An electrospray/ultraviolet lamp dual-source ion trap mass spectrometer was developed for the rapid detection of gas and liquid samples. The instrument used the discontinuous atmospheric pressure sampling technique that both the electrospray ions and gaseous analytes were sampled and transferred using a pinch valve device. The two ionization sources were generally suitable for different kinds of analytes and provided complementary applications.Electrospray was used for the ionization of polar compounds in solution, while the UV ionization source was mainly applied for the analysis of gaseous organic compounds.A variety of samples such as anisole, toluene, 2,4-dimethylaniline, arginine, reserpine and aspartame were employed to test the performance in different working mode of the instrument. The results showed that the two sources were feasible for ionization of different types of samples, and different types of molecular ions could be generated when 2,4-dimethylaniline was analyzed. The two ionization sources could be used alternately or simultaneously without interference, and the working mode was also switched to fit the application requirements. The dual-source configuration was an effective way to extend the applications for miniature mass spectrometers. It did not significantly increase the size of the instrument, but provided more versatile analysis to meet the need for the measurement of different types of samples.

Objective To assess the diet quality of pregnant women during the second trimester using the dietary inflammatory index ( DII) and to explore the correlation between the DII in second trimester of preg-nancy and preterm delivery. Methods A total of 253 women with singleton pregnancy in 16-20 gestational weeks who had received routine prenatal care between August 2014 and April 2015 at the First Affiliated Hospi-tal of Anhui Medical University were enrolled with cluster random sampling. The included women were asked to recall daily dietary intake in the 3 days prior to the survey. All dietary data were analyzed for energy and nutri-ents intake with a nutritional analysis software, followed by calculation of DII according to previous reports. Based on quartiles of the DII scores, the participants were divided into three groups, namely the anti-inflam-matory group (DII<-5.10), intermediate group (DII -5.10--2.55), and the pro-inflammatory group (DII>-2.55).The participants were followed up until delivery and the pregnancy outcomes were recorded. The relationship between the DII in second trimester of pregnancy and preterm delivery were analyzed. Results DII scores of the 253 pregnant women during the second trimester of pregnancy ranged from -7.913 to 3.872.The risks of preterm birth in the anti-inflammatory diet group, the intermediate group, and the pro-inflammatory diet group were 0, 1.6%, and 6.3%, respectively, with statistically significant differences among the groups (P=0.034).The higher DII scores (pro-inflammatory) were associated with higher inci-dence of preterm birth ( P<0.05 ) .Conclusion DII may be used to assess diet quality of pregnant women during the second trimester and to predict the risk of preterm birth.
7.Advances in diagnosis and treatment of celiac disease
Chinese Journal of Applied Clinical Pediatrics 2017;32(19):1452-1455
Celiac disease(CD) is a chronic intestinal malabsorption syndrome by ingestion of wheat bran material in a susceptible population which is immune-mediated.At present, the incidence of CD in North America and Europe has reached about 1%-3%,the disease is considered to be very rare in China, but now some studies have confirmed the existence of the disease.Children suffering from CD can lead to failure in growth and development, and patients can be significantly improved by gluten-free diet. Therefore, early diagnosis and timely treatment is great significance.

Background:Gross target volume of primary tumor (GTV?P) is very important for the prognosis prediction of patients with nasopharyngeal carcinoma (NPC), but it is unknown whether the same is true for locally advanced NPC patients treated with intensity?modulated radiotherapy (IMRT). This study aimed to clarify the prognostic value of tumor volume for patient with locally advanced NPC receiving IMRT and to ifnd a suitable cut?off value of GTV?P for prognosis prediction. Methods:Clinical data of 358 patients with locally advanced NPC who received IMRT were reviewed. Receiver oper?ating characteristic (ROC) curves were used to identify the cut?off values of GTV?P for the prediction of different end?points [overall survival (OS), local relapse?free survival (LRFS), distant metastasis?free survival (DMFS), and disease?free survival (DFS)] and to test the prognostic value of GTV?P when compared with that of the American Joint Committee on Cancer T staging system. Results:The 358 patients with locally advanced NPC were divided into two groups by the cut?off value of GTV?P as determined using ROC curves: 219 (61.2%) patients with GTV?P≤46.4mL and 139 (38.8%) with GTV?P>46.4mL. The 3?year OS, LRFS, DMFS, and DFS rates were all higher in patients with GTV?P≤46.4mL than in those with GTV?P>46.4mL (allP<0.05). Multivariate analysis indicated that GTV?P>46.4mL was an independent unfavorable prognostic factor for patient survival. The ROC curve veriifed that the predictive ability of GTV?P was superior to that of T category (P<0.001). The cut?off values of GTV?P for the prediction of OS, LRFS, DMFS, and DFS were 46.4, 57.9, 75.4 and 46.4mL, respectively. Conclusion:In patients with locally advanced NPC, GTV?P>46.4mL is an independent unfavorable prognostic indi?cator for survival after IMRT, with a prognostic value superior to that of T category.

Objective:To explore the current status of central venous catheter (CVC) maintenance knowledge, attitude and practice among Intensive Care Unit (ICU) nurses, and analyze its influencing factors. Methods:From March to May 2021, convenience sampling was used to select 445 ICU nurses from 9 ClassⅢ Grade A hospitals in Anhui Province as the research object. The self-designed General Information Questionnaire and CVC Maintenance Knowledge, Attitude and Practice Questionnaire for ICU Nurses were used to investigate the nurses. Multiple linear regression was used to explore the influencing factors of CVC maintenance knowledge, attitude and practice among ICU nurses. A total of 445 questionnaires were distributed, 11 invalid questionnaires were excluded, and 434 valid questionnaires were recovered, and the valid recovery rate was 97.5%.Results:Among 434 ICU nurses, the scores of CVC maintenance knowledge, attitude, and practice dimensions were (22.14±3.16) , (37.31±3.28) , and (75.65±6.35) , respectively. The results of multiple linear regression analysis showed that hospital category and CVC training times were the influencing factors of ICU nurses' CVC maintenance knowledge ( P<0.05) . CVC training times, educational background, ICU working years, and ICU category were the influencing factors of ICU nurses' CVC maintenance attitude ( P<0.05) . ICU working years, CVC training times, professional title and hospital category were the influencing factors of ICU nurses' CVC maintenance practice ( P<0.05) . Conclusions:ICU nurses have a good attitude to CVC maintenance, but the level of knowledge and practice needs to be improved. Nursing managers should focus on ICU nurses with low educational background, short ICU working years, low professional titles, non-teaching hospitals and few trainings, strengthen relevant knowledge training, increase the level of CVC maintenance practice, and then improve the quality of nursing.

Objective:To understand the status quo of postoperative exercise adherence in patients with non-small cell lung cancer (NSCLC) and analyze the pathways through which disease perception, exercise self-efficacy, and coping style influence postoperative exercise adherence based on the Revised Common Sense Model (CSM) .Methods:NSCLC patients who underwent surgery in the Department of Thoracic Surgery of the First Affiliated Hospital of Anhui Medical University from March to September 2023 were selected by convenience sampling. Data were collected using a general information questionnaire, the Postoperative Exercise Adherence Questionnaire for Lung Cancer Patients, the Brief Illness Perception Questionnaire (BIPQ), and the Exercise Self-Efficacy Scale for Lung Cancer Patients. Results:A total of 245 questionnaires were distributed, with 241 valid responses collected. The postoperative exercise adherence score among the 241 NSCLC patients was (39.70±9.55). Exercise self-efficacy and coping style partially mediated the relationship between disease perception and exercise adherence, accounting for 81.31% and 13.64% of the total indirect effects, respectively. Exercise self-efficacy and coping style also had a chain mediating effect between disease perception and exercise adherence, accounting for 5.05% of the total indirect effects. Conclusions:The level of exercise adherence in NSCLC patients needs improvement. Negative disease perception not only directly negatively affects exercise adherence but also indirectly affects it through the mediating roles of exercise self-efficacy and coping style. Clinical healthcare providers should focus on guiding patients to develop healthy disease perceptions, increase their confidence in participating in exercise, and encourage the adoption of positive coping styles to enhance exercise adherence levels.
