1.Removal of Organic Mutagen in Tap Water

Objective To understand the effect of the organic extracts of tap water deeply treated with O3-BAC on DNA damage. Methods During June to July 2005, water samples were collected from 6 sites in waterworks A treated with O3-BAC, the raw water, the pre-chlorination water, the filtration water, the post-ozonation water, the BAC water and the tap water respectively and 4 sites in waterworks B treated by general treatment, the raw water, the pre-chlorination water, the filtration water, and the tap water respectively. The test was carried out on extracts of water sample from waterworks A with dosage(7.00, 3.00, 1.50, 1.00, 0.75, 0.38 L/plate)and waterworks B with dosage(7.00, 3.00, 1.00 L/plate)using S.typhimurium strains TA98 and TA100. Cytokinesis-blocked micronucleus (CBMN) test, Comet assay were used on extracts of water sample from waterworks A with dosage(3.00, 1.50, 0.75, 0.38 L/plate). Human embryo lung fibroblast (KMB17 strain) p53 ELISA were used with dosage(3.00, 1.00, 0.3 L/plate). Results Ames test showed that in the waterworks A, at the dose of 7.0 L, the revertants of the raw water, pre-chlorination water, the filtration water on TA98-S9 and 7.0 L, 3.0 L/plate, the revertants of the raw water, pre-ozonation water, filtration water on TA98+S9 were twice more than that of solvent control; in waterworks B, at the dose of 7.0 L/plate, the revertants of the raw water, pre-chlorination water, filtration water on TA98-S9 and 7.0 L, 3.0 L/plate the revertants of the tap water on TA98-S9, and 7.0 L/plate pre-chlorination water on TA98+S9 were twice more than that of solvent control. 2.Segmental anterior cervical decompression with fusion for treating multilevel cervical myelopathy: Comparison of fusion rates among three methods

BACKGROUND:As a traditional treatment for multilevel cervical myelopathy,nterior long-segmental decompression has the shortcomings of great operative trauma,high difficulty,low fusion rate,etc.,which can affect the postoperative efficacy.OBJ ECTIVE:To evaluate the clinical effects of three different anterior surgeries on multilevel cervical myelopathy.DESIGN:A comparative observation.SETTING:Department of Orthopaedics,Changzheog Hospital,the Second Military Medical University of Chinese PLA.PARTICIPANTS:Thirty-six patients with multilevel cervical myelopathy of 3 consecutive segments,who were surgically treated,were selected from the Department of Orthopaedics,Changzheng Hospital,the Second Military Medical University of Chinese PLA from June 1999 to June 2003,including 25 males and 11 females,35-62 years of age,the disease course ranged from 3 to 26 months. According to the clinical manifestations and imaging esults,they were diagnosed as multilevel cervical myelopathy,and they were not suffering from consecutive ossification of posterior longitudinal ligament and ossification of ligamenta flava. Informed contents were obtained from all the patients and their relatives.METHODS:All the patients were grafted with utologous bone. Autologous ilium or cancellous bone excluding vertebral body was filled into titan net or Cage,which were made of titan and characterized by high intensity,tolerance to decay,good biocompatibility,etc. According to the operative manner,the patients were divided into 3 groups:① two-level corpectomy with fusion group(long-segmental decompression group,n =11):There were 4 cases grafted with long-titan net,and 7 cases grafted with autologous iliac bone. Sub-total two-level corpectomy with fusion was performed. ②segmental decompression group(n =16):including 12 cases of titan net+cage graft,4 cases of autologous bone+cage graft. One-level decompression and sub-total single corpectomy with fusion were performed. ③three-level decompression group(n =9):Only discectomy without corpectomy was performed. After complete decompression,3cages were used to fill artificial bone or grafted with autologous bone.MAIN OUTCOME MEASURES:Cervical anteroposterior and lateral radiographies,flexion and extension radiograph were reexamined within 1 week and at 3,6 and 12 months postoperatively. The neurological function was assessed using the Japanese Orthopaedic Association(JOA) scoring method preoperatively and 3 months postoperatively. The total score was 17 points,the higher the score,the better the neurological function. The duration of operation,perioperative bleeding amount,length of stay,cost of hospitalization,graft fusion at 3 months postoperatively,improved JOA score at 3 months postoperatively were recorded in the three groups. The occurrence of postoperative complications was observed by means of return visit.RESULTS:All the 36 patients with multilevel cervical myelopathy were involved in the analysis of results. The mean duration of operation,mean perioperative bleeding amount and mean length of stay in the segmental-decompression group and three-level decompression group were obviously fewer or shorter than those in the long-segmental decompression group(P < 0.05),and the average cost of hospitalization was obviously higher than that in the long-segmental decompression group(P < 0.05). The postoperative improved JOA score and graft fusion rate were close among the groups(P > 0.05).CONCLUSION:Segmental anterior cervical decompression is a recommendable technique for multilevel cervical myelopathy by comprehensively considering the fusion rate,recovery of neurological function,duration of operation,perioperative bleeding and length of stay.
3.MiR-150 downregulation and its significance in mantle cell lymphoma

Objective: This study explores the miR-150 expression and its clinical significance in mantle cell lymphoma (MCL). Methods: The miR-150 and c-Myc expression was measured in 29 primary MCL tissue samples and 7 normal donors through quantita-tive real-time polymerase chain reaction. MiR-150 expression was detected in Mino and HBL-2 cells after c-Myc was knocked down by small interfering RNAs. MiR-150 was analyzed in tet-treated P493-6 cells with Myc turned off. The number of tumor colonies in the BL-2 cells transfected with pre-miR-150 was determined through colony formation assay, and c-Myb was detected through Western blot. Results: Compared with the normal donors, miR-150 expression was significantly downregulated and c-Myc was considerably overexpressed in MCL. Moreover, MCLs with high Myc expression had a significantly low miR-29 expression. MiR-150 was upregu-lated in the Mino and HBL-2 cells with c-Myc knockdown. MiR-150 was evidently upregulated in P493-6 cells after c-Myc was turned off. MiR-150 overexpression suppressed colony formation and c-Myb expression of HBL-2. Conclusion: MiR-150 is downregulated in MCL, which may be related to c-Myc overexpression. The recovery of miR-150 suppresses MCL cell survival. These results indicate that miR-150 may be a potential therapeutic target of MCL.
4.Comparison of thoracoscopic 2 hole and 3 hole for congenital pulmonary bulla resection

Objective To compare the effects of thoracoscopic 2 hole and 3 hole for congenital pulmonary bulla resection.Methods 38 cases of congenital pulmonary bulla patients,both in the VATS downlink congenital pulmonary bulla resection.According to the number of holes,thoracoscopic operation were divided into the two groups. To observe the use of group 19 cases of 2 holes,19 cases 3 holes were adopted in the control group.Pull the chest tube operation time,operation time of the two groups were compared after.and the average hospitalization time after opera-tion,postoperative analgesia drug application.Results The observation group operation time,operation time,pulling the chest tube after operation the average hospitalization time, analgesic drug application rate respectively were (46.89 ±9.11)min,(3.95 ±0.85) d,(7.37 ±1.34) d,21.1%,The control group were (66.05 ±12.09) min, (4.37 ±0.98)d,(7.32 ±1.57)d,52.6%.There were statistically significant differences in rate of the two groups in operation time,analgesic drug application.(t=-5.516,χ2 =4.071,P<0.05);38 cases were cured,followed up for 3-24 months,no recurrence occurred in 1 cases.Conclusion Video assisted thoracic descending congenital pulmona-ry bulla resection,the 2 hole 3 hole more than minimally invasive,short operation time,postoperative analgesic use rate is low.
5.Expression and significance of Smad4 in mice with hepatic alveolar echinococcosis ZHANG Ya-lou,MA

Objective To investigate the expression and significance of Smad4 in peripheral hepatocytes of lesions in mice with hepatic alveolar echinococcosis.Methods Eight mice in the test group were inoculated with alveolar echinococcosis and 8 mice in the control group were injected with normal saline.The expression of Smad4 protein in the hepatic tissue of the mice was detected by immunohistochemistry method,and the data were analyzed by chi-square test.The effect of alveolar echinocoeeosis on the expression of Smad4 protein was investigated.Results Smad4 was detected in cell nuclei and partly in the cytoplasm.Six months after the establishment of the mice model for alveolar echinococeosis,the expression of Smad4 in the hepatic tissue in the test group was significantly higher than in the control group(x2=19.869,P<0.05).The number of Smad4 with positive expression in the hepatocytes in the test group was slightly higher than in the control group,and the expression intensity of Smad4 in the test group was greater than in the control group(x2=58.3 10,P<0.05).Conclusions The increase of the expression of Smad4 protein in the periphery hepatocytes and tissues of hepatic alveolar echinococcosis lesions may play a role in hepatic cirrhosis and immune evasion.
6.Clinical analysis of surgical treatment for the aged patients with primary NSCLC: a report of 35 cases

Objective To investigate the clinical feature of perioperative management of non-small cell lung cancer(NSCLC) in aged patients and improve the efficacy of surgical treatment.Methods The clinical data of 35 aged patients with NSCLC were retrospectively analyzed.The risk factors of postoperative complications were analyzed by single factor analysis,the factors had statistical significance were included in Logistic regression analysis.Results Postoperative complications occurred in 10 cases,accounting for 28.6%,and 1 case died,accounting for 2.9%.Logistic regression analysis showed that smoking,chronic bronchitis,coronary heart disease,pulmonary lobectomy were independent risk factors of postoperative complications.ConCLusion Correct staging before operation,strict surgical indication,choose the standard surgical method to reduce surgical trauma,strengthen the perioperative management can still achieve satisfactory therapeutic effect in surgical operation for aged patients with NSCLC.
7.Prevalence rate of ovine hepatic cystic echinococcosis in Quaker Wusu area of Bayinbuluke of Xinjiang, 2014

Objective To investigate the prevalence rate of ovine hepatic cystic echinococcosis (HCE) in sheep in Quaker Wusu area of Bayinbuluke of Xinjiang by ultrasonography and provide evidence for the prevention and control of HCE in sheep.Methods The prevalence screening of HCE in sheep was conducted based on ultrasound images in this area in July 2014.The sheep were divided into different groups by dental age to calculate the age specific prevalence rate of HCE and analyzed the correlation between the dental age and the prevalence rate.Results The total prevalence rate of HCE in sheep in this area was 36.9%.The prevalence rates of none-calcified HCE and calcified HCE were 7.3% and 29.6%,respectively.The prevalence rates of none-calcified HCE in different age groups were 1.2% (1-2 years old),1.4% (2-3 years old),14.0% (3-4 years old),10.0% (4-5 years old),15.6%(5-6 years old) and 4.2%(>6 years old) respectively.The prevalence rate of calcified HCE in different age groups were 9.9%(1-2 years old),16.2% (2-3 years old),31.6%(3-4 years old),47.8%(4-5 years old),42.2%(5-6 years old) and 41.7%(>6 years old) respectively.The prevalence rate of HCE in 1-2 years old group was lower than those in other groups,the prevalence rate of HCE in age groups >3 years increased significantly.There was positive correlation between the prevalence rate of HCE and dental age (r=0.372,R2=0.107,F=44.176,P=0.000).Conclusion HCE is highly endemic in Quaker Wusu area.The prevalence rate of HCE is low in sheep with young age and high in sheep aged 3-4 years.It is necessary to conduct early prevention of HCE in sheep in this area.
8.Identification and biological characterization of a Streptococcus parasuis strain

Objective:To identify a strain isolated from the cerebrospinal fluid of a patient and to investigate its biological characteristics.Methods:The strain was analyzed by several methods including Gram staining, biochemical identification, 16S rRNA and recN gene sequencing, average nucleotide identity (ANI), antibiotic susceptibility testing and detection of drug resistance and virulence genes. Results:The strain was Gram-positive cocci and formed α-hemolytic colonies on the blood plate. It was identified as Streptococcus parasuis by 16S rRNA, recN gene and whole-genome sequencing. It was sensitive to multiple antibiotics and carried the genes encoding a variety of virulence factors such as adhesion. Conclusions:Streptococcus parasuis could cause human infection and be identified by whole-genome sequencing.
9.Radiomics based on arterial-venous mixed images derived from dual-energy CT data in diagnosis of lymph nodes metastasis of papillary thyroid cancer

Objective:To explore the diagnostic value of radiomics based on arterial-venous mixed images derived from dual-energy CT (DECT) data in diagnosis of cervical lymph nodes (LNs) metastasis of papillary thyroid cancer (PTC).Methods:From June 2017 to December 2018, eighty-four patients with preoperatively DECT scanning and pathologically confirmed PTC (129 non-metastatic LNs and 97 metastatic LNs) in the First Affiliated Hospital of Nanjing Medical University were included in this study. The clinical and imaging data of all patients were retrospectively analyzed. The training cohort consisted of 62 PTC cases with 156 LNs (91 non-metastatic LNs and 65 metastatic LNs). An independent validation cohort consisted of 22 PTC patients with 70 LNs (38 non-metastatic LNs and 32 metastatic LNs). Semi-automatic LNs segmentation was conducted on arterial-venous mixed images derived from DECT using Syngo.via Frontier Radiomics software. Totally 1 226 radiomics features were extracted from arterial-venous mixed images for each LN. The least absolute shrinkage and selection operator (LASSO) regression was applied for radiomics features selection and signature building. The logistic regression modeling was used to construct diagnostic models based on the CT image features of LNs (model 1), the radiomics signature (model 2) and the combination of the CT image features and radiomics signature (model 3). An intuitive nomogram was plotted for model 3. The ROC curve analyses and area under the curve (AUC) were performed to evaluate the diagnostic efficiency of the three models, with the performances compared using the Delong test.Results:Model 1 was developed with LNs shape, degree of enhancement, pattern of enhancement, calcification and extra nodal extension. Three arterial phase radiomics features were selected and used to establish radiomics signature using LASSO regression (model 2). Model 3 was developed with LNs size, shape, degree of enhancement and radiomics signature. In both the training and validation cohort, model 3 showed the best diagnostic performance (AUC=0.965, 0.933), followed by model 2 (AUC=0.947, 0.910), and both these two models significantly outperformed model 1 (AUC=0.850, 0.846) (training cohort, Z=4.066 and 3.758, P both<0.001; validation cohort, Z=2.871 and 1.998, P=0.017 and 0.042) respectively. Conclusion:The radiomics model based on arterial-venous mixed images derived from DECT data can realize effective diagnosis of LNs metastasis in patients with PTC; and the combination model of radiomics signature with CT image features can further improve the diagnostic accuracy.
10.Accurate Measurement of Agatston Score Using kVp-Independent Reconstruction Algorithm for Ultra-High-Pitch Sn150 kVp CT

Objective:
To investigate the accuracy of the Agatston score obtained with the ultra-high-pitch (UHP) acquisition mode using tin-filter spectral shaping (Sn150 kVp) and a kVp-independent reconstruction algorithm to reduce the radiation dose.
Materials and Methods:
This prospective study included 114 patients (mean ± standard deviation, 60.3 ± 9.8 years; 74 male) who underwent a standard 120 kVp scan and an additional UHP Sn150 kVp scan for coronary artery calcification scoring (CACS). These two datasets were reconstructed using a standard reconstruction algorithm (120 kVp + Qr36d, protocol A; Sn150 kVp + Qr36d, protocol B). In addition, the Sn150 kVp dataset was reconstructed using a kVp-independent reconstruction algorithm (Sn150 kVp + Sa36d, protocol C). The Agatston scores for protocols A and B, as well as protocols A and C, were compared.The agreement between the scores was assessed using the intraclass correlation coefficient (ICC) and the Bland–Altman plot. The radiation doses for the 120 kVp and UHP Sn150 kVp acquisition modes were also compared.
Results:
No significant difference was observed in the Agatston score for protocols A (median, 63.05; interquartile range [IQR], 0–232.28) and C (median, 60.25; IQR, 0–195.20) (p = 0.060). The mean difference in the Agatston score for protocols A and C was relatively small (-7.82) and with the limits of agreement from -65.20 to 49.56 (ICC = 0.997). The Agatston score for protocol B (median, 34.85; IQR, 0–120.73) was significantly underestimated compared with that for protocol A (p < 0.001). The UHP Sn150 kVp mode facilitated an effective radiation dose reduction by approximately 30% (0.58 vs. 0.82 mSv, p < 0.001) from that associated with the standard 120 kVp mode.
Conclusion
The Agatston scores for CACS with the UHP Sn150 kVp mode with a kVp-independent reconstruction algorithm and the standard 120 kVp demonstrated excellent agreement with a small mean difference and narrow agreement limits. The UHP Sn150 kVp mode allowed a significant reduction in the radiation dose.
