1.Overseas Education in Military Medical Universities:Working Summary and Further Reflections

In light of the situations of overseas education in the 21st century,this paper summarizes the general working guidelines for and concrete measures of the overseas education in military universities from four aspects:tapping the full potentials of the university to carry out comprehensive education;renovating training modes to implement individualized teaching;insisting on being human-centered to show the theme of friendship;exerting great efforts to develop a first-rate overseas training base.
2.Analysis on Monitoring of Serum Concentration of Carbamazepine in Epileptic Children

OBJECTIVE:To approach the correlations of carbamazepine concentration in serum with clinical therapeutic effect,dosage,sex,age and body weight in epileptic children.METHODS:158 monitoring records for serum concentration of carbamazepine in children with epilepsy in our clinical pharmacy department were consulted retrospectively.Data such as serum concentration,sex and age were recorded into data base and were analyzed using SPSS 13.0 software.RESULTS:No significant correlation was noted between serum concentration of carbamazepine in epileptic children and sex.However,serum concentration of carbamazepine was closely correlated with clinical therapeutic effect,dosage,age and body weight.CONCLUSION:The individual difference in the metabolic processes of carbamazepine was concerned with age and body weight,so it's necessary to make individualized regime according to child's specific condition and monitoring results.
3.Panax notoginseng saponins can inhibit apoptosis of renal cells induced by cisplatin through path of mitochondrion

Aim To investigate the effects of panax nonstaining saponins ( PNS ) on the apoptosis of renal cells induced by cisplatin through the path of mitochon-drion . Methods Male Sprague-Dawley( SD) rats were randomized divided into normal control group, cisplatin model group and the cisplatin+PNS group,with 12 rats of each group. Animals were sacrificed to determine the N-acetyl-β-D-Glucosaminidase ( NAG ) in urine, blood urea nitrogen ( BUN) and serum creatinine ( Cr) concentrations after 8d of intraperitoneal injection. HE-staining was employed to observe renal pathologies. Transmission electron microscopy ( TEM ) was em-ployed to observe the mitochondria of the rats′ injured kidney region. TUNEL staining was employed to detect the distribution of apoptotic cells. Immunnohistochem-istry was used to detect Bax and caspase-9 expression, and expression of Bcl-2 protein was detected by West-ern blot. Results Compared with the normal control group, contents of BUN, serum Cr and urinary NAG levels of rat in the cisplatin model group were increased ( P <0. 01 ) , and some mitochondria of the epithelial cells of renal tubules got injured seriously. The apopto-sis rate of kidney cell was increased ( P<0. 01 ) . The expression of Bax,caspase-9 and Bcl-2 proteins was in-creased ( P < 0. 01 ) . Compared with the cisplatin model group, contents of BUN, serum Cr and urinary NAG levels of rats in the cisplatin model group were decreased ( P <0. 01 ) , and some mitochondria of the epithelial cells of renal tubules were significantly im-proved. The apoptosis rate of kidney cell was decreased ( P<0. 01 ) . The expression of Bax and caspase-9 pro-tein was decreased (P<0. 01),but Bcl-2 protein was increased ( P < 0. 01 ) . Conclusion PNS may in-crease the expression of Bcl-2 protein and decrease the expression of Bax and caspase-9 proteins, which may play a protective role in cisplatin nephritic damage.
4.Investigation of Irrational Infusion Prescriptions for Outpatients from 2005 to 2007 in Wuhan Children's Hospital

OBJECTIVE: To investigate the irrational use of the infused drugs in our out-patient clinic.METHODS: In this retrospective study,the infusion prescriptions in our outpatient clinic from 2005 to 2007 were randomly selected for a statistical analysis of the irrational ones based on our knowledge on clinical pharmacology and literature.RESULTS: The problems manifested by the total 81 172 prescriptions were unsuitable solvent,improper dosage regimen,irrational drug combination and repeated drug of medicines etc.CONCLUSION: The use of infused antibiotics in our out-patient clinic were rational on the whole,however,there were some problems which remain to be solved and improved.
5.Pediatric Drug Counseling Telephone Service:Analysis of 542 Cases

OBJECTIVE:To probe into the pediatric drug counseling service and the characteristics of adverse drug reactions (ADR). METHODS:The telephone records of drug consultation from Feb. 2006 to Apr. 2008 in our hospital were statistically analyzed and the problems related to adverse drug reactions were classified. RESULTS:Among the 542 cases of drug consulting telephone records,the top 3 consulting questions were the usage and dosage of drugs (29.15%),recommendation and choice of drugs (13.28%) and drug interactions (10.52%). Of which,41 cases of ADR problems were about the use of antibacterials and Chinese traditional patent drugs. The most common manifestation of ADR was the lesion of digestive system. CONCLUSION:Drug consulting telephone service can not only enhance children patients' medication compliance and promote rational use of drugs but also improve the learning of clinical pharmacists.
6.Practice and reflection of pathophysiology teaching based on curriculum integration of organ system

In order to prevent the teaching reform of organ system curriculum from fall of teaching quality, the teaching effect of organ system curriculum and subject curriculum system in teaching the basic knowledge of pathophysiology was compared. In organ system curriculum teaching, there was no difference between the grades of students in Batch 2011 and Batch 2012 who conformed to the teaching principle of "gradual improvement" and those of the students taught with subject curriculum system. On the contrary, the students of Batch 2013 and Batch 2014 with insufficient curriculum content integration had a decreasing trend or a significant reduction in the teaching effect of organ system curriculum compared with that of subject curriculum system. After the supplementary for curriculum knowledge was made, the teaching effect of the organ system was significantly improved, which was better than that of the subject curriculum system. In conclusion, we have summarized and reflected on the effectiveness of the teaching reform of organ system curriculum, once again proved that basic medical teaching must also follow the inherent law of medical education, which is the teaching principle of "gradual improvement."
7.Establishment of Gestational Diabetes Rat Model Induced by High-fat Diet

Objective To establish a gestational diabetes rat model by feeding the rats with high-fat diet.Methods A total of 55 female SD rats were divided into four groups:NV group,NP group,FV group and FP group.Three months after normal feeding,the female rats in NP and FP group were put into the same cage with the male rats at the ratio of 2∶1 and were given high-fat diet or normal diet as usual.Before pregnancy and day 1,7,14,20 in pregnancy,fasting plasma glucose and body weight of rats were detected.The fasting serum insulin and serum c-peptide levels were monitored by enzyme immunoassay and insulin resistance index was calculated.At late pregnancy,glucose tolerance and the indicator of fat were tested.Liver and pancreas were dyed to be observed under microscope.FResultS Body weights of the rats raised with high-fat diet were significantly higher than those of control group and body weight during pregnancy significantly increased (P<0.05).Fasting glucose,fasting insulin and serum C-peptide in FP group were signifieantly higher than those in NP group and insulin resistance was evident (P<0.05).The area under curve of GTT in FP group was significantly larger (P<0.05).The levels of serum lipids in FP group were higher than those in normal group.CorncluSiornS The gestational diabetes rat model induced by high-fat diet can be successfully established.The model presents major pathophysiological manifestations of GDM and can be used as a good model of GDM in relevant research.
8.Impact of safety organization on occupational embeddedness among ICU nurses

Objective To understand the degree and influencing factors of safety organization on occupational embeddedness among intensive care unit (ICU) nurses.Methods A total of 1007 ICU nurses in 15 tertiary hospital in Zhejiang province were selected to get scores of the questionnaire by the safety organizing scale and occupational embeddedness scale for nurses in May 2016. The SPSS 18.0 software was used for date management and analysis for the relationship between the scores of two scales.Results The scores of safe organization was (37.85±7.84) and the scores of occupational embeddedness was (53.96±10.87). Correlation analysis showed that the safety organization positively correlated with the items of occupational embeddedness in matching,connection,sacrifice (r=0.248,0.282,0.253,0.207;P<0.01).Conclusions Higher level of safe organization in ICU nurses,higher level occupational embeddedness. Clinical nurse managers in ICU can make the measures from the perspective of safety organization to increase the level of occupational embeddedness.
9.Optimized surgical treatment for osteoporotic hip fractures in the elderly

BACKGROUND:Active surgical treatments are preferred for elderly hip fractures. Individual fixation method is chosen according to fracture site, type, age and whether there are basic diseases in internal medicine, which plays an important role in the successful treatment of elderly hip fractures.
OBJECTIVE:To explore the effects of optimized surgical treatment on osteoporotic hip fracture in the elderly.
METHODS: Totally 176 patients with osteoporotic hip fracture were treated by different methods between January 2000 and January 2012, including 63 males and 113 females, with a mean age of (76.7±6.3) years. Out of the 84 cases of interchanteric fracture, seven cases were treated with conservative methods, 34 cases were treated with dynamic hip screw internal fixation, 18 cases were treated with cannulated screw internal fixation, seven cases were treated with anatomical plate internal fixation, 12 cases were treated with bipolar femoral placement, and six cases were treated with total hip arthroplasty. Out of the 92 cases with femoral neck fractures, 40 cases were treated with bipolar femoral placement, 37 cases were treated with total hip arthroplasty and 15 cases were treated with cannulated screw internal fixation. Modified Harris hip function scores were used to evaluate the therapeutic effects of different treatment methods. Complications were observed.
RESULTS AND CONCLUSION:Seventy-six cases of interchanteric fractures and 85 cases of femoral neck fractures were fol owed-up for 8-26 months with an average of (5.7±1.3) months. Three cases suffered from post-operative infection, and one case died due to cardio-pulmonary failure in 10 days after operation. Both intraoperation and postoperative complications included femoral head cutting, intraoperative fracture, internal fixation and prosthetic loosening, postoperative fracture, avascular necrosis of femoral head, coxa vara, legs shorten, and delayed fracture healing. The incidence rates of complications in patients undergoing bipolar femoral placement and total hip arthroplasty were significantly lower than those treated with conservative methods, dynamic hip screw internal fixation, cannulated screw internal fixation, and anatomical plate internal fixation (P<0.05). The outcomes of the treatment according to Harris scoring criterion were better in patients treated with bipolar femoral placement and total hip arthroplasty than in patients treated with conservative methods, dynamic hip screw internal fixation, cannulated screw internal fixation, and anatomical plate internal fixation (P<0.05). No significant differences were found in the incidence rate of complications and Harris scores among patients treated with conservative methods, dynamic hip screw internal fixation, cannulated screw internal fixation, and anatomical plate internal fixation (P>0.05). No significant differences were found in the incidence rate of complications and Harris scores between patients treated with bipolar femoral placement and total hip arthroplasty (P>0.05). These findings indicate that the treatment of osteoporotic hip fracture in the elderly can achieve satisfactory results if the comprehensive therapies are given. Bipolar femoral placement and total hip arthroplasty are preferred for elderly femoral neck fractures.
10.Effect of blood flow restriction combined with low-intensity plyometric jump training on functional ankle instabil-ity

Objective To investigate the effect of blood flow restriction combined with low-intensity plyometric jump training(LI-PJT+BFR)on lower limb dynamic postural control of functional ankle instability(FAI)in college students. Methods From March to May,2023,40 FAI college students were recruited from Xi'an Physical Education University,and randomly divided into high-intensity plyometric jump training(HI-PJT,n = 14)group,low-intensity plyomet-ric jump training(LI-PJT,n = 13)group and LI-PJT+BFR group(n = 13).All the groups finished the six-week corresponding training.The maximum voluntary isometric contraction(MVIC)of tibialis anterior,peroneus lon-gus,lateral head of gastrocnemius,gluteus maximus,vastus lateralis,biceps femoris and semitendinosus were measured,and the root mean square(RMS)of electromyography of these muscles was measured during the sin-gle-leg landing(SLL),using wireless surface electromyography before and after intervention.Moreover,they were assessed with Y-balance test and Cumberland Ankle Instability Tool(CAIT). Results MVIC and RMS of the target muscles improved after intervention in all the groups(t>2.218,P<0.05),except MVIC and RMS of peroneus longus,gluteus maximus,biceps femoris and semitendinosus in LI-PJT group,and RMS of peroneus longus in LI-PJT+BFR group;and MVIC and RMS of the target muscles were the least in LI-PJT group(F>3.262,P<0.05),except those of peroneus longus.The extension scores of Y-balance test and the total score improved after intervention(t>2.485,P<0.05),and they were the least in LI-PJT group(F>5.042,P<0.05).The CAIT score improved after intervention(t>5.227,P<0.001),and it was the least in LI-PJT group(F = 4.640,P<0.05). Conclusion LI-PJT+BFR could improve lower limb dynamic postural control of FAI college students,which is similar to HI-PJT.
