2.Influence of preoperative biliary drainage on severe jaundice patients undergoing pancreaticoduodenectomy

The hospital stay in drainage group[ average 71(43-101)days] was significantly longer than that in non- drainage group [ average 47(29-81)days](P=0.05). Conclusions If a sufficient preoperative preparation is performed,one stage PD operation is a benificial method for peri-ampullar carcinoma patients complicated with severe obstructive jaundice.
3.Protective effects of sinomenine on renal ischemia/reperfusion injury in mice

Objective To evaluate the protective effect of sinomenine (SIN) on renal ischemia/reperfusion (I/R) in mice. Methods In the experiment one, 12 C57BL/6 mice were randomly divided into 2 groups: SIN group (mice were injected with 200 mg/kg SIN by tail vein) and control group (mice were injected with equal volume of saline). Six and 24 hs later, the serum was collected and the contents of alanine aminotransferase (ALT) and creatinine (SCr) were determined. In the experiment two, C57BL/6 mice were randomly divided into 3 groups: sham-operated (SO) group, SIN group (mice were injected with 200 mg/kg sinomenine just before ischemia induction) and saline group (mice were injected with equal volume of saline at the same time). At the 6th h after reperfusion, the sera and renal samples subject to IR injury were collected. The SCr and BUN levels in serum were determined and renal histological changes were also examined. The apoptosis of renal tubular epithelial cells was measured by using terminal deoxynucleotidyl transferase mediated dUTP nick end labeling assay. The infiltration of F4/80 positive macrophages was measured by using immunohistochemistry and that of neutrophils with myeloperoxidase (MPO) kits. The mRNA expression of tumor necrosis factor (TNF)-α, chemokine CXC ligand (CXCL)-10, intercellular adhesion molecule (ICAM)-1 and IL-17 was detected by using real-time reverse transcription PCR. The activation of transcription factor NF-κB was measured by using Western blotting. Results In the experiment one, there was no significant difference in ALT and SCr between the two groups at 6 or 24 h. In the experiment two,levels of SCr and BUN were lower in SIN group (P<0. 05 or P<0. 01 ), histological damage was milder (P<0. 01 ), and apoptosis rate of renal tubular epithelial cells apoptosis was lower than in saline group (P<0. 05). The infiltration of macrophages, neutrophils and the mRNA expression of TNF-α, CXCL-10, ICAM-1 and IL-17 in the renal tissue in SIN group were reduced as compared with saline group (P<0. 05 or P<0. 01 ). The activation of NF-κB in SIN group was significantly downregulated as compared with saline group. Conclusion SIN can ameliorate the renal IR injury without hepatic or renal toxicity, which is associated with inhibition of acute inflammatory response induced by reperfusion.
4.Expression and Clinical Significance Of Selenoprotein-P In The Tissues of Colorectal Cancer

Objective To discuss the effect of selenoprotein P (SeP) on occurring and developing of colorectal cancer and its clinical significance by detecting the expression of SeP in adjacent mucosa of colorectal cancer, and in the cancer, metastatic lymph node and metastasis hepatic tissues.Methods Tissue microarray was constructed, the expression of SeP in adjacent mucosa of colorectal cancer, and in the cancer, metastatic lymph node and metastasis hepatic tissues were detected by means of immunohistochemistry.Results The positive rate of SeP were 48.4%,69.5%,66.7% and 61.5% in adjacent mucosa, tumor cell, lymph node metastasis and hepatic metastasis. 5.Relationship between the genetic variants in SLCOI B1 gene and the response to repaglinide treatment in newly-diagnosed type 2 diabetic patients

Objective To investigate the effect of solute cartier organic anion transporter family, member 1B1 (SLCOIBI) gene variants on the response to therapy with repaglinide in type 2 diabetes. Methods 100 newly-diagnosed type 2 diabetic patients were treated with repaglinide during a course of 48 weeks. Anthropometrie parameters and indices related to glucose metabolism were measured periodically. Genotypes of SLCO1B1 D130N and V174A were detected by PCR-restriction fragment length polymorphism (RFLP) and sequencing respectively. Results Eighty-nine patients accomplished the 48-week follow-up visits. D130N variant in SLCO1B1 gene was associated with repaglinide treatment, DD genotype had better HbA1C lowering effect than N allele carrier [△HbA1C: (-2.29±0.23) % vs (-1.49±0.21)%, P<0.05]. No association was detected between D130N and the other effects of repaglinide on glucose metabolism related phenotypes. Conclusion D130N variant in SLCO1B1 gene is associated with the response to repaglinide treatment in patients with type 2 diabetes. DD homozygotes had a better effect than N allele carriers.

Objective:To discuss clinical effect of Zhuyu Zhixuetang to chronic endometritis (CE) with syndrome of Qi deficiency and blood stasis and to study improvement effect to pregnancy outcome. Method:One hundred and forty-four patients were randomly divided into two groups, patients in control group were 72 cases and in observation group were 72 cases. In control group, 66 patients completed the therapy because of 4 falling off or missing visit and 2 eliminated, and in observation group, 65 patients completed the therapy because of 3 falling off or missing visit and 5 eliminated. In two groups. anti-infection treatment was gicen to patients. Patients in control group got Fuke Qianjin Pian, 6 tablets/time, 3 times/day. Patients in observation group got Zhuyu Zhixuetang, 1 dose/day. The treatment was continued for 3 months and the follow up was recorded for 6 months. Before and after treatment, changes of menstrual volume, period and cycle were recorded. And hysteroscopy and color Doppler ultrasound of vagina were made, and endometrial morphology and endometrial receptivity were evaluated [endometrial thickness, resistance index (RI), pulsation index (PI) and blood flow index (FI)] were evaluated, and pathology of endometrial were tested. 8.PENG Peichu's Experience in Staged Differentiation and Treatment of Prostate Cancer

This paper summarized Professor PENG Peichu's experience in the differentiation and treatment of prostate cancer in three phases and four stages. It is considered that prostatic cancer is categorized into root deficiency and branch excess, with depletion of healthy qi as the root, and the accumulation of cancer toxin as the minifestation. Clinical diagnosis and treatment of prostatic cancer can be divided into three phases and four stages according to the exuberance and decline of pathogenic and healthy qi and the changes of deficiency and excess of yin and yang. In the initial accumulation phase of cancer toxin (yang excess stage), the key pathogenesis is the accumulation of dampness, heat and static blood, and internal generation of cancer toxin, and the treatment should be resolving toxins, fighting cancer and dispelling yang excess. In the phase of healthy qi deficiency and toxin accumulation (yin deficiency stage), with the lung and kidney yin deficiency, dampness, heat and static toxin accumulation as the key pathogenesis, the treatment should be centered on mutual generation between metal and water to nourish kidney yin, supplemented with the method of clearing heat and draining dampness, activating blood and resolving toxins, for which self-made Nanbei Formula(南北方)is usually used. In the phase of yang deficiency and cold stagnation (yang deficiency stage and yin excess stage), with the spleen and kidney yang deficiency, cold dampness stagnation, static heat and toxin accumulation as the key pathogenesis, the treatment should be warming and tonifying spleen and kidney to dissipate cold accumulation; for deficiency of both yin and yang, and excess pathogen obstruction, modified Yanghe Decoction(阳和汤) is recommended, while for yang deficiency, cold congealing and blood stasis, self-made Wenshen Sanjie Formula(温肾散结方) can be used, and for cold dampness binding with cancer toxin, and cold complex with heat, self-made Quanan Formula (泉安方) is advised.
9. Measurement of spino-pelvic anatomical parameters and finite element analysis of lower lumbar vertebra in lumbarization patients

Objective To measure the sagittal anatomical parameters of the spine and pelvis based on EOS imaging system, and to evaluate the stress of the lower lumbar spine by finite element analysis (FEA). Methods A total of 44 subjects examined by EOS imaging system were included, including 11 sacral lumbar vertebra patients and 33 normal subjects. The sagittal plane parameters of lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT) and sacral slope (SS) were measured and compared in both groups. Pearson test was used to analyze the correlation between PI and LL in the two groups. At the same time, the finite element model of the lower lumbar vertebra was established. The stress condition of the lumbar spine model during forward bending, backward extension and left and right bending was evaluated by FEA method. Results The differences of PI, PT, SS and LL between the two groups were statistically significant (P<0.05). The correlation study found that there was a positive correlation between the two groups of subjects' PI and LL, including the lumbarization group (r = 0.69, P<0.05) and the normal group (r = 0.52, P<0.05). Under the conditions of forward bending, backward stretching, left bending and right bending, the bending moment of the model was 2 Nmm, and the stress concentration gradually decreased from bottom to top. The maximum stress concentration point was located at the lower articular process. Conclusion The physiological curvature and stress distribution of the lumbar spine in lumbarization population were different than normal, especially the stress concentration of the transitional intervertebral disc and articular process joint was obvious, and early degeneration of the spine was easy to occur.
10.Prognosis of primary liver carcinoma treated with local resection.

OBJECTIVETo discuss the prognosis of primary liver carcinoma treated with local resection and factors affecting prognosis.
METHODSThe data of 130 patients who had been treated from October 1989 to October 1995 were analyzed retrospectively. We analyzed the concrete operating methods, the cause of postoperative complications and factors affecting prognosis.
RESULTSCurative local resection was performed in 130 patients. Operation-related mortality was 0.8% and the incidence of complication was 16.1% (n = 18). The overall 1-, 3- and 5-year survival rates were 82.1%, 60.6% and 48.2%, respectively. Involvement of blood vessels or liver capsules and the elevation of AST before operation were the significant factors affecting prognosis (P < 0.05).
CONCLUSIONLocal resection is a safe method characterized by little damage, rapid, less blood loss, low complication rate and good prognosis.
