1.Hemoperfusion in treatment of overdose digoxin poisoning

AIM: To observe the efficacy of hemoperfusion (HP) to treat one case suffering from overdose digoxin poisoning. METHODS: HA 330 ml and YT 160 hemoperfusion cartridges were used and blood vessel was femoral vein, single needle and two chamber catheter. Four HP were done. RESULTS: The blood digoxin level were 16 ?g?L -1 before and after the 1st HP, 9.22 ?g?L -1 at the 2nd, 12.4 ?g?L -1 before the 3th, 10.45 ?g?L -1 after the 3th, 3.22 ?g?L -1 before the 4th, and 2.84 ?g?L -1 after the 4th HP. CONCLUSION: This case demonstrats that multiple HP at different period may decrease the blood digoxin level step by step.
2.Relationship between the changes of angiotensin Ⅱ and vascular endothelial growth factor in plasma and the occurrence of diabetic nephropathy

AIM: To measure the concentrations of angiotensinⅡ(ATⅡ) and vascular endothelial growth factor(VEGF) in plasma of diabetics and investigate the relationship between ATⅡ and VEGF and the role of VEGF in occurrence and development of diabetic nephropathy(DN). METHODS: 98 type Ⅱ diabetics were divided into two groups: 43 were in diabetic nephropathy group and 55 in non-diabetic nephropathy (NDN) group, and 25 healty persons were in control group. We measured the levels of ATⅡ, VEGF, glycosylated hemoglobinC (GHbAlc) in plasma and urinary microalbumin(UmALB), respectively. 3.Economic Analysis of the Cost of Cephlosporin Injection's Skin Test in Our Hospital

OBJECTIVE:To provide reference for standardized management of skin test of cephalosporin injection in our hos-pital. METHODS:In retrospective analysis,skin test of 8 kinds of cephalosporin [cefotaxime(1.0 g),cefotaxime(0.5 g),cefoti-am (1.0 g),cefotiam (0.5 g),ceftazidime (0.5 g),cefminox (0.5 g),cefminox (1.0 g),ceftriaxone (1.0 g)] and the cost of skin test, related cost of allergic reaction induced by cephalosporin injection were analyzed statistically during Sept. 1st, 2015-Aug. 31th,2016. The cost of skin test of cephalosporin injection was compared with allergic reaction cost reduced by skin test. RESULTS:The positive rate of 100330 patients who used above 8 kinds of cephalosporin injections was 6.27%;the rate of skin test was 82.49%;the direct cost of skin test was 3434411.72 yuan;the indirect cost was 141985.12 yuan;the cost of pa-tient was 3162901.44 yuan;the cost of medical insurance was 259096.28 yuan;the cost of the whole society was 3576396.84 yuan. From the perspective of the whole society,the cost of skin test of cephalosporin injections was(447049.61±247395.07)yuan, and allergic reaction cost reduced by skin test was (316075.48 ± 260600.49)yuan. The cost of skin test was significantly higher than allergic reaction cost reduced by skin test,with statistical significance(P<0.05). CONCLUSIONS:Skin test of cephalosporin injection is low in positive rate,has high expense. The government standardizes the management of skin test of cephalosporin injec-tion and reduces the economic burden of patients under the premise of ensuring the safety of drug use.
4.A STUDY OF THE RELATIONSHIPS BETWEEN VARIOUS EDIBLE OILS AND ATHEROSCLEROSIS

The effects of 4 kinds of edible oils on serum lipid levels, morphological changes of cardiovascular tissues, fatty acid compositions of various tissues and platelet function in rats had been observed. 40 adult Wistar rats fed semi-synthetic diet containing edible oil to supply 41% energy were divided into 5 group, i. e. control group (18.7% energy from fat), soy bean oil group, peanut oil group, lard group and rape seed oil group. The animals were fed the diets and water ad libitum for 2 months. The results showed that the lard gave the most serious detrimental effect but the soy bean oil was the least. The difference between these two groups was significant. The platelet number was 223?109/L and aggregation rate was 21.8% of the soy bean oil group but. the lard group 149?109/L and 30.2%. The composition (%) of PUFA in tissues was higher and that of saturated fatty acid was tower in soy bean ou group than those in lard group.Electron microscopical studies showed that the animals with vascular endothelial cells changes were more and the pathological changes were more serious in the lard group than those in the soy bean group.All groups except rape seed oil group increased weights in the experi-mental period at the similar rate. Rape seed oil group gained very little weights and aggregated more C22:1 especially in myocardium.
5.Re-evaluation of the immunogenicity of meningococcal serogroups A and C polysaccharide vaccine among a healthy population aged 5 to 59

Objective To re-evaluate the immunogenicity of meningococcal serogroups A and C polysaccharide vaccine among a healthy population of age 5 to 59.Methods Pre and post-vaccination ser-um samples were collected from the subjects involved in a randomized , controlled clinical trial conducted in 2005 at Hechi, Guangxi, China.Enzyme-linked immunosorbent assay (ELISA) was performed for the quan-titative detection of specific anti-capsule IgG antibody against meningococcal serogroups A and C in serum samples.Serum bactericidal assay ( SBA) was used to measure the bactericidal antibody activity against se-rougroups A and C bacterial strains .Results Geometric mean concentrations (GMCs) of specific anti-cap-sule IgG antibody were 23.66 μg/ml and 78.83 μg/ml for serogroup A (P<0.05), and 1.23 μg/ml and 51.25 μg/ml for serogroup C (P<0.05) in serum samples of pre and post-vaccination, respectively.Be-fore immunization , 99% of serum samples ( 97/98 ) showed serogroup A-specific IgG concentration ≥2μg/ml, which reached up to 100%(98/98) after vaccination (P>0.05).The percentage of serum sam-ples with serogroup C-specific IgG concentration ≥2 μg/ml rose from 20%to 99% after vaccination ( P<0.05).The ratio of positive serum samples with at least four times of increases in concentration of specific IgG against serogroup A and serogroup C after vaccine immunization were 34% and 100%, respectively . The rSBA geometric mean titers ( GMTs) for serogroups A and C in serum sample of post-vaccination were respectively elevated from 1164 to 5530 (P<0.05), and from 4 to 6225 (P<0.05).The percentage of se-rum samples with rSBA GMTs≥128 increased from 91% to 99% for serogroup A (P>0.05), and from 14%to 96%for serogroup C (P<0.05) after vaccination.The rSBA GMTs with at least four times of in-creases after vaccination were detected respectively in 53% and 100% of serogroups A and C vaccinated subjects.Pearson correlation coefficient between IgG concentration and rSBA GMTs was r=0.15 (pre) and r=0.23 (post) for serogroup A, and r=-0.14 (pre) and r=0.58 (post) for serogroup C (P<0.05). Conclusion This study has demonstrated an efficient and sustained immunogenicity with meningococcal sero -groups A and C polysaccharide vaccine as evidenced by the data from standardized assays of ELISA and SBA .

Attention deficit hyperactivity disorder (ADHD) is a common neurodevelopmental disorder in children characterized by deficits in cognition.The examination of the disease mainly relies on clinical observation and scale testing.Functional near-infrared spectroscopy (fNIRS)is an emerging non-invasive brain function detection technology which can evaluate brain function more objectively.Its biggest advantage is that the test results are less influenced by body movements,especially suitable for children.Recently,fNIRS has been widely used in children with ADHD both in scientific research and clinical work,and has application value in its cognitive function detection,drug treatment influence,auxiliary diagnosis and adjuvant therapy.
7.Investigation on injury of liver and kidney among the workers exposed to terephthalic acid, ethylene glycol and(or) dowtherm A.

OBJECTIVETo study injury of liver and kidney among the workers exposed to terephthalic acid(TPA), ethylene glycol(EG) and(or) dowtherm A(DOW), and research for early biological monitoring indexes.
METHODSBy using the method of occupational epidemiology, an investigation of industrial hygiene in a chemical fibre corporation was carried out and the changes of the liver and kidney functions were analyzed among the workers who had been exposed to TPA, EG, DOW.
RESULTSThe values of serum gamma-glutamyl traspetidase(GGT) and total bile acid(TBA) in TPA + EG + DOW group men were (35.45 +/- 16.09) U/L, (10.29 +/- 6.76) mumol/L respectively and the values of serum alanine transaminase(ALT) and TBA in TPA + EG + DOW group women were(30.68 +/- 8.58) U/L, (9.53 +/- 6.63) mumol/L respectively, significantly higher than those in TPA, DOW and control groups(P < 0.05, P < 0.01). Compared with TPA, DOW and control groups, the values of urine N-acetyl-beta-D-glucosaminidase(NAG) and beta 2-2-microglobulim (beta 2-MG) in TPA + EG + DOW group of both men and women increased significantly(P < 0.05, P < 0.01), with(5.68 +/- 4.01) U/mmol Cr and (23.49 +/- 13.44) mg/mol Cr, and(6.68 +/- 4.68) U/mmol Cr and (22.80 +/- 13.00) mg/mol Cr, respectively. Analysis of regression indicated that both liver and renal injuries of the workers were evidently correlated with their exposure to TPA, EG and DOW after adjustment for the confounding factors such as sex, smoking, drinking, etc(P < 0.001).
CONCLUSIONBased on available knowledge, it is reasonable to assume that the joint actions should be considered on the injury of liver and kidney caused by TPA, EG and(or) DOW among the workers. Serum ALT, GGT, TBA, urine NAG and beta 2-MG should be suggested as biomarkers for liver and kidney damage.

8. Association between maternal tea consumption in pregnancy and birth outcomes

Objective:
The purpose of this study was to explore the relationship between maternal tea consumption and birth outcomes.
Methods:
From January 2005 to December 2010, pregnant women were recruited from Nanjing Maternity and Child Health Hospital. The basic information and the situation of tea consumption during pregnancy were investigated using questionnaire and the birth outcomes of newborns were followed up. Finally, 500 pairs of mothers and infants with complete and standard-compliant data were included in the analysis. The differences of birth outcomes between the tea consumption group and the non tea consumption group were compared and the associations between tea consumption and birth outcomes were analyzed using multivariate logistic regression.
Results:
Mother's tea consumption rate was 32.8% (164 cases) during pregnancy. The rate of low birth weight in the tea consumption group was (5.5%, 9 cases) and higher than that in the non-tea consumption group (2.1%, 7 cases) (

10.Classification and reconstruction of bile duct in pediatric split liver transplantation

Objective To investigate the anatomical classification of left intrahepatic bile duct (LHD) and the pattern of bile duct reconstruction during pediatric split liver transplantation and their relationship with postoperative biliary complications. Methods Clinical data of 75 pediatric recipients undergoing split liver transplantation were analyzed retrospectively. Before splitting the donor liver, iopromide injection was used for retrograde cholangiography through the common bile duct. According to the patterns of intrahepatic bile ducts in the second, third and fourth segments, the anatomical classification of LHD of the donor liver was determined. The biliary reconstruction regimens for different classification types of LHD were summarized. The incidence and treatment of biliary complications after pediatric split liver transplantation were analyzed. Results Among 75 donor livers, the anatomical classification of LHD included 57 cases (76%) of type Ⅰ, 9 cases (12%) of type Ⅱ, 4 cases (5%) of type Ⅲ and 5 cases (7%) of type Ⅳ LHD, respectively. Among 75 pediatric recipients, 69 cases (53 cases of type Ⅰ, 8 type Ⅱ, 4 type Ⅲ and 4 type Ⅳ) underwent the left hepatic duct-jejunum Roux-en-Y anastomosis, 1 case received common bile duct-jejunum Roux-en-Y anastomosis (type Ⅳ), and 5 cases underwent the left hepatic duct-common bile duct end-to-end anastomosis (4 cases of type Ⅰ and 1 type Ⅱ). Postoperative biliary complications occurred in 6 cases (8%), including 3 cases of biliary anastomotic stenosis, 2 cases of biliary anastomotic leakage and 1 case of bile leakage on the hepatic resection surface. Among 6 recipients, 4 cases were classified as type Ⅰ and 2 cases of type Ⅲ LHD.
