1.Treatment of spontenous rupture of hepatocellular carcinoma(a report of 64 cases)

Objective To study the treatment of sponteneous rupture of hepatocellular carcinoma(SRHCC).Methods The clinical data of 64 patients with SRHCC treated by non-operation or operation were analysed retrospectively.Results The mean survival time of 14 patients treated by conservative therapy was 7 days(12 hours~12 days), the mean surival time of 31 patients treated by packing and 6 patients treated by ligation of hepatic artery was 11 days(4 hours-3 months)and 2.5 months(8 days-6 months);and the 1-month survival rate was 3.2% and 66.7% respectively.1 patient underwent transhepatic artery embolism surviving 9 days.The 1-year survival rate of 12 patients treated by hepatectomy was 66.7%;the mean survival time was 36 months(4 days-25 years and 3 months).Of the 12 patients,11 survived for over one and a half months whose liver function was Child-Pugh grade A before operation; 1 died of liver function failure 4 days after operation,whose liver function was Child-Pugh grade B before operation.Conclusions Emergent hepatectomy should be reserved for patients with an easily resectable lesion whose liver function is in Child-Pugh grade A.Early two-stage or delayed hepatectomy would be suitable for those patients who are unsuitable for emergent operation.Conservative therapy and other therapies may be used for patients with terminal stage of SRHCC.
2.Analysis on Therapeutic Effects of 209 Cases of Allergic Asthma Treated with Acupoint Application

209 cases of allergic asthma were randomly divided into two groups, which received respectively traditional medicinal vesiculation and drug application based on syndrome differentiation, and their short - term and long - term therapeutic effects were investigated and compared, and the relation between the clinical therapeutic effects of the two application therapies and classification of TCM syndrome differentiation was further analysed. Results show that the total effective rate of the short -term of drug application based on syndrome differentiation is higher than that of medicinal vesiculation, and the clinical thertapeutic effect is closely related to classification of syndrome differentiation, the therapeutic effect of the short -term is the highest for lung -heat type and the worst for Yang-deficiency of kidney.
3.Problems in continuing medical education in grass-roots hospitals and solutions

With the burgeoning of continuing medical education, grass-roots hospitals are confronted with quite a number of problems in the implementation of the task, such as a general rush into action with each department going its own way, incontrollable educational expenses that are beyond the hospitals, difficulties in developing programs at the hospital level because of the limitation of academic disciplines, and the buying and selling of credits. In view of the above problems, the authors put forward some suggestions for formulating a series of continuing medical education management strategies that both conform to the situation in our country and suit the needs of health professionals at grass-roots institutions.

5.Effect of electroacupuncture and dihydroergotoxine on patients with vascular dementia

ObjectiveTo study the effect of the electroacupuncture on patients of vascular dementia (VD). Methods68 patients suffering from VD were randomly divided into two groups, the electroacupuncture group(34 cases) and the medicine group(dihydroergotoxine, DHET, 34 cases). Scores of HDS R(Hasegawa Dementia Scale ), FAQ(Functional Activity Questionaire), ADL(Activity of Daily Living)and Nervous Dysfunction were examined and compared before and after treatment. The course was 42 days. ResultsThe total effective rate was 79.41% in electroacupuncture group and 20.58% in DHET group(P<0.01). All the above mentioned indices were more significantly improved in electroacupuncture group than those in DHET group(P<0.01). ConclusionsIt suggests that the electroacupuncture may improve the VD patients in intelligence, ability of functional activity and quality of life, etc.

7.Clinical Study of Synergistic Effect of Xianglian Suppository Combined with Miconazole Suppository in Treating Simple Vulvovaginal Candidiasis

Objective To observe the antibacterial and synergistic effect of Xianglian Suppository with miconazole suppository for the treatment of simple vulvovaginal candidiasis ( VVC) , and to explore the advantages of integrative Chinese and western medicine for the treatment of simple VVC. Methods According to the principle of randomized trial, 65 simple VVC patients were divided into two groups, 35 cases in test group and 30 in control group. The test group was given Xianglian suppository and miconazole suppository, and the control group was treated with miconazole suppository, the medication lasting for 7 days. The changes of symptom scores before and after treatment were observed, and the therapeutic effect and safety were also evaluated in both groups. Results ( 1) After treatment for one course, the markedly effective rate and total effective rate were 60.00%, 88.57% in the test group, and were 23.33%, 60.00% in the control group, respectively, the differences being significant between the two groups ( P<0.01). ( 2) After treatment, symptom scores were decreased in both groups ( P<0.01 compared with those before treatment) , and the decrease in the test group was superior to that in the control group (P<0.01). (3) One case of the test group had the complaint of severer pruritus vulvae after medication, and the results of physical examination showed the case had vulvovaginal flush but had no edema, blister or other discomfort. The manifestations disappeared when the medication continued, indicating that the case had no allergic reaction. No case had allergic reaction in the control group. Conclusion For the treatment of simple VVC, Xianglian suppository combined with miconazole suppository can obviously relieve the clinical symptoms, and is effective and safe.

9.The Effect of Chinese Herbal Capsule on Experimental Genital Herpes

Objective To study the mechanism of Chinese herbal capsule in the treatment of genital herpes.Methods Guinea pig model for genital herpes infection was established.HSV DNA from the cerebrum,spinal cord,cervical canal and vagina of guinea pigs was detected by fluorescent-quantitative PCR.Results The HSV DNA could be detected from all four kinds of tissues,with different copies of the virus in the different tissues,the highest in spinal cord,then in vagina,the least in cerebrum and cervical canal.There was statistical difference of HSV-DNA copies between herbal capsule group and control group(normal saline),whereas no statistical difference between herbal capsule group and aciclovir group.Conclusion The mechanism of the anti-virus herbal capsule in the treatment of experimental genital herpes might be related to blocking the latent infection and suppressing the replication of HSV.

The relationship of dose and effect in TCM formulae was an important subject in the TCM modernization research.Some characteristic properties of TCM dose-effect relationship were proposed in this paper: the dose-effect relationship of TCM had a specifi c regulation from the whole-view idea in traditional medical theories besides common properties,nonlinear character from the compatibility of medicine properties,and multi-effects varied with the different compatibility from the alternation of doses.The subjects of dose-effect relationship included single medicine,drug pair and formulae.The study on the dose-effect relationship of TCM might be helpful to the explanation of conception,connotation,ideas and methods of TCM,provide scientifi c supports and instructions for the TCM clinical practices,and provide research bases for the TCM innovation.
