1.Validation of the integration of health belief model and planned behavior theory

Objective:To establish and validate a new model of health belief model(HBM) combined with theory of planned behavior(TPB).Methods:Path analysis was applied to set up a new model predicting iron-fortified soy sauce consumption behavior using baseline survey data among women in rural and urban areas in Beijing,and the model was validated in follow-up survey.Results:It was proved that health values had powerful direct effect on behavior identity and had relatively strong direct effect on attitudes towards behavior;behavior identity had strong effect on behavior barriers,mostly in a direct way,and on behavior intention in a direct or indirect way;control belief was an important external factor influencing behavior intention;behavior intention was the most direct and most important one of factors influencing actual behavior;and convenience to buy was an important external factor influencing actual beha-vior.Conclusion:The integrated model of TPB and HBM explains behavior better and may be attempted in other similar researches.
2.Bone mesenchymal stem cells with allogeneic bone to repair canine mandibular defects:detection of osteogenic ability

BACKGROUND:Al ogeneic bone has anatomical appearance and biological features similar to autogenous bone, which is an excel ent biological scaffold material. Mesenchymal stem cel s originating from autogenous bone marrow have mutli-lineage differentiation potential, can differentiate into osteoblasts and chondrocyte, and thus can accelerate the formation of bone tissue and cartilage tissue. OBJECTIVE:To establish the osteogenic ability of al ogeneic bone with autogenous bone marrow mesenchymal stem cel s for repairing major mandibular defects. METHODS:The left mandibular teeth of 24 beagles were extracted, and at 2 months after wound healing, mandibular defects were made artificial y. The beagles were divided into two groups:control group treated with lyophilized al ogeneic bone, and experimental group with autogenous bone marrow mesenchymal stem cel s and lyophilized al ogeneic bone. Densitometry with CT and Micro-CT was conducted 4, 12, and 24 weeks after surgery. RESULTS AND CONCLUSION:Compared with the control group, the bone density of the mandible was significantly higher in the experimental group at 12 weeks after transplantation (P<0.05). Over time, the bone densities in the two groups were both increased, but the bone density in the experimental group was always higher than that in the control group. Bone structure parameters were progressively increased or decreased in the two groups, especial y in the experimental group. At 24 weeks after surgery, the degree of trabecular separation in regions of interest was higher in the experimental group than the control group (P<0.05), but the bone volume fraction, number of trabecular bone, and bone trabecular thickness were significantly lower in the experimental group than the control group (P<0.05). These findings indicate that bone marrow mesenchymal stem cel s are capable of accelerating the reconstruction of al ogeneic bones.
3.Lyophilizing canine allogeneic mandible:meeting the requirements for rebuilding strength, shape and support capabilities

BACKGROUND:The favorable structure and biological characteristics of al ogeneic temporomandibular joint become an effective solution for condylar defect, but immunologic rejection and slow ossification are the main problem for the presence of bone al ograft. OBJECTIVE:To meet the requirements of rebuilding mandible defect by lyophilizing canine mandible. METHODS:The periosteum, soft tissue, and cartilage of 12 canine mandibles were removed. 1 mm diameter hole was dril ed with 1 to 2 cm intervals in their cortical bones with a fissure bur. After washing, they were placed into a-4 ℃ refrigerator for 12 hours, and then stored at-80℃gradual y for 1 week. The mandibles were put in a drier. When the moisture content of osseous tissue decreased to 5%, the bones were packed in aseptic environment, radio pasteurized, and stored in vacuum container at atmosphere temperature. A biomechanical test was conducted after the lyophilization. RESULTS AND CONCLUSION:The maximum shifts of lyophilized mandibles in compression and bending tests were slight according to the steep load-shift curve. The plastic zone was insignificant and fractures appeared immediately when the pressure exceeded the plastic zone. The maximum load, maximum shift, and rigidity in the compression test were (5 163.10±730.16) N, (0.78±0.19) mm and (11 069.17±1 758.12) N/mm, respectively. The data in bending test were (486.67±134.12) N, (0.67±0.15) mm and (1 151.67±256.46) N/mm, respectively. It is concluded that the dehydrated and lyophilized canine mandibles have good shape and support ability and can meet the mechanical requirements in repairing and reconstructing mandibular defect.
4.Effects of elderly patients with long-term use of proton pump inhibitors for osteoporosis

Objective To investigate the effect of long-term proton pump inhibitor on osteoporosis in elderly patients.Methods A total of 150 patients with peptic ulcer treated in our hospital from January 2011 to January 2015 were selected as the observation group.150 healthy subjects were selected as the control group.The age,height,body weight and PPI time of the two groups were recorded.The changes of bone mineral density before and after treatment were measured by bone mineral density analyzer,ineluding lumbar L1-4,radial density and ulna density.The changes of bone mineral density were observed and recorded in the observation group before treatment,six months,1 year and 2 years after treatment.Results After treatment,the levels of gastrin were significantly increased in the observation group,and the serum calcium concentration and bone mineral density were significantly decreased (P<0.05).The density of lumbar vertebrae,radius and ulna was significantly lower in observation group than those of control group (P<0.05).With the prolongation of PPIs,lumbar vertebrae,radius and ulna density in observation group showed a decreasing trend.Conclusion Long-term application of proton pump inhibitors in elderly patients can cause bone loss.
5.Factors Influencing the Quality of Standardized Treatment for Patients with Post-Cardiac Arrest Syndrome.

BACKGROUND AND OBJECTIVES: For the present study, we investigated the factors that influence the quality of standardized treatment for patients with post-cardiac arrest syndrome (PCAS) to improve the quality of PCAS treatment. SUBJECTS AND METHODS: We collected data on patients with cardiac arrest (CA) who were admitted to the intensive care units (ICUs) of 11 hospitals-Class II Grade A or above-in Suzhou from January to October 2013. Indexes of standardized treatment were observed within 72 hrs of CA. We analyzed monitoring techniques, monitoring frequency, ICU human and material resources, and intensivists' knowledge of PCAS treatment to explore how those factors affected the management of patients with PCAS. RESULTS: The bed/nurse ratio and the frequency with which core temperature was recorded correlated closely with the implementation of therapeutic hypothermia (TH) within 6 hrs of CA. The bed/doctor ratio and intensivists' knowledge about PCAS correlated closely with high-quality blood glucose control within 6 hrs of CA. Furthermore, the frequency with which core temperature was recorded was an independent factor influencing the quality of TH implementation, and the number times blood gas was analyzed was an independent factor influencing how well partial pressure of carbon dioxide was kept within the normal range in the 6 hrs after CA. CONCLUSION: The frequency of core temperature measurements and the number of times blood gas is analyzed are the most important factors influencing the quality of standardized treatment for patients with PCAS.

6.Correlation analysis between drug resistance of Helicobacter pylori and the clinical eradication efficacy of bismuth-based quadruple therapies

Objective To investigate the correlation between the drug resistance of Helicobacter pylori (H .pylori )and clinical eradication efficacy of bismuth-based quadruple therapies,and to guide clinical rational drug use in the region.Methods A total of 260 patients with H .pylori infections were collected.H .pylori from biopsied gastric mucosa tissues were isolated and cultured.Drug resistant rates of isolated H .pylori to metronidazole,clarithromycin,amoxicillin,levofloxacin and furanzolidone were tested.Patients were randomly divided into clarithromycin,levofloxacin,furanzolidone and metronidazole groups by completely randomized design.All patients received bismuth potassium 220 mg,esomeprazole 20 mg and amoxicillin 1 000 mg twice daily,and according to group received clarithromycin 500 mg, levofloxacin 200 mg,furanzolidone 100 mg and metronidazole 400 mg,twice daily,espectively.The treatment course was 10 days.At least four weeks after treatment,13 Curea breath test or 14 Curea breath test was taken.According to the intention to treat (ITT)and per-protocal (PP),the eradication rate of each group was caculated.Chi square test was performed to compare the differences between groups. Results The drug resistant rate of H .pylori to metronidazole,clarithromycin,amoxicillin,levofloxacin and furanzolidone was 94.2% (146/155 ), 21 .3% (33/155 ), 2.6% (4/155 ), 5 .8% (9/155 ) and 1 .9%(3/155),respectively.According to ITT analysis,the eradication rate of clarithromycin group, levofloxacin group,furanzolidone group and metronidazole group was 81 .5 %(53/65 ),90.8%(59/65 ), 93.8% (61/65 )and 75 .4%(49/65),respectively.And according to PP analysis which was 84.1 %(53/63),92.2%(59/64),95 .3%(61/64)and 79.0%(49/62 ),respectively.The differences between furanzolidone group and metronidazole group,clarithromycin group were staistcally significant (χ2ITT =8.509 and 4.561 ;χ2PP = 7.592 and 4.323,all P < 0.05 ).There was no statistical significance in the H .pylori eradication rate between resistant strains and sensitive strains of each group.Conclusion Bismuth-based quadruple therapy can overcome antibiotic resistance,the eradication rate of protocal with furanzolidone is higher and with good safety,which can be the first-line treatment for H .pylori eradication.
7.Efficacy analysis of different bismuth-based quadruple therapies for two hundred and forty cases of Helicobacter pylori eradication

Objective To evaluate the efficacy and safety of different bismuth-based quadruple therapies for Helicobacter pylori (H.pylori) eradication.Methods From December 2012 to October 2013,240 patients with H.pylori infection were collected and evenly divided into clarithromycin group,levofloxacin group,furanzolidone group and metronidazole group.Each group received bismuth potassium citrate 220 mg,esomeprazole 20 mg and amoxicillin 1 000 mg twice daily.In addition,each group received clarithromycin 500 mg,levofloxacin 200 mg,furanzolidone 100 mg,and metronidazole 400 mg,respectively.The course of treatment was 10 days.At least four weeks after the end of therapy and withdrawal the medicine,patients underwent fasting 13C-urea breath test or 14C-urea breath test.The negative result indicated as successful H.pylori eradication.The adverse effects were observed and recorded during treatment.The rate of H.pylori eradication was analyzed by the intention to treat (ITT) analysis and per protocol (PP) analysis.Chi-square test was performed for eradication rate comparison among groups.Results According to ITT analysis,the eradication rate of clarithromycin group,levofloxacin group,furanzolidone group and metronidazole group was 81.67% (49/60),88.33% (53/60),93.33% (56/60) and 73.33% (44/60),respectively,and according to PP analysis which was 85.96% (49/57),89.83% (53/59),94.92% (56/59) and 75.86% (44/58),respectively.The differences among four groups were statistically significant (x2 =10.13 and 9.89,both P<0.05).The differences between furanzolidone group and metronidazole group were statistically significant (x2 =8.64 and 8.55,both P<0.01).There were no statisticaly significant differences in adverse effects among the four groups (x2 =0.47,P>0.05).Conclusion The H.pylori eradication rate is high in furanzolidone contained bismuth based quadruple therapy and with good safety,which could be the first line treatment for H.pylori eradication.
8.Impact and challenges of ChatGPT on medical science research management

Objective:To explore the influence and challenge of ChatGPT technology on medical scientific research management, and to provide references for better meeting the change of scientific research paradigm generated by artificial intelligence.Methods:Through literature research and case testing, combined with the main characteristics of ChatGPT, the possible application scenarios, risks, and challenges of ChatGPT technology in medical scientific research management were analyzed.Results:Countermeasures and suggestions were proposed from the perspective of medical institutions and research managers.Conclusions:ChatGPT technology will have a broad application prospect in medical scientific research management, and bring new risks and challenges. It is suggested to comply with the trend and actively face it by improving laws, regulations, and institutional norms, strengthening research and supervision, and improving the ability and quality of medical scientific research management personnel.
9.An experimental study on segmental defects reconstruction of canine mandible with allogenic bone marrow mesenchymal stem cells combined with lyophilized bone.

OBJECTIVETo investigate the effect of segmental defects reconstruction of canine mandible with allogenenic bone marrow mesenchymal stem cells (BMSC) combined with lyophilized bone.
METHODSA 30 mm segmental defect was created on the left mandibles of beagles. Beagles were randomly divided into three groups. Allogeneic bone marrow mesenchymal stem cells with lyophilized bone were used for segmental defects reconstruction in group A. Autologous bone marrow mesenchymal stem cells with freeze- dried bone were used for segmental defects reconstruction in group B. The defects of group C were repaired with lyophilized bone only. Every three beagles were sacrificed 4, 12, 24, and 48 weeks after surgery respectively. The reconstruction effect was evaluated by CT and histopathological examination.
RESULTSCT examination showed that new bones formed in group A and group B 12 weeks after surgery, but not in group C. The form of the reconstructed mandibles in the three groups recovered in 48 weeks. The small pores on the bone graft were filled with new bones in group A and group B. In group C, the pores were still evident. Histopathological examination showed that bone trabecula between allogeneic bone and autogenous bone was completely joined in group A and group B. A large number of fibrous tissue appeared around the implanted bone and new bones were formed. In group C, the lyophilized bone resorption was still not obvious, the new bone formation was significantly slower than the other two groups. There was no difference between group A and group B.
CONCLUSIONSBoth allogeneic bone marrow mesenchymal stem cells and autologous mesenchymal stem cells could accelerate the bone formation.

10.Evaluation and applications of antibody variable stability

Objective To study the intrinsic relationships between the binding energy of the antibody light and heavy chains and the conformational characteristics , physical and chemical properties , and to establish a corresponding mathemat-ical model and evaluate the thermal stability of the antibody molecules , which contribute to the antibody design , optimiza-tion and affinity maturation .Methods Based on bioinformatics and computational biology methods , the antibody′s structur-al information with the crystal diffraction data was analyzed .The conformational character of the variable domain of the antibody was studied using distance geometry and computer graphics technology .With the aid of the intermolecular hydrogen bond formation theory and the reaction free energy theory , the dynamic structure and energy characteristics be-tween the heavy and light chain variable regions of the antibody were studied .Furthermore , using nonlinear fitting and regression analysis, a mathematical model was set up .Results According to simulation and statistic analysis , there was a linear relationship between the binding energy and the number of the intermolecular hydrogen bonding , Van der Waals interaction of the heavy and light chains of the antibody .There was polynomial correlation between the binding energy and the physicochemical properties of the antibody .Using the frequency of amino acid position and the established model , the humanized anti-ricin antibody , which could not obtain the stable engineering cell line , was evaluated and optimized .The stable engineering cell line of the humanized anti-ricin antibody was obtained in the experiment .Conclusion The self structure of the antibody variable region ( conformation and physicochemical properties ) has much effect on its stability . The antibody stability can be improved by structural optimization .
