1.Health Input, Technical Efficiency and Health Performance:Based on the Estimation of Stochastic Frontier Analysis Method on Health Input Efficiency

Objective: To support the distribution of health resources in China better. Methods: According to the related data of resident health, health resources and marketing index in China from 2005 to 2010, related conclusion was given by using stochastic frontier analysis and fixed effect panel analysis method to establish the change trend of technical efficiency in different provinces and cities. Results and Conclusion: (1) The distribution of technical efficiency in different provinces and cities are asymmetric and generally low;(2)there are obvious differences of technical efficiency among different areas;(3)government input in medical and health field should be focused on central and western areas.
2.Retrospective Analysis of82Cases of Anaphylactic Shock from Trasylol

OBJECTIVE:To discuss the prevention and cure of anaphylactic shock caused by trasylol.METHODS:The anaphylactic shock was statistically analyzed among3862cases that were given slight dosage of trasylol before operation by intravenous drip.Among which,82cases had anaphylactic shocks,which accounted for2.12%of the total.RESULTS&CONCLUSION:The incidence rate of anaphylactic shock from trasylol by intravenous drip is quite high,thereby preparation of prevention and emergency treatment should be ready.
3.Acoustic radiation force impulse imaging in differential diagnosis of small solid thyroid nodules coexisting with Hashimoto's thyroiditis

Objective To investigate the application value of acoustic radiation force impulse imaging (ARFI) in differential diagnosis of small solid thyroid nodules coexisting with Hashimoto's thyroiditis (HT).Methods A total of 136 patients with 155 small solid nodules (≤1 cm in diameter) confirmed by pathology were included.The small solid nodules were divided into HT group and normal group.All patients underwent virtual tissue imaging (VTI) and virtual touch tissue quantification (VTQ).The stiffness of small solid thyroid nodules in two groups was respectively scored based on VTI images,and the area ratios of nodules in VTI images to gray scale images were calculated.Taking pathological diagnosis as the gold standard,the ROC curve were performed to explore the optimal points and efficacy of area ratio and shear wave velocity (SWV) in the diagnosis of malignant nodules in two groups respectively.Results Regarding VTI score≥4 as the diagnostic standard of malignant nodules,the sensitivity of HT group and normal group in diagnosis of malignant nodules had significant difference (64.29%[27/42] vs 81.69%[58/71],P<0.05).In two groups,the area ratio and SWV of malignant nodules were significantly higher than those of benign nodules (all P<0.05).The area ratio and SWV of thyroid benign nodules and malignant nodules had no significantly difference between two groups (all P>0.05).Taking 1.41 as the cutoff point of area ratio,the sensitivity and specificity in diagnosis of malignant nodules in HT group were 85.7% and 88.9%.Taking 2.87 m/s as the cutoff point of SWV,sensitivity and specificity in diagnosis of malignant nodules in HT group were 85.7% and 94.4%.Taking 1.40 as the cutoff point of area ratio,the sensitivity and specificity in diagnosis of malignant nodules in normal group were 91.5% and 95.8%.Taking 2.67 m/s as the cutoff point of SWV,the sensitivity and specificity in diagnosis of malignant nodules in normal group were 88.7% and 95.8%.Conclusion ARFI technology can evalu-ate the hardness of the small solid thyroid nodules through the intuitive VTI images and objective SWV values in HT,which is helpful in diagnosis of malignant nodules in HT backgroud.

The teaching methods in anesthesiology practice to traditional Chinese medicine students was discussed.Watching for the first step, then simulation and manipulation on patients should be abided by. Paying more attention to patients’ safety and avoiding any fault to patients should be emphasized . The one to one and hand by hand teaching method should also be used to train the students' clinic thought and practical ability.

OBJECTIVE:To prepare rufloxacin eye drops METHODS:The content of rufloxacin was determined with HPLC method The therapeutic effect of the eye drops was observed in 150 out-patients with severe ocular infections RESULTS:Determined by HPLC method,the average recovery of rufloxacin was 100 12%(RSD=1 18%) After treatment,130 patients recovered,16 improved and had no efficiency with a total effective rate of 97 33% CONCLUSION:The eye drops have definite therapeutic effect The HPLC method is simple,rapid and accurate in determination of the content and no special reagent is needed
7.Cloning and expression of the σC gene of avian reovirus

The σC gene of ARV S1133 was designed to amplify by reverse transcription chain reaction(RTPCR).The σC gene was inserted into the vector pMD19-T,identified by PCR method and restriction enzyme,and sequenced.It showed that the insert cloned gene fragment was the σC gene of ARV.Then the gene was inserted intc the pET32a(+) and indicated that fusion expression vector pET32a-σC was constructed.The recombinant fusion protein was highly expressed in E.coli BL21 induced by 1.0 mmol/L IPTG for 5 hours in the form of inclusion bodies.The weight of recombinant fusion protein molecular is 54 000.Western-blot with ARV antibodies against the fusion protein showed the recombinant protein has a favourable reactivity.
8.Dry weight evaluation in hemodialysis patients

Objective To estimate dry weight (DW) and prevent dialysis-related hypotension and hypertension with the on-line monitoring of relative blood volume (RBV) and other judgments.Methods One hundred and eight maintenance hemodialysis patients were assigned to three groups according to their blood pressure:normal blood pressure group (A group,n=43),hypotension group (B group,n=35) and hypertension group (C group,n=35).The level of hemoglobin,serum albumin,dialysis adequacy were determined.Systolic blood pressure,diastolic blood pressure,mean arterial pressure,heart rate,ultrafiltration volume,relative blood volume changes and the corresponding clinical symptoms were monitored during hemodialysis in all patients.Each of the patients was continuously monitored of the indicators above for 10-12 times.At the observing period,the inferior vena cava diameter (IVCD),brain natriuretic peptide (BNP) and cardiothoracic ratio(CTR) were measured.Then according to the monitoring results,appropriate clinical interventions were given under on-line blood volume monitoring guidance.Results (1)The shape of RBV curve in group A showed doubleexponential curve early,then down to the final linear decling ended during hemodialysis.(2)The RBV curve in group B was stable in the former two hours,then rapidly linear declined.RBV changes were significantly higher in group B than group A (P < 0.05),but when changes in RBV were plotted against ultrafiltration volume,there was no significant difference in the two groups.The level of RBV reduction at which symptomatic hypotension occurred showed considerable inter-individual variability (P < 0.05,coefficient of variation=0.28).(3)The RBV curve in group C slowly linear declined.At the end of dialysis,RBV changes were significantly lower in group C than group A (P < 0.05).(4)The IVCD values in three groups of patients before dialysis were greater than normal,significantly decreased after the dialysis (P < 0.05),but that in group B and group C were still greater than that in group A (P < 0.05).The BNP values were significantly greater in three groups before and after dialysis (P < 0.05),but after dialysis,the values decreased significantly than that before dialysis (P < 0.05).(5)After appropriate clinical intervention were given under on-line blood volume monitoring in hemodialysis,the patients of group B controlled weight gain,and even cut dry weight,the RBV change significantly decreased at the end of dialysis and significantly reduced the incidence of hypotension events (P < 0.05); When the patients of group C cut dry weight,increased ultrafiltration,the RBV change increased,the mean arterial pressure decreased significantly than before (P< 0.05).Conclusions (1)Hemodialysis patients with symptomatic hypotension show larger RBV decline rate in the forth hour and lager total RBV changes,which provides important information for forecasting the symptomatic hypotension in hemodialysis.(2)IVCD and CTR have certain significance to the adjustment of dry weight,but the BNP has guiding significance to volume change.(3)On-line monitoring of RBV can effectively guide the adjustment of dry weight,reduction of symptomatic hypotension occruence,and controlling of refractory hypertension in hemodialysis.
9.Improving the dissolution rate of poorly water-soluble resveratrol by the ordered mesoporous silica.

The aim of this study is to synthesize the ordered mesoporous silica (OMS) as drug carrier to improve release property of insoluble drug and investigate the dissolution profile of insoluble drug from the porous carrier. The OMS was obtained by using cetyltrimethyl ammonium bromide as the template and resveratrol was selected as the model drug. The resveratrol-loaded OMS (Res-OMS) were characterized by scanning electron microscopy (SEM), transmission electron microscopy (TEM), N2 adsorption-desorption, X-ray diffraction (XRD) and FT-IR spectroscopy. In vitro drug release behavior was also investigated. It was found that the synthesized OMS showed a large surface area, a narrow pore size distribution and an important mesoporosity associated to hexagonally organized channels. Compared with physical mixture and crystalline powder, resveratrol was in amorphous or molecular form after loading into OMS. The release rate ofresveratrol from drug-loaded OMS was significantly increased suggesting the great potential application of OMS for the formulation of poorly soluble drugs.
10.Surgical experience in the treatment of refractory cholelithiasis

Objective To investigate the operative techniques for refractory cholelithiasis. Methods The clinical data of 521 patients with refractory cholelithiasis who had been admitted to People's Hospital of Hunan Province from January 1990 to December 2007 were retrospectively analyzed. Results All patients received surgery. After the operation, 3 patients died of liver and kidney failure, the remaining 518 patients were cured without severe complications. The results of B ultrasound and computed tomography showed residual stones in 78 patients (15.1%). Four hundred and twenty-three patients (81.7%) were followed up for 5 months to 17 years (mean, 7.5 years), and the rate of positive effect was 90.1% (381/423). Conclusions Most of the refractory cholelithiasis can be cured radically. Individualized surgical planning, fine and standard surgical procedure are key to the treatment effect.
