1.Effect of ultrasonic cleaning of post space on the apical microleakage following treatment with two kinds of root canal sealers

BACKGROUND:The post space cleaning using ultrasonic file can improve the bonding strength of fiber postscemented with the self-adhesive resin cement, but the effect on apical sealing ability is stil unclear. OBJECTIVE:To evaluate the effect of the post space cleaning using ultrasonic file on apical microleakage fol owing treatment with two kinds of root canal sealers at two different residual lengths of root canal fil ing. METHODS:Eighty extracted human maxil ary anterior teeth with single-rooted canal were selected, and equivalently randomized into groups A and B (n=40 per group), fol owed by fil ed with Cortisomol or AH-plus, respectively. Groups A and B were randomly subdivided into four groups according to the residual length of root canal fil ing and with or without ultrasonic cleaning (n=10 per group):A1, B1 groups:residual 5 mm, no ultrasonic cleaning;A2, B2 groups:residual 5 mm, ultrasonic cleaning;A3, B3 groups:residual 3 mm, no ultrasonic cleaning;A4, B4 groups:residual 3 mm, ultrasonic cleaning. Subsequently, the apical microleakage was measured using dye method and transparent specimen technique under stereomicroscope. RESULTS AND CONCLUSION:The apical microleakage of each group was as fol ows:A1 (0.73±0.21) mm, A2 (1.37±0.55) mm, A3 (1.18±0.36) mm, A4 (2.05±0.62) mm, B1 (0.34±0.18) mm, B2 (0.47±0.14) mm, B3 (0.59±0.23) mm, B4 (1.39±0.50) mm. The three-factor analysis of variance revealed that the ultrasonic cleaning, root canal sealers and residual length of root canal fil ing al had a significant effects on the apical microleakage (P<0.05), and there were significant differences in the apical microleakage between groups A1 and A2, groups A3 and A4, groups B3 and B4, groups A2 and A4, groups B2 and B4 (P<0.05). These results indicate that the ultrasonic cleaning increases the apical microleakage fol owing the use of Cortisomol at 5 and 3 mm residual length, as wel as the use of AH-Plus at 3 mm residual length. Moreover, for the same root canal sealer, the longer residual length of root canal fil ing indicates, the less microleakage after ultrasonic cleaning.
2.Association of tumor necrosis factor-alpha gene of G-308A genotype and mRNA expression with serum uric acid in hyperuricemic patients

BACKGROUND: Hyperuricemia (HUA), serving as a component of metabolic syndromes (MS), has a major regulatory role in inflammatory responses. However, the correlation between G-308A tumor necrosis factor-α (TNF-α) and MS is still controversial.OBJECTIVE: To study the correlation among G-308A TNF-α genotype distribution, TNF-α mRNA expression and C-reactive protein (CRP) in patients with HUA.DESrGN: Patients with HUA were selected as the subjects, while normal peoplewere taken as the controls.SETTING: Tongji Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology.PARTICIPANTS: Subjects were selected from 5 003 people, including 188 patients with HUA and 51 healthy people.Patients with HUA included 40 patients with simple HUA, 42 patients with HUA and dyslipidemia, 80 patients with HUA and dyslipidemia and hypertension, and 26 patients with gout (administration of allopurinol was stopped at one week before the experiment).METHODS: TNF-α gene promoter G-308A genotype was determined by using PCR application followed by Nco/digestion.TNF-α mRNA expression was studied by RT-PCR with semi-quantitative analysis. Antibody sandwich method and enzyme linked immunosorbent assay (ELISA) were adopted to determine the CRP concentration. And the serum uric acid was measured by uricase method, while the insulin resistance index (IRI) was evaluated with homeostasis model.MArN OUTCOME MEASURES: ① Hardy-Weinberg equilibrium test of genotypic frequency. ② Changes in distribution of G-308A TNF-α genotypes and TNF-α mRNA expression in patients with HUA. ③ Changes in TNF-α mRNA expression and other parameters in G-308A TNF-α AA+GA and GG groups. ④ Multiple stepwise regression analysis.RESULTS: A total of 188 patients and 51 normal people were involved in the analysis of results. ① G-308A TNF-α genotypes distribution in 239 enrolled subjects accorded with Hardy-Weinberg equilibrium. ② AA+GA was compared with GG due to lower frequency of AA in population. The results revealed the HUA with dyslipidemia and hypertension group displayed a greater prevalence in G-308A TNF-α AA+GA genotype (30%) than that in the control group (12%) (P < 0.05),while there were no significant differences among other groups. ③ TNF-α mRNA expression and CRP concentration were the highest in the gout group, and HUA accompanied by dyslipidemia and hypertension group listed the second,and then was the HUA and dyslipidemia group, while simple HUA group listed the last, all of which were higher than those in the normal control group (P < 0.05-0.01). TNF-α mRNA expression in HUA accompanied by dyslipidemia and hypertension group, gout group was higher than that in the normal control group (P < 0.05-0.01). The WHR, SBP, DBP,serum uric acid, triglycerides, TNF-α mRNA expression and CRP concentration in AA+GA group were remarkably higher than those in the GG group (P < 0.05-0.01). ④ After adjustment of age, gender, systolic pressure, diastolic pressure,fasting blood glucose, and multiple variable linear stepwise regression showed that triglycerides, uric acid, TNF-α 308 A carrier were related with quantityof TNF-α mRNA expression. Meanwhile, the uric acid concentration, WHR and the quantity expression of TNF-α were significantly in positive association with CRP concentration.CONCLUSTON:The genotype of TNF-α 308 A carriers is associated with TNF-α expression and plasma CRP levels in HUA patients.
3.Efficacy of simplified binding pancreaticojejunostomy

Objective To investigate the methods and techniques of simplified binding pancreaticojejunostomy for patients with periampullary malignant tumor after radical pancreatoduodenectomy (RPD). Methods From March 2005 to May 2010, 323 patients with periampullary malignant tumor received RPD at the Tongji Hospital of Huazhong University of Science and Technology, and their clinical data were retrospectively analyzed.Simplified binding pancreaticojejunostomy was applied after RPD: the distal end of pancreas was freed for 3-4 cm;a No. 6 or No. 8 silicone urinary catheter was inserted into the pancreatic duct for 4-5 cm, and the remaining urinary catheter (6-8 cm) out of the pancreatic duct was sutured to the pancreatic stump with absorbable sutures.The cutting end of the jejunum (2-3 cm) was everted, and the everted mucosa of the jejunum ( 1 cm) was injured by electrocautery, then the everted jejunum was reverted to its normal position. The cutting end of the mesentery of jejunum and its opposite side, as well as the mid-point of these two parts were sutured symmetrically with the lower and upper edges of the pancreas, and with the capsule of pancreas between them. The everted jejunum was wrapped over the pancreatic stump and sutured it to the pancreas for fixation. The cutting end of the jejunum was bound to the pancreatic stump with 1-0 absorbable suture after confirming the jejunum was completely invaginated into the pancreas. The alimentary tract was reconstructed by using Child's method. Results Simplified binding pancreaticojejunostomy was successfully completed in all patients, Pancreatic fistula was detected in one patient who was complicated with anastomotic bleeding on the third day after secondary laparotomy. The patient was discharged with catheter and spontaneously recovered one month later. Pancreatic fistula was also detected in two patients with distal bile duct carcinoma and two patients with carcinoma in the uncinate process of pancreas at postoperative day 3, 6, 8 and 11, and they were cured by expectant treatment. The incidence of pancreatic fistula was 1.5% (5/323). Conclusion Simplified binding pancreaticojejunostomy is simple, safe and feasible, and it can significantly reduce the incidence of pancreatic fistula.

5.The effect of transforming growth factor β1 in the transition of bone marrow-derived macrophages into myofibroblasts during renal fibrosis

To study which subgroup of bone marrow derived cells formed myofibroblasts and the mechanism that transforming growth factor β1 (TGFβ1) regulates the formation of bone marrow derived macrophages into myofibroblasts during renal fibrosis.Chimeric mice were generated by lethally irradiation of C57 mice followed by transfusion of green fluorescent protein (GFP) labeled bone marrow cells.Complete marrow reconstitution was developed until 12 weeks after transplantation.The mice were randomly divided into Sham operation group,unilateral ureteral obstruction (UUO) 3 days group,UUO5 days group,UUO7 days group and UUO7 with TGFβ1 treatment group.Each group had four mice.Flow cytometry was used to evaluate cell components.Compared with Sham operation group the proportions of GFP + CD+14 oα-smooth muscle actin(α-SMA) + cells,GFP + CD+44 CD+105 α-SMA+ cells and GFP+ F4/80+ α-SMA+ cells in each UUO group were progressively increased and the transformation rate in UUO7 day group was the highest.The GFP+ F4/80 + α-SMA+ cells accounted for the largest population.TGFβ1 promoted the transformation of bone marrow derived macrophages into myofibroblasts.Compared with Sham operation group or UUO7 day group,the proportion of GFP+ F4/80+ α-SMA+ cells increased in UUO7 day TGFβ1 treatment group.Compared with Sham operation group (or UUO7 days group) the protein expressions of F4/80,oα-SMA,Collagen Ⅰ increased in UUO7 with TGFβ1 group.Bone marrow derived macrophages are considered as the main type of myofibroblast precursors during the development of renal fibrosis.TGFβ1 regulates the transformation of bone marrow-derived macrophages into myofibroblasts.This process contributes to progressive renal fibrosis and deterioration of renal function.
6.Influencing factors of serum uric acid and the critical concentration of serum uric acid to prevent and treat metabolic syndrome in female inhabitants from coastal area of Shandong province

BACKGROUND: What are the influencing factors of serum uric acid of inhabitants from coastal area? What is the critical concentration of serum uric acid to prevent and treat metabolic syndrome?OBJECTIVE: To probe into the relationship between serum uric acid and metabolic syndrome in female inhabitants aged more than 20 years from coastal area of Shandong province.DESIGN: A clusting stratified random sampling survey.SETTING: Department of Endocrinology, Affiliated Hospital of Medical College of Qingdao University.PARTICIPANTS: The survey was carried out in the female inhabitants of five cities from coastal area of Shandong province (Qingdao, Rizhao, Yantai, Weihai and Dongying) between May and October 2004. The inhabitants, aged 20 to 80 years, lived there for 5 or more than 5 years, and they were natural crowd taking family as unit.METHODS: Investigations in the manner of entering every family and being on the spot were combined. Questionnaires were filled in on the first day, and fasting blood was taken to perform serum uric acid examination on the morning of the second day. For those with serum uric acid higher than normal, they were given rechecking on the third day, and education about prevention and treatment of gout and hyperuricemia was conducted at the same time.MAIN OUTCOME MEASURES: ① Investigation on general condition:Including health status, diet, physical activity, labour intensity and economics. ② Investigation on nutrition: Consists of food intake frequency and dietary. ③ Body height, body mass, waistline, hip circum, blood pressure and body mass index. ④ Levels of fasting blood glucose(FBG), serum uric acid, total cholesterol (TC), triacylglycerol (TG), high-density lipoprotein cholesterol(HDL-C), urea nitrogen (UN) and creatinine(Cr).RESULTS: ① With the increase of concentration of serum uric acid, levels of systolic pressure (SP), diastolic pressure (DP), pulse pressure (PP),body mass index(BMI) , waistline, waist-to-hip ratio(WHR), UN, Cr, TG,TC were all gradually increased. Each index was significantly higher in the serum uric acid 280-319 μmol/L group, serum uric acid 320-349 μmol/L group and serum uric acid > 350 μmol/L group (hyperuricemia group)than that in serum uric acid < 280 μmol/L (P < 0.05-0.01 ), While the concentration of HDL-C was decreased with the increase of concentration of serum uric acid (all P < 0.01). ② The incidence of hypertension, lipid metabolic disorder, overweight and obesity, glycometabolism disorder as well as metabolic syndrome was increased with the increase of uric acid;Compared with serum uric acid < 280 μmol/L group, OR value of metabolic syndrome was 2.29(95%CI: 1.81-2.89),4.15(95%CI:3.10-5.55),4.96 (95%CI:3.85-6.39) in the serum uric acid 280-319 μmol/L group,320-349 μmol/L group and hyperuricemia group , respectively. ③Noncondition Logistic multiple stepwise regression analysis showed that age,hypertension, shellfish intake volume, UN, Cr, TG and WHR and light physical activity were the independent risk factors for female patients with hyperuricemia and HDL-C was protective factor.CONCLUSION: The incidence of metabolic syndrome is increased with the increase of concentration of serum uric acid in female inhabitants from coastal area of Shandong province. 280 μmol/L should be as the critical concentration of serum uric acid to prevent and treat metabolic syndrome.Controlling metabolic syndrome and reducing intake of shellfish and other marine products which contain high level of purine is one of means to prevent hyperuricemia. Clinical physicians should paid more attention to the pathopoiesis of hyperuricemia.
7.Research on the pancreatic injury after autologous liver transplantation in rats

Objective To investigate the causes of pancreatic injury after autologous liver transplantation in rats. Methods Forty-two SD rats were randomly divided into post autologons liver transplantation 1-hour group, 6-hour group, 12-hour group, 24-hour group, 48-hour group, 72-hour group and sham group (6 rats per group). The plasma concentrations of amylase and lipase were measured to assess pancreatic exocrine function. The histomorphological changes of pancreatic tissue were studied under optical and electron microscopes. All data were analyzed via one-way ANOVA. Results The plasma concentrations of amylase and lipnse in post autologous liver transplantation 1-hour group were significantly higher than those in sham group, and they gradually increased as time passed by. The plasma concentrations of amylase and lipase reached peak at hour 48, after which they decreased gradually. There was a significant difference in the plasma concentration of amylase and lipase among the 7 groups (F = 538.622,489.417, P < 0.05). Acute edematous pancreatitis was observed 1 hour after autolognus liver transplantation, and acute hemorrhagic necrotic pancreatitis was observed 6 hours after transplantation. The degree of injury reached a peak 48 hours after transplantation. The number of mitochondria was increased, and endoplasmic reticulum and Golgi apparatus were swollen 1 hour after transplantation, and the area, perimeter, specific surface area and mean gray value of mitochondria were (312±40) mm~2, (80.3±3.8)mm, 0.332±0.039 and 113±11, respectively. As time passed by, the injury of the pancreatic cells was aggravated and autophagosomes were observed. The injury was most severe 48 hours after transplantation, and the area, perimeter, specific surface area and mean gray value of mitochondria were (466±7) mm~2, (108.8±3.7) mm, 0.298±0.009 and 195±12, respectively. There were significant differences in the specific surface area and mean gray value among all the groups (F = 9.322, 76.560, P < 0.05). Conclusion The pancreatic injury after autologous liver transplantation is related to the energy metabolism of the pancreatic cells induced by hypoxia.
8.Effect of hypoxia and deferoxamine on transplanted liver

Objective To compare the protecting effect of hypoxia preconditioning(HP) and deferoxam-ine preconditioning (DP)on transplanted livers and to search a protecting method which is suitable for clini-cal practice.Methods SD rats are randomly divided into four groups:sham operation group (S),analogic transplantation group (AT),deferoxamine preconditioning group (DP) and hypoxia preconditioning group (HP).Twenty-four hours after operation,several factors of each group were measured respectively,inclu-ding deterioration of histomorphology,hepatic zymogram in the serum,hypoxia inducing factor-1 (HIF-1α)and malonaldehyde (MDA) in hepatic tissue.Results The levels of alanine aminotransferase (ALT),glu-tamic oxalacetic transaminase (AST) and alkaline phosphatase (ALP) in AT group,DP group and HP group were higher than S group's.Levels of ALT,AST,ALP in AT group were dramatically high,hepatic tissue suffers from edema,degeneration even necrosis.By contrast,the pathological change is milder in DP group and HP group.HIF-1α in hepatic tissue of AT group was lower than that in DP group and HP group,MDA was higher than two latters (P < 0.05).Conclusion DP can protect transplanted liver as same as HP,however,DP is more suitable for clinical practice.
9.Detection of immunoglobulin and TCR gene rearrangements by PCR using BIOMED-2 multi-plex protocols in T lymphoblastie lymphoma and acute lymphoblastic leukemia

Purpose To investigate the sensitivity of BIOMED-2 primer system in T lymphoblastic lymphoma ( T-LBL) and acute lym-phoblastic leukemia ( ALL) patients immunoglobulin ( Ig) and T-cell receptor ( TCR) gene rearrangement, and to analyze the co-rear-rangement pattern. Methods Amplification of rearranged Ig and TCR gene was performed in standard PCR in 35 T-LBL/ALL pa-tients. PCR products were analyzed by heteroduplex and polyacrylamide gel electrophoresis. Results 16 cases (45. 7%) of 35 sam-ples were detected to have TCR gene rearrangements, including 6 cases (37. 5%) of TCRβgene monoclonal rearrangements, 4 cases (25. 0%) of TCRγ gene monoclonal rearrangements, 3 cases (18. 8%) of TCRβ and TCRγ gene double rearrangements, 2 cases (12. 5%) of TCRδ gene monoclonal rearrangements and 1 case (6. 3%) of TCRγand TCRδgene double rearrangements were detec-ted. 4 cases (11. 4%) of 35 samples detected to have clonal immunoglobulin and TCR gene rearrangements. 11 cases (39. 3%) of 28 T-LBL patients were detected to have TCR gene rearrangements, 6 cases (85. 7%) of 7 T-ALL have TCR gene rearrangements. Con-clusions BIOMED-2 multiplex PCR analysis strategy is a useful technique in the T-LBL patients.
10.Effect on the micro- and ultramicro-structure of intrahepatic bile duct mediated by hypoxic preconditioning after liver graft
