1.Identification and virulence analysis of a Streptococcus suis strain isolated from human blood

To identify and analyze the virulence of a bacteria strain isolated from the blood of a patient with suspected Streptococcus suis (S.suis) infection in a hospital of Beijing,we inoculated the bacteria strain isolated from the blood of the patient to the Columbia with sheep blood agar plate,after Gram staining and microscopical examination,serum agglutination test,VITEK 2 Compact microbial identification system test and matrix assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) test,S,suis species specific gene 16SrRNA,S.suis species serotype 2 specific virulence gene capsule polysaccharide 2J (cps2J) and virulence gene muramidase-released protein (mrp),hemolysin (sly),extracellular factor protein (ef),glutamate dehydrogenase (gdh) genes,fibronectin-binding protein (fbps),glyceraldehyde-3-phosphate dehydrogenase (gapdh) genes and virulence correlated gene orf2 were further detected by PCR.Results showed that the suspicious bacteria strain of S.suis was identified as S.suis type 2 (S.suis 2) by conventional methods,MALDI-TOF-MS and PCR.PCR results showed that cps2J,sly,ef,gdh,fbps,gapdh and or f2 genes were positive,and mrp gene was negative.In conclusion,the bacteria strain isolated from the patient's blood is sly+/ef+/mrp-virulent S.suis 2.

3.Improvement of neurological function and stress in patients with acute massive cerebral infarction by mild hypothermia: a prospective randomized controlled study

To study the effect of mild hypothermiaon neurological function and oxidative stress in patients with acute massive cerebral infarction. Methods Sixty patients with acute massive cerebral infarction (within 12 hours of onset, the age < 70 years old) admitted to the First Hospital of Qinhuangdao from August 2017 to August 2018 were enrolled. Patients were divided into control group and observation group according to random number table method, with 30 patients in each group. The control group was given conventional treatment such as lowering intracranial pressure, anticoagulation, improving microcirculation, neurotrophy and anti-stress ulcer, while the observation group was received mild hypothermia (after 72 hours of treatment at 32-35 ℃, it increased about 1 ℃ every 6 hours until the normal temperature). The neurological impairment (the National Institutes of Health Stroke Scale, NIHSS), motor function (Fugl-Meyer score), daily living ability (Bathel index) and coma degree (Glasgow coma scale, GCS) were observed before treatment and 72 hours after treatment. The levels of plasma malondialdehyde (MDA), superoxide dismutase (SOD) and advanced oxidative protein products (AOPP) were measured at same time. After 72 hours of treatment, the therapeutic effect was judged according to the NIHSS score. Results There was no significant difference in NIHSS score, Fugl-Meyer score, Bathel index and GCS scores, and the levels of plasma MDA, SOD and AOPP between two groups before treatment. Compared with before treatment, NIHSS score and plasma MDA and AOPP levels in the two groups were significantly decreased after treatment, while Fugl-Meyer score, Bathel index, GCS score and plasma SOD activity were significantly increased; and the change of observation group was more significant than those of control group [NIHSS score: 9.78±3.24 vs. 13.44±2.63, MDA (mmol/L): 8.70±2.79 vs. 10.50±3.81, AOPP (μmol/L):51.10±17.81 vs. 72.60±18.71, Fugl-Meyer score: 55.68±8.38 vs. 46.25±3.92, Bathel index: 66.63±11.25 vs. 56.71±10.81, GCS score: 13.78±1.31 vs. 10.49±1.03, SOD (kU/L): 134.70±29.87 vs. 104.60±25.75, all P < 0.05]. The total effective rate of the observation group was significantly higher than that of the control group [93.3% (28/30) vs. 86.7% (26/30), χ2 = 6.119, P = 0.020]. Conclusions Mild hypothermia was effective in treating acute massive cerebral infarction. It could significantly improve the neurological function and oxidative stress of patients, improve the quality of life, and was safe and reliable.
4. Risk factors for portal vein thrombosis in cirrhotic patients and the influences of anticoagulation on esophagogastric variceal bleeding

Objective:
To investigate the associated factors and the independent risk factors for portal vein thrombosis (PVT) in cirrhotic patients and assess the influences of anticoagulation on esophagogastric variceal bleeding (EGVB) in these patients.
Methods:
From January 2012 to December 2012, 239 cirrhotic patients were diagnosed in our hospital. According to the presence or absence of portal vein thrombosis (PVT), they were divided into thrombus group (33 cases) and control group (206 cases). According to the presence or absence of EGVB in thrombus group, they were divided into bleeding group (10 cases) and non bleeding group (23 cases). According to whether anticoagulant therapy was used in thrombus group, they were divided into anticoagulant group (10 cases) and non anticoagulant group (23 cases). The risk factors of each group and its control group were observed and compared.
Results:
The thrombus group had a lower level of the albumin (ALB) , higher level of count of platelet (PLT), diameter of main portal vein (MPV), propotion of diabetes prevalence and history of splenectomy compared with the control group (
5.Incidence and Survival Trend of Malignant Tumors in Shenyang Urban Area from 2011 to 2018

Objective To analyze the incidence and survival trend of malignant tumors in urban residents of Shenyang from 2011 to 2018. Methods The Shenyang tumor registration report system was used to collect the onset data and survival data of patients with malignant tumor from 2011 to 2018.The crude incidence, age-standardized rate, cumulative rate (0-74 years old), truncated rate (35-64 years old), survival rate, and incidence and survival rank were calculated.The observed survival rate was calculated by the life table method.The expected survival rate and relative survival rate were calculated by EdererⅡmethod.Using Joinpoint 3.5.3 software, the annual percentage change in incidence rate and survival rate (APC%) were calculated.SPSS23.0 software was used for the chi square tests of males and females. Results The crude incidence of malignant tumors in Shenyang, age-standardized incidence rates by Chinese standard population and world standard population were 364.70/10 million, 190.00/10 million and 185.63/10 million, respectively.The cumulative rate (0-74 years old) was 21.17%, and the truncated rate (35-64 years old) was 311.66/10 million in the years 2011-2018.The top five incidence rates of males are lung, colorectal, liver, stomach, and bladder cancer, whereas those of females were breast, lung, colorectal, uterine, and thyroid cancer.The incidence rate of malignant tumors increased in 8 years (

Gynecological diseases are one of the important factors that threaten women's reproductive health.Endoplasmic reticulum(ER)stress is a stress response induced by the accumulation of unfolded or misfolded proteins in ER cavity when the female body is in a disease state,and the effects of different stress levels on the female reproductive system and the increase in the risk of female disease will vary greatly.Studies have shown that ER stress is closely related to the occurrence and development of gynecological diseases.Based on the relevant research reports at home and abroad in recent years,this paper summarizes the role of ER stress in several common gynecological diseases to provide new research ideas for preventing,diagnosing,and treating gynecological diseases.
7.Bridging-combined internal fixation for extremity fractures

Objective To explore the application of bridging-combined internal fixation for extremity fractures.Methods From May 2010 to August 2017,56 patients were treated by bridging-combined internal fixation for 58 different extremity fractures at Department of Orthopaedics,Military Orthopaedic Trauma Institute,Kunming General Hospital of PLA.They were 34 males and 22 females,aged from 16 to 83 years (average,48 years).There were 48 comminuted and 16 open fractures.The interval from injury to surgery ranged from one to 34 days (average,8 days).Early functional rehabilitation was encouraged for the patients.The therapeutic outcomes were evaluated in terms of fracture union time,postoperative infection,and postoperative functional recovery.Results Fifty-two of the patients were followed up for 12 to 28 months (average,16 months).The fractures united after 3 to 6 months (average,3.5 months).Comprehensive evaluation of the therapeutic outcomes resulted in 36 excellent,12 good,and 4 fair cases,yielding an excellent to good rate of 92.3%.Follow-ups revealed no postoperative infection,bone nonunion,loosening or breakage of the implants.Conclusion The bridging-combined internal fixation may lead to fine therapeutic outcomes for traumatic extremity fractures,because this system is flexible and easy to apply,and leads to fine reduction and limited postoperative complications.
8.Transplanted low-set perforating branch propeller flap of fibular artery for repairing calcaneal soft tissue defects

Objective:To investigate the clinical efficacy of low-set perforating branch propeller flap of fibular artery for repairing calcaneal soft tissue defects.Methods:A retrospective case series study was conducted on 28 patients with calcaneal soft tissue defects admitted to 920th Hospital of Joint Logistics Support Force of PLA from January 2013 to December 2019. There were 19 males and 9 females,aged 13-69 years[(33.8±13.7)years]. The size of soft tissue defects ranged from 5 cm×2 cm to 18 cm×12 cm,and the size of flaps ranged from 12 cm×4 cm to 24 cm×10 cm. The soft tissue defects were repaired with the low-set fibular artery perforator propeller flap after emergency debridement or flap transplantation at the second stage after debridement according to the wound surface conditions. The survival of the flap,wound healing at the recipient and donor sites and complications were recorded. The function of the affected ankle was assessed according to American Orthopedic Foot and Ankle Society(AOFAS)ankle-hindfoot score before operation and at the final follow-up. The degree of satisfaction of flap appearance was assessed by Thankappan flap appearance satisfaction score at the final follow-up.Results:All patients were followed up for 2-40 months[(17.4±5.9)months]. All flaps survived and wounds at both donor and recipient sites were healed well,with satisfactory color and texture of the flaps,without thinning operation performed. Three patients experienced numbness on both sides of the incision at donor sites within 1 week after operation and returned to normal on their own about 2 months after operation,with all flaps restoring partial light pain sensation 1 year after operation. No complications occurred such as blood flow disorder of the affected limb or compartment syndrome. The AOFAS ankle-hindfoot score was 56-95 points[(82.3±15.2)points]at the final follow-up,significantly higher than the preoperative score of 17-68 points[(35.7±13.4)points]( P<0.01). The results were excellent in 21 patients,good in 5 and fair in 2,with excellent and good rate of 93%.The appearance satisfaction score was 4-10 points[(7.6±2.9)points]according to Thankappan flap appearance satisfaction score. The results were excellent in 6 patients,good in 15 and fair in 7,with the excellent and good rate of 75%. Conclusions:The low-set perforating branch propeller flap of fibular artery for reconstruction of calcaneal soft tissue defects has advantages of reliable blood supply and high survival rate of the flap as well as good restoration of the appearance and function of the affected limb.
9.Ginsenoside Rg5 enhances the radiosensitivity of lung adenocarcinoma via reducing HSP90-CDC37 interaction and promoting client protein degradation

Ginsenoside Rg5 is a rare ginsenoside showing promising tumor-suppressive effects.This study aimed to explore its radio-sensitizing effects and the underlying mechanisms.Human lung adenocarcinoma cell lines A549 and Calu-3 were used for in vitro and in vivo analysis.Bioinformatic molecular docking prediction and following validation by surface plasmon resonance(SPR)technology,cellular thermal shift assay(CETSA),and isothermal titration calorimetry(ITC)were conducted to explore the binding between ginsenoside Rg5 and 90 kD heat shock protein alpha(HSP90α).The effects of ginsenoside Rg5 on HSP90-cell division cycle 37(CDC37)interaction,the client protein stability,and the downstream regulations were further explored.Results showed that ginsenoside Rg5 could induce cell-cycle arrest at the G1 phase and enhance irradiation-induced cell apoptosis.It could bind to HSP90α with a high affinity,but the affinity was drastically decreased by HSP90α Y61A mutation.Co-immunoprecipitation(Co-IP)and ITC assays confirmed that ginsenoside Rg5 disrupts the HSP90-CDC37 interaction in a dose-dependent manner.It reduced irradiation-induced upre-gulation of the HSP90-CDC37 client proteins,including SRC,CDK4,RAF1,and ULK1 in A549 cell-derived xenograft(CDX)tumors.Ginsenoside Rg5 or MRT67307(an IKKe/TBK1 inhibitor)pretreatment suppressed irradiation-induced elevation of the LC3-Ⅱ/β ratio and restored irradiation-induced downregulation of p62 expression.In A549 CDX tumors,ginsenoside Rg5 treatment suppressed LC3 expression and enhanced irradiation-induced DNA damage.In conclusion,ginsenoside Rg5 may be a potential radiosensitizer for lung adenocarcinoma.It interacts with HSP90α and reduces the binding between HSP90 and CDC37,thereby increasing the ubiquitin-mediated proteasomal degradation of the HSP90-CDC37 client proteins.
10. Clinical characteristics and prognosis of 34 cases of acute myeloid leukemia with FLT3 internal tandem duplication and MLL gene rearrangement

Objective:
To analyze the clinical characteristics and prognosis of 34 cases of acute myeloid leukemia (AML) with FLT3 internal tandem duplication (FLT3-ITD) and MLL gene rearrangement.
Methods:
The clinical data of 34 AML patients with FLT3-ITD and MLL gene rearrangement was compared and analyzed for the therapeutic efficacy, prognostic factors when treated with chemotherapy, chemotherapy combined with targeted therapy or allogenic hematopoietic stem cell transplantation (allo-HSCT).
Results:
Of the thirty-four cases with median age 41 (4-71) years old, 63.6% presented with white blood cells (WBC) greater than 30×109/L, 39.4% greater than 50 × 109/L respectively on admission. M5 (35.3%) made up the highest proportion. The cytogenetic abnormality reached 61.8%, of which the complex cytogenetic abnormality accounted for 11.8%. Eleven patients (32.35%) had both FLT3-ITD and MLL gene abnormalities. In addition to FLT3 and MLL abnormalities, 23 patients (67.6%) had one or more other gene abnormalities (multiple gene abnormalities). Of the 34 cases, 29.4% patients went into complete remission (CR) after two courses of chemotherapy. 20.6% (7 patients) went into CR after 3 or more courses of chemotherapy. The rate of early relapse in the CR group was 52.9%. Patients with WBC>50×109/L or multiple gene abnormalities had a lower remission rate (7.7%, 5.4%) after two courses of chemotherapy.
