3.Application of CO2 laser and self-made laryngeal dilator in the treatment of the laryngeal stenosis.

OBJECTIVE:
To study the treatment effect of CO2 laser and self-made laryngeal dilator on the laryngeal stenosis.
METHOD:
Twenty-five patients with laryngeal stenosis were treated with CO2 laser and self-made laryngeal dilator. The laryngeal cavity scar was resected as much as possible by using CO2 laser and the laryngeal cavity was exposed by placing laryngeal dilator under general anesthesia.
RESULT:
The 24 cases of all were successfully decanulated 1 to 12 months after operation and could breathe freely. One case had second operation and was successfully decanulated 9 months after operation. The follow-up for 1 to 2 years showed all cases speaking well and 3 cases recurrence.
CONCLUSION
The method for treating laryngeal stenosis using CO2 laser and self-mande laryngeal dilator is effective, fewer complication, less invasive, and faster recovery. The laryngeal dilator is made simply and conveniently.

Objective To investigate the in vitro inhibitory effect of LIGHT on human esophageal carcinoma cells. Methods LIGHT Fc expression vector was transfected into human esophageal squamous carcinoma cell line, Eca109. The inhibitory effect of LIGHT gene on cell growth was detected by MTT and cell growth curve. The expressions of LT?R and HVEM were detected by RT PCR. Results Expression of LIGHT Fc gene could inhibit Eca109 cell proliferation. The growth curve of Eca109/LIGHT was significantly lower than that of the control group in the culture medium containing 1% FCS. Objective To investigate the inhibitory effect of recombinant LIGHT-Fe gene on the proliferation of human esophageal carcinoma cell line Eea109. Methods LIGHT-Fc expression vector was transfected into human esophageal squamous carcinoma cell line Eca109 by using DOTAP liposomal transfection reagents. The effects of LIGHT-Fc gene on the proliferation of esophageal carcinoma cell line Eca109 in vitro were detected by cell growth curve and MTr assay. Forty-five nude mice were equally divided into Eea109/Wt group, Eca109/neo group and Eca109/LIGHT group. Carcinogenesis and pathological expression of the esophageal carcinoma tissues were observed. Results The expressions of LIGHT receptors were detected in Eca109 cells. The proliferation of Eca109 cells was inhibited after trasfecting LIGHT-Fc gene into Eca109 cells. The numbers of tumors generated in Eea109/Wt group, Eca109/neo group and Eca109/LIGHT group were 12, 11 and 5, with statistical significance between Eca109/LIGHT group and the other two groups (X2 =6.652, 4.821, P <0.05). The result of histopatholagical examination indicated that the tissue necrosis appeared significantly in tumors derived from Eea109/LIGHT cells. Conclusions The growth of esophageal squamous carcinoma cell line Eca109 can be suppressed by LIGHT-Fc gene whether in vitro or in vivo.
9.Comparison of the effects of penequinine hydrochloride and atropine on the cardiovascular stability about preoperative in children

Objective To compare the effects of penequinine hydrochloride and atropine on the cardiovascu-lar stability about preoperative in children by monitoring and recording the HR, MAP, SpO2, ECG changes. Methods 40 children underwent selective operation were randomly divided into two groups, 20 in each group. Penequinine hydrechloride group(group A) and atropine group(group B) were injected intramuscularly with penequinine hydro-chloride 0. 015mg/kg or atropine 0. 015mg/kg respectively, at 30 minutes before operation. Patients' HR, MAP, SpO2 and ECG changes were recorded at 10min,20min,30min and 40min after injection. Results The HR,MAP,SpO2 levels of group A and SpO2 level of group B had no significant changes. There are significant changes in HR,MAP of group B. Conclusion Penequinine hydrochloride can be used as preoperative medication and it is conducive to the maintenance of cardiovascular stability.
10.Efficacy of domestic gemcitabine combined with Lobaplatine in salvage treatment of taxanes-refractory advanced lung squamous carcinoma

Objective To evaluate the effiacy and adverse reactions of domestic gemcitabine combined with lobaplatine in salvage treatment of taxanes-refractory advanced lung squamous canceroma.Methods 71 patients with squamous carcinoma of lung,all had been failer to combined chemotherapy with taxanes treatment before,received the treatment with domestic gemcitabine (1 000mg/m2,d1,8) combined with lobaplatine (30mg/m2,d2),21-28 days as a cycle,at least 2 cycles should been recieved.The therapeutic efficacy was evaluated after 2 cycles of chemotherapy,according to RECIST1.0 standard.Results All of 175 cycles had been observed.In terms of the treatment efficacy,68 patients could be evaluated objectively of 71 cases patients,partial response (PR) was 15 cases (22.0%),stable disease(SD) was 18 cases(26.4%),with an overall response rate(RR) was 22.0% (15/68),disease control rate (DCR) was 48.5 % (33/68).The median time to progression (TTP) was 13.6 weeks.the median survival time (OS) was 7.9 months.One year survival rate was 42.6% (29/68).On 71 cases for a total of 175 cycles of chemotherapy,the major toxic reaction was hematological toxicities,including myelosuppression about 38.3% (67/175) for grade Ⅲ and Ⅳ.Conclusion The combination treatment of domestic gemcitabine plus lobaplatin is a feasible and active scheme in salvage treatment of taxanes-refractory advanced quamous carcinoma of lung.
