased in the group with pravastatin added. The research showed that there was potential drug interaction between pravastatin and scutellarin. The drugs may compete for oatp2 mediated transport pathway consisted in the uptake of scutellarin in liver.
2.Clinical Observation of Nitroprusside Combined with Dopamine in the Treatment of Acute Heart Failure

OBJECTIVE:To observe the clinical efficacy and safety of nitroprusside combined with dopamine in the treatment of acute heart failure(AHF). METHODS:96 patients with AHF were randomly divided into control group and research group. All patients were given ACEIs,digoxin,diuretics,statins,aspirin and other conventional treatment. On this basis,control group was given nitroprusside 12.5-25.0 mg,adding into 5% Glucose solution 50 ml by micro pump and started at 6.25 μg/min,and then maintained at 12.5-50.0 μg/min based on the blood pressure adjustment;research group was given dopamine 100 mg based on the treatment of control group,adding into 5% Glucose solution 50 ml at rate of 2 μg/(kg·min)by pump,and maintained at 2-10 μg/(kg·min). The course of both was 5-7 d. The clinic data was observed,including clinical efficacy,hemodynamic parameters(cen-tral venous pressure,pulmonary artery systolic pressure,heart rate and cardiac output index)before and 0.5,3,12,24 and 48 h af-ter treatment,renal functions(serum potassium,blood urea nitrogen,serum creatinine and 24 h urine)before and 24,48 and 72 h after treatment,and the incidence of adverse reaction. RESULTS:The total effective rate in research group was significantly higher than control group,with significant difference(P<0.05). The central venous pressure,pulmonary artery systolic pressure,heart rate,serum sodium and potassium in 2 groups at different time points were significantly lower than before;cardiac index,blood uric acid nitrogen,creatinine and 24 h urine were significantly higher than before,with significant difference(P<0.05). However, there were no significant differences on the hemodynamic parameters and renal functions at different time points between 2 groups (P>0.05). There were no significant differences in the incidence of adverse reactions(P>0.05). CONCLUSIONS:Nitroprusside combined with dopamine has more obvious efficacy than only nitroprusside in the treatment of AHF,and can obviously reduce the cardiac stress,increase cardiac output and improve renal function,with similar safety.
3.Clinical Observation of Mifepristone Cycle Therapy in the Treatment of Perimenopausal Dysfunctional Uter-ine Bleeding

OBJECTIVE:To observe the efficacy and safety of mifepristone cycle therapy in the treatment of perimenopausal dysfunctional uterine bleeding. METHODS:Totally 116 patients with perimenopausal dysfunctional uterine bleeding were randomly divided into control group and observation group. Control group was given Mifepristone tablet 10 mg from 3 d after diagnostic cu-rettage on an empty stomach for continuous 90 d,once a day. Observation group was given Mifepristone tablet 10 mg from 3 d af-ter diagnostic curettage on an empty stomach,once a day,and it was stopped after continuous 5 d. Then it continued 5 d from the first day of menstruation,and lasted 3 menstrual cycles. The clinic data was observed,including clinical efficacy,estradiol(E2),fol-licle-stimulating hormone (FSH),luteinizing hormone (LH),progesterone (P),hemoglobin (Hb),endometrial thickness,men-strual conditions and recurrence in 2 groups before and after treatment were observed,and the incidence of adverse reactions were recorded. RESULTS:After treatment,E2,FSH,LH and P in 2 groups were significantly lower than before,and observation group was lower than control group,the differences were statistically significant(P<0.05). Hb in 2 groups was significantly higher than before,endometrial thickness was significantly lower than before,the differences were statistically significant(P<0.05);however, there was no significant difference between 2 groups(P>0.05). Total effective rate in observation group significantly higher than control group,normal proportion of menstruation were significantly better than control group,the differences were statistically sig-nificant(P<0.05). There was no significant difference in the incidence of a dverse reactions and dysfunctional uterine bleeding be-tween 2 groups(P>0.05). CONCLUSIONS:Mifepristone cycle therapy has significant efficacy in the treatment of perimenopausal dysfunctional uterine bleeding,can significantly improve the hormone level and the establishment of normal menstrual cycle and re-duce endometrial thickness,with good safety.
4."Ethical Consideration on the Researches Using Human Biological Specimen-Revision Suggestion and Enlightenment of ""Common Rule"" for Subject Protection in the United States"

The use of human biological specimen has great significance to the development of biomedicine. Meanwhile, the powerful electronic data set with sophisticated analytic techniques creates challenge to the protec-tion of private information. As for the research with human biological specimen, how to facilitate the research con-duct on the basis of ethical principles is one of the key considerations when the US Department of Health and Hu-man Service initiated the revision of Code of Federal Regulations ( CFR) -theCommon Rule for subject protec-tion. The Notice of Proposed Rulemaking ( NPRM) proposed:Respect for autonomy will be enforced by broad con-sent to enforce and waiver of consent intend to be rare;If the research risks only involve privacy protection, review process will be simplified under the premise of ensuring the implementation of privacy laws and other protective measures. The paper introduced the revisions related to the researches using human biological specimen in NPRM, analyzed the terms of broad consent, exemption and exclusion, and explored the elicitation to ethical review prac-tice in China.
5.Surgical treatment of superior shoulder suspensory complex injury

Objective To explore the method and effects of operative treatment of superior shoul-der suspensory complex(SSSC)injury.Methods A total of 18 patients with SSSC injury were enrolled. There were six cases of scapular neck fracture combined with comminuted clavicle fracture,five cases of scapular neck fracture combined with acromial fracture,and two cases of comminuted clavicle fracture com-bined with acromial and glenoid fracture.These patients were treated with open reduction and internal fixa-tion.Three patients had acromioclavicular joint dislocation combined with coracoclavicular ligament rupture and they were given hook plate fixation and ligament repair.One patient with scapular neck fracture com-bined with acromioclavicular joint dislocation,and one patient with comminuted clavicle fracture combined with coracoclavicular ligament rupture,were treated with the open reduction,internal fixation and the liga-ment repair.Results All patients were followed up for 3 to 14 months,with an average of 9 months.Ana-tomical reduction and healing were all achieved within 7 to 12 weeks,with an average of 8.6 weeks.Con-clusion Anatomical structure of SSSC is very complex and surgery is given priority to internal fixation, which restores the stability good therapeutic effect.
6.Application of standard quantized integration in the comprehensive assessment of nursing

We have enhanced the quality and efficiency of nursing through the management of the work by means of standard quantized integration. The measures include formulation in the first place of various quantized standards and detailed assessment rules and then the working out of assessment methods. The adoption of these measures in our hospital has led to the improvement of its managerial expertise, the enhancement of its nursing personnels quality and competition awareness, the stimulation of its staffs enthusiasm, and the strengthening of its nursing work in teaching and research management. As a result, nursing in the hospital has become more scientific and efficient.
8.Applicability of capacity of sexual self-defense assessment scale in psychiatric patients without mental retardation.

OBJECTIVE:
To explore the applicability of Capacity of Sexual Self-defense Assessment Scale (CSSAS) in psychiatric patients without mental retardation.
METHODS:
Eighty-seven cases required for assessment of the capacity of sexual self-defense were collected and evaluated by trained professionals according to CSSAS, and the results were compared to the experts' opinions.
RESULTS:
The scores of three grades of capacity of sexual self-defense (none, limited and whole) were 5.97 +/- 5.83, 32.22 +/- 10.15 and 61.60 +/- 13.02, respectively, which were statistically significant among them (P < 0.01). The correlation coefficients between items and total score were 0.59 to 0.91, the Cronbach's alpha value was 0.96, the spilt half Cronbach's alpha value were 0.94 and 0.91, respectively, and the relation coefficient between them was 0.94. According to the original cut off scores, the Kappa value between scale results and experts' opinions was 0.32 (P < 0.01). By redefining the cut off scores, the Kappa value was improved to 0.84 (P < 0.01). Two factors were extracted by means of the factor analysis. The explanation rates of variance were 46.15% and 28.93%, respectively. The accuracy of retrospective cumulative square was 94.30%.
CONCLUSION
The CSSAS could be applied in the psychiatric expertise of the capacity of sexual self-defense in psychiatric patients without mental retardation; however, the cut off scores and the factor weight should be redefined.

9.Clinical significance of human telomerase RNA component and C-myc gene expression in cervical cancer and precancerous lesions

Objective To investigate the expression level of human telomerase RNA component (hTERC) and C-myc in cervical lesions.Methods Fluorescence in situ hybridization (FISH) was applied to detect hTERC and C-myc expression in 62 cases of cervical tissue including 35 cases of cervical intraepithelial neoplasia,8 cases of invasive cervical cancer,19 cases of inflammation as controls.Results The positive rates of hTERC on chromosome 3 in chronic cervicitis organizations,CIN Ⅰ,CIN Ⅱ-Ⅲ and cervical cancer were 5.3 % (1/19),16.7 %(2/12),87.0 % (20/23),87.5 % (7/8) (x2 =36.299,x2 =40.237,P <0.01).The positive rates of C-myc in chronic cervical inflammation organization,CIN Ⅰ,CIN Ⅱ-Ⅲ and cervical cancer were 5.3 % (1/19),8.3 % (1/12),78.3 % (18/23),62.5 % (5/8) (x2 =30.200,x2 =34.224,P <0.01 ).The differences among groups were statistically significant.hTERC had a positively correlation with C-myc expression (r =0.514,P < 0.01).Conclusion The expression of hTERC and C-myc on chromosome 3 is closely related to cervical cancer progression.
10.Neoadjuvant chemotherapy for breast cancer and it's non-randomized clinical observation: Report of 76cases

Objective To investigate the breast cancer neoadjuvant chemotherapy for clinical applica-tion.Methods Given suitable breast cancer patients anthracycline-based combination chemotherapy, 2 ~ 4 cy-cle after the surgery were observed to assess their clinical efficacy and conform to conditions and are compatible with the same period did not receive neoadjuvant chemotherapy for a comparative analysis of cases.Results Ac-ceptance of neoadjuvant chemotherapy group of cases the total effective rate was 89.47%, 1-year and 3-year over-all survival rate significantly improved, 17 cases due to neoadjuvant chemotherapy benefit received axillary lymph node dissection to narrow the scope of the modified radical mastectomy for breast cancer, five cases of implemen-tation of the breast retain on the radical operation of breast cancer.Conclusions Neoadjuvant chemotherapy can effectively reduce the clinical stage of breast cancer, some patients benefit from breast-implementation, such as radical mastectomy or surgery to narrow the scope, postoperative survival and prognosis of breast cancer is there-fore improved.
