1.Diagnosis and treatment of huge gastrointestinal stromal tumor combined with large inguinal hernia

Gastrointestinal stromal tumors (GISTs) are relatively common mesenchymal tumors of the digestive tract characterized by c-kit mutations and the stomach is the commonly involved site.Clinical and pathological diagnosis of GISTs can be achieved by B sonography,computed tomography and immunohistochemical detection of marker CD117.Surgical resection of GISTs has been the most effective therapy.Target therapy with tyrosine kinase inhibitors may reduce the development of recurrence or progression of GISTs.The optimized method of diagnosis and treatment of huge GISTs is still controversial.In this paper,the diagnosis and treatment for huge GIST combined with large right inguinal hernia were discussed.
2.Down-regulation of microRNA-187*expression in colorectal cancer and its roles in promoting cell ;apoptosis

Background and purpose: MicroRNAs (miRNAs) play an important role in tumor biological behavior. miRNAs are down-regulated or up-regulated in various cancer types, triggering abnormal cell differentiation, proliferation and apoptosis. This study was designed to investigate the expression and clinical signiifcance of miR-187*in colorectal cancer (CRC), and further to investigate its roles in promoting cell apoptosis. Methods:The expressions of miR-187* in 40 CRC cases were examined by real-time quantitative reverse transcription-PCR (qRT-PCR). The relationship between miR-187*expression and clinical features of CRC was analyzed. HCT116 cells were transfected with a miR-187*mimic and the apoptosis of the transfected cells were examined by lfow cytometry (FCM). Results:The expression of miR-187*was down-regulated in CRC tissues 0.165 (0.106, 0.428) compared with those in normal tissues 0.334 (0.211, 0.712) (P<0.05), especially in mucinous carcinoma and older age CRC (P<0.05). Transfection of HCT116 cells with a miR-187*mimic up-regulated the expression of miR-187*and increased cell early apoptosis (P<0.05). Conclusion: The expression level of miR-187* was lower in CRC. miR-187* expression correlates with histological type and age. Transfection of HCT116 cells with a miR-187*mimic accelerates apoptosis of tumor cells, suggesting that miR-187*is a potent tumor suppressor.
3.Progress in diagnosis and treatment of locally advanced pancreatic cancer

Pancreatic cancer,one of the common malignant tumors of the digestive system,is secretive in incidence and morbidity.Eighty percent of pancreatic cancer patients have already missed the opportunity of eradicative resection surgery when they were diagnosed,among whom,thirty to fifty percent of patients are in locally advanced tumor stage (specifically signifying the local extensive infiltration of tumor with serious vascular invasion and without distant metastasis) with a five-year survival rate of five to ten percent after excision.The pathogenesis of pancreatic cancer is still unclear,with a low rate of early diagnosis due to its lack of characteristic clinical manifestations.This paper reviewed the relevant studies on the clinical therapy of pancreatic cancer patients in locally advanced tumor stage,which is the key to improve the prognosis of pancreatic cancer.
4.A clinical treatment strategy for ruptured hepatocellular carcinoma

Objective To investigate the prognoses of patients with ruptured hepatocellular carcinoma (HCC) after resection combined with interventional therapy.Methods This retrospective study was conducted on 50 consecutive patients with ruptured hepatocellular carcinoma treated from March 2013 to December 2016 in the First Affiliated Hospital,Anhui Medical University.The patients were divided into two groups according to the different therapies,they underwent:Group A resection after interventional therapy (n =20) and Group B interventional therapy after resection (n =40).The COX regression multivariate analysis was performed and the 1-year,2-year,3-year overall survival rates (OS) were calculated.Results The 3-year OS rates were significantly different (OS:34% vs.0%,P =0.044).Multivariate survival analysis showed that age (HR 1.376,P <0.001,95% CI 1.191 ~ 1.589) and blood transfusion (HR 1.001,P < 0.05,95% CI 1.000 ~ 1.003) were the two prognostic factors which affected OS rates of patients.Conclusions Combined surgical resection and interventional therapy was effective in treating patients with ruptured hepatocellular carcinoma.Resection after interventional therapy gave a better overall survival rate than interventional therapy after resection.
5.Efficacy and safety of Jiuhua hemorrhoid suppository plus diosmin for the treatment of hemorrhoid hemorrhage: a multicenter, randomized, and controlled trial

Objective:
To compare the efficacy and safety of combining diosmin with Jiuhua hemorrhoid suppository versus diosmin alone for the treatment of hemorrhoid hemorrhage.
Methods:
The Jiuhua hemorrhoid suppository study was conducted in 10 medical centers across China from April 1, 2019 to June 30, 2020. Patients with hemorrhoid bleeding were randomized in a ratio of 1 : 1 to either receive Jiuhua hemorrhoid suppository and diosmin tablets (the study group) or diosmin tablets alone (the control group). The suppository was used once a day after defecation or at bedtime after rinsing the anus with warm water. Diosmin tablets were administered only once a day (0.9 g). The primary endpoint of the study was the assessment of hemorrhoid bleeding relief 7 ± 2 days after treatment, classified as “very effective” “effective” and “ineffective”. The secondary endpoint included the evaluation of pain alleviation using the visual analogue scale (VAS, with scores ranging from 0 to 10) and edema (with scores ranging from 0 to 3). The safety of the two treatment regimens was evaluated 14 ±
2 days after drug administration.
Results:
The full analysis set (FAS) comprised 107 participants in the study group and 111 in the control group, while the per-protocol set (PPS) included 106 participants in the study group and 111 in the control group. In terms of hemorrhoid bleeding, the proportion of very effective and effective cases in the study group were significantly higher than that in the control group [106 (99.06%) vs. 91 (81.98%), P < 0.0001] in the FAS, and the PPS results [105 (99.06%) vs. 91 (81.98%), P < 0.0001] were comparable to the FAS results. The pain VAS scores at day 7 after treatment were comparable between the two groups (0.80 ± 1.17 vs. 0.80 ± 1.20, P = 0.2177). The majority of the participants in both groups had an edema score of 0 at day 7 after treatment [96 (89.72%) vs. 99 (91.67%), P = 0.370 5]. Adverse events (AEs) occurred in 9 patients (8.4%) in the study group and 3 patients (2.7%) in the control group. In addition, 5 AEs in the study group and 1 AE in the control group were possibly in association with the study drug.
Conclusion
Compared with the administration of diosmin oral tablets alone, the addition of Jiuhua hemorrhoid suppository to the tablets demonstrates enhanced efficacy in addressing hemorrhoid bleeding, with satisfactory patient adherence and acceptable safety.
