1.The Advance of Comorbidity of Metabolic Syndrome and Psychiatric Disease

The frequent co-occurrence of metabolic syndrome (MS) and mental disorders has become increasingly common, which is known clinically as comorbidity. Epidemiological survey showed that schizophrenia and mood disorders are the most common co-existing psychiatric disorders of metabolic syndrome. The pathogenesis of comorbidity of mental dis-orders and MS is related with hypothalamic-pituitary-adrenal axis dysfunction, unhealthy lifestyles and related applications of antipsychotics. An important reason for the metabolic syndrome associated with schizophrenia is heredity. However, in-flammation is the key factor in the relationship of mood disorders and MS. The intervention of unhealthy lifestyle, psychologi-cal counseling and rational use of drug should all be included in the treatment of comorbidity of psychiatric disorders and MS. The follow-up of metabolic index in psychiatric patients and psychological conditions in patients of MS should be paid more attention during the whole course of treatment.
2.Clinical Application of Fritillaria thunbergii Miq. Granule in Refractory Acute Leukemia during Perichemotherapy

Objective To translate the findings from fundamental research into clinical application and to evaluate the clinical efficiency and safety of Fritillaria thunbergii Miq. granule as adjunctive means of chemotherapy during peri chemotherapy of refractory acute leukemia. Methods Patients in multiple hospitals were randomly divided into two groups with Fritillaria thunbergii Miq. granule treatment or synchronous control at three days before chemotherapy respectively, according to random approaches for medical treatment. The clinical therapeutic effects were then determined after one course of treatment. Results According to the research project, 138 patients were analyzed statistically, 72 in Fritillaria thunbergii Miq. granule group and 66 in control group. The complete remission rate (CR) of Fritillaria thunbergii Miq. granule group and control group were 36.8% and 25.8% respectively, while the total effects were 77.8% and 53.0%, which was significantly different (P
3.Progress in Treatment for Advanced Gastric Cancer

Gastric cancer is the fourth most common cancer and is the second leading cause of cancer death worldwide. Gastric cancer is usually in advanced stage at diagnosis, without opportunity for curative resection. Chemotherapy is the major treatment for advanced gastric cancer (AGC), and other treatments include palliative surgery, radiotherapy, chemotherapy, biologic and immunologic therapy as well as Chinese medicine. Palliative therapy is used to reduce tumor load or complication occurrence, while radiotherapy is effective for locoregionally advanced gastdc cancer. At present, chemotherapy plays a leading role in AGC treatment and has a palliative effect on symptomatic patients. For advanced or metastatic gastric cancer, chemotherapy has always been the focus of studies. Results of studies of FAX, ECF/DCF regimens and Phrase ill clinical trials of REAL-2, ML17032 and SPIRITS have been released. With the application of EOX, XP, S-1/CDDP, IF, FOLFOX and XELOX regimens, the outcomes of advanced gastdc cancer are improved. However, there is no standard regimen accepted as supedor over others. Biologic and immunologic therapy are beneficial supplements to surgery, chemotherapy and radiotherapy, but are not yet the mainstream. All of these treatments have obvious side effects. Other comprehensive methods are of equal importance, such as Chinese herbal medicine, acupuncture and moxibustion, and psychological intervention. Combined with western medicine, the above methods have certain merits in relieving clinical symptoms, reducing toxicity,increasing effectiveness, improving quality of life, preventing metastasis and recurrence, reversing multidrug resistance of tumor cells, and curing ascites and managing cancer pain.

Blood hypercoagulabale state of malignant tumor was the pathological basis of tumor blood stagnation,which led to tumor proliferation and metastasis and also influenced the therapeutic effect and prognosis of tumor.Based on knowledge of thrombin and tissue factor system,We discussed the mechanism blood hypercoagulabale leading to tumor proliferation and metastasis,and the influence of traditional Chinese drugs of activating blood circulation to dissipate blood smsis on tunlor proliferation and metastasis.We concluded that traditional Chinese drugs have sound effects on prevention and treatment of tumor proliferation and metastasis.

Objective To summarize adverse reactions of common traditional Chinese medicine injection for the treatment of cardiovascular and cerebrovascular diseases,and put forward the countermeasures.Methods Adverse reactions of common traditional Chinese medicine injection for the treatment of cardiovascular and cerebrovascular diseases were arranged,and the reasons were analyzed.Results Adverse reactions were mainly skin and cardiovascular system diseases and so on,and were related with the quality of medicinal material,preparation process,and so on.Conclusion We should strengthen every link of production,from the medicinal plant to clinical use,improve the quality of traditional Chinese medicine injection,and strengthen the supervision to reduce the incidence of adverse reactions and ensure drug safety.
6.The Changes of HIF-1α and VEGF-A in Myocardial Tissue of Rats with Arrhyth-mias

Objective T o observe the expression changes of hypoxia inducible factor-1α (H IF-1α) and vascular endothelial grow th factor-A (V E G F-A ) in rats w ith arrhythm ias, and to explore the differences of the expression pattern in the tw o indicators of acute m yocardial ischem ia caused by arrhythm ias and coronary insufficiency. Methods T he arrhythm ia w as induced by C aC l2, and the expression changes of H IF-1α and V E G F-A w ere detected by im m unohistochem istry, W estern blotting and real-tim e PC R w ithin 6 h after the arrhythm ia in rats. Results T he expression of H IF-1α and V E G F-A show ed diffuse in the m yocardial tissue of rats died from arrhythm ias. B oth of them increased in the early arrhythm ia, then decreased. E xtensive m yocardial ischem ia happened at the beginning of arrhythm ia occurrence and its range didn't expand w ith tim e. Conclusion T he expressions of H IF-1α and V E G F-A in m yocardium of the rats w ith arrhythm ia can provide evidence for the differential diagnosis of acute m yocardial is-chem ia caused by fatal arrhythm ia and coronary insufficiency.
7.Study on the effect of new tracheal tube fixation device in "double fixation" of oral tracheal intubation

Objective:To understand the effect of a new tracheal tube fixation device in the "double fixation" of oral tracheal intubation for critically ill patients in adult emergency department.Methods:A retrospective analysis of patients with oral tracheal intubation admitted to our EICU from January 2016 to June 2017 was performed using traditional tape and inch band fixation as a control group; the experience of treatment from July 2017 to December 2018 for patients with oral tracheal intubation, the new double-fixation method of tracheal tube fixation device was used as the observation group. The degree of displacement, oral cleanliness, facial skin allergies and injuries, and comfort of the two groups of patients was compared.Results:The rate of tracheal displacement in the experimental group was mildly 11.2% (11/98), moderately 5.1% (5/98), and the control group was mildly 24.2% (15/62), moderately 14.5% (9/62) and severe 6.5% (4/62). The incidence of facial skin allergy and skin damage was 6.1% (6/98), 0, and the control group was 24.2% (15/62), 11.3% (7/62), the difference was statistically significant ( Z value was -4.021, χ2 value was 24.48, P<0.05). The comfort score of the test group was (1.05±1.01) points, which was significantly higher than (2.10±1.71) points of the control group ( t value was 4.920, P<0.01). Conclusions:The new type of tracheal tube fixation device "dual fixation" for critically ill patients with mechanical ventilation through oral endotracheal intubation is visually beautiful, and has good firmness and comfort. It can effectively reduce the occurrence of adverse events and has good application value.
8.Kongsheng Zhenzhong dispensing granule on tumor-related insomnia pa-tients with deficiency in heart and kidney

Objective To evaluate comprehensively therapeutic effect of Kongsheng Zhenzhong Pill (dis-pensing granule)(KSZZ),a classic TCMherbal formula,on tumor-relater insomnia patients with TCM pattern of deficiency in heart and kidney,so as to provide foundation for clinical application.Methods Sixty tumor-related insomnia patients with deficiency in heart and kidney who were conformed to the diag-nostic criteria were randomly divided into control group and treatment group (each n =30).Patients in treatment group took orally KSZZ twice daily,one pack 30 minutes after breakfast and dinner,and those in treatment group took orally estazolam at dose of 1 mg at night,for consecutive 28 days.Pittsburgh sleep quality index scale (PSQI)and TCMPattern Integral were used before treatment ,on Day 1,Day 14 and Day 28 after treatment,so as to record PSQI score,TCM clinical pattern and change of single symptom.Then the quality of sleep and TCM pattern curative efficacy were calculated by using SPSS 18.0 software.Results Compared with the control group,PSQI scores and TCM pattern integral decreased significantly (P <0.001).The curative efficacy of KSZZ on insomnia or TCMpattern was high-er than that of estazolam(P <0.001 ).The scoring of individual symptoms on Day 14 or Day 28 were lower obviously than those on Day 1(P <0.001),except sleep efficiency on Day 14 and Day 28 (P >0.05).Conclusion The curative effect of KSZZ on improving sleep quality of tumor-related insomnia patients has been proved,and KSZZ could be worthy of wide clinical application.
9.Expression regulation and forensic application of Brain Natriuretic Peptide

Brain natriuretic peptide (BNP), a natriuretic peptide hormone, is mainly secreted by cardiomyocytes when they are pulled by mechanical force. BNP acts to increase natriuresis and decrease vascular resistance, thereby decreasing blood volume, systemic blood pressure and afterload. It is not only the main indicators of cardiac function which is widely used in clinical, recent studies have also shown that it has a very important value in identifying sudden cardiac death(SCD). However, the exact expression of the regulation mechanism is still unknown. This paper reviewed the structure, expression of the regulation mechanism of BNP, and the application progress in clinical and forensic.
10.Ketogenic Diet in Infants with Early-Onset Epileptic Encephalopathy and SCN2A Mutation

Research has shown mutations in the voltage-gated sodium channel gene SCN2A to be associated with developmental delays and infantile seizures in patients with early-onset epileptic encephalopathies (EOEEs). Here, we report the case of an infant with a de novo SCN2A mutation with EOEE who had medically refractory seizures that improved with a ketogenic diet (KD) implemented at an age less than 2 months. On the day of his birth, the infant presented with a pattern of convulsions with dozens of episodes per day. An initial video electroencephalogram revealed poor reactivity of background activity, with multiple partial episodes starting from the right temporal region, and abnormal electrical activity in the right hemisphere. The seizures previously were not controlled with successive therapy with phenobarbital, topiramate, and levetiracetam. Genetic testing revealed the presence of a mutation in the SCN2A gene (c.4425C>G, p.Asn1475Lys). The infant’s seizures decreased significantly with a combination of KD and medication. The present case exemplifies the potential for personalized genomics in identifying the etiology of an illness. Furthermore, the KD appears to feasible in infants younger than 2 months and might elicit good responses to EOEE associated with SCN2A mutation.
