Objective:To survey the enablement level of elderly patients in the community and its influencing factors. Methods:A total of 460 patients aged ≥60 years visiting Changfeng and Fenglin Community Health Service Centers of Shanghai during September 3 to 15, 2018 were invited for a questionnaire survey. A self-made health status questionnaire, Patient Enablement Instrument (PEI), Chinese Health Questionnaire (CHQ-12), the Consultation and Relational Empathy Measure (CARE) were applied for survey. Results:All valid 460 questionnaires were retrieved. The positive rate of patient enablement was 73.91% (340/460). The median score for PEI was 3, Q1 was 0 and Q3 was 6. The univariate analysis showed that patient′s gender( Z=-2.32, P=0.02), age ( H=9.73, P<0.01), reason for visit ( H=42.06, P<0.01), health status in the past year ( H=8.28, P=0.02), consultation time ( H=22.92, P<0.01), sufficiency of communication with the doctor as patient thought ( H=41.83, P<0.01), detailed inquisition ( H=81.11, P<0.01), attending a contracted doctor ( Z=-3.42, P=0.04), the satisfaction of the visit ( H=38.64, P<0.01), total score of CHQ-12 ( Z=-2.01, P=0.04) and the score of CARE were significantly associated with the enablement level of the patient after the visit ( H=288.06, P<0.01). Multivariate linear regression showed that age 60-79( P=0.03), visit for acute disease( P<0.01), good or moderate health status in past year ( P<0.01), communication with the doctor<3 min or 3-<5 min ( P<0.01), sufficient or moderate time for communication with the doctor as patient thought ( P<0.01), detailed or moderate inquisition( P=0.03 or P=0.01), attending a contracted doctor ( P<0.01), CHQ-12 ≥4 ( P<0.01), middle or high CARE score ( P<0.01)were associated with the enablement of elderly patients after visit, with a cumulative contribution rate of 73.40%( F=50.08, P<0.01). Conclusion:The level of enablement of elderly patients after visiting the general outpatient clinic of the community health service center is relatively low. Patient′s age, types of disease, health status and the time of communication with the doctor, the sufficiency of communication time as patient thought, the detailed inquisition, attending a contracted doctor, the mental health status, and the empathy expression of the doctor are associated with the enablement of patients.

OBJECTIVE:To investigate clinical efficacy and safety of Teng medicine combined with Gongyankang granules in the treatment of chronic pelvic inflammatory disease(CPID). METHODS:A total of 141 CPID outpatients were selected from our hospital during Jul. 2015-Jun.2016,other patients were divided into control group(65 cases)and observation group(76 cases)ac-cording to random number table. Control group was given Gongyankang granules 9 g orally, bid;observation group additionally re-ceived Teng medicine hot compress on lower abdomen for 30 min until it was cool, once a day,on the basis of control group. Both groups began the medication since third day after the menstrual period, for consecutive 20 d. TCM syndrome, local sign scores and clinical efficacies were compared between 2 groups before and after treatment, and the occurrence of ADR and recur-rence were observed in 2 groups. RESULTS:Before treatment, there was no statistical significance in TCM syndrome scores or lo-cal sign scores between 2 groups (P>0.05). After treatment,TCM syndrome scores and local sign scores of 2 groups were de-creased significantly, and the observation group was significantly lower than the control group, with statistical significance (P<0.05). Clinical total response of observation group were 92.11%,which were significantly higher than 70.77% of control group, rate recurrence rate of observation group were 2.99%,which were significantly under than 15.79% of control group, with statistical significance (P<0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Teng medicine hot compress combined with Gongyankang granules show significant efficacy for CPID,can improve TCM syndrome and local sign and reduce recurrence rate with good safety.

Objective: To provide reference for promoting the use of Tengyao in the treatment of pelvic inflammatory diseases (PID). Methods: From January to March 2017, 111 health workers in department of gynaecology were investigated. The quantitative research of KAP questionnaire was used, and the content of questionnaire included personal information, knowledge, attitudes and practices. Results: A total of 111 copies of questionnaire were distributed and 95. 5% of them were effective. Interviewees' overall knowledge was at low level while the attitude was at high level; the main way to obtain knowledge was coming from colleagues (76. 42% ); working lifetime and speciality classification had significant impacts on the level of knowledge (P<0. 05); the majority of interviewees were willing to learn the relevant knowledge through training sessions (54. 72% ) and new media (27. 36% ); 48 inter-viewees would like to continue to use Tengyao for treating PID. The main problem of Tengyao in the treatment of PID was inconvenient operation (63. 64% ). Totally 83. 96% of interviewees considered that the dosage forms of Tengyao should be improved, and 50. 94% of interviewees considered that the clinical efficiency evaluation should be studied. Conclusion: Interviewees'overall knowledge is still at low level, especially in the junior medical staff. Pharmacists should improve the knowledge level of medical staff by meeting train-ing, and study how to improve the dosage forms and evaluate the clinical efficiency of Tengyao.

Objective:To analyze the efficacy of enzyme replacement therapy and anti-drug antibody production in children with Fabry disease. Methods:The clinical data of 7 children with Fabry disease treated with enzyme replacement therapy for more than 1 year at Children′s Hospital of Zhejiang University School of Medicine from July 2021 to June 2024 were retrospectively analyzed. The basic information and the changes of related clinical indicators before and after treatment were collected. Paired sample t test was used to compare renal function, left heart mass index, pain score and other related indexes before and after treatment. The anti-drug antibodies were detected by enzyme-linked immunosorbent assay. Results:A total of 6 boys and 1 girl were included. The age of diagnosis was (12.2±1.8) years. After 1 year of enzyme replacement therapy, the abnormal substrate globotriaosylsphingosine and brief pain inventory scores of all children were significantly lower than those before treatment ((16±11) vs. (63±42) μg/L, 22±19 vs. 45±29, t=3.88, 3.43, both P<0.05). There were no significant differences in glomerular filtration rate, urinary microalbumin to creatinine and left heart mass index before and after treatment ((124±35) vs. (136±26) ml/(min·1.73 m 2), (9.3±8.3) vs. (3.8±2.5) mg/g, (38±9) vs. (33±6) g/m 2.7, t=1.33, 1.74, 1.19, all P>0.05). Patients 4, 5 and 6 developed anti-drug antibodies at 1 month, 4 months and 1 month after medication, respectively. Patient 4 had persistently high anti-drug antibody titers (absorbance 3.65-3.73) accompanied by urticaria, elevated globotriaosylsphingosine and worsening clinical symptoms. Conclusions:The enzyme replacement therapy can effectively improve the clinical symptoms and reduce the level of globotriaosylsphingosine in children with Fabry disease. The anti-drug antibody is common in patients after long-term enzyme replacement therapy and may diminish the efficacy, which needs dynamic monitoring.
