3.Trends in the development of gastrointestinal surgery in the era of minimally invasive surgery

The development of science and technology dramatically promotes the application of minimally invasive technique in the field of gastrointestinal surgery.Today,almost all kinds of gastrointestinal operations can be accomplished through or with the assistance of a laparoscope.Compared to traditional open surgery,lapm'oscopic surgery,which is accomplished by experienced surgeons.can leads to a similar or even better surgical outcome.Although the endoscopic technique was firstly invented as a diagnostic method,its application has expanded to many different medical areas.Now,endoscopic gastrointestinal polypectomy is extensively applied in primary hospitals.Moreover,endoscopic mucosal resection and endoscopic submucosal dissection are also ca~ied out in many large medical centers in China.More and more patients are benefiting a lot from the minimally invasive technique.As the concept of minimally invasive technique is widely accepted,minimally invasive surgery will be a major aspect in the development of gastrointestinal surgery.As a contemporary gastrointestinal surgeon,it is imperative to handle the minimally invasive technique proficiently.
4.Correlations of fundus lesions with prognostic significance in acute leukemia

Objective To determine retinal charges in newly diagnosed acute leukemia patients and to eval-uate the diagnostic value of retinal findings in patients with central nervous systerm leukemia(CNSL) and intracranial hemorrhage(IH). Methods 123 patients with acute leukemia(86 had myeloid leukemia and 37 had lymphoid leuke-mia) aged 14~77 years were examined with direct/indirect ophthalmoscopy for the presence of intraretinal hemorrha-ges (IRH), white-centred hemorrhages (WCH) and cotton-wool spots (CWS) within 5 days of diagnosis. Results CNSL and IH were more common in patients with retinopathy than those without (27.1% vs 9.1%, P=0.046; 29.2% vs 9.1%, P=0.029;respectively). Complete remission(CR) rate in patients with IRH was lower than those without(52.5% vs 80.5%, P =0.08). No association was found between the presence of WCH or CWS and CR rate. Conclusion The present study shows that patients with IRH is an indicator of poor prognosis in acute leukemia. Leukemic retinopathy usually is seen in patients with IH. Invasion of the eye by leukemia is not rare and indicates the need for early ophthalmological examination.

So plastic surgery does not equal to shape body according to the formal principles of aesthetics, it has to be directed by somaesthetics. Prudence in dealing with the cultural and time-varying differences of physical beauty embodies and even creates the culture of physical beauty.

7.Development of cytokine-induced killer cells in hematological oncology

Cytokine-induced killer ( CIK ) cells are a cluster of heterogeneous cells prepared from human peripheral blood mononuclear cells ( PBMC ) with the co-stimulation of some kinds of cytokines.CIK cells are characterized with rapid proliferation rate, high antitumor effects, broad tumoricidal spectrum, and few side effects.CIK cells have particularly applied in hematology.Now it starts a new generation of adoptive immunotherapy.In this article new development of CIK cells in department of hematology is reviewed.
8.Clinical comparison of proximal humeral locking plate and traditional AO plate in treatment of the elderly with proximal humerus fracture

Objective To compare the clinical efficacy of proximal humeral locking plate and traditional AO plate in treatment of the elderly with proximal humerus fracture.Methods 96 elderly patients with proximal humerus fracture were divided into observation group and control group by random number table,48 cases in each group.The control group was treated with traditional AO plate,while the observation group was treated with proximal humeral locking plate.Results 1 month,3 months and 1 year after operation,the SPADI index were (75.3 ± 3.1) points,(57.2 ± 2.7) points and (35.1 ± 2.2) points in the observation group,which were significantly lower than (87.1 ±2.8) points,(68.8 ± 2.9) points and (44.6 ± 2.5) points of the control group (t =8.442,7.025,6.293,all P <0.05).Excellent clinical efficacy of the observation group accounted for 72.92%,which was significantly higher than 54.17% of the control group (x2 =3.64,P < 0.05) ; the poor efficacy of observation group accounted for 8.33%,which was significantly lower than the 22.91% of the control group (x2 =3.87,P < 0.05).Conclusion The proximal humeral locking plate has good clinical efficacy for the treatment of the elderly with proximal humerus fractures,and it is worth to be promoted in clinical.
9.The application of limited decompression,fixation and fusion on the patients with degenerative lumbar spinal stenosis

Objective To study the effectiveness of limited decompression ,fixation and fusion on the patients with degenerative lumbar spinal stenosis .Methods 92 patients with degenerative lumbar spinal stenosis were ran-domly divided into two groups,the observation group(n=46 cases) who were operated through limited decompres-sion,fixation and fusion,and the control group(n=46 cases) who were operated through the lamina resection decom-pression and internal fixation and interbody fusion .Two groups were observed about surgery time ,blood loss and length of hospital stay.Follow-up for 6 months,the two groups of JOA score was compared.Results In the observation group,operative time,blood loss and length of hospital stay were shorter than that of the control group ( t=6.092, 7.114,3.714,all P<0.05).The average follow-up of six months,the two groups of preoperative JOA score and VAS scores were not significantly different (all P>0.05).After 1 month,3 months after operation,the JOA score was no significant difference between the observation group and the control group (t=0.810,0.901,P>0.05).JOA score in the observation group after 6 monthe of operation was higher (t=4.014,P<0.05).After 6 months,excellent rates were 78.3%in the observation group and 67.4%in the control group .The observation group was significantly higher than the control group (χ2 =7.210,P<0.05).Conclusion Limited decompression,fixation and fusion had a low trauma and significant effect for patients with degenerative lumbar spinal stenosis .
