1.Algorithm expression on the optimal reconstruction increment in coronal multi-planar reformation of temporal bone for multislice CT

Objective To study algorithm expression of the optimal reconstruction increment with which images could be reconstructed without loss of z-direction spatial resolution for multislice spiral CT.Methods Using Gauss function and signal sampling principle,an algorithm expression was deduced to calculate the optimal reconstruction increment with which images could be reconstructed without loss of zdirection spatial resolution.Spiral slice sensitivity profile (SSP) phantom was scanned using Somatom Sensation 64-slice spiral CT and temporal bone protocol as those used for clinic,axial images were reconstructed with slice thickness of 0.600 mm and increment of 0.100,0.300,0.400 and 0.500 mm respectively.Then SSPs and full width at half maximum (FWHM) were measured and modulation transfer functions were obtained by Fourier transfer from SSPs.Axial CT scan of 7 normal temporal bones in 6 patients were obtained by the same CT system and parameters as above.Coronal MPR images of temporal bone out of different reconstruction increment were obtained and the quality of reconstructed images were independently assessed by three senior radiologists using a four-point scale and blinded the information of reconstruction.Experimental data were processed and two-way ANOVA(in which Dunnett t test was selected for multiple comparisons) was performed with statistic software SPSS10.0.P < 0.05 was considered as significant difference.Results (1) The measured FWHM of reconstruction slice thickness of 0.600 mm was 0.665 mm,so the optimal reconstruction increment calculated with the algorithm expression in this article was 0.296 mm(≈0.300 mm).Objective evaluation showed that there was obviously aliasing in high spatial frequency range of MTF curve when reconstruction increment was more than 0.300 mm,i.e.both 0.400 and 0.500 mm.Clinical scores of coronal MPR images of temporal bone reconstructed with different increment had significant difference(F =505.374,P < 0.01).Treating reconstruction increment of 0.100 mm as a control group and comparing another groups against it,there was no significant difference for reconstruction increment of 0.300 mm (t =-0.222,P > 0.05),while there were significant difference for 0.400 and 0.500 mm (t =-1.333、-15.889,P < 0.05,P < 0.01).Conclusion It was proved that the angorithm expression deduced in this article was proper because of the consistence between the calculated value and the results from objective and clinical evaluation.The optimal reconstruction increment without loss of z-direction spatial resolution is 0.445 times as large as FWHM for multislice CT.
2.Percutaneous transluminal angioplasty of tibioperoneal arteries for severe limb ischemia

Objective The purpose of this study was to assess the technical feasibility, safety and effectiveness of percutaneous transluminal angioplasty (PTA)for the treatment of severe limb ischemia caused by tibioperoneal arteries occlusion. Methods From June 2004 to May 2007,35 patients with critical limb ischemia (CLI)were treated on an intention-to-treat basis with tibioperoneal arterial PTA. Main outcome measures were technical success rate, primary patency rate, limb salvage rate and complications. Results The technical success rate of tibioperoneal arterial PTA Was 83%.Mean follow-up time was 11.5 months, primary patency rate and limb salvage rate were 57%and 82%,respectively.Mean length of tibioperoneal arterial PTA was 9.5 cm(4.5~14 cm),concurrent treatment of iliac artery or femoral-popliteal artery was carried out in 19 patients via either angioplasty or combined with stenting.There were three complications including arterial spasm and thrombosis in one, which Was relieved by thrombolysis and antispasmodics, and artery perforation in 2 cases, which was treated conservatively. Conclusion In tibioperoneal arterial occlusions in CLI patients, PTA is feasible with a high initial technical success rate and high limb salrage rate,tibioperoneal arterial PTA carries a lower morbidity and less severe complications. PTA is a safe, effective and good treatment alternative for CLI patients of tibioperoneal arterial occlusions.
3.Subintimal angioplasty for the treatment of chronic ischemia with arteriosclerosis occlusions in the lower extremity

Objective To assess the technical feasibility, patency and outcomes of subintimal angioplasty (SIA) for treatment of the lower extremity arteriosclerosis occlusions. Methods During the period from December 2003 to August 2008, 122 lower extremities with arteriosclerosis occlusions (median length, 10.25 cm;range, 4.5 to 28 cm) were treated on an intention-to-cure basis with SIA. Twenty-three lower extremities had disabling claudication and ninety-nine had limb-threatening ischemia. Patient history, demographics, procedural details, complications, and follow-up information were collected and analyzed. Patency, limb salvage, sustained improvement in claudication was determined by Kaplan-Meier analysis. Results The technical success rate of SIA was 83%. Following successful SIA, the mean ankle-brachial index increased from 0.19±0.11 to 0.67±0.29 (P<0.01). Primary patency at 12 and 24 months was 54%±5% and 45%±4% respectively, the clinical effective rates (rates of limb salvage and improvement in claudication) at 12 and 24 months were 82%±5% and 79%±4% respectively. No serious complications occurred. Conclusion In a selected group of patients, SIA is feasible with a high technical success rate and the clinical effective rates are satisfactory, there were no severe complications. SIA is a good treatment alternative in patients suffering from chronic ischemia with arteriosclerosis occlusions in the lower extremity.
4.Effect of different dispensing methods on content of omeprazole sodium for injection

Objective To select omeprazole content changes smaller with dispensing method and to seek for rationality of off-label uses.Methods To measure content change of omeprazole sodium for injection mixed by different subscriptions at different time through HPLC, and compared effect of different dispensing methods on content of omeprazole sodium for injection.Results 10 mL 0.9%sodium chloride injection was chosed as dissolvent,the change of omeprazole sodium for injection content would be minor, and stability of drug solution was superior.Conclusion Dispensing methods of drug impact on its'security and validity, which is part of discuss category about medicine rational use as well.Off-label uses could not vest in unreasonable use, which should contingent on specific document,data and actual environment of medical treatment.
5.EFFECTS OF BU GU ZHI(PSORALEA CORYLIFOLIA L) AND BAI ZHI (DAHURIAN ANGELICA ROOT) EXTRACTS ON MELANOCYTE ADHESION AND MIGRATION

Objective To find direct effect of Chinese herbs Bu Gu Zhi (Psoralea corylifolia L) and Bai Zhi (Dahurian angelica root) Extracts on melanocyte adhesion and migration in vitro. Methods Ethanol extracts obtained from two kind of Chinese medicinable herbs were tested. Human melanocytes were obtained from neonatal foreskins and 48-well culture dish covered with fibronectin were used to perform melanocyte adhesion assay; Motility was assessed using the micropore filter method. Results: The extracts of Bu Gu Zhi(Psoralea corylifolia L), Bai Zhi(Dahurian angelica root) obviously showed an effect in increasing of human melanocyte adhesion and migration on fibronectin. Conclusion It is suggested that Buguzhi(Fructus Psoraleae) and Baizhi(Radix Angelicae Dahuricae) might induce melanocyte adhesion and/or migration in the treatment of vitiligo.
6.Reconstructive options for critical limb ischaemia in infrapopliteal arteries

Objective To assess reconstructive options for critical limb ischaemia in infrapopliteal arteries. Methods A retrospective review of all CLI patients who underwent infrapopliteal reconstruction was carried out. Patient history, demographics, procedure details, complications, and follow-up information were collected and analyzed. Patency, limb salvage rate was determined by Kaplan-Meier analysis. Results During the period (from December 2003 to January 2008 ), 123 CLI patients with arteriosclerosis occlusions were treated on an intention-to-treat basis with infrapopliteal percutaneous transluminal angioplasty (PTA).Thirty-three thromboangiitis obliterans and twenty-three arteriosclerosis occlusions suffering CLI were treated by infrapopliteal bypass procedures. Primary patency and limb salvage rate of infrapopliteal PTA at 6, 12 and 24 months was 67%, 54%, 49% and 91%, 85%, 78% respectively, Primary patency and limb salvage rate of infrapopliteal surgical bypass at 6, 12 and 24 months was 90%, 83%, 79% and 92%,87%, 80% respectively, the patency of infrapopliteal PTA was lower than infrapopliteal surgical bypass (P <0. 01 ), but the limb salvage rate of infrapopliteal PTA and open surgery was no significant difference (P > 0. 05 ). Conclusion Endovascular treatment (PTA) in patients with infrapopliteal arteriosclerosis occlusions and critical ischaemia is safe, effective. Infrapopliteal PTA can be used as the choice of therapy and surgical bypass reserved in those endovascular treatment failed. While in CLI patients with thromboangiitis obliterans infrapopliteal artery bypass remains the best treatment option.

AIM: To determine the effect of hydrogen peroxide(H_2O_2) on voltage-gated potassium channel currents(IKv) in pulmonary vascular smooth muscle cells(PASMCs).METHODS: Using whole cell patch-clamp technique,IKv was recorded in freshly isolated rat PASMCs with acute enzymatic digestion method.The effect of hydrogen peroxide on IKv in PASMCs was investigated in normoxia.RESULTS: IKv in PASMCs was increased significantly by H_2O_2 and the increase depended on the concentration in normoxia.Current-voltage relationship curve shifted to the left.CONCLUSION: Hydrogen peroxide is an important K~+ channel opener.

This experiment aimed to investigate the effect of adrenergic system in the subnucleus commissuriu of nucleus solitrius tractus (CNTS) on renal nerve discharges. Norepinephrine (NE) was microinjected into the CNTS of rabbits and mean arterial blood pressure (MAP) and renal nerve discharges (FRND) were synchronously recorded. The results indicated that (1) microinjection of norepinephine into the CNTS of rabbit could significantly attenuate the frequency of renal nerve discharge, and at the same time decrease markedly the mean arterial pressure. (2) Microinjection of 0.3 nmol yohimbin into CNTS had no significant influence on FRND and MAP, but could attenuate and even reverse the effects of NE on FRND and MAP. These results suggest that microinjection of NE into CNTS may activate the alpha-adrenorecptor located in CNTS and secondarily produce a depressor effect by attenuating the activity of periphenal sympathetic nervous system.

10.Effects of 17β-estrogen on expressions of C-reactive protein and its mRNA of vascular smooth muscle cells in rats.

OBJECTIVETo investigate the effects of 17β-estrogen on expressions of C-reactive protein (CRP) and its mRNA in vascular smooth muscle cells(VSMCs).
METHODSImmunocytochemistry was used to detect CRP level in normal VSMCs. The expressions of C-reactive protein and p-ERK1/2 in Ang-II-stimulated VSMCs were evaluated with Western blot. C-reactive protein mRNA was examined with RT-PCR.
RESULTS17β-estrogen had no effect on cell morphology and C-reactive protein expression in normal VSMCs; however, C-reactive protein and mRNA, as well as p-ERK1/2 were decreased in Ang-II-stimulated VSMCs after 17β-estrogen treatment in a concentration-dependent manner.
CONCLUSION17β-estrogen may inhibit the expression of C-reactive protein and its mRNA in Ang-II-stimulated VSMCs via ERK1/2 signal transduction pathway in a concentration-dependent way.
