1.Correlations of fundus lesions with prognostic significance in acute leukemia

Objective To determine retinal charges in newly diagnosed acute leukemia patients and to eval-uate the diagnostic value of retinal findings in patients with central nervous systerm leukemia(CNSL) and intracranial hemorrhage(IH). Methods 123 patients with acute leukemia(86 had myeloid leukemia and 37 had lymphoid leuke-mia) aged 14~77 years were examined with direct/indirect ophthalmoscopy for the presence of intraretinal hemorrha-ges (IRH), white-centred hemorrhages (WCH) and cotton-wool spots (CWS) within 5 days of diagnosis. Results CNSL and IH were more common in patients with retinopathy than those without (27.1% vs 9.1%, P=0.046; 29.2% vs 9.1%, P=0.029;respectively). Complete remission(CR) rate in patients with IRH was lower than those without(52.5% vs 80.5%, P =0.08). No association was found between the presence of WCH or CWS and CR rate. Conclusion The present study shows that patients with IRH is an indicator of poor prognosis in acute leukemia. Leukemic retinopathy usually is seen in patients with IH. Invasion of the eye by leukemia is not rare and indicates the need for early ophthalmological examination.
2.Effect of Blood-nourishing and Wind-expelling Therapy Combined with Epidermic Grafting for Vitiligo

[Objective] To observe the influence of blood-nourishing and wind-expelling therapy on the therapeutic effect of epidermic grafting for vitiligo. [Methods] Sixty-two patients with vitiligo were allocated to group A (n = 32) and group B (n = 30) with simple random method. Group B was treated with epidermic grafting. Group A was treated with oral use of Baibofeng Decoction (mainly composed of Radix Angelicae Sinensis, Caulis Spatholobi, Radix Saposhnikoviae, Fructus Psoraleae, Radix Paeoniae Rubra , Radix Salviae Miltiorrhizae, etc.) 2 weeks after epidermic grafting was performed, one dose every other day, and with external application of Fubu tincture (composed of Fructus Psoraleae, Radix Angelicae Dahuricae, Radix Salviae Miltiorrhizae and Flos Carthami) bid. After treatment, the short-term and long-term therapeutic effect was observed in the two groups. Objective To establish the GFR reference range of solitary kidney by 99 Tcm?DTPA SPECT imaging and explore factors influencing renal compensation. Methods A total of 108 patients ( 62 males, 46 females, age range:5-83 (47±19) years) with only one functional kidney who underwent 99Tcm?DTPA dynamic imaging from January 2011 to September 2015 were retrospectively analyzed. The GFR was measured to establish the reference range. Meanwhile, the potential influence factors were analyzed, inclu?ding age, gender, location of the functional kidney, removal and causes of the non?functional kidney. Two?sample t test, one?way analysis of variance and Bonferroni test were used. Results The GFR were (71.81± 23.23) ml/min in the 108 patients. There were no significant differences of GFR between male and female patients ( t=0.496) , between patients with left functional kidney and those with right functional kidney( t=0.999), between patients with and without removed non?function kidney(t=-1.966), and among patients with different causes of the non?function kidney( F=1.894;all P>0.05) . 4.Effect of Berberine on AQP4 Expression in Focal Cerebral Ischemia Area in Diabetic Rats

Objective to investigate the effect of berberine on the expression of AQP4 and neuronal injury after focal cerebral ischemia in diabetic rats. Methods Male SD rats were randomly divided into four groups:control(sham surgery),middle cerebral artery occlusion and reperfusion (MCAO/R),MCAO/R treated with vehicle(DMSO),MCAO/R treated with berberine. the transient focal ischemia/reperfusion was induced by in-troducing silicone-coated monofilament nylon suture from the right external carotid artery into the origin of the middle cerebral artery,which was re-moved after 60 min. In group treated with berberine,the rats were injected with berberine before and after suffered from cerebral ischemia. Similarly, in group vehicle,the animals received DMSO vehicle at the same time. the score of neurological behavior was evaluated 24 h after reperfusion. Mean-while,the rats were sacrificed for Nissl staining. to estimate cerebral edema,the wet-dry ratio was measured. the expression of AQP4 in the border of the infarct region in different groups was observed by Western blot. Results Compared with the model group,berberine improved neurological deficits(P < 0.05). Berberine treatment inhibited the neuronal deformation shown by Nissl staining(P < 0.05). Berberine significantly decreased the wet-dry ratio and reduced the expression of AQP4(P < 0.05). Conclusion these results suggested that berberine could induce neuroprotection against ischemic injury by inhibiting the expression of AQP4 in diabetic rats.
5.Evaluation of caudal block with dexmedetomidine mixed with ropivacaine for management of perioperative analgesia in children

Objective To evaluate the efficacy of caudal block with dexmedetomidine mixed with ropivacaine for the management of perioperative analgesia in children.Methods Sixty pediatric patients,aged 1-5 yr,of ASA physical status Ⅰ or Ⅱ,scheduled for elective hypospadias repair,were equally and randomly assigned into 2 groups using a random number table:ropivacaine group (group R) and dexmedetomidine mixed with ropivacaine group (group DR).Each patient received a single caudal dose of 0.25% ropivacaine 1 ml/kg in group R.Each patient received a single caudal dose of 0.25% ropivacaine 1 ml/kg mixed with dexmedetomidine 2 μg/kg in group DR.Postoperative analgesia was assessed using FLACC scale,and the degree of motor block was assessed using modified Bromage scale within 24 h after the end of operation.The duration of analgesia (the time from onset of caudal block to first requirement for the rescue analgesic) and development of side effects were recorded.Results Compared with group R,the duration of analgesia was significantly prolonged,and the incidence of bradycardia and oversedation was increased in group DR.Hypoxemia,hypotension and postoperative motor block were not found in either group.Conclusion Addition of dexmedetomidine 2 μg/kg to caudal ropivacaine can significantly optimize the efficacy of caudal block with ropivacaine alone for the management of perioperative analgesia in children.

8.The effectiveness and influencing factors of 131I treatment for cervical lymph node metastases of papillary thyroid carcinoma

Objective To evaluate the effectiveness and influencing factors of 131I treatment for cervical lymph node metastases of papillary thyroid carcinoma (PTC).Methods Eighty-nine post-operative PTC patients (24 males,65 females,11-72 years) with cervical lymph nodes as the only site of metastasis (negative serum TgAb,1-4 times of 131I treatment) on 131I planar Rx-WBS or SPECT/CT were retrospectively reviewed.Results of 131I Rx-WBS or SPECT/CT,serum Tg levels under both TSH-suppressed and TSH-stimulated conditions were measured and recorded.CR and PR were recognized as effective therapy.x2 test and logistic regression analysis were used to analyze the data.Results The effective rate of 1-4 times of 131I treatment was 57.30%(51/89),73.03%(65/89),80.90%(72/89),83.15%(74/89),respectively (x2=18.792,P<0.01).Logistic regression analysis suggested that therapeutic effectiveness was related to the size of metastatic lymph nodes and whether metastatic lymph nodes could be visualized on the post-ablative 131I Rx-WBS or SPECT/CT.Conclusions 131I treatment is an effective modality for PTC patients with cervical lymph nodes as the only site of metastasis.The cumulative effective rate increased within a limited times of treatment.The size of metastatic lymph nodes and whether metastatic lymph nodes could be visualized on the first post-ablative 131I Rx-WBS or SPECT/CT are the key factors for the therapeutic effectiveness.
9.Regulating Effects of Shen-Xiong Hua-Yu Capsule Preconditioning on Expression Subtype of NOS in Cerebral Ischemia-reperfusion Injury among Rats

This study was aimed to observe the regulating effect of Shen-Xiong Hua-Y u (SXHY) capsule precondi-tioning on the expression of subtypes of nitric oxide synthase (NOS), including endothelial nitric oxide synthase (eNOS), nervous nitric oxide synthase (nNOS), inducible nitric oxide synthase (iNOS), in cerebral ischemia-reperfu-sion injury (CIRI) among rats, and to further clarify the mechanism of protective effect by SXHY capsule on acute CIRI rats. Rats were randomly divided into the sham operation group, CIRI group, and SXHY capsule of high-, medium-, and low-dose preconditioning group (480, 240, 120 mg·kg-1). Each group was further randomly divided into different subgroups, which were the 3, 6, 12, 24, 48, 72 h group after 2 h CIRI (n=6). Intragastric administration was given once a day for 7 days. The middle cerebral artery occlusion (MCAO) and reperfusion model was repro-duced by an intraluminal filament method on the 7th day. The protein expressions of eNOS, nNOS and iNOS were measured by immunhistochemical method. The results showed that compared with the sham operation group, expres-sions of eNOS, nNOS and iNOS in the CIRI group were increased at different time points (i.e., 3, 6, 12, 24, 48, 72 h, P< 0.05 or P< 0.01). Compared with the CIRI group, eNOS expression increased at different time points in SX-HY capsule group (P< 0.05 or P< 0.01). The nNOS and iNOS expression decreased at different time points (P<0.05 or P < 0.01). Among them, the high-dose group was the group with the most obvious effect. It was concluded that SXHY capsule preconditioning had protective effect on CIRI. Its mechanism may be related to the regulation on protein expression of NOS subtypes.
10. Effect of age on prognosis of hepatocellular carcinoma patients with diabetes mellitus: A propensity-score matching analysis

Objective To explore the effect of age on prognosis of patients with hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) complicated with diabetes mellitus (DM) after radical resection. Methods The clinical data of 250 patients with HBV-related HCC complicated with DM who received radical resection were collected from the First Affiliated Hospital of Nanchang University and the Eastern Hepatobiliary Surgery Hospital and were retrospectively analyzed. The patients aged 30-80 years old and were divided into the elder group (≥60 years n=58) and the non-elder group (<60 years, n=192). Propensity score matching (PSM) method was used for covariate balance and the overall survival (OS) rates were compared between the two groups. Cox regression was used to analyze the risk factors influencing the prognosis of HCC. Results In the PSM cohort (n=53 in elder group, n=85 in non-elder group), the 1-, 3-, 5-year OS rates were not significantly different between the two groups (P =0. 15). The results of Cox multivariate regression analysis revealed that age was not significantly associated with OS of patients with HBV-related HCC complicated with DM. Conclusion Elder age seems not to decrease the OS of patients with HBV-related HCC complicated with DM after radical resection; elder patients can obtain similar survival as the non-elder ones.
